INTERNATIONAL ACCREDITATION A pillar of our international presence is IQNet. Its 36 partners – the largest international network of certification bodies – have issued approximately one third of all certification in the world – that is more than 310,000 certified organisations. With IQNet, DQS Group customers can take advantage of over 40 international accreditations and additional services that are offered in cooperation with IQNet partners.
The DQS Certificate Set your mark! DQS and IQNet certificates as well as the copyrighted certification symbols are the outer markings of a successful certification – and enjoy an excellent reputation anywhere in the world. The symbols may be used in a variety of ways: you can put them on your letterhead stationary, on your brochures, or on your vehicles. You can also use them in all your advertisement, at trade fairs or on the Internet. Please note: •
Use of the certification symbols requires valid certificates or other declarations of conformity issued by DQS.
•
If your certificate does not apply to the whole of your organisation, you are requested to indicate the restricted scope for which it is valid.
•
The symbols should always be used in connection with the name of your company.
•
When using a neutral certification symbol, the underlying standards or specifications needs to be specified in the immediate vicinity of the symbol.
•
The symbols may not be changed in any way – however, you may choose which colour to use.
•
The IQNet symbol may be used along with the DQS certification symbol, but not by itself.
•
We recommend appending the certificate registration number to the symbol.
•
The symbols may not be used for the labelling of products.
•
The impression of a product certification must be avoided at all costs.
The DQS and IQNet logos (symbols without the rings) are not certificate symbols. They are protected as proprietary marks virtually world-wide, and DQS and IQNet reserve them for their own use.
ROADMAP TO CERTIFICATION SUMMARY Step 1
Basic Data Sheet (DQS will send you the document called Basic Data Sheet. For ISO standard and each business unit e.g. ISO 14001 and BS OHSAS 18001, the document needs to be filled in and send back to DQS)
Step 2
Cost Estimate will be worked out for the 3 year audit cycle on each standard required for certification. DQS uses the AMDAC table approved by the accreditation body TGA to quote the required days in doing the assessments. The Cost Estimate (Quotation) is sent to the client for approval.
Step 3
Once the Cost Estimate has been approved, assessments are then planned according to the client requirement and assessment process
Step 4
Assessment Process is as follows:

