Last night my friend and endodontic colleague, Dr. William Nudera, gave his third webinar in a series of four on endodontic protocols. I was really looking forward to another one of Bill’s impeccable presentations. However, since I have practiced endodontics myself for over thirty years, and at the risk of sounding arrogant, this old dog was quite doubtful that she was going to learn any new tricks. But, hear me out. Don’t judge. Read on.

Bill and I do not use the same instrument systems, materials or techniques. We have had many discussions about the latest rotary instruments. We have agreed to disagree about how to fill the root canal system. Yet, we both get predictable results. The truth is that there really are no new tricks. Yes, there are plenty of new gadgets. If you doubt this, I can show you a closet in my office that I fondly call the “endo garage.” It is where old first generation apex locaters and the like are put out to pasture. What Bill and I agree on is far more important than NiTi and gutta percha.

Here are 7 habits that we both agree are the key to endodontic excellence.

1. Begin with the end in mind

This is one of the basic principles touted by Dr. Stephen Covey in his book the Seven Habits Of Highly Effective People and the namesake of this article. Translated into endospeak, that means study the tooth in question before ever picking up a handpiece both radiographically and clinically. For example, look at the complexity of the case, recall the variables in canal anatomy for the tooth in question and picture what the root canal will look like upon completion. Ask yourself if the tooth is restorable and whether it has adequate periodontal health to support a long-term result. It is not as important to question whether a tooth could have a root canal, as it is to question whether a tooth should have a root canal.

2. Administer profound anesthesia

Despite our quite slanderous reputation, we really want our patients to have a pain free procedure. While the scope of this article does not permit me to go into great detail, I will give you a few pearls in this department.

- On lower teeth, sometimes a numb lip is not a sign that a patient is numb enough for root canal therapy. It is only a sign that the lip is numb. Test the tooth with ice after giving a mandibular block. If they still feel the cold, they will need supplemental anesthesia.

- In addition to buccal infiltration, upper teeth usually need palatal anesthesia.

Our topical only works on mucosa. Press your index finger or mirror handle firmly in the targeted area on the palate for a few seconds until it blanches slightly.

This pressure makes the injection much more tolerable (or should I say palatable).

- Use a new needle for each injection.

- Learn how to give intra-ligament injections and intra-osseous injections.

These work like a charm on “hot” teeth.

3. Always use a rubber dam

Okay. I have to ask. What is the big deal here? Surveys have revealed that most dentists do root canal therapy without a rubber dam. I will make this easy for you. You need the following clamps: a #56 for molars or a 14AT, a # 1AT for premolars and fat canines, and a #9AT for anterior teeth and teeth with crown preps. You punch one hole, and voila.

In addition, you render your patient almost speechless (always a good thing), you don’t have them following your handpiece with their tongue, you have better visibility, and your gloved fingers don’t get wet and slippery. Besides, would you consider yourself clean if you took a bath in dirty water? You cannot consider a tooth clean if it is contaminated with saliva.

Most importantly, it is the legal and ethical standard of care. Nothing will ruin your day more than if a patient swallows (and/or aspirates) a file.

4. Shape the canal to facilitate cleaning

It is a fallacy that instruments clean canals. Instruments are used to selectively remove dentin to provide a space for optimum irrigation.

A continuous minimally tapered funnel-form preparation should be prepared where each cross- sectional diameter narrows as you approach the canal terminus.

Care must be taken to leave the apical constriction in its original position. This preparation should be as conservative as possible so that the tooth remains structurally sound. Once the canal is negotiated to the terminus with a small, pre-curved hand file, a glide path is created. This allows each subsequent instrument to move smoothly from the coronal orifice to the apical constriction unimpeded. A glide path allows us to place all subsequent instruments and move them smoothly from the coronal orifice of the canal in an unimpeded progression to the apical constriction.

To maintain a glide path and insure that you will not block or transport the apical foramen, patency needs to be established after each rotary file. Patency is defined in the American Association of Endodontics’ glossary of terms as ‘a canal preparation technique where the apical portion of the canal is maintained free of debris by recapitulation with a small file through the apical foramen’. To achieve apical patency, a pre-curved, small No. 6-10 K-file is passed through the apical foramen following irrigation.

5. Clean the canal with copious irrigation

Last night Bill stressed that depth of penetration of your syringe tip is the key to canal cleaning. The bleach cannot disinfect areas it cannot reach. Bleach (sodium hypochlorite solution) is the irrigant of choice and is often used in conjunction with 17% EDTA to remove the smear layer. There are other irrigants and lubricants available. Each clinician has their favorite regime. But I think we can say that if you do not use a lot of bleach, and get it to its target area, your canal will not be cleaned sufficiently. Irrigation is best done using a safe-ended side vented syringe tip with care not to engage the walls or lock it into the apical constriction. Irrigation of the canal system after each instrument is recommended, followed by re-establishing patency as described above.

6. Obturate in 3 dimensions

The ultimate goal of canal obturation is to fill the root canal system in all dimensions to create a fluid tight seal, thus preventing bacterial ingress and their toxic byproducts. While gutta percha is not a perfect material, it, along with sealer is widely used to accomplish this task.

I am often asked whether it is better to overfill a canal or under-fill a canal. It is never good to underfill a canal, however there are several types of underfilling.

