In cats the pruritus associated with infection may be associated with moder ate to severe selftrauma to the head (Figures 3.16, 3.17). In some cases, perhaps in cats more than in dogs, the mite causes clinical signs distant from the ear. Scott 8 considered that this might be a consequence of cats sleeping in a curled position so that the ear is apposed to the tailbase. Two syndromes may be associated with ectopic infection: • Crusted papules, i.e. miliary dermatitis. • Patchy alopecia. Asymptomatic infection may be a feature of older cats where very high numbers of mites may be found with apparently no associated clinical signs10. The presence of asymptomatic carriage in dogs has not been considered a major problem in veterinary dermatological texts, but in light of the discussion above it should be borne in mind, particularly when treatment protocols are discussed.

The mite is relatively large and may be easily seen in the external ear canal with the aid of an auroscope (Figure 3.18). Direct observation may not always result in a diagnosis: • The degree of discharge may make direct observation difficult. • There may be so few mites that direct observation is not possible. In these situations, microscopic examin ation of discharge may be necessary. Gentle maceration of collected samples in mineral oil will aid diagnosis, and microscopic examination under low power should show evidence of infestation. Aetiology and Pathogenesis of Otitis Externa
Demodex canis, D. felis, and D. gatoi
Demodex canis has been reported as a rare cause of otitis externa in dogs. It may occur as part of a generalized condition, in isolation, or as a longterm complication of juvenileonset generalized demodicosis which has apparently responded to treat ment11. A history of demodicosis should alert the clinician to the possibility of otodemodicosis but cases arising de novo should not be discounted. In cats, demodicosis is more usually associated with erythema (Figure 3.20) and crusting on the pinnae and head, rather than otitis externa. However, D. gatoi may be asso ciated with a ceruminous otitis externa in cats. Diagnosis is based on recovery of demo decid mites (Figures 3.21–3.23) in skin scrapes and on cotton swabs from the external ear canal. Punch biopsy samples would also give appropriate material for a diagnosis.

Checking the anesthetic machine and breathing circuit for leaks Checking for leaks in the anesthetic machine and breathing circuit can be divided into positive pressure and negative pressure leak checks. POSITIVE PRESSURE LEAKS • Checking the anesthetic machine and breathing circuit for leaks is easy and should be performed prior to each anesthesia case. A leaking machine or breathing circuit is not only inefficient for anesthetizing a patient properly, but it also pollutes the environment. An incorrectly connected anesthetic machine and breathing circuit could be fatal for the anesthetized animal. • To check for leaks, ensure that the anesthetic outlet fittings of the vaporizer are tightly in place and that the breathing hoses are connected to the anesthestic machine properly. The pop-off valve of the rebreathing circuit is closed, the ‘Y-piece’ of the rebreathing circuit occluded, and the oxygen flowmeter turned on to pressurize the breathing circuit to 30 cmH2O (77). Once this pressure is reached, the flowmeter is turned off. The breathing circuit and the machine should hold this pressure for at least 10 seconds to be considered free of leaks. • In a non-rebreathing circuit, the patient end of the breathing circuit is occluded. The non-rebreathing circuit does not contain a pressure manometer. However, a manometer can be placed between the reservoir bag and the breathing hose of the non-rebreathing circuit to perform the leak check in a similar fashion as for the rebreathing circuit (78, 79). The pop-off valve is closed and oxygen flow continued until the pressure manometer reads 30 cmH2O. The oxygen flow on the flowmeter is then turned off while continuing to occlude the breathing circuit system. If the pressure remains at 30 cmH2O for 10 seconds, there are no leaks in the system. A breathing circuit is unacceptable if it leaks at ≥200 ml/minute. • Do not use the oxygen flush valve to inflate the reservoir bag, but instead turn on the oxygen flowmeter. When the oxygen flush
valve is depressed, oxygen bypasses the flowmeter and vaporizer and goes directly into the reservoir bag via the fresh gas outlet. This allows a leak between the flowmeter and the vaporizer, or from the vaporizer to the fresh oxygen outlet, to remain undetected. • If leaks are detected, several areas should be checked first. The priority areas are the reservoir bag itself, the corrugated breathing hoses and their connectors, the filler cap on the vaporizer, the end caps on either side of the vaporizer, or the CO2 absorbent canister. A leak can be easily detected by spraying soapy water (use a diluted Nolvasan antiseptic soap solution) onto these areas. The leak check procedure is repeated to pressurize the breathing circuit and look for bubbles in the soapy water (80).

