rom where I sit, I feel as though I should be wearing a safari pith helmet, because sometimes the zoo-like quality of family life is overwhelming! At times it’s like my house is a monkey house: jumping all around, squawking, carrying on, making a mess. The antics should be broadcasted: “When last we visited the Day household, mother Susan’s arms were elbow-deep in her latest pair of rubber gloves, shoved deep into a bucket of hot sudsy water to scrub away the remains of the day!” Meanwhile monkey #4 (my youngest) miscalculates his aim at the potty again (as young boys are want to do) and monkey #3 has spilled his milk at the dinner table ... again. Father Day (that would be Dad) stands over the household, bewildered, and says in his oblivious way, “Honey, wouldn’t it be great to have a puppy?” You just have to laugh. You have to grin and bear it. Smile and wave. Perfectionism must be escorted to the door when it tries to be a guest, and none to kindly either — sometimes you have to drop kick it away. You have to GIVE IN to your wild family life or it becomes unforgiving of you. “Here Mama,” monkey #1 says, offering me a wrapper he picked up off of the floor after a recent tirade by the zoo keeper about what a mess our cage had become. The monkey smiles at me in a “I’m-agood-boy-aren’t-I?” kind of way. I smile weakly. Monkey #2 proudly presents me a stale crust of bread he’s pulled out from the sofa cushions. “Just trying to help,” he offers. My shoulders sag. Life in the trenches, that’s what this means. What does it matter anyway, that we should get all tied up in knots about how “just right” we want our lives to be? What’s the point? That we should have picture-perfect lives, homes, cars, husbands, kids? Doesn’t the world remind us that we’re not in charge anyway? We can try to be ... we can be control freaks ... but often we are reminded that really, the big stuff is NOT up to us. The big stuff is love. So life in the trenches means giving in. While I want my children to learn once and for all that wrappers and messes need cleaning up after, I don’t want them to hand me wrappers! I want them to kiss my cheeks, hug me often and try their best like I do. I want to be there for them as the mom they need me to be: I want to run with them, play with them, laugh with them and cry with them. The point is, you CAN’T beat the craziness of it all, so you HAVE to join in at the zoo. And every once in awhile, you have to close the bathroom door and take a good, long bath.

irque du Soleil returns to Greater Cincinnati with OVO! When a mysterious egg appears in their garden, the insects are curious, and then fascinated by the creature contained inside. With flying trapezes, acrobatics, lively music and an enormous mechanical flower, the show explores the cycle of life. “OVO is without any doubts the most family-friendly Cirque du Soleil show. You can feel the energy in the performers, in the different acts and in the music,” says Marie-Claude Asselin, touring publicist for OVO. “To see human-size insects is great for the kids and not frightening at all.” Performances are April 21 - May 8 under the blue and yellow Grand Chapiteau (Big Top) at Coney Island (6201 Kellogg Ave.). he Education First (EF) Foundation for Foreign Study needs families to host high school exchange students, and you don’t have to have a teen in the house to be a host family. The EF Foundation looks for all kinds of families willing to share their homes; ones with young children reap major benefits. “It’s been a real positive experience for us,” says Heather Herman, mother of two girls, ages 5 and 8. “Without a fellow teenager in the house, we didn’t have to worry about whether or not they would get along. He plays with our daughters when he’s here, but he’s been able to make his own friends, and it keeps home less stressful. Plus, it’s been good for us to be around a teenager!” For additional details, visit effoundation.org.

On April 9 from 12 - 4 p.m., kids will get the chance to check out sports like baseball, swimming and soccer, or learn about cooking, yoga and home exercise. “A lot of people with autism are not involved in sports and fitness, which puts them at greater risk for health issues,” says event organizer Doug Blecher. Adding that sports has always been a big part of his own life, Blecher says that along with introducing families to sports organizations for children with autism, he hopes the event will inspire new programs as well. Of Bubbles, Raspberries, & Speech How Blowing Bubbles and Razzies Helps Your Child Develop Communication Skills

T

BY LAURA AMANN he first time it happens, you don’t expect it. Your baby looks at you, sticks out her tongue and makes a gentle razzing sound. It’s almost impossible not to smile and make a raspberry back at her. Just try. What starts as a fun and silly game between parent and baby actually sets the foundation for language, social skills and fine motor skills such as eating and drinking from a cup. So pucker up those lips and help your baby begin to experience a whole new world.