The first is a filling that is too short. The second is a filling that is overextended past the end of the root, but underfilled because it does not seal it. Since a radiograph is a 2 dimensional picture of a 3 dimensional object, the filling could even look like it is right at the end of the root, but still be underfilled because it does not obturate the space. As far as overfills are concerned, there is a big difference between extruding some material past the end of the root during condensation of the gutta percha vs. pushing it out the end because your master cone does not fit properly. A bit of sealer or gutta percha that has extruded during condensation is fine as long as the terminus is sealed. If the gutta percha is pushed beyond the root apex and it does not seal the terminus, it will result in an underfilled canal with an overextension of material.

7. Never force an instrument

Have you ever noticed that more endodontic instruments separate at the end of the day, than at the beginning? That is because of fatigue, rushing and lack of attention. It is when you might inadvertently push too hard because you have blocked your glide path by either not irrigating after each file, or by not re-establishing patency. If the instrument does not glide naturally, remove it, irrigate, establish patency and try again. Develop a light touch. Remember, slow and steady wins the race.

Excellence in dentistry takes discipline, hard work, and lots of practice. Highly successful clinicians strive to improve their habits every day. They are never satisfied with the status quo and always see room for improvement. Even after 30+ years of practicing endodontics, I still find root canal therapy challenging and tedious. So when I look at a radiograph of what appears to be a completely calcified upper second molar on a patient with limited opening, I take a deep breath. Then I begin, with the end in mind.

We sat down and had a chat with Dr. Crissy Markova in her outstanding office with equipment help from DC Dental. The following is the conversation with Dr. Markova: DC Dental: What motivated you to go into orthodontics?

DC Dental: What do you find is a misperception about orthodontics?

Dr. Markova: I shadowed with an orthodontist right before I went off to undergrad and everyone just seemed so happy in the office. They loved what they were doing, it was very busy, and there was a lot going on. I just loved it and I didn’t change my mind when I went to dental school, so it was great!

Dr. Markova: The biggest misperception is that we are just cosmetic and we just straighten teeth, but there is so much more to it than that, especially when we are talking about kids and child growth and monitoring development.

It’s not just about a tooth that is crooked and we want to straighten it, there’s quite a bit that goes into it.

DC Dental: How do you make your patients feel more comfortable and less anxious during their visit?

Dr. Markova: I spend a little more time with them and answer all of their questions. I think a lot of times, especially with orthodontics, it is kind of more rushed because there are more people, so I will take the time to talk to them and answer their questions and explain everything that I am about to do. For children, we bring out the fun tricks, the rainbows and the puppies!

DC Dental: Where do you see orthodontics in 5 years with trends and technology?

Dr. Markova: I think we are already getting there to some extent. I see it going towards more and more customized appliances such as customized bracket options. There will be a lot more technology in terms of 3D printing and scanning which is already happening a lot more.

DC Dental: To date what has been your proudest moment in your dental career?

DC Dental: If you could, what advice would you give to yourself upon graduating?

Dr. Markova: I think it was when I have caught a couple of significant things that had nothing to do with orthodontics or dentistry. There was a tumor growing in one person or a lesion that might have gotten missed along the way. So that to me was a pretty defining moment.

Dr. Markova: To be patient. Opening up an office and everything else in life requires a little bit more patience. I always want to get stuff done right away and sometimes you just need to take a second and breathe and it will happen.

DC Dental: How do you use your skills and talents to give back?

Dr. Markova: We have partnered with quite a few of the local schools in the area and support their sports teams. We also do the Juvenile Research Diabetes Walk with our team and make contributions as well as get our patients involved.

DC Dental: What might someone be surprised to know about you?

Dr. Markova: I played Powder Puff Football in high school. It is a little unexpected because I’m not a football kind of chick, but that’s pretty unique!

DC Dental: How did you hear about DC Dental?

Dr. Markova: It was completely random actually. I was on a forum looking at an X-ray machine and DC Dental was a distributor for that machine, so I reached out and actually stopped in within the day and I talked to Eddie who was amazing. Him and I just think about things the same way and we just clicked right away. He was not trying to oversell me on products, so when I had that first initial meeting with him I knew I wanted to work with DC Dental going forward.

DC Dental: Would you recommend DC dental to your colleagues?

Dr. Markova: Eddie is very knowledgeable and exactly in tune with what I needed for a new start-up office. He was able to help me navigate pros and cons of different equipment and different companies. DC Dental is also value focused. As a start-up, I don’t have a lot to be investing in equipment or supplies that I don’t need at the moment. I think Eddie was very customer oriented in terms of working with me for exactly what I needed and giving me products that ended up saving me thousands of dollars. It was a huge savings for very high quality equipment! Even in terms of recommendations for something like chairs, Eddie was so open and would let me know about his experiences and what other doctors use. I was looking at chairs by myself and there are 20 different companies and I wouldn’t know which one is better since I haven’t tried all of them. Eddie was patient with me when I was making selections.

Steve, my technician, has been wonderful as well. It is so great to have an assigned technician because they know the in’s and out’s of your office. I have nothing but positive things to say about the DC Dental team! I have had such a good experience.

Dr. Markova: Yes, I already have! I tell everyone to go talk to DC Dental!

I really like that DC Dental has a big supplies catalog which is very cost effective.

It shows that you do everything that you can do to help customers. The sales reps don’t push products and they are focused on individual doctors which is rare to find in this industry.
