Your organization will be instructed to provide a list of patients for whom you would like to receive alerts. You may receive an alert telling you which hospital the patient is admitted. You can also receive an alert once the patient is being discharged. Once the alert is received you can choose to login to the Connie portal and view the patient’s clinical information that the hospital has shared with the HIE.
Patients have the right to
opt-out of any notification and HIE service. Your organization will need to acknowledge participation in the Health Information Exchange (HIE) and provide step-by-step instructions if a patient chooses to opt out.
You can only receive notifications on patients that you have a current treatment or previous care coordination with.
WHAT IS PROMPT? PROMPT (Proactive Management of Patient Transitions) is a tool that was developed to provide clinicians with an easy-to-use interface to access notifications and other important patient health information from the Encounter Notification Service (ENS).

Subscribing organizations are required to provide a panel list of who they have appropriate authorization to receive encounter alerts. The template and specifications for creating a patient list will be provided, along with directions. Organizations acting as data sources and subscribers may choose to auto-subscribe to the creation of patient panel lists via their ADT interface.