Raspberries: More Than Just a Fruit, They’re a Workout Most babies start blowing raspberries and bubbles between 5 - 6 months of age. After a few tries, they usually catch on quickly, particularly if you encourage them. And you should blow back; besides being darling, those raspberries teach a variety of important skills. “Razzies really teach babies how to regulate their voice, how to turn it on and off, change the volume and the pitch. It shows them how to navigate the diaphragm, mouth, lips and tongue,” explains Tara Kehoe, a speech and language pathologist at Easter Seals. All that noise gives the jaw a great workout by exercising the muscles needed to move lips independently of the jaw and tongue. That’s a crucial skill for when they start using a spoon and eating chunkier foods.

Have Fun with this Stage But what parents notice most about the raspberry stage is that it’s just plain fun. Babies laugh and giggle in response to their parents lip blowing and then they do it back. It’s the early foundation for the back and forth rhythm of a conversation. Frequently, this is when older siblings begin to realize that the baby is capable of interaction. “My then 3- and 5-year-olds had a great time at this stage. They would be smiling and laughing and just go back and forth. It was really a great bonding time,” says Carrie Raeside, a mother of five. Blowing raspberries is almost nature’s way of ensuring that you join in. “When babies produce ‘raspberries,’ it provides an opportunity for early language skill development of turn-taking, imitation, eye contact, and attending,” says Marsha Lee-White, a speech language pathologist with Cincinnati Children’s Hospital Medical Center. “When the children produce ‘raspberries’

and then the parent responds, the children begin to learn that when they make a sound, another person will respond and engage in interaction. The children also begin to learn that they are able to make differing sounds.”

New Sounds, New World Once they’ve mastered the raspberries, be ready for language to start developing. “Babies begin to learn that when they make a sound someone may respond,” says Lee-White. “The parent also usually responds by getting physically closer to the child and may smile at the child during this interaction. The parent may also laugh and be animated, which may encourage the child to produce more ‘raspberries.’ Eventually, the children may begin to produce different sounds, some of which may be early speech sounds.” Courtney Romano, mother of a 7-month-old, states, “I think blowing the bubbles has taught her how to use her mouth and enhance her language skills. Right after blowing bubbles she started using different sounds like her Gs and Bs in her baby talk.” Early speech usually entails repeated constants and vowels with no discernable meaning. Much to the delight of mom and dad, the “m,” “d,” and “a” sounds are frequently the earliest. Hence “mama” and “dada” are often two of the earliest words. That soon develops into long strings of words such as “bababababa” and then eventually combined consonants for nonsense words such as “takomamano.” They frequently combine gesture with the sound so you might get “ah” with the arms raised for “up.”

Reason for Concern According to Lee-White, babies may begin to make ‘razzies’ as early as 3 or 4 months, but more typically by 5 or 6 months. If your baby isn’t producing these kinds of sounds by 7 months of age, Lee-White suggests contacting your pediatrician. Some babies may skip the raspberry stage but they should make some type of sound that plays with their lips and their mouth. If not, it could be an indication of delayed speech development or a hearing issue. Otherwise, enjoy those raspberries and bubbles. Play other games of cause and effect with your baby and get them to communicate back to you. It’s an amazing and wonderful time of life, enjoy every sweet smile and laugh!  Laura Amann is a freelance writer.

c-sections explained “Women are the drivers for how their pregnancies will go.” — Michael Marcotte, M.D. with TriHealth

D

elivery by Cesarean Section (C-section) is far from unusual: 30 percent of all babies in the U.S. are delivered by C-section — the most ever, according to the National Center for Health Statistics. What’s propelling the trend? For one, fewer women who’ve had a C-section are having vaginal births. Technology is also a factor. With the advent of fetal heart rate monitoring during labor, obstetricians can see whether a baby is in distress. Doctors can also use ultrasound in the last weeks of pregnancy to predict whether a baby is too big to fit through Mom’s pelvis. And while vaginal births were once the norm for breech babies, most doctors now think they’re too risky. But when it comes to C-sections, it’s important to educate yourself so you can make the best decisions for you and your baby.

