Fundraisers are important sources of financial support for CHaD, and many of our programs rely on philanthropy to provide the best care for our patients and families. Every dollar donated and every hour volunteered represents an investment in strengthening the care of our children and the experience of our families. Here are just some of the programs and initiatives we fund.
Complex Care Program The Complex Care program focuses on families and CHaD kids who face a multitude of diagnoses while working with various pediatric specialists. This team of providers and nurses integrates itself into the daily lives of these patients by assisting with all aspects of the patient's care from: · Booking and scheduling appointments with various specialists on the same day to reduce the frequency of trips to CHaD and, therefore, improving the patient's quality of life while reducing stress on the family. · Arranging and sharing medical records & communication between specialists. · Coordinate home visits from CHaD providers while communicating with and educating local rural hospitals with the patient's diagnosis & care plan. · Facilitating communication with insurance companies for families. When a family faces complex treatment and diagnoses, the Complex Care team ensures the focus remains on the child rather than the administrative side of their medical care.
Child Advocacy & Protection Program [CAPP] CAPP cares for the most vulnerable patients in our region: children who may be victims of physical abuse, psychological/emotional abuse, sexual abuse, and/or neglect. CAPP's multidisciplinary team responds to reports of suspected child abuse and/or neglect throughout New Hampshire and Southeast and Southern Vermont and provides psychosocial evaluations, medical evaluations and treatment, and referrals for further services. CAPP's goal is to help children lead happy and healthy lives, free of abuse and medical complications. Through skilled forensic interviewing or medical testing and diagnosis, CAPP helps to reassure children that they are safe. When medical care is needed, it is sensitive, child-focused, and delivered by a team of experts trained in child abuse. Each year CAPP cares for several hundred children and their families.
