There is no specialty in dentistry called family dentistry. Pediatric dentistry is one of eight recognized dental specialties. A pediatric dentist receives two additional years of specialty training beyond a four-year dental degree. This additional training focuses on growth and development, child management, child psychology, sedation, hospital dentistry, interceptive orthodontics, growth guidance, advanced restorative procedures, etc. etc. as it pertains to growing children. Pediatric dentists are trained to work closely with pediatricians and other medical specialists in a hospital environment where medically compromised and special needs children often have to be treated. The pediatric dental staff must be highly trained. And, the pediatric dental office must be designed to be a child-friendly environment. The goal of every pediatric dentist is to make dentistry as pleasant an experience as possible for not only the child, but also the parent.
Dr. Pete, my child just got his braces off and he has prominent white spots on his teeth. What are these and can bleaching get rid of them?
A:
These white spots are called decalcifications. These are caused by poor oral hygiene. When a child has braces and does not brush and floss properly plaque builds up around the braces. If poor oral hygiene persists this plaque build up around the appliances begins to damage teeth. When the braces are removed the damaged areas appear white. The undamaged areas, or the areas protected by the braces, appear healthy or tooth colored. These “white spots” can only be fixed by restoring these affected areas with tooth colored fillings. Bleaching will not cure the damage done by poor brushing and flossing habits. The good news is that these “white spots” are totally preventable. Proper monitoring of a child’s oral hygiene by the patient, the parent, and the orthodontist will prevent any permanent damage to the teeth.

O
ver the years I’ve experienced Christmas in different ways, and I always start out with high expectations because I love Christmas, everything about it. I loved it when I was a child — the giving and yes, the getting — and now I love sharing it with my family. But there was one Christmas in particular that I’ll never forget. Now that my firstborn’s a freshman in college — I can marvel at the memory. I had that college freshman years ago on December 17. It was a bitter cold Friday evening and there were lots of holiday parties going on — in fact, my obstetrician was hoping to get to one because at one point during the late afternoon he gave the order to “up” the pitocin drip in my IV which rapidly increased my contractions. At 5:30 p.m. he came in to deliver Alexandra wearing a full body shield ... later it dawned on me that he must have had party clothes on underneath ... but that’s just a snippet of the memory I recall. I was in and out of the hospital with my newborn in a speedy 24 hours after a lactation nurse pronounced us good to go — only we were not. At home alone with my infant, I didn’t realize she wasn’t latching on ... a groggy nurser, she’d eventually grow sleepy and I’d just pop a pacifier in her mouth. I held her in my Baby Bjorn front ways and vacuumed because my in-laws were arriving soon. I was crazy, sleep deprived ... and ignorant. How much I loved this little baby, but how much I also wanted everything to be just right and for my family to see me as a success. I was way off. Alexandra was NOT getting what she needed and the next day found us both choking with tears and frustration — just in time for my in-laws arrival. Here I was, trying to be Mrs. Perfect and I was an utter failure. I was determined to breastfeed, but now I couldn’t relax. Over the phone, the lactation consultant told me heating pads would help ... or ice. Huh? That confused me further. I had no Christmas shopping done, no cookies baked and everything was caving in around me — along with my expectations and my family’s confidence. That’s when I stopped all of my striving and took to my bed with the baby. I’d nurse until the cows came home. We would get this thing if it took forever. At last, in the middle of the night, both of us bleary-eyed and exhausted, she latched on — it was, for me, a miracle. Come Christmas Eve, my mom-in-law had delicious things simmering in the kitchen, piles of presents wrapped under the tree and I was beginning to get the knack of breastfeeding ... finally experiencing let down, but still a mess. An over-achiever from birth, I was nursing every hour and having a hard time getting used to not being able to do anything else. When my husband came home from his hard day at work, I coaxed him into a private meeting in our bedroom. “I have no presents!,” I whispered, whining. “We don’t have anything to give anyone!” My young, hard-working husband watched me fall apart. He’d hoped to just be at home with his wife, new baby and parents, but he took me in his arms and said kindly, “It’s OK ... I can still go to Wal Mart.” I don’t think I ever loved him so much as I did at that moment. Thinking back, I marvel at my selfishness. Out he went into the awfulness that is Wal-Mart on Christmas Eve, and he returned two hours later with the things I’d requested to give — he sat and wrapped everything, too. Late that night, as his parents slept, we curled together on the sofa and watched the fire die down. I nursed our precious baby girl. I marveled at a stream of milk gliding down her cheek — liquid gold. This was our first Christmas as a young family and we loved it. We would be OK. It was the BEST Christmas ever.

