Dial 9-1-1 for Emergency and Non-emergency Medical Needs

October is fire prevention month. Our organization’s name, Dubuque Fire Department (DFD), represents just one of our core missions, to protect lives and property from fire; however, your fire department delivers more services than just fire suppression and prevention. In Fiscal Year 2016, the department responded to 6041 calls for service EMS represented 73% of all responses. Our EMS services often generate many questions from our citizens on how this operation works within our community. DFD has four ambulances staffed and ready to respond 24 hours a day, 7 days a week. The ambulances are certified with the State of Iowa at the ACLS (Advanced Cardiac Life Support) level, which means you and your loved ones will receive the most advanced medical treatment available for your medical or traumatic emergency. This is often equated to bringing the emergency room to you, since our staff will be able to start IVs to give fluids and medications, provide defibrillation to correct arrhythmias in the heart and secure the airway to provide ventilations for patients that cannot breathe on their own. The city has in place a 28E Mutual Aid Agreement with our surrounding fire departments, providing access to an additional 17 ambulances during heavy call demand periods or major events. This ensures that when you call 9-1-1, you can expect a reliable system of trained professional emergency responders answering your needs. The department presently has 89 uniformed personnel, with 79 members certified at the Paramedic level. This is the highest level of pre-hospital providers. Did you know that the national survivability rate from a cardiac arrest, according to the American Heart Association in 2015, was around 10.6%? The Dubuque Fire Department takes pride in having a cardiac arrest survivability rate of 14.29% of all for the same time frame. This is a testament to the advocacy effort that have been implemented within our community by encouraging bystanders to initiate “Hands Only CPR” prior to the arrival of emergency responders. We know we can improve our cardiac arrest survival rate and work each day to promote early CPR and having more automatic external defibrillators (AEDs) available in our community. When you dial 9-1-1, trained emergency dispatchers will answer your call. They follow an industry standard process called Emergency Medical Dispatching (EMD), with specific questions based on the caller’s needs. The answer to the questions will assist the 9-1-1 dispatcher to send the nearest emergency vehicle(s) to the incident. The dispatcher provides guidance in how the unit(s) will respond, emergency or non-emergency, to the scene based on the responses they receive during their questioning. This ensures that vehicles get to the scene as quickly and safely as possible based on the nature of the situation. The highest level of response is dispatched as a “Code 1”. This is when all responding vehicles travel to the scene with their lights and sirens activated. During the “Code 1 call” DFD will respond with an ambulance, a fire engine assigned to cover that district, and a command vehicle. This response is designed to get the closest responder to the scene in a timely manner, and the providers work as a team to deliver expedient treatment to help the patient. Non-emergency transfers between facilities and/or a private residence is another service delivered by the Dubuque Fire Department. For non-emergency calls, responders will come to the scene without lights and sirens activated. The Fire Department is funded by property taxes. While the ambulance service does bill for services provided to the patient, many of the ambulance fees are paid by insurance and Medicare. The revenue generated by the fees are used to offset the property tax need for the Fire Department. The Dubuque Fire Department’s mission statement says it all: “To Protect, Assist and Educate our Community and Visitors with Pride, Skill and Compassion.” We believe prevention and education are part of our important duties. Providing great customer service during your time of need is integrated into our mission. Every member of the Dubuque community can expect nothing but the best when they dial 9-1-1 for emergency and non-emergency medical needs.

Dubuque Fire Department Receives 2017 Mission Lifeline EMS Gold Award

The Dubuque Fire Department has received the 2017 Mission: Lifeline EMS Gold Award from the American Heart Association for implementing quality improvement measures for the treatment of outof-hospital heart attack patients. Of the 931-emergency service (EMS) services in the state of Iowa, the Dubuque Fire Department is one of three to meet the criteria to receive the Gold Designation in 2017. Mission: Lifeline Emergency Medical System Recognition seeks to acknowledge the work, training, and commitment by EMS agencies and medical first responders (MFRs) to improve the overall quality of care for “STEMI” patients. According to the American Heart Association, STEMI is a common name for ST-elevation myocardial infarction, which is a more precise definition for a type of heart attack. STEMI has a substantial risk of death and disability and calls for a quick response. As a participating EMS agency, DFD staff receive training in STEMI identification, provides access to 12 lead ECG machines, and develops protocols derived from ACC/AHA STEMI guidelines. Dubuque Fire Department medics are driving improvements in the care and outcomes of STEMI patients. The 2017 Mission: Lifeline EMS Gold Level Recognition Award is based upon achievements identified through an agency’s online submission of data representative of the care in 2016. The criteria to achieve the Mission: Lifeline EMS Gold award is based on meeting achievement measures aggregated annually for two years with no single measure below 75 percent. Mission: Lifeline is a coordinated, proactive system of care that turns first responders, hospitals, and other healthcare providers into a team that uses proven guidelines and best practices to treat patients suffering from acute-coronary syndrome (NSTE-ACS) heart attacks.
