What are venous disease, venous reflux, and symptomatic varicose veins?

Venous reflux disease, also known as venous insufficiency, is a medical condition affecting normal outflow circulation of blood in the lower extremities. Valves contained within veins normally facilitate blood emptying from the legs towards the heart. In the case of venous reflux, these valves no longer function properly, allowing blood to pool in the legs.

This produces back pressure and congestion, otherwise known as venous hypertension.

Venous reflux and venous hypertension can cause swelling, aching, heavy legs to serious recurring ulcers that can be limb threatening.

Varicose veins are often the first recognition of venous disease, but when absent, can frequently result in delayed diagnosis. Varicose veins are enlarged surface veins that become engorged over time as a result of valve failure.

What are the causes?

The underlying problem, common to most of these patients, is venous valvular incompetence.

Valves are present in the heart and veins to facilitate unidirectional flow that maintains efficient circulation. Incompetent valves in the heart are responsible for regurgitant flow, which can lead to dilated heart chambers that can ultimately impair heart muscle contraction.

Veins, responsible for returning blood that has been distributed to the body, dilate in response to incompetent valves. This impairs venous function, delaying return of blood to the heart and overloading the tissues in the leg which causes congestion. Heredity, female gender, pregnancy, weight, and professions requiring prolonged standing and sitting are contributing factors.

Venous disease is quickly recognized in patients with large protruding varicose veins, but in their absence, venous disease can be overlooked and interpreted as another problem. The face of venous disease can be a 30-year old woman, thin and healthy appearing, with a history of multiple pregnancies. It can also be a 30-year old man with four years of recurrent ankle ulcers and severe skin changes. And, then again, it can be a 61-year old woman with a 10 year history of leg pain misdiagnosed as neuropathy and fibromyalgia. In some cases, varicose veins are dismissed as a cosmetic problem.

Venous insufficiency can significantly impact lifestyle. Occupations that require long periods of standing or sitting, thought to contribute to the development of venous reflux, provoke intolerable symptoms of aching fatigue, throbbing, swelling, skin changes, and ultimately painful ulcers. Salesmen, teachers, hairdressers, nurses, and shipyard workers are among those affected. Market research indicates that over 2 million workdays are lost annually in the US and $1.4 billion is spent each year on this common medical condition.

Of the 25 million Americans with venous insufficiency, approximately 7 million exhibit serious symptoms such as swelling, skin changes, and venous ulcers. It is estimated that 72% of women and 42% of men will experience varicose veins by the time they are 60.

What are the symptoms?

Varicose veins often herald the presence of venous reflux. Legs can ache and fatigue due to reflux without the obvious bulging veins.

Discomfort usually worsens toward the end of the day. Sometimes, restless and aching legs at night are the predominant complaint.

Although few patients describe pain per say, the discomfort can be fairly disturbing and interfere with desired activities, including exercise, work responsibilities, and keeping up at family events. People in their fifties and older occasionally dismiss the symptoms to aging and ‘slowing down.’ Other features of venous reflux disease include burning and itching skin of the lower extremities, leg swelling (edema), skin changes including darkening and thickening, and ulcers usually at the ankle level.

What are the answers?

Recommendations are generated based on individual case histories, physical exam, and usually duplex ultrasound evaluation. ❖

Being overweight increases your risk for a host of ailments, including type 2 diabetes, heart disease and stroke. According to the American Diabetes Association, losing as little as 10 to 15 pounds can make a significant difference for people looking to reduce their risks of developing type 2 diabetes. When attempting to lose weight, men and women should recognize that making lifestyle changes is a more effective way to shed pounds and keep weight off than fad diets that may promise quick weight loss but tend to be less effective at keeping that weight off over the long haul. Successful weight loss typically involves a combination of physical activity and a healthy diet. Include physical activity as part of your daily routine several days per week, taking it slow at first if you have not exercised regularly in quite some time. As your body begins to adapt to exercise, you can gradually increase the intensity of your workout routines.

Adopting a healthy diet is another way to lose weight and maintain that weight loss. A diet low in calories and fat is a good start. Adding more fiber to your diet is another diabetes. Foods that are high in fiber tend to make people feel fuller, reducing the likelihood that you will overeat. Fiber also helps the body control its blood sugar levels, and fiber can lower a person’s risk of heart disease. Many foods include fiber, but some high-fiber foods include beans, fruits, nuts, and vegetables.

avoid reFined carbohydrateS Studies have shown that diets rich in refined carbohydrates increase a person’s risk of developing diabetes, while additional studies have shown that diets rich in whole grains protect the body against diabetes. Researchers examining the results of several studies that explored the relationship between whole grains and diabetes found that eating an extra two servings of whole grains each day can reduce a person’s risk of type 2 diabetes by as much as 21 percent. Refined carbohydrates, which can be found in white bread, white rice, mashed potatoes and many cereals, cause sustained spikes in blood sugar and insulin levels, which can increase a person’s risk of diabetes. n

s part of its ongoing commitment to bring the latest technology to the Hampton Roads community, the Sentara Cancer Network has introduced 3D mammography. Highly trained physicians use this breakthrough technology to detect early breast cancer at two Peninsula locations: the Sentara Williamsburg Comprehensive Breast Center and the Dorothy G. Hoefer Comprehensive Breast Center in Newport News.

