Achieving the

“Pot of Gold” with High Fear Patients

In the best-selling, classic book, entitled Three Feet from Gold, by Napoleon Hill, a young man gives up on his dreams of becoming rich, while prospecting for gold, when he is a mere 3 feet from hitting a major gold vein. Caring for the fearful dental patient can be a circumstance in which dentists often give up before digging deeper to achieve success. Anxiety and dental fear can literally paralyze patients and keep them from seeking dental care. As dental care providers, we need to be able to uncover the cause of their distress, while building trust, which is truly achieving the “pot of gold.” Each part of the appointment is an opportunity to build the relationship and add to the level of trust and comfort the patient experiences while in our care.

Roxanne is a patient much like ones that you might see every day in your own practice--one that expects high quality dentistry, while hoping that the appointment proceeds in an efficient and comfortable manner.

She is a 48 year old woman with many posterior crowns, due to breakage, bruxism, and years of dental neglect. To complicate matters, Roxanne has high anxiety. However, she has been steadfastly working to improve her mouth, since she became a patient in my practice 6 years ago. She has completed the work in three quadrants so far, and today she presents to finish her dental restorative treatment in the lower left quadrant.

Several different products and adjunctive treatment modalities have been uniquely beneficial for me as I work on anxious patients (and non-fearful patients alike). Each of these aid my team and myself in providing them the comfort and efficiency they deserve during their dental appointments.

1. Nitrous oxide and behavorial modifications 2. EMLA (Eutectic Mixture of Local Anesthetic) topical cream—for supreme soft tissue comfort prior to anesthetic placement 3. Isolite—quick and easy isolation of the area 4. Seek and Sable (Ultradent) caries indicator—to ensure complete caries removal 5. Reflectys (Itena) hybrid composite—butter-like consistency, esthetic and strong composite for buildups and anterior/posterior composite fillings.

6. Dentotemp temporary cement (Itena)—robust temporary cement for short preps or bruxers

Roxanne and I spent time talking at length before the appointment about her fears to understand how I could alleviate her anxiety before the procedure. She admitted that her biggest fear was the anesthesia and that she had been hurt before by her childhood dentist during administration and had never forgotten the experience. At this point, I applied the topical cream and allowed it to sit for several minutes to thoroughly numb the soft tissue. Then I “kicked it up a notch” with the N2O before I anesthetized both the inferior alveolar nerve with lidocaine, and infiltrated around the lower left quadrant with septocaine to insure comfort throughout the appointment. Taking time to go slow, and gently shaking the cheek while administering the first drops of anesthetic, does wonders to distract the patient and decrease most, if not all, sensation at the site of administration. The nitrous oxide not only allays mild to moderate dental anxiety, but it also provides a bit of analgesia, so that the injection is felt even less.

I also use EMLA (Eutectic Mixture of Local Anesthetic), a prilocaine/ lidocaine blended cream that provides incredible soft tissue anesthesia.

My local pharmacy compounds this for me, but it is available through other channels. This is the same topical cream that pediatricians use before stitching up kids with minor dermal lacerations. The beauty of EMLA is that it penetrates the dermis and (even more so) the mucosa, so well that it makes the injections almost entirely pain-free (although I like to think that my amazing skills have something to do with it!) With the beneficial combination of the N2O and the EMLA, Roxanne did very well with the anesthesia. She stated that she felt more at ease than she ever had before, and was pleased that she had not felt the injection.

Roxanne has deep caries, fracturing, breakage and large existing amalgams in teeth #18 and #19, and caries in the disto-occlusal aspect of #20 (Figures 1-3). I have planned to do full coverage porcelain fused to metal crowns on #18 and #19, and a DO composite filling on #20. The caries are very deep on the disto-facial aspect of #18, so I decided to proceed with porcelain fused to metal crowns for optimal strength.

