SUCCESS STORY

Document-Ready Clients

(This process takes about 3-6 months on average to gather ID and Social Security cards, birth certificates, and medical documentation

A young mother with three children has been working with City Net for the past two months. City Net referred to the SPA 7 family solutions center, where The Whole Child reached out to connect with the family Since the family needed their vital identifying documents updated to complete their intake, they worked with their case managers to get their birth certificates reordered (Continued on page 3)

In March, teams found the most traction with outreach around: an Arco Gas on Firestone Boulevard, and the San Gabriel Riverbed Bridge near a car dealership. The team continues to make referrals to Salvation Army to help people get off the streets, recognizing that these areas are highly trafficked areas for people experiencing homelessness.

SUCCESS STORIES

• (Continued from page 1) City Net was able to cover associated costs for the mother's notarized authorization form for her birth certificate from another county. With City Net's help, the family could rest assured that they were document-ready for the housing process by the end of March.

• Downey's Homeless Solutions Manager referred an older woman who had been living on her own to City Net after she had spent time at one of Downey's Emergency Rooms during the recent cold weather. Upon meeting the case managers, the client shared how she was interested in getting a shelter referral in the nearby area. She explained that she had a disability that required her to visit the doctor often, and having a shelter placement near the office would make it easier for her to keep up her regular visits. Her case managers referred her to the Salvation Army shelter and secured a same-day enrollment. Since the client was ready to enroll right away, City Net provided transportation to the shelter and saw her through the intake process.
