Complete the information required on the form 5. Select “Terms and Conditions” and “Submit” All serial numbers must be submitted online. This offer is valid on promotional products purchased by a medical physician or medical facility through an authorized Midmark Medical dealer. Upon each submission a confirmation email will be sent for your records. Please keep the confirmation until incentives are received. Incentives must be submitted online by July 31, 2014. * Midmark must receive a copy of Physician or Medical Facility invoice (stating the physician’s name and date purchased). Physician or Medical Facility invoice must be dated February 3 – June 30, 2014. Incentives will not be processed until invoice is received.
