REPLACED TABLE SALE AND TRANSFER AGREEMENT:

*Midmark must receive a copy of the invoice (stating the physician’s name and date purchased). The invoice must be dated September 1, 2016 – January 31, 2017. Incentives will not be processed until the invoice is received.

In exchange for the trade-in rebate amount as payment from Midmark Corporation, I hereby agree to sell and transfer my old table, as owned and/or used in my medical or clinical practice, to Midmark Corporation. I hereby acknowledge and certify one of the following disposal actions regarding the above table that has been purchased by Midmark Corporation:
