GERMANTOWN ACADEMY S UMMER P ROGRAMS R EBATE P ROGRAM Recipients Name:
This certificate entitles the above named recipient to a $100.00 rebate. Rebate recipients must be registered and have attended the camp for the summer of 2006. This award is not transferable and it must be used for the 2006 camp season only. To be eligible for the rebate, this certificate must be presented with the application for a new camper (who is enrolling in at least two weeks of camp) between March 1, 2006 and April 1, 2006 to Germantown Academy Summer Programs . The rebate will be paid to the recipient after August 1, 2006, on the condition that the new camper has attended at least two weeks of camp, and the recipient has attended camp. Rebates will be cancelled if the new applicant does not complete at least two weeks of camp, and/or the recipient does not attend camp in 2006. Applications that are received as part of the rebate program can not be processed on the web site. The application and the rebate certificate must be mailed or delivered to the office for processing. The $15 registration fee will be waived for the new applicant.