in response to “proper grammar” letter Dear Editor, I was disappointed by the way you responded to the reasoned critique offered by Terri Lynn Merritts regarding your pronoun choices in Nashville Parent. The fact of the matter is that Merritts is right — using language that chooses the masculine category as the “neutral” category sends a message to women and girls that they are feminine deviations from the masculine “norm.” I would not suggest, as she does, that we replace “he” with “she” as the neutral category, but rather alternate pronouns, use he/she or substitute the pronoun “they.” All three of these solutions to the problem of gendered pronouns are cited as acceptable in the most recent MLA style guide. Other professional organizations like the American Psychological Association, the American Philosophical Association, and the American Sociological Association specifically caution NOT to let “he” and “his” stand for all people. I am curious to know what style guide you have consulted that has told you that this is acceptable practice. I have not seen one published since the 1980s that actually suggests sticking to masculine pronouns. For what it’s worth, too, choice of pronouns is a style issue, not a grammar issue. Proper style for an editor responding to a reader’s criticism would dictate avoiding the use of text speak like “LOL,” too. Carly E. Schall, Ph.D.

by good grammar including the use of the word mankind. Really?! Is this how insecure we have become? Do we need magazines and books to validate us as women by using she instead of he or police officer instead of policeman? Our girls know they can be anything they want to be because we teach them so, not because of political correctness. Why does the use of mankind mean women are inferior? And all the talk about the majority of the population being female so we should use the feminine version is ridiculous. Should you only have pictures of white children on your cover because that is the majority of the population? I am a confident woman. My girls are confident in themselves. They know they can accomplish anything their brothers can. Grammar has absolutely nothing to do with that. By the way, should we stop using the word human (Gasp! I started with sharing with them. If I brought a new toy in I would play with it, then slowly share with them. But they would have to share something with me then with each other. We continued this and the kids are great with sharing now. Didnt start that way! GINA REEL

I set two boxes in the room and I tell my 6-year-old daughter that there are less fortunate children who would love to play with the toys she doesn’t play with anymore. I let her decide which ones to keep and which to give away. She loves doing this. Some time later, she remembers a toy and wants to play with it. She asks me about it, but then remembers she gave it away. She is sad for just a second and then says that the little girl who received that toy is probably very happy and having fun. Q
My new granddaughter’s gums look uneven. I’m concerned her teeth may grow in crooked, but my daughter says it’s nothing to worry about. Is she right? By the time babies are born, all of their tooth buds are developed. These tooth buds are located just beneath the gums and are what give the bumpy and, sometimes, uneven appearance of a newborn’s gums. It is from these tooth buds the primary teeth (baby teeth) will emerge beginning around 6 - 12 months of age. The appearance of the gums and tooth buds prior to the eruption of baby teeth does not necessarily predict whether the teeth will come in straight, crowded or crooked. Although less common, there are some teeth and/ or gum conditions during the newborn period that may cause the gums to appear abnormal or uneven. These include natal teeth (temporary teeth present at birth), benign gingival (gum) cysts — including Bohn nodules or mucoceles — and Epstein pearls (keratinized-filled bumps). Your child’s doctor will easily recognize these benign conditions and provide reassurance. A pediatric dentist is also an excellent resource for evaluations and those with concerns regarding gum and teeth conditions of infants and children.

