Placement User Guide
Many of us struggle with the decision to move our loved one to a care facility, whether it is a semi-autonomous setting or full care facility. These tools will guide you in assessing which facility is best for your loved one.
It is recommended you visit at least 3 facilities before making a decision. Try to go during a mealtime, and even have a meal at the facility. It will help you get the “feel” of the facility.
Once you have identified your facility of choice, it is also a good idea to visit the facility once again, without making an appointment. This enables you to see how smoothly things are run, when they aren’t expecting you. You may also want to consider your return visit occurring at a different time of day from your first visit.

Consultation with a care provider: placement: Use this tool every time you visit or call the facility. It will structure your conversation with the care provider to ensure you are obtaining all of the updated information on the overall functioning of your loved one.

Consent to Disclose Information: Use this form to establish permission. Have your parent sign a form for EACH of their care providers and request that a copy is placed in your parent’s chart. Keep a copy for yourself in case you need to show or fax it to a care provider in the future. Please note that some provinces or states may have another form that is legally required.
