¿Son todas las COBERTURAS DE MEDICAMENTOS en medicare iguales?

NO. En Medicare, los medicamentos están cubiertos por las reglas federales de la Parte D que cubren medicamentos orales. Proporcionan pautas de cobertura que son la base de cómo las compañías privadas de atención médica cubren los medicamentos. Hay varios tipos de coberturas de medicamentos y solo se puede usar uno a la vez: (1) PDP (Planes de Medicamentos Recetados) (2) HMO (Medicare Advantage), (3) PPO (Medicare Advantage). La cobertura de medicamentos es muy diferente entre estos 3 tipos de compañías. Por ejemplo, los PDP cobran primas adicionales por su cobertura, mientras que Medicare Advantage no lo hace en Florida. Además, los PDP cobran un deducible por medicamentos de marca, mientras que Medicare Advantage no cobra deducibles en la Florida.

Las pautas de la Parte D detallan 4 etapas de cobertura: Etapa 1: Deducible anual ($480 en 2022), Etapa 2: Límite de cobertura inicial (ICL): donde usted paga un copago o co-seguro según el nivel del formulario de su cobertura. Una vez que los costos de su bolsillo más lo que el plan paga por sus medicamentos alcanza los $4,430 (cantidad de 2022), ingresa en la Etapa 3: Brecha de cobertura “Período sin cobertura”. Esta etapa es donde los costos de desembolso aumentan a medida que el plan cubre menos. Además, los planes de salud crean una lista de medicamentos que cubrirán y NO cubrirán. Esto se llama “Formulario” y cada empresa tiene derecho a incluir o excluir medicamentos, por lo que no todos los formularios son iguales. Luego hay 5 “Niveles” y cada compañía le asigna un Nivel a cada medicamento: Nivel 1: Genéricos preferidos, Nivel 2: No preferido, Nivel 3: Marca preferida, Nivel 4: Marca no preferida, Nivel 5: Medicamentos especializados, es decir, como las medicinas auto-inyectables.

Hay varias razones por las que la cobertura de medicamentos puede ser diferente según la compañía. La primera es que no todas las empresas cobran el deducible. Además, el monto de la Etapa 2 (límite de cobertura inicial) es diferente según la compañía. No puede ser inferior a $4,430, pero si puede ser mucho mayor, como $10,000 para una empresa en particular en el condado de Miami-Dade. Cuanto mayor sea esta cantidad, más tarde entrará en el período sin cobertura en el cual usted paga mas. Además, cada compañía puede colocar los mismos medicamentos en diferentes niveles, y cuanto más alto sea el nivel, mayor será su costo. Entonces, si su medicamento es de Nivel 3 en una compañía y de Nivel 4 en otra, el mismo medicamento de Nivel 4 le costará más que en la compañía donde es de Nivel 3. Obviamente, la Parte D es complicada y requiere que un agente de Medicare con licencia y capacitado lo ayude a comparar sus costos, ya que difieren de un plan a otro, y/o por tipo de plan, es decir, HMO versus PDP. Las personas con Medicaid están cubiertas por un subsidio federal que cubre la mayoría de los costos de los medicamentos, pero si usted no tiene Medicaid, asegúrese de buscar a alguien que pueda ayudarlo a comparar estos costos.

Why is the city not transparent in its communications?”, we expressed our concerns about the lack of transparency in the communications to the residents regarding upcoming zoning changes and high-rise construction on 41st and 97th.

The DCC believes in facts and therefore investigated the case of the OASIS at Doral project and made public record requests to verify what was already evident. Once those records where received our concerns where confirmed, the City and the Mayor deceived the residents. In fact, according to emails from the City received by public records requests (PRR) the staff in the city has been guiding the developer of this project since early February 2023, weeks before the meeting with residents in March.

As mentioned in the April article, there is a process prior to an official and legal application being filed with the city. Part of this process is having the support of the Mayor and key staff members (in Planning and Zoning and Legal) supporting the project. Once this support is given the applicant/developer will be guided to ensure their formal application will go smoothly and ultimately approved by council. While this same project was brought to the previous administration, it was not given any support and therefore never moved forward to staff or pre-application. But, as we know, the administration has changed, and several of the same staff who approved the massive developments during the 2014-1016 years in Doral, are now back and supportive of more high-density development in our city.

Let’s review the events of the last few months:

During the months of January through April, many emails and meeting have taken place regarding this project moving forward. For the Mayor to have stated to the residents that there is NO application is disingenuous at best. We want to make this very clear, there is no law that states the City of Doral and its staff must help developers change zoning, or move projects forward. This project is moving because it has the green light from the Mayor’s office and staff has been asked to follow up. One of these staff members is the newly rehired Julian Perez, who is a big supporter of high-density downtown areas.

Residents must continue to be vigilant and inform themselves on what these changes will bring to the entire City. Ask yourselves why the City changed the focus of the Charrette in March to ONLY focus on the area east of 87th? Why didn’t they want to discuss the entire Boulevard as stated in the first invitation to the residents. Why discuss the beautification of an area that has already been approved and almost complete? Be careful of promises made, such as the promise of a “beautiful project” for our main roads when the massive building on Doral Blvd was built, known us 8800 AMLI. The same staff members recently rehired made many promises during those years of development approvals in Doral. Let’s not repeat the history that took place in 2014-2016!