Conventional mammography creates a two dimensional image of the breast much like an x-ray, making it difficult to find small cancers hidden among layers of breast tissue. This new advanced technology is an extension of 2D mammography and creates 3D breast reconstructions so radiologists can view the breast in paper thin layers. As the radiologist views each layer of breast tissue, early cancers are revealed that would not be visible using other mammography.

A recent study published in the June 2013 issue of the American Journal of Roentgenology (AJR) found the use of 3D mammography increased breast cancer detection by 35 percent. Even more significantly, it increased the detection of the more deadly, invasive cancer by 53 percent. The study of 13,856 women also reduced call back rates by 38 percent. Those are times when a woman is called back for more images, and most times, no cancer is found.

hen it comes to making boneheaded choices, teenagers usually win society’s award for overall poor decision-making.

Yet a study published recently in the journal PNAS suggests that our ability to make wise choices changes over time, and actually declines with old age.

In fact, the study found that in certain situations, the decision-making ability of people older than 65 was worse than that of adolescents. While teens were more apt to drive carelessly, study authors argued that seniors were more likely to make poor medical or financial decisions.

“We found that even the healthiest of elders show profoundly compromised decisionmaking,” wrote senior study author Ifat Levy, an assistant professor of comparative medicine and neurobiology at Yale University in Connecticut.

The study involved 135 men and women between ages 12 to 90. Each was assigned a sum of money -- $125 -- and then asked a series of “lottery questions.” The questions were designed to gauge risk aversion, consistency of thought and rationality.

Seniors “disturbingly” chose irrational wager options 25% of the time, according to the study authors. An example of an irrational choice would be to bypass a sure gain of $5 in favor of an ambiguous or risky choice to win the same amount of cash.

By contrast, adolescents chose irrational options 10% of the time, while young and midlife adults chose them only 5% of the time.

Seniors were far more cautious than the other age groups when choosing between two possible cash gains. If faced with the option of receiving $5 for certain, or a lottery that paid $20 with a 50% chance of winning, seniors chose the $5.

When seniors faced a choice between two losses, however, such as losing $5 for certain or choosing a lottery in which they stood a 50% chance of losing $20 or nothing, they chose the riskier option with the higher potential loss.

Seniors also lacked consistency, the authors found. When a certain question was asked four times, seniors switched their answers “significantly” more than all other age groups and roughly twice as often as adults ages 21 to 50, the authors found.

The authors argued that the pattern of decision-making among elder participants in the study was not a function of illness or age-related dementia. The 36 seniors who participated in the study were all screened and determined to be healthy.

Levy said the findings fall in line with a growing body of research that suggests older adults make decisions detrimental to the wealth, health and general well-being. For instance, they generally borrow at higher interest rates, incorrectly estimate property values and pay more fees to financial institutions.

The reason for such behavior remained a mystery, but it did raise potential policy issues, considering that seniors are more likely to fail to choose health plans correctly and more likely to make voting errors, the authors wrote.

“The decrease in consistency and rationality may be due to a general reduction in cognitive function, which occurs even in healthy aging, although we were surprised that it is so extreme,” Levy said.

“As for the risk preferences, it may be that as they are getting closer to the end of their life, people assume that it is less likely for uncertain events to actually happen to them, which drives them to take less risks with gains, but more risks with losses,” Levy said. “But that is pure speculation – more research is needed to understand these phenomena.” n

others came to intensive care after suffering a stroke, a sudden turn in an ongoing illness, or injury.

The authors of the study, physicians at Vanderbilt University Medical Center and Thomas Hospital in Nashville, were cautious in explaining why ICU patients so often emerge with substantial cognitive impairment.

While suspicion has long fallen on the widespread use of powerful painkillers and sedatives in the ICU, they found few clear links between the medications a patient got in the ICU and his or her subsequent cognitive health.

The new research, which quantifies a phenomenon long observed by critical-care physicians, found that three months after leaving the hospital, 4 in 10 patients continue to have cognitive problems on a par with those seen in cases of moderate traumatic brain injury. And more than a quarter experience a decline in mental function akin to that seen in patients with mild Alzheimer’s disease, the study says.

For 58% of patients who’ve experienced a stay in the ICU, those intellectual deficits are still there a year after they have been released from the hospital, the study found.

The latest research, published Wednesday in the New England Journal of Medicine, found that the sudden decline in mental function was as likely to affect young patients as it was those who were older. And it was no more likely in patients who were ill

even before their hospitalization than it was in those who had been healthy.

In fact, only one factor appeared to predict which patients would sustain mental deficits after an ICU stay: the length of time the patient experienced delirium, a common occurrence among the hospitalized critically ill. The study tracked for a year 821 patients who landed in the ICU from a wide range of circumstances. Some were admitted following surgery;

At the same time, the study authors observed a weak relationship between post-ICU mental decline and the use of benzodiazepines — anti-anxiety and hypnotic medications including diazepam (marketed as Valium). While that link “should be interpreted cautiously,” the authors said, its weakness should not encourage physicians to minimize the use of such drugs where possible.

The researchers suggested that delirium itself may contribute to loss of mental function, given that it is also linked to inflammation and the death of brain cells, which may either start a downward slide in brain health, or serve as a signal of that process. n