Once I ensured that Roxanne was comfortably numb, I placed the Isolite (Fig. 4). The Isolite has reduced the time I spend on each restorative appointment stopping and going, because it suctions continuously, retracts in all the right spots, illuminates the field, and provides my assistant and I Figure 4: Isolite in place- Notice how it holds the soft tissue and tongue away with greater visibility in the area in which from tooth.

I am working. Patients initially look at it a bit funny, surely wondering how that contraption is going to fit in their mouth. Once I explain all that it does and that the appointment will go FASTER for them, however, they are converts.

First, I reduced #18 and 19 occlusally, to allow for enough space for the new crowns to fit into her bite. Then, I prepared the premolar (#20) for the DO composite. Once I removed the occlusal aspect of #18 and #19, I could see the fractures and the “gaps” around the existing amalgams much better (Fig. 5). I took care to remove the decay completely, and then I dried well, and applied a small amount of Seek and Sable caries indicator (Ultradent) to the entire prepared area (Fig. 6 & 7). I like the certainty it gives me when I’m removing affected and infected dentin, especially when there is compromises from amalgam tattooing and fracturing.

Now that the foundation is free of any contaminants, I can work to put Roxanne’s teeth back together again. After caries removal, I etched, bonded and filled #20 with my go-to composite, Reflectys (Itena).

Reflectys is an exceptionally esthetic nanohybrid composite, which I use regularly in both the anterior for its esthetic capabilities, and in the posterior for its durability. The easy handling of the material makes me think of butter, in that it “melts” right into the preparation without sticking to my instruments or gloves; it molds effortlessly, and is quick to polish. I completed the buildups on #18 and 19 with Reflectys, and since it is so easy to manipulate the composite, I freehand adapted the margins in no time. I like to place #00 retraction cord around my crown preparations to more clearly delineate the margins and to see if there are any rough spots.

After the preparations are checked for occlusal height, (Fig. 8) I removed the Isolite and took a two step impression with polyvinyl siloxane impression material (heavy and light body). Between the two steps the retraction cord was removed. My assistant then began the fabrication of the temporary crowns for #18 and 19.

I use Luxatemp (DMG America) to make high compressive and flexural strength temporaries that endure throughout the temporization process.

Using the clear, full contour, pre-operative impression of the teeth as a matrix from which to craft the temporary crowns, Luxatemp is injected into the matrix and seated over the preparations. It sits in the mouth for 1.5 minutes, after removal from the mouth, it sets up in the impression for 2-3 minutes. Only 4 minutes (from initial mixing), we begin to trim, polish, and finish the temporary crowns (Fig. 9).

Dentotemp (Itena) is a long-term temporary cement, with lots of brawn. The way I use it most often is for patients with short clinical crowns and with bruxing issues. These patients typically need nothing short of a prayer and a very fast lab to fabricate the final crown! Since using Dentotemp, I have found the “repeat offender” appointments, in which patients continually knock off their Figure 8: Final preparations temporary crowns, to be markedly decreased.

Originally developed for use in Europe, where patients oftentimes will be gone on “holidays” for months at a time, Dentotemp has been utilized

by European dentists for years. It also functions as an ideal implant cement as it allows for optimal retrievability of the implant.

Roxanne presented for the cementation of #18 and 19 three weeks after the preparation.

Her temporaries were still in place (Fig. 10) and functioning well, and she had experienced no problems in the lower left quadrant.

She said that she felt so comfortable at her last appointment that she did not feel the need to use N2O at her crown cement appointment. My assistant removed the temporaries and completed the preliminary steps for crown cementation. Once the crowns were ready, I did the final verification steps. I made sure that margins were seamless, the interproximal contacts were appropriate, and that her bite was comfortable and balanced. I used Relyx (3M) resin-modified glass ionomer cement to cement the porcelain fused to metal crowns. Roxanne was thrilled to finish her final quadrant of dental work. (Fig. 11 & 12) Using the right products, at the right time, in the right way leads to a systematic approach that allows you to treat both the predictable and the complex cases in an efficient manner. Anxiety and fear can generate lots of stress during an appointment, so it is always beneficial to have tried and true methods and materials to rely on to hasten the appointment along, while ensuring quality results. With these tools in hand, may each appointment flow by with ease, and enable you to reach out and grab the elusive “pot of gold” results you have been looking for with high fear patients.