Reasons for C-Sections Michael Marcotte, M.D., medical director of quality for maternity services at TriHealth, says there are several reasons for an emergency C-section, including problems with delivery — such as the baby not fitting through the birth canal — or the baby’s elevated heart rate. Fever on your part, or if your labor fails to progress, may also result in a C-section. Your doctor may schedule you for surgery if you’re carrying multiples, or if your baby is breech, very large, or has been diagnosed in utero with a birth defect or placental abnormality. C-sections are also preferred if you’ve had pregnancy complications such as severe preeclampsia or if you’ve had a previous C-section.

Risk and Recovery While the good news is that most C-sections don’t encounter many complications, Marcotte recommends avoiding it whenever possible. “It’s really better not to have a C-section if you can help avoid it,” he says. Aside from the risks that go along with any surgery, such as infections and blood clots in the legs or lungs, Marcotte says additional risks include a slower recovery time, a longer hospital stay, and additional pain medications compared to women who have vaginal deliveries. Babies born by C-section have more fluid in their lungs than babies born vaginally, and are more susceptible to respiratory problems. Also, following a C-section, Mom and Baby may be separated for a longer period of time than those who deliver vaginally. This may cause concern for those who opt to breastfeed, and who hoped to nurse the baby right away, however, the La Leche League reports that moms who deliver via C-section are just as successful at breastfeeding as moms who deliver vaginally, if they remain highly committed. Risks rise in women who experience multiple C-sections, according to

16 April 2011

CF 14-17 (C-Sections).indd 16

Marcotte, with the possibility of experiencing placenta accreta (the placenta growing into an older C-section scar which may lead to hemorrhaging and even an emergency hysterectomy). Mothers often hope to deliver vaginally after having a C-section — this is known as VBAC — but there are risks associated with this as well. The biggest danger doctors worry about during a VBAC is uterine rupture — a potentially life-threatening tear in the uterus. And although Marcotte says the majority of women who attempt a VBAC are successful, he urges expecting moms to evaluate their risk factors with their doctors. Women who aren’t induced, have had only one C-section and are at least 18 months past their previous delivery have the lowest risk of rupture.

Patient Choice C-Sections Women may opt for a “patient choice C-section” for a number of reasons — fear of complications during labor, fear of labor itself, or timing Baby’s arrival so that family is able to help. But a patient choice C-section can cause problems if you haven’t properly calculated your baby’s due date. An incorrect due date might mean a premature delivery (before 39 weeks), which may lead to problems including underdeveloped brain and lungs; vision and hearing problems; and low birth weight. While the number of patients who choose to schedule a C-section isn’t large — less than five percent of C-section deliveries according to a 2005 Healthgrades study — it’s still greater than it should be, in Marcotte’s opinion. It also may not help Ohio’s preterm birth rate, most recently given a “D” grade from the March of Dimes (Kentucky got an “F”). Although the rate of premature births has dipped in recent years, recent prematurity rates are still higher than they were three decades ago. The local chapter of the March of Dimes is working with area hospitals on several initiatives to improve the “D” grade, including the Ohio Perinatal Quality Collaborative (OPQC), a statewide effort to reduce the number of scheduled births between 36 and 38 weeks that lack a medical reason. The effort has shown signs of success — from September 2008 through September 2010, about 12,000 babies who might have been delivered early were actually delivered at 39 weeks.

Know Your Pregnancy Whether you’re delivering vaginally or via C-section, the most important thing to know is that you are leading the way. Marcotte is an advocate of women educating themselves on all the options. “Women are the drivers for how their pregnancies will go,” he says. “We use their leads to make decisions, so it’s important for them to be informed of all concerns, and how to minimize any risks.” J Sandra Gordon is a writer and frequent contributor to our publications.
