What Is the Prostate? The prostate is a small gland in men that is part of the reproductive system. It’s about the shape and size of a walnut and sits below the bladder and in front of the rectum. It surrounds part of the urethra, which is the tube that carries urine from the bladder.
What does the prostate do?
The prostate functions to produce the fluid that will carry sperm cells from the testicles and along with fluids form other glands make up semen. Its muscular contractions also ensure that semen is pushed out through the urethra during ejaculation.
What is BPH?
Benign prostatic hyperplasia (BPH) is the excessive growth of the cells of the prostate. The prostate first grows during puberty where it almost doubles in size and then begins to grow again after the age of 25. The cells continue to grow throughout a man’s life and can eventually cause compression of the urethra resulting in difficulty urinating.
The increased resistance that occurs with the obstruction can result in changes to bladder function, which can result in increased urinary infections and significantly affect the quality of life of older men. Generally, a normal prostate is typically 15-30ml in volume and glands that are larger then 30 ml are considered enlarged, however the extent of enlargement and onset of symptoms varies considerably between patients. About 50% of men at 50 years of age will have some evidence of enlargement of the prostate gland and at least 30% will require some form of treatment by 70. PROSTA GOLD 3

What Causes BPH? The exact cause of BPH is complicated and poorly understood but there are several theories. BPH does not occur in males who have had their testes removed prior to puberty, which had lead researchers to believe that there is a connection between male hormones produced in the testes and the prostate. The main hormone in the prostate is dihydrotestosterone (DHT), which is created from the conversion of testosterone by the enzyme 5-alpha reductase. The level of DHT provides the signal for the prostate cells to grow and continues to remain high with aging. BPH likely occurs due to a dysregulation in the balance of DHT action resulting in signals to increase cell growth outweighing the signals for cell death. Many of the current pharmaceutical treatments for BPH focus on blocking the conversion of testosterone to DHT to try and limit the levels of this hormone in the prostate. Men who have metabolic syndrome and/or are obese are also at increased risk of developing BPH, as higher rates of prostate growth were observed among this population compared to healthy men. While the exact physiological relationship between metabolic syndrome/obesity and BPH has yet to be determined, a potential explanation is a greater level of inflammation present in the body. Those with higher levels of inflammation in the blood showed an increased risk of lower urinary tract symptoms. Specific inflammatory factors have been shown to stimulate the growth of prostate cells in lab studies and patients with highgrade levels of inflammation were noted to have significantly higher prostate symptom score values that those with low-grade inflammation. Recent genetic studies have identified specific variants which are associated with more aggressive growth of the prostate. Those with these genetic variants are at a 4-fold increased risk of developing BPH.

What are the signs and symptoms of BPH? When the prostate increases in size it can cause an obstruction to the flow of urine. Irritation of the prostate occurs because the when the prostate is enlarged the bladder has to work harder in order to empty.
Obstructive Symptoms
• Hesitancy or trouble initiating urination when trying to urinate despite having the urge to urinate. • Straining or needing to bear down in order to urinate • Weak stream or interrupted stream of urine. • Often unable to completely empty the bladder when urinating
Irritation Symptoms
• Increased urinary frequency- usually urinating more than 8 times a day • Inability to sleep through the night without having to urinate several times • A sense of intense urgency to urinate and often unable to delay urination • Pain after ejaculation or during urination

Serious Effects of BPH • While a rare occurrence, BPH can lead to bladder distension which in turn can lead to kidney issues • Significant disruption to lifestyle can occur due to increased urinary frequency, physical discomfort and decreased sexual function • Having BPH does not mean that you are at increased or decrease risk of prostate cancer; they involve growth of different cells in the prostate. - BPH Prostate cells grow inwards towards the urethra which is why urinary symptoms start to occur - Prostate Cancer starts on the outer layer of cells and grows outwards and is thus often silent, where no urinary symptoms are felt
How is BPH Diagnosed? In general your physician should perform a full history and physical exam, which should include a digital rectal exam (DRE) where the physician will exam the size, consistency and shape of the prostate. Lab work will also likely be ordered to help with the diagnosis. A prostate symptom severity score is also a helpful tool for your physician to determine what level of treatment you may need.
