To redeem incentives, please follow the instructions below: 1. Visit midmark.com/promotions 2. Select the 2016 Medical Procedures Table Promotion icon 3. Select “Redeem Offer” 4. Upload invoice* 5. Complete the information required on the form 6. Select “Submit” All serial numbers must be submitted online. Upon each submission a confirmation email will be sent for your records. Please keep the confirmation until incentives are received. Incentives must be submitted online by July 29, 2016. * Midmark must receive a copy of the physician or medical facility invoice (stating the physician’s name and date purchased). The invoice must be dated February 1 – June 30, 2016.

Incentives will not be processed until the invoice is received.

This offer constitutes a discount or reduction in price on the purchased product(s). Buyer/Lessee/ Lessor is obligated to: (i) fully and accurately disclose the amount of this discount in Buyer/Lessee/ Lessor’s cost reports or claims for reimbursement to Medicare, Medicaid or other federal healthcare program that requires such disclosure; and (ii) provide documentation of the discount to representatives of the Secretary of the Department of Health and Human Services and state agencies upon request.