My 6-year-old son is continuously constipated. What could be causing this and what do you suggest to remedy the situation?
Constipation in children is a common problem characterized by infrequent bowel movements or hard, dry stools. It can develop for a number of reasons and does not normally indicate anything is physically wrong with your child. The most common reason for constipation in this age group is a diet consisting of too much dairy, too little fiber, and too few vegetables and fruits. Children who don’t drink adequate amounts of water are also prone to suffer from constipation. Encouraging your child to make simple dietary changes — such as eating more fiber-rich fruits and vegetables and drinking more fluids — can go a long way toward alleviating constipation. If your child is a picky eater and does not care for fiber-rich fruits and vegetables, try sneaking these foods into a fun smoothie drink. Just toss some fruits such as pears, oranges and grapes along with a handful of spinach into a blender, then mix with ice and 100 percent juice. Your child will love it! Remember: the goal for children suffering from constipation is for them to have regular, soft and easy-to-pass bowel movements. Children with constipation are more likely to avoid the bathroom if they have a history of painful/difficult poops — a behavior that leads to even bigger and harder stools making the next attempt to poop more painful, and the vicious cycle continues. To break this cycle, especially if dietary changes are difficult and/or not working, your child’s doctor may recommend a stool softener. A common stool softener used in children is polyethyline glycol 3350. This stool softener is a tasteless, odorless powder which can be mixed with water and is well tolerated by most children. It is always important to discuss your child’s constipation with his doctor as occasionally constipation may be a symptom of an underlying medical condition requiring further evaluation.

t’s that time of year, where you may find yourself prowling the pharmacy aisle in search of an over-the-counter (OTC) medication, for your child ... but is it OK to give OTC meds along with a doctor’s prescription?
ACETAMINOPHEN ADVICE In summer 2011, some makers of liquid acetaminophen revised their infant and children products to contain a standard amount of medicine. Prior to that, infants’ medicine was three times as strong as the children’s version. Why the change? “Overdoses were happening,” says Rosy Thind, M.D. She says parents were giving toddlers and kids the same medicine they were giving to their infants, and not realizing that the infant drops were more concentrated so that infants didn’t have to consume as much liquid. The concentration of the infant drops is now diluted to match the children’s concentration (160mg/5mL), so be sure to check the dosing amounts before administering any medication. Thind advises parents to steer clear of ibuprofen for babies younger than 6 months, as there haven’t been any real studies to test the effects. She also says parents should not give acetaminophen to infants younger than 2 months of age. “If they display symptoms like a fever or are irritable, then they need to be seen, as they could have a serious infection.” Parents should always call their pediatrician if their infant younger than 2 months of age has a fever of 100.4 degrees or higher, and call if

your child’s fever reaches 103 degrees. Thind also advises parents to take into consideration how their child appears. “Do they perk up after some medication? What other symptoms are present?” she says.
HELP AT HOME Thind says it is OK to give an OTC medication like ibuprofen or acetaminophen along with a doctor’s prescription – just make sure your pediatrician knows about it. “It’s OK with antibiotics, there’s no drug interaction,” she says, adding that antibiotics won’t be prescribed for a cold caused by a virus. Which unfortunately means that your child will have to suffer through it. But there are some home remedies that might help ease his symptoms, along with acetaminophen or ibuprofen for fevers or pain. Thind says a teaspoon of honey for kids older than 1 year is good for congestion and coughs, and tea or warm soup is a good way to thin mucous. “Parents can also sit in a steamy bathroom with their child and read a book.” You can also break out the vaporizer or humidifier. “Either one is helpful for colds caused by a virus,” says Thind, “as they both put moisture in the air.” Vaporizers typically put out warm steam, while humidifiers provide a cool mist. Make sure you keep them clean, though! “Mold can build up if they’re not cleaned and drained,” says Thind, adding that parents should always read the manufacturer’s instructions for proper cleaning guidelines. — sherry hang