Force Majeure
In the event that DQS cannot, within reasonable time, fulfill its obligations arising from an agreement reached with "the client" due to strikes, accidents, fire, flood, tempest or other acts of God, which could not be foreseen or overcome by DQS, DQS and "the client" may mutually agree to cancel the order; in which case DQS will be entitled to full payment for all items delivered to date and/or all progress payments and milestone payments that were payable at the date of the event, as well as any commitment made which cannot be canceled. DQS shall not be held liable for damages or losses suffered as a result of the delay or cancellation.
Agreement
No verbal agreement or undertakings made by any employee of DQS, nor any variation or cancellation of these terms will be binding on DQS, unless it is confirmed in writing by an authorized DQS employee.
Stage 1:
System Analysis (document review and site tour): to determine whether the organization has established, implemented and is maintaining the ISO/BS OHSAS standard as per scope of application. The System Analysis is minimum 1 or maximum 5 man-days at the client premises depending on the number of employees. Time period between System Analysis and System Assessment is maximum 3 months
Arbitration and disputes
In the event of any dispute or disagreement arising between "the client" and DQS during the execution of the order or the carrying out of the terms of the order or any matters arising there from, such dispute shall, if it cannot be settled by mutual agreement, be submitted to and decided by arbitration in terms of the Arbitration Act, Act No 42 of 1965, as amended. DQS and "the client" shall nevertheless be entitled to depart from this provision by mutual consent.
Pre-Assessment
(not mandatory but is used as Gap Analysis of your Management System.) The man-days are agreed between the client and DQS. After the assessment the report is compiled with recommendations and deviations that need to be corrected before the certification assessment (System Assessment) is carried-out.
Stage 2:
Certification Audit (on-site assessment of the documented procedures and process assessment). The auditor will normally record areas of improvements, observations and action plans (findings). The Action Plans need to be closed/ addressed within 90 days or alternatively a project plan must be established if the action plan will take more than 90 days to be addressed.
Stage 3:
1st (Stage 1) and 2nd (Stage 2) Advancement Assessment (surveillance assessment) is carried-out on year 2 and year 3 to complete the assessment cycle. The number of mandays for the advance assessment is far less than the System Assessment mandays. Addressing the action plans raised is the similar process as on system assessment.
Stage 4:
Re-Assessment (re-certification on year three) is carried out to recertify the management system. Sample taken during this assessment is much larger than the one taken during the advancement assessment.
Indemnity
Clients listed below and DQS mutually indemnify each other against direct damage or injury to property to the extent caused by negligent acts or omissions of either party, its subcontractors, agents or employees whilst on the designated site for the purposes of delivery or installation of the quoted goods or service. Any consequential damage or loss will be for the customer’s account.
Precedence
If any of the Standard Terms and Conditions of Sale as outlined in this document differs from specific Terms and Conditions of Sale outlined in our proposal, the conditions contained in our proposal shall take precedence.
AUDIT PROCESS Customer service representative
DQS-office prepares everything necessary for the conduct of the assessment process. It selects authorized auditors, assigns them to perform the separate assessment stages and supplies them with necessary documentation. On behalf of the DQS managing directors, it concludes an assessment contract with the customer, based on the Order Confirmation and the General Conditions of Business. Following the on-site assessment, customer service representatives review all documents for completeness and perform the necessary steps to continue the assessment process.
Group leader / assessments
The respective group leader serves as contact person for auditors during the assessment process and, in cases of doubt, shares responsibility with them in making decisions.
Lead auditor
Lead auditors represent DQS on site. They are responsible for the assessment process at the customer’s, the audit team’s performance, their discipline and compliance with DQS directives. Among other items, this includes:
Conduct
In case of a difference of opinion between lead and co-auditor at the customer’s, lead auditor will make the final decision. In case of a separation during the assessment, lead auditor will accept coauditor’s results without reservation. During the interviews, the checklist will be used to identify documents reviewed during the assessment, to include quality records; clearly identified samples and comments will be annotated on form 750D1_N Findings (findings pad), in order to be used as a basis for evaluation after conclusion of the interviews. During joint auditing, one findings pad suffices to record findings. Auditors will provide feedback to interview participant’s ref. their statements. Whenever auditors operate separately during an assessment, each auditor will keep one findings pad. Evaluation is done jointly by both auditors following completion of individual segments. At the end of the assessment, lead auditor will integrate the filled-in parts of coauditor’s findings pad. In the end, there will be only one findings pad for each assessment. Reviewing confidential documents such as recipes and special operating instructions requires customer’s consent. Such a review is usually not necessary during an assessment. • •

Being familiar with the customer, their needs and existing agreements (e.g. on the basis of data provided by customer service staff), in order to convey a clear and consistent image of DQS and its auditors. Arranging for date and place of conduct Preparing the audit schedule according to 750E1 XSampleSchedule-System-Analysis resp. 750E1_YSample- Schedule-Audit (Please note comments within the documents)). In case only one of the auditors in the audit-team covers the scope, it needs to be ensured during preparation of the audit schedule that sector specific areas or processes will only be assessed by the auditor covering the scope. At least one member of the audit team needs to have suitable command of the local language. If that is not the case, lead auditor, in concert with the organization to be certified, needs to arrange for an independent interpretation during audit planning.
Lead auditor is responsible for the arrival, the assessment beginning on time, its proper conduct and necessary resources. This includes: • Co-ordination with customer to ensure agreed-upon date can be kept • Forwarding information to co-auditor, if one has been assigned, and agreeing on place and time for preassessment consultation • Reviewing audit documents for completeness in accordance with 750E1_L Documents for Assessment. • Reviewing directives for completeness • Forwarding information about results / agreements arrived at on site to customer service representative • The requisite protective equipment The lead auditor will receive copies of all necessary documents and available customer correspondence, along with the assessment order, from customer service representative at least one week prior to the agreed-upon date.

Return all original documents and quality records to DQS within seven working days. Stay in touch with customer Forward information about results / agreements made on site to DQS-office
Co-auditor Co-auditors represent DQS on site and support lead auditor during the audit. Their duties commence only after having been assigned for an assessment by the customer service representative and include i.e.: • • • • • •
Being familiar with system documentation Co-ordination with lead auditor Participation in opening and final meetings Conducting interviews Documentation of findings (in co-ordination with LA) Co-operation in the evaluation / explanation of actions to be taken
Whenever auditors operate separately during an assessment, co-auditors are responsible for their part.
