Crosby Scholars Senior Program Checklist To graduate high school as a Crosby Scholar, you are expected to complete the following checklist. Your Senior Advisor will guide you! o o o o o o o o o

Complete 1 Senior Academy by December 12 Complete 5 hours of Community Service and Submit Form^ Complete the College Fair/College Visit requirement^ Submit Target List of Colleges to Crosby Scholars Senior Advisor Meet with Senior Advisor to complete all Conference Notes Submit at least 1 College Application* Complete the Residency Determination Service (RDS)* Complete the Free Application for Federal Student Aid (FAFSA)* Submit a Last Dollar Grant Application or at least 1 Scholarship Application*

SENIOR YEAR IS HERE.

^ College Fair/College Visit may be completed in Sophomore or Junior years. Community Service may be completed in Junior year. * If your post-secondary pathway is not applying to a 2-year or 4-year school, your Crosby Scholars Senior Advisor can provide alternative ways to fulfill these expectations.

Agree to random drug-screening Fulfill terms of Partnership Agreement Maintain enrollment in and graduate from a Forsyth County public school Complete the Program with a cumulative weighted GPA of 2.0 or higher to be eligible to apply for Last Dollar Grant funding

Scholars who do not meet these 5 expectations will be dismissed from the Program.

• Attend one Think Like A Senior session* • Submit Senior Advisor Form* • Visit College Campuses • Attend Individual Senior Advisor Meeting* • Elect Crosby Scholars Presidents • Parent to submit Finish the FAFSA Agreement • Senior Academies begin*

• Create and submit Target List of Colleges* • Retake the ACT and/or SAT • Write drafts of College Essays, if required for your College Applications • Review decision plans (EA/ED/REA/RD), Application Types (Common App, Coalition, etc.) and Application Deadlines • Create a FSA ID to sign your FAFSA • Refine your Activities Resume • Complete the Residency Determination Online Interview (RDS)* • Individual Senior Meetings continue* • Senior Academies continue*

WINTER • Community Service Deadline is January 31* • Continue to apply for Crosby Scholars scholarships • Attend at least one Senior Academy by December 12* • Submit at least one College Application*
