And that the medical costs for treating fall-related injuries last year was $34 billion and is expected to grow to $64 billion by 2020? This is

Dr. Daniel Deems

FYZICAL Principal and Medical Advisor ddeems@fyzical.com

Savvy Otolaryngologists Creating by Making Balance Their Business As a practicing Otolaryngologist for 19 years, Dr. Dan Deems is keenly aware of the balance challenges many of his patients have. None more so than Dolores M., who came into his OTO practice using a walker because she had recently broken her hip from a fall. Dr. Deems discovered that although she was receiving physical therapy to rehab her hip, she wasn’t receiving therapy to fix the balance issue that made her fall in the first place. Tired of constantly seeing their patients in lifethreatening danger from falling, Dr. Deems and his partner Viengsouk Phommachanh decided to do something about it.

Wealth

Dr. Deems and his partner Viengsouk Phommachanh SO: What made you decide to open a balance center within your Otolaryngology practice? It just made sense to me and my partner, Viengsouk (Vien) Phommachanh. There is such a huge need for it among the elderly population who suffer from disequilibrium and balance challenges. And for us Otos, otology including study of the vestibular system is a significant part of all of our training. So who better than Otolaryngologists to provide balance therapy? As surgeons, we like to “fix stuff,” and you can’t always “fix” a patient suffering from imbalance, they may come back 2, 4, 8 weeks later with the same complaint. Not only are they unhappy, but their primary care doctors

DD:

are unhappy, too. If you can’t offer them a treatment plan, what can you offer them? But now, I can offer them a treatment plan; for patients with vertigo and those patients who need balance retraining.

SO: How can a balance center help your patients? The death and injury rates from falls continue to increase – and it’s absolutely treatable! We know now that neuro-regeneration can occur in the nervous system, losing one’s balance and falling is not an inevitable result of aging. Instead of just diagnosing and sending my balance-challenged patients away for therapy, crossing my fingers and hoping for the best, now I can offer treatment for them in my own facility and see their progress for myself—it’s very rewarding.

Amazing! We’ve been open only 4 weeks and have already seen 62 new patients just for balance therapy. The phone is ringing off the hook. And we haven’t done any marketing yet! Within one hour of becoming part of an insurance registry, we had a referral for PT from a physician who doesn’t normally refer to us. Unbelievable.

For example, a patient of mine, who is 90 years old, hunched over and incessantly falling, within two weeks of balance therapy our PT has him doing the ragtime jazz dance sidestep. It’s amazing!

of the work is being done by others in our employ, which means whether I’m there or not, or Dan is on vacation, our practice is still generating income. It’s a great opportunity for boosting our revenue.

SO: How does having a balance center benefit your Otolaryngology practice?

SO: What’s it been like since you opened your balance center?

It never occurred to Dan and me before we knew of FYZICAL’s turnkey balance program that we could even do balance therapy. But now we see the opportunity to grow our practice and help our patients without the burden being placed on our backs of learning how to run a new business. FYZICAL’s business model, support staff, and balance experts make it easy. And the benefits are enormous: Medicare reimburses for balance testing and therapy; the increased foot traffic will likely lead to more hearing testing and hearing aid sales, and with this type of ancillary business, the majority

Within one hour of becoming part of an insurance registry, we had a referral for PT from a physician who doesn’t normally refer to us. Unbelievable. the antiquated “foam and dome” balance therapy. That’s old school. FYZICAL is the future. FYZICAL uses state-of-art, proprietary technology and programs to effect real change in patients.

SO: What does the future look like for you and your Otolaryngology practice? Boundless. And exciting! We’re thrilled to be able to provide this type of service to our patients and the older community in general. It is an absolutely untreated patient population; 27,000 people last year died from a fall. That’s more than gastric cancer and esophageal cancer combined! So providing balance therapy for fall prevention is like curing cancer, and it doesn’t take hundreds of thousands of dollars of chemotherapy. It’s people dancing. Its people getting out and just going back to the lifestyle they used to have.