was fortunate to be raised in a wooded area where outside my backdoor was 3,600 acres of a natural playground,” says Peter Gros, co-host of Mutual of Omaha’s Wild Kingdom, which your kids can watch weekly on Animal Planet. Gros brings his lifelong love of animals and nature to TPAC on Friday, Dec. 7. Gros intends his show to introduce children to the good things in the natural world and the necessity to protect it. “It’s an exciting mix of beautiful footage from the wild interspersed with live animals,” Gros says. W
hat’s a new mom to do when all she wants for Christmas is a nap? Plan ahead. Simplify. Reprioritize. And get help. Let’s take it one step at a time, but let’s also remember what matters most: Children are tiny newborns once in a lifetime, but Christmas comes around every year. Get your priorities straight so you can focus on your baby and allow some of the holiday hoopla to fall to the wayside. For instance, don’t waste countless hours writing, stuffing, stamping and mailing holiday cards. Instead, combine your Christmas
cards and birth announcements into a single photo card with a pre-print-
ed message like, “Look what Santa brought us!” on the front. Everyone who receives one, even if it doesn’t arrive until Valentine’s Day, will be delighted. Online photo companies like Shutterfly and Ofoto offer the option of creating a custom picture card, uploading your address list and mailing all of them out for you! Although its tempting to run out and get the standard “Baby’s First Christmas” ornaments, a monogrammed stocking and a pile of over-hyped toys that will “stimulate your newborn’s brain and enhance his reflexes,” realize that your baby has no clue what the holiday means and is just happy to snuggle quietly with you, so don’t overdo it for his sake. Your relatives will inundate you with baby ornaments and there is nothing your parents or in-laws would love more than to have a stocking made for their new grandbaby, so let them. (If you don’t like it, you can have a new one made next year.) While you’re in the simplifying mood, gracefully bow out of the neighborhood
Christmas lights competition this year. Just hang a wreath or put a bow on the
mailbox and consider yourself jolly. If putting up a tree seems like an overwhelming task, don’t do it. Most folks have a friend or neighbor who goes crazy for holiday decorating and already put the finishing touches on her house the day after Thanksgiving, so ask your local domestic diva devotee to add her creative flair to your home so you can bathe the baby or go for a walk.
Jeannie Casey, certified postpartum doula (http://nashvillechildbirth.com), offers this sage advice: “Stop worrying about coffee
stains on the counter or whether you put a wreath on your door.” When you’re in your final years, she says, you won’t remember the wreath, the gifts you sent or whether you had turkey or ham that year. You’ll remember falling asleep with your baby on your chest or staring at tiny toes in awe. So do the important things and let the everything else go.
Put You and the Baby First Reprioritizing means putting the needs of yourself and your new baby first. Obstetrician Susan E. Mackey of Women’s Medical Associates of Nashville recommends that new mothers strive to get eight hours of sleep a day. “If you don’t get it, your energy level is low, your stamina is lower, you’ll gain weight more easily and your decision making will be negatively affected,” she warns.” Sleeplessness also adds to the feeling of being overwhelmed. Casey agrees, “You have to re-establish
your priorities when you have a new baby. Life is hard going from store to store
with a newborn who needs to eat, take naps and have frequent diaper changes. And Mom needs food, drinks and naps, too. If a mother doesn’t take care of herself, she can get stressed out, irritable and depressed — all of which, if you’re breastfeeding, can affect her milk production.” Plenty of rest, food, liquids and quiet time is the recipe for a healthy mother, but infants have immature immune systems and need extra protection around the holidays. Pediatrician Kris Rehm, M.D., of Old Harding Pediatric Group, says, “Having a newborn over the holidays is such a joy, but it may mean that a family needs to stay home to prevent exposing the newborn to viral illnesses that are common at this time of year.” That means forgoing holiday
travel until the baby is at least 2 months old and letting someone else brave
the stores. As tempting as it may be to get a picture with Santa, it’s probably not a good idea. Santa has had dozens of children on his lap, one of which may have had a cold, flu or stomach bug. Germ protection extends to friends and family, too. Rehm advises, “It’s really important to make
sure that anyone who holds the newborn is healthy, because a little cold in an adult can be a serious illness to a newborn. That means the parents need to be vigilant about making sure that all family members with a runny nose or cough steer clear.” Don’t feel guilty asking someone to admire from afar; your primary responsibility is to keep your baby healthy. Strict rules about
hand washing are key!
Say Yes to Help Caring for a baby is a full-time job, and getting through the holidays can be another job in itself, so consider adding an extra pair of hands to your household to make things run more smoothly. You can seek out a professional doula, or, if that’s out of your price range, Casey suggests moms “get to know the teenage girls in your neighborhood and to have one come over after school each day. You can hold the baby on the couch and direct her in putting up decorations or doing laundry. When you feel comfortable with her, she can hold the baby while you nap. The $8 an hour is a great deal compared to professional doula prices. Check around with local church youth groups, too. Some of them perform community service by cleaning homes or doing light yard work for families with new babies Mackey counsels mothers to, “‘Let others
take care of you and let go this year. Ask for help from friends, family and your husband.” Request that visitors
bring frozen lasagna or a package of diapers when they come to see the new arrival. Whenever anyone says, “What can I do to help?” ask them to pick up a gallon of milk for you or grab a last minute Christmas gift while they’re at the mall. Most people love to be useful and will gladly help in any way they can ... but you have to ask!
The Best Gift’s Already Yours If you miss out on the caroling, church service or a family feast this year, remember that the holiday celebrates the birth of a baby to poor parents in a stable without an ounce of fanfare, so take heart that your holiday has something in common with the very first Christmas ... besides, you’re holding the greatest gift of all. RUFFLE UP A CHRISTMAS TREE WHAT YOU’LL NEED Ribbon (one with wire works best) Cone (made from a piece of cardstock or paper plate) Tape Hot glue
WHAT YOU’LL DO 1. Take a piece of cardstock and a plate and trace a circle onto it. 2. Cut that circle out and cut it in half. If you want to use a paper plate, just cut that plate in half. 3. Fold the half circle until the flat edges meet and then tape those sides together. 4. Take a really long piece of pretty ribbon (one with wire in it or a wide ribbon) and tape one end of the ribbon to the bottom of your cone with the ribbon going slightly over the edge of the cone. 5. Continue taping the ribbon around the cone and pleating it every so often. Be sure to put a new piece of tape right where you pleat the ribbon to hold it in place. 6. As you go around the cone, be sure to work your way upward so that the ribbon just slightly overlaps the layer below. 7. Once you reach the top, cut the ribbon. 8. Fold the end and then use hot glue to glue it in place. When you are all done, you will have a frilly, pretty Christmas tree! Be creative and have fun with different kinds of ribbons. This is an easy craft that kids can do, but a parent’s help is needed when it comes time for hot glue.

