Failure Analysis Workshop Avoid future failures and extend the life of your facility DNV provides leading corrosion and materials experts to help you extend the life of your facility with valuable information provided in a one-day workshop Why Attend DNV’s Failure Analysis Workshops explain and demonstrate how to perform an adequate failure analysis and provide guidance on avoiding future failures, making appropriate repairs, and aiding in the collection of information for remaining life and fitness-for-service assessments. The six-hour course covers: ✓

Establishment of an action plan for performing failure analyses and using the results

A systematic approach to root-cause analysis

Procedures for metallurgical examination

Practical examples of actual failures

Who Will Benefit People from the oil & gas, transmission pipeline and chemical processing industries will find this tutorial beneficial. The course is geared towards: •

Engineers

Common types of failures in mechanical equipment including piping, pressure vessels, tanks, valves, fittings and rotating machinery

Technicians

Personnel involved with failure analysis

Common material failure mechanisms including corrosion, stress corrosion cracking, brittle facture, fatigue, hightemperature creep, wear, and hydrogen embrittlement

Personnel involved with maintenance

Personnel involved with integrity

Failure Analysis Workshop Avoid future failures and extend the life of your facility Who Are the Instructors The workshop will be led by DNV staff members John A. Beavers, Ph.D., FNACE and Michiel P. Brongers, P.E.

Workshop Outline Welcome and Introduction Chapter 1: Review of Common Types of Failures Chapter 2: Action Plan—Be Ready When a Failure Occurs Chapter 3: General Steps of Failure Analysis Chapter 4: Root-Cause Analysis Chapter 5: Metallurgical Examination

Dr. Beavers holds a Ph.D. in metallurgical engineering from the University of Illinois and is a Fellow of NACE International. He is the director of the Failure Analysis department at DNV in Dublin, Ohio. During his tenure he has helped operators develop data to enhance their integrity management programs and optimize testing and inspection activities. In addition, he has directed numerous research and engineering programs for failure analysis and laboratory and field evaluations. Dr. Beavers holds two patents and has over 30 years of failure analysis experience.

Chapter 6: Laboratory Analyses of Field Samples Chapter 7: Example Failure Analyses How to Schedule a Failure Analysis Workshop For more information or to schedule a Failure Analysis Workshop for your staff, contact Valerie Mays at (614) 761-1214 or valerie.mays@dnv.com. For more technical information, contact Dr. Beavers or Mr. Brongers at john.beavers@dnv.com, michiel.brongers@dnv.com or (614) 761-1214.

Mr. Brongers holds a masters degree in materials science and engineering from Delft University of Technology in the Netherlands. He currently serves as a senior project manager for DNV in Dublin, Ohio. He has been with DNV since 1997 and has performed numerous failure analyses.