For us, it’s rejuvenated the practice, and it’s rejuvenated me. Most of us as physicians, we started to despise medicine because of the EMRs, PQRS, and government regulations, and so we weren’t enjoying practicing. Now, I’m loving practicing, and Vien is just giddy with excitement.

We hosted the seminar in our practice two weeks ago and it was attended by 30 people— and every one of them wanted to get back in here and be scheduled for balance therapy! There was no cost of acquisition; we just gave a fun lecture from the heart about our passion for balance. There is such a huge need for this that there is certainly no shortage of patients.

SO: How did you know the best layout for your balance center and what equipment to buy?

FYZICAL helps with all of that. FYZICAL’s team of balance experts has extensive expertise in the design, layout, and build-out of balance centers. DD:

SO: Do you have much competition from other Otolaryngologists doing balance therapy? No, nobody else is really doing balance therapy in the area. If any of them are, they’re most likely doing

DD:

That’s true! (He laughs). It’s just so exciting and rewarding to be doing this for our patients.

Every Otolaryngologist should be in the business of balance. It’s the perfect complement to your practice. You’re saving lives, helping people regain their lives and remain independent, all while also making passive income. It’s literally a Iife changer; for my patients and for me. he numbers are alarming and disturbing. When people over the age of 65 suffer a fall that leads to injury, as more than 30 percent of that population do each year, one in five will not survive the injury. A fall can be a death sentence. The good news is that balance therapy after a fall can significantly increase odds of survival, and balance therapy before a fall may just well prevent one. Indeed, as Dr. Michael B. Siegel, M.D. says, “Fall prevention through balance rehabilitation saves lives.”

practitioner have an ENT practice in South Dakota where they see more than 350 patients each week, including a significant number of them with balance issues. Unfortunately, prior to adding a FYZICAL balance center, they were only able to test and treat a handful of balance patients each week.

“All of my hearing aid patients are also balance patients. Many of the new balance patients that are referred to my office are also hearing loss patients. It just makes sense.”

Unmet Need

Over $34 billion was spent on fall-related injuries in 2015. In addition, more than 63 million Americans need balance therapy. And it’s not just the elderly that can

6

benefit greatly from balance therapy. For middle age folks, one of the most frequent complaints in primary care is dizziness and vertigo and is often unexplained. For children, especially those that play sports, vestibular deficiencies are common in those who have sustained a concussion.

SUCCESSFUL OUTCOMES I SavvyOTO.com

Vestibular damage by disease or injury can have a major negative impact in a child’s growth and development and result in musculoskeletal injuries. Just ask Dr. Sarah Powell. Powell and her two physician partners and a nurse

Powell says that new patients who “can’t figure out what’s wrong with them” are now able to come in for balance testing without seeing an ENT.

“The community is full of people in need, with not enough time to serve them. FYZICAL has shared with us how to meet the needs of these patients with physical therapists. We do not have to turn away patients now.” Powell states. As a private practice Otolaryngologist, you already have the two most important ingredients for the recipe to fast-track

success with FYZICAL Balance & Dizziness Centers: (1) a steady flow of patients and (2) administrative support. All that’s left is to add a physical therapist to the mix. At first, Mike Siegel didn’t see the connection. “Adding physical therapy to my practice really didn’t make sense until I understood how the people at FYZICAL specialize in helping patients with balance issues,” Siegel said. “All of my hearing aid patients are also potential balance patients. Many of the new balance patients that are referred to my office will also be hearing loss patients. It just makes sense.”