Risk factors
• Balance between practicalities of shoeing for continued training/racing (avoiding pulled shoes) and prevention of longer-term deterioration in shape of foot; not always compatible. • General rule: style of shoe used should be the simplest that successfully keeps horse sound in short/medium term. May require experimentation over several shoeings. • Many cases manageable with correct trimming (to prevent excessive length of toe and crushing of heel tubules) combined with shoes that distribute load better through back of foot (more length at heel) and ease breakover. • Bar shoes (straight/egg bar/heart bar) +/− sole packing for more troublesome cases. • Flat- or reverse-inclination of P3: if poor response to above, may be appropriate to wedge heels (5–6°) with a wedge-shoe or pads. Can sometimes yield good medium-term soundness, although subsequent increased pressure on coronet of heels may reduce rate of heel growth.
History • Typically develops over months/years. • More rapid loss of foot shape may occur in a shorter period (weeks) with inappropriate foot care in susceptible horse; commonly during rest period from training.

Diagnosis • Clinical findings are definitive (Figure 2.3) • External appearance of hoof is poor guide to alignment of internal structures. • Diagnostic blocking: lameness usually abolished with palmar digital nerve block. • Imaging rarely needed but recommended if lameness is persistent or recurrent. • Radiography: may reveal ‘reverse rotation/ inclination’ of P3; palmar processes of P3 closer to solar margin than the toe is (on LM projection) (Figures 2.4a, b). • MRI: occasionally justified in poorly responsive cases to rule out other injuries. Signs of laminar disruption and concussive strain involving the P3/ navicular bone are typical.

• Dorsopalmar foot imbalance is very common. • Risk factors include shoeing practices (frequency of shoeing/overdressing of heels/shoeing close at heels to avoid lost shoes) and training itself (fast work linked to lowering of heel angle). • Individual susceptibility may have a genetic component.
• Biotin supplementation: small improvements in hoof growth/quality with long-term (months) daily supplementation (20–60 mg/day). • Problematic cases may benefit from being rested barefoot in deep bedding to encourage heel growth when out of training.
Prognosis • Condition not reversible but progression can usually be halted. • Most cases can be kept sound in training with appropriate management.
Quarter crack These are longitudinal cracks of variable length in the hoof wall (Figure 2.5) that can develop anywhere from the medial or lateral quarter to the heel. They may be full or partial thickness; involvement of sensitive tissue is common due to the thin hoof wall at the quarters. Cracks usually originate in the upper half of the hoof wall or close to the coronet. They are most common in the front feet.

The duration of this phase is highly variable depending on the horse and the duration of the stance phase of the forelimbs. This, in turn, is dependent on the height of the jump and the speed at which it is cleared. There is always an intermediary state between the stance phase of the forelimbs and the subsequent stance phase of the hindlimbs, which can be characterized by several features: The lumbosacral and thoracolumbar junctions reach maximal flexion, which is often greater than the flexion that occured during take-off (15.6, 15.8). This results in intense compression of the vertebral bodies and intervertebral discs (15.9). Furthermore, the interspinous ligaments are placed under tension and the supraspinous ligament undergoes intense traction. These mechanical stresses appear to be one of the main causes of back pain in show
jumpers. One condition in particular that can result in back pain is desmopathy of the supraspinous ligament at its insertions (15.9). The erector spinae muscle undergoes maximal lengthening. Flexibility and elasticity of this strong muscle dictate the degree of spinal flexion and contribute to efficient re-establishment of equilibrium. In order to improve this movement, training of the horse must aim to improve the suppleness of the topline as well as the tone of the ventral muscle chain (see Chapter 4, Lowering of the neck). The cervicothoracic junction undergoes an initial extension, which assists in lifting the neck. This results in a reduction of the load on the forehand and assists in the establishment of equilibrium. This movement is a result of concentric contraction of the dorsal cervical muscles (15.10).
