Include all dates of treatment. Patient ID numbers are also needed. Physician’s names, addresses and phone numbers are required. Include any medical tests you have had, the dates of your appointments and the requesting physician or healthcare professional. 4. Information on all medicines you are taking and the prescriber. 5. List up to five employers you had in the last 15 years. Include dates worked and duties. 6. Just remember to be very inclusive and detailed with all information on the application and all documents that you provide. You have the option to save information and complete later. Information does not need to be complete when submitted, but be advised you must complete the information before the agency reviews your case. You must also sign a medical information release so the agency can acquire any additional information.
