othing can ruin a day off or a weekend more than that dreaded phone call from a patient who reports that their temporary crown came off. Sometimes that is actually the good news. The bad news is that they swallowed it. Not only will you have to re-cement it, you will have to remake it. Even if this occurs during your office hours, it can disrupt your schedule and is a nagging, unproductive use of chair-time.

I often will remove a crown and reassemble it with resin so that I can use it for a provisional restoration. Any time that you are “retrofitting” a restoration, a long-term temporary cement is preferable because the crown now fits like a boot on a rooster, and you need something strong to take up the empty space. Nothing is more destructive to the dentition than parafunction. We know what clenching, grinding, chewing finger nails, biting pencils, and the like does to tooth structure. These unnatural forces are bound to wreak havoc on a plastic provisional. All the more reason to use a more retentive, longer lasting temporary cement.
Cements for provisional restorations come in many formulations and their indication for use varies widely. I am not a big fan of short-term eugenol based cements for several reasons. It is widely known that eugenol interferes with bond strength because it penetrates the dentinal tubules and doesn’t allow the adhesive to cure properly. In addition, it makes the temporary mushy and if it leaks, it tastes and smells like dead fish. I know that the sedative effect it has on the dentin can be a plus, even a necessity, but in general the cons outweigh the pros on my list of priorities.

I prefer to use a resin based long-term temporary formulation when I cement a provisional restoration. Perhaps the biggest reason that I use a stronger, longer lasting product is, of course, its superior retention. However, it is particularly efficacious when the walls of the preparation are short or convergent. patient is going on vacation or they will be in provisionals for perio/pros purposes or other treatment plan delays, maintaining marginal integrity is a must.
We all have our preferences when it comes to products. My long-term temporary cement of choice is, in fact, DentoTemp. I happen to really like the ItenaTM line of products, and this particular long-term cement is a rock star. It is highly retentive due to high bond strength, and yet is easily retrieved. The working time is just right and it is easy to clean. When removed, the cement stays in the temporary, and leaves the tooth clean and free of debris. It is resistant to dissolution in oral fluids, radiopaque, and it actually dries with a pinkish tint so the color blends into the gingiva giving it a nice esthetic effect. Oh, and as an added bonus it is a superb product for permanently cementing implant restorations. Furthermore, it passes the taste and sniff tests… which means no more dead fish. Sizing Ads must be built to the correct size specifications. We cannot re-purpose ads designed for other media. Oversized or undersized ads will be adjusted to fit the space purchased.