Tennessee State Museum: Continuing through Tuesday, Jan. 8, 2013 is a wonderful new exhibit, Elvis at 21. The exhibit features 56 poster-size photographs of Presley taken in 1956 by Alfred Wertheimer. Presley’s rise to stardom began in 1956 at the age of 21, and this amazing collection of pictures documents both on- and off-stage moments in great detail. Parents and kids alike are sure to enjoy this excellent documentation of the King of Rock’s breakthrough year. The museum is open Tue - Sat 10 a.m. - 5 p.m., Sun 1 - 5 p.m.; admission is free. Presley recorded more than 200 songs there, and guests get the inside scoop on lots of Presley history, including the Christmas connection to the studio’s colored lights. This year, the theater company is gives the production something new on the Franklin Theatre stage, Dec. 6 - 16. Managing Director Jake Speck promises a “fresh interpretation” this season while Artistic Director Matt Logan says, “We hope to ignite the imagination by telling this beloved story with bits and pieces from our existing lives.” Interesting ... Williamson County Parks and Recreation has a new offering for kids ages 3 - 17 interested in recitalbased dance. The Rhythm & Spirit Dance Program has classes in ballet, tap, jazz and hip hop with options in individual or small group instruction. Spring session runs Jan. 7 - May 11, 2013, and registration takes place this month on Saturday, Dec. 1 (8 a.m. - 1 p.m.) and Saturday, Dec. 8 (1 - 6 p.m.) at the Franklin Recreation Complex, 1120 Hillsboro Road. Children may also sign up during Rhythm & Spirit’s premiere “Winter Wonderland” performance on Saturday Dec. 15 at 6 p.m. at Rolling Hills Community Church in Franklin.