DR. GINA PHAM

Dr. Gina Pham grew up in Denver, Colorado. She received a Bachelor of Science degree in Electrical Engineering from University of Colorado in Denver. Following a career change, Dr. Pham attended Howard University, College of Dentistry in Washington DC. After receiving her Doctor of Dental Surgery (DDS) degree from Howard University, she completed one of the best pediatric dentistry residency programs in the country at the University of Pittsburgh, School of Dental Medicine. Following several years in private practice on the East Coast, Dr. Pham and her husband decided to settle down in Northern Virginia. In 2009, Dr. Pham started her own dental practice, BriteStars Pediatric Dentistry located in Woodbridge, Virginia where she continues to provide dental care for infants, toddlers, kids, adolescents, and special-needs children.

DC Dental: What or who influenced you to go into dentistry?

DC Dental: How does your office differentiate itself from competition?

Dr. Pham: I went into engineering first and I liked the technical field, but then I had a friend that went into medical school and she had a friend that went into dentistry. I asked her about it a little bit and I thought it would be something that I would like. After I finished with engineering degree, I decided that I wanted to switch.

Dr. Pham: The most important thing is that we have a selected team of girls that loves kids. We didn’t have to think about competition because our patients love us and we keep them for a long time. All of the girls in our office adore kids and they play with the kids so it all comes natural to them.

DC Dental: What made you go into pediatric dentistry?

Dr. Pham: I love doing restorative and if necessary, sedation. I know sedation is something that not a lot of doctors on the east coast feel comfortable doing but sometimes that’s the only way to complete the procedures on children who are fearful of pain. I have gained so much experience in restorative and sedation, so I am good at it and I enjoy it.

It is very rewarding to see kids that behave badly at first but turn out to love us.

Dr. Pham: I love kids. I can’t describe it, but I’m at ease with them. I’m goofy with them and I can do anything and they will probably laugh at me. I have the personality that works well with kids. Kids make me very calm.

DC Dental: What is your favorite dental story?

Dr. Pham: We had a snow day recently and we tried to notify parents that we were closed. One of the mothers did not get the message so she drove her kid all the way here and it made me very sad. The child kept saying to her mother “I have to see the tooth fairy!” She was crying because she didn’t want to go home! She told her mom to go downstairs and knock on the door because maybe the tooth fairy is downstairs. Another favorite story of mine is that a child wanted to be a pediatric dentist when she was only 3 years old. She is now eight and still comes to see me and still wants to be a pediatric dentist. We must influence them in a positive way sometimes.

DC Dental: What has been your proudest moment to date in your dental career?

Dr. Pham: I have proud moments all of the time. Every time a child is in pain and their whole mouth has cavities with nursing bottle caries and at the end of their dental treatment plan they are happier and healthier.

Before the treatment they were really thin, but after their teeth become healthier their whole body grows stronger and healthier. I just think to myself “Wow, I made a difference!” I’m proud of myself and that is just one moment that has consistently driven me throughout my career.

DC Dental: What is your favorite procedure?

DC Dental: What is your favorite dental supply item?

Dr. Pham: I love composite and really any 3M product. We love it when there is a sale on 3M crowns! We use a lot of 3M from the varnish to Clinpro 5000 and everything else!

DC Dental: How did you hear about DC Dental?

Dr. Pham: I have a friend from Alexandria, Virginia and I asked her when I first opened my practice which supplier is the best, helps you a lot, and has a reasonable price. She said that she only orders from DC Dental.