Increase Your Ancillary Revenue While Saving Lives

The perfect ancillary business for otolaryngologists, a balance center is similar to how an audiologist drives income with hearing aid testing, sales and service. With a FYZICAL Balance and Dizziness Center, a physical therapist can also drive income by providing comprehensive balance testing and rehabilitation in a world-class, state-of-the-art facility. “In my ENT practice we see hundreds of patients each month and many of them have balance issues… and that’s not even including the patients that come in for dizziness. I saw a patient the other day for a nasal fracture. I asked him what happened and he told me he had been having problems with his ankle and calf and he lost his balance and fell. In the past I would have done a closed reduction of a nasal fracture and sent him on his way,” Siegel said. “I really never thought about it. I was sending him out to fall again and the next time it might be worse.” “FYZICAL is special because what we are doing will save lives,” says Siegel. Simply put, now is the time for otolaryngologists to get into the business of balance.

The Time is Now

A FYZICAL Balance and Dizziness Center is the only balance franchise in the U.S. that can both save lives and expand patient care while tapping into a near $68 billion industry. Finally, there is a way to help patients with balance problems. There has never been a better time for Otolaryngologists to get into the business of balance. The benefits are real and they are huge. Are you ready to join other savvy Otolaryngologists who are taking part of one of the greatest opportunities to ever exist for their profession? Visit SavvyOTO.com to discover how you can help save lives and expand patient care.

4 Reasons Otolaryngologists are Making Balance Their Business 1. Enhanced patient care

Nearly 30% of your existing patients suffer from dizziness and balance dysfunctions. These people are in critical need of balance therapy to circumvent life-threatening falls.

2. Increased ancillary revenue Medicare and most insurances companies cover Balance therapy. Reimbursements are earned for the initial evaluation, balance testing and for the actual treatment allowing you to generate substantial amounts of ancillary income.

3. Employees become significant income producers

Perhaps the best part of owning a balance center is your employees produce the revenue whether you are in the office, operating room, or on vacation. This steady flow of income creates a secure, diversified business that increases the value of your practice.

4. Maximum revenue per patient

Welcoming new patients into your practice for balance retraining leads to more hearing testing, hearing aids, and other services such as voice, swallowing and sinus evaluations. This “cross-pollination” of services for each patient stimulates horizontal growth within your practice.

great feeling it was to be able to do that. It was all just a really good experience. I’m so glad I went to FYZICAL. You just can’t beat the people and balance program they have. Everybody who worked with me at FYZICAL is just amazing: Mitch and Jill, Beth, Scott and Dave... all just first class!

PETER’S PERSPECTIVE: I’ve been a very energetic person my whole life. I’m a doer and a shaker! I’ve been going to the gym for 25 years. I love my independence and like to get around by myself; but it was getting difficult. I drive a Mini Cooper, and that’s a small car. It’s hard to get in and out of, especially with a cane or walker. I like to go to church, but, I know people are just being nice and respectful, but when I’m using my walker, and they’re always holding the door for me,…and how they look at me… well, I just don’t like not being able to do things on my own.

“He had a wonderful attitude. He was very energetic and determined. After completing a task, he would just light up. And, when he saw he could walk, and jog, he would light up even more!” – Beth Dole, ATC – FYZICAL Traverse City, MI

I was prescribed physical therapy because I was told I had vertigo. I went to FYZICAL Therapy & Balance Centers and when they did their initial evaluation on me, they said: “Who told you that you have vertigo? You don’t have vertigo…you have a severe balance problem!” What struck me was how serious they were about balance, and how important balance therapy is—they convinced me. I could see they were passionate and sincere. I had been sent to a hospital PT clinic once. They weren’t as serious about balance there. Weren’t serious at all about whether I did balance therapy or not. And they definitely did not have all the fabulous equipment FYZICAL has. So, I immediately started my balance therapy with the PTs at FYZICAL. It wasn’t always easy. They made me do things I hated doing, and some days I just didn’t want to go. But, they were just so committed to me and passionate about my progress. Soon, I started looking forward to going. After starting my therapy still using my walker, I was eventually able to switch to only using my cane. After a few weeks, I was able to ditch the cane, too. After a few months of balance therapy, I could walk on my own; I could jog on my own! What a

I’m a different person now for having gone through it. I recommend it to everyone. I feel sorry for someone who doesn’t experience FYZICAL.

