5. Select “Terms and Conditions” then “Submit” All serial numbers must be submitted online. Upon each submission a confirmation email will be sent for your records. Please keep the confirmation until incentives are received. Incentives must be submitted online by January 31, 2014. * Midmark must receive a copy of Physician or Medical Facility invoice (stating the physician’s name and date purchased). Physician or Medical Facility invoice must be dated September 15, 2013 – December 20, 2013. Incentive will not be processed until invoice is received. This offer constitutes a discount or reduction in price on the purchased product(s). Buyer/Lessee/Lessor is obligated to: (i) fully and accurately disclose the amount of this discount in Buyer/Lessee/Lessor’s cost reports or claims for reimbursement to Medicare, Medicaid or other federal healthcare program that requires such disclosure; and (ii) provide documentation of the discount to representatives of the Secretary of the Department of Health and Human Services and state agencies upon request.