PT PERSPECTIVE (AS TOLD BY BETH DOLE)

Peter was 95 years old when he came to see us. He was having trouble with falling and BPPV. He was dependent on a walker and was shuffling his feet, taking small, unsteady steps. He told us his kids wanted to put him in a nursing home, but that wasn’t for him. He wanted to remain independent. His primary goals were to get rid of the walker and be able to get in and out of chairs easier. We started his balance therapy primarily utilizing the SOS harness system. We also did some core strengthening. We saw the biggest difference in his balance in just 4-6 weeks of therapy. He was doing the speed ladder on the floor: stepping in and out, stepping over and back and back lateral. He was doing 5-8 different patterns— that’s impressive for a 95 year old! He had a wonderful attitude. He was very energetic and determined. After completing a task, he would just light up. And, when he saw he could walk, and jog, he would light up even more! His grandkids came in to watch him and were amazed at his progress. He did about 10 weeks of balance therapy in total, and he’s doing great. He still exercises regularly at the gym, he drives, and he’s involved with his church. He’s very happy that he’s able to remain independent. It’s amazing to see patients make that much progress and really make a difference in someone’s life. When I watch the video of Peter and see his big, long beautiful strides and that fist pump….it’s an incredible feeling, especially knowing that just a few months earlier he shuffled into our practice on a walker. 941.227.4313 I SUCCESSFUL OUTCOMES

What Happens in Vegas Should NOT Stay in Vegas... by Ellyn Santiago

YZICAL Balance Center of Las Vegas specializes in helping patients with dizziness, balance disorders, and those in need of vestibular rehabilitation, changing lives in the process. A young boxer named Eric Gonzalez couldn’t figure out why he had these “weird feelings” that included fatigue and dizziness; symptoms that were not only often debilitating but caused the otherwise healthy young athlete to faint and fall at work. Three months of “we can’t help you, we can’t help you,” left him not only frustrated but afraid. Finally an otolaryngologist told him, “I don’t know exactly what’s wrong with you, but I think I know who will.” Eric found answers and the solution at FYZICAL Balance Center, Las Vegas. After his helicopter was shot down in Afghanistan, decorated U. S. Air Force pilot Christopher Aguilera suffered myriad major traumatic and injuries; his right foot

FYZICAL: Leading the charge. Transforming healthcare from “Sick Care” to “Well Care” – Jim Abrams

J

ames Abrams, co-founder of FYZICAL, is a preeminent business leader and builder who has led eight companies to national prominence, largely from the ground up going up against entrenched and well-capitalized competitors, utilizing robust and solid business management, marketing and customer service practices. A former teacher and college professor, Abrams is acclaimed for his incredible ability to teach, advise, and coach small-business owners on how to become highly successful even in the most competitive of markets. Abrams successes in business are almost unbelievable: He co-founded FYZICAL, LLC, in early 2013 and is on the Board of Directors. He is also co-founder and CEO of BizZoom, a private equity firm he started in 2012. In 1998, Abrams founded Clockwork Home Services, Inc. In 10 years serving as President and CEO, Clockwork became the largest service company in North America, growing to over 1200 employees, $600 million revenue under brand (over $200 million in company owned locations). Abrams sold this company for $183 million. Prior to Clockwork Home Services, Abrams was the President and Founder of AirExperts, which he grew into the largest residential service company in the nation. Abrams then founded Service Experts, taking it public on NASDAQ & NYSE and it grew to over $900 million. From 1975 to 1980, Abrams worked with Trane where in five years he rose through the ranks to become the National

Abrams has created an innovative business model based on a Nobel Prize-winning formula that is “designed to help you provide the best in quality patient care as well as achieve astonishing business success. Abrams’ philosophy is with the help of medical professionals, business experts, and physical therapists, FYZICAL is leading the charge to transform healthcare, as he puts it, from one of “sick care to well care” by providing services focused on quality outcomes, total well-being and prevention. On FYZICAL Balance and Dizziness Centers, Abrams says, investing in prevention is critically important. And with an aging population that is susceptible to falls due to balance issues, FYZICAL is the answer. “The best time to treat someone with balance issues is before they fall, not after,” he says wisely. “Let’s prevent them from falling in the first place.” Given Abrams track record and expertise as a business builder, there’s nowhere for FYZICAL to go but up. In fact, FYZICAL was listed as the fastestgrowing health care franchise in America in Entrepreneur magazine’s 2016 Franchise 500.

Is Balance

Right For You? What educational credentials should a balance rehab trainer possess? A balance and dizziness center should have at least one Doctor of Physical Therapy (DPT) or a Physical Therapist (PT).

How do I hire a Doctor of Physical Therapy or a Physical Therapist? Physical Therapy is one of the fastest growing professions in the U.S. generating a large pool of quality applicants. The opportunity for PTs to work with Otolaryngologists as part of a comprehensive medical team where they can diagnosis and treat patients creates a rewarding and fulfilling environment. When you factor in the steady flow of patients that already exist from your current client base, you will attract the most talented PTs because of the secure working environment that creates peace of mind and job stability.

Do I have to hire a PT experienced in balance and vestibular therapy? It is not necessary and perhaps even a benefit to hire a PT with little balance and vestibular training. FYZICAL has developed a proprietary balance program evidenced to provide the best patient rehab experience. FYZICAL’s comprehensive Balance Certification program ensures the proper training of PTs in balance therapy and retraining and instills in them the desire to continue to learn and elevate their level of expertise. Simply hire a PT who is compassionate and eager to learn, and FYZICAL will provide the training to turn them into a master in balance therapy.

How much space is required to start a balance center inside my existing practice? FYZICAL provides a variety of different size models to meet the individual needs of each practice. With as little as 500 square feet, you can start providing a valuable, indemand service and build a very profitable ancillary business.

How do I design my Balance Center? FYZICAL’s team of balance experts has extensive expertise in the design, layout, and build-out of balance centers. Simply provide us with a floor plan and we will assist you in designing the optimum balance center for your space to maximize patient care and revenue.

What equipment is needed? The square footage you have dedicated to your balance center will determine your equipment needs, and the FYZICAL support team will guide you on what your Balance Center will need for optimum patient benefit and revenue. As your Balance business grows, you can expand your balance center to meet the growing demand.

What is the equipment investment to start a balance center? With different levels of balance and dizziness programs, FYZICAL can design a balance center to meet virtually any budget. For as little as approximately $29,000, you can have the equipment necessary to be in the business of balance. Perhaps even less if you are able to acquire previously-owned, quality equipment.

What is the typical number of visits for balance retraining? There are many factors that determine the number of visits a patient needs to regain optimum health, but typically you can expect 8-12 visits per patient on average.

Is balance therapy covered by insurance? As a general rule of thumb, most balance and dizziness therapy is reimbursable by insurance.

What is the typical reimbursement for balance therapy? Amounts vary per insurance carrier and state, but typically a visit will be $80 $120 per patient visit. If your patient visit average is 10 visits, you could expect $800 - $1,200 of revenue per patient being generated by your therapist. When you include additional services provided by your audiologists, the amount per patient could substantially increase.

How much is the investment to own a FYZICAL? The investment for a Balance Center is determined by many factors. Such as the level of patient care you wish to provide, your space allocation, equipment needed, and of course the population of the exclusive area you select. Once you secure your area, you lock out all other healthcare providers giving your Otolaryngology practice a huge competitive advantage in patient care and income earning opportunities. Upon submitting an application and subsequent approval, the FYZICAL team will discuss your goals and objectives with you to determine the proper investment to maximize your highest R.O.I.
