Welcome to the Fall 2023 issue of Insight, a newsletter celebrating the accomplishments of our community of scholars. We aim to spotlight insights from people at the Dugoni School working in all areas of scholarship, including clinical or biomedical research, the scholarship of teaching and learning, improvement of the health care system, and professional partnerships that advance the field.
Faculty Member Receives NIH Award to Study Immune Biomarkers for Predicting Oral Cancer Dr. Xiaoyuan Han, assistant professor in the Department of Biomedical Sciences at University of the Pacific, Arthur A. Dugoni School of Dentistry, was recently awarded a Support for Research Excellence-First Independent Research (SuRE-First) Award from the National Institutes of Health (NIH) on the topic “Single-cell immune landscape in the oral dysplasia’s malignant transformation.” Han is the first researcher at University of the Pacific to receive the SuRE-First award, which aims to provide students, especially those from underrepresented groups, with research opportunities and enrich the research environment at institutions having limited NIH research support.

The award grants Han $702,000 for a four-year research project to identify immune features that predict the malignant transformation of cells in the mouth to oral cavity squamous cell carcinoma (OCSCC) using state-of-the-art single-cell analysis and bioinformatic AI tools. “Our students will develop multiplex immune imaging assay on clinical dental biopsies and apply innovative bioinformatic AI tools on data analysis in my research project, which is a collaboration with Stanford School of Medicine,” said Han. “OCSCC is an aggressive cancer with a poor prognosis, and Dr. Han’s work could help to identify patients needing early care and increase our ability to provide personalized medical care to treat these patients,” said Dr. David Ojcius, assistant dean for research and co-chair of the Department of Biomedical Sciences.
Pacific Collaborating with Stanford to Train Next Generation of Clinical Oral Health Researchers University of the Pacific, Arthur A. Dugoni School of Dentistry is collaborating with Stanford University on an ambitious program aimed at bridging the gap between biomedical research and clinical practice. A $3.8 million grant was recently awarded by the NIH’s National Institute of Dental and Craniofacial Research (NIDCR) to fund the Pacific-Stanford PRIMED Program. New knowledge on oral disease prevention and clinical care is constantly emerging, but much of it does not make it into clinical practices. The five-year grant will allow Pacific to train the next generation of clinical oral health researchers who are equipped to translate research into practice. There is also a growing need for more clinician-scientists in the workforce. “As a clinician, it’s profoundly gratifying to help one patient at a time, but as a researcher we can help populations at a time,” said David Lam, professor and chair of the Dept. of Oral & Maxillofacial Surgery and associate dean for medical integration. Lam is co-leading the grant with Rebecca Moazzez, professor and chair of the Dept. of Preventive and Restorative Dentistry and director of the Center for Innovation and Translation at Pacific and two researchers from Stanford— Ruth O’Hara, professor and senior associate dean for research and John Sunwoo, professor and associate dean for academic affairs. “We are extremely excited to formalize our research and educational collaboration with our colleagues at Dugoni,” Sunwoo said. “We have collaborated informally for several years with Dr. David Lam and are thrilled to be part of this new initiative to train the next generation of oral health clinician-scientists.” The partnership between the schools draws on Pacific’s strength in dental education and training and Stanford’s robust clinical and translational research. “Putting the two together, we synergistically help each other,” Lam said. The program will transform dental education at the Dugoni School by making clinical research training mandatory for all first-year students and residents. “Our students will be in a different league compared to dental graduates around the country,” Moazzez said. “Not only will they have their excellent dental training, but they will now also receive excellent clinical research training, together with a certificate from Stanford University, which will equip them for practice-based research here at the dental school, and in the future, in their private practices.”

Research studies conducted as part of the training will focus on three areas: oral cancer, sleep medicine and developing a Pacific-Stanford biobank. The oral cancer study is aimed at detecting cancer earlier. Sleep apnea research will expand work currently underway at the Dugoni School to evaluate the effectiveness of new treatment options. The biobank project involves collecting blood, saliva, tissue, and demographic information to create a robust collection of patient samples. “At every patient appointment, students will be trained on obtaining informed consent and collecting saliva or blood samples,” Moazzez said. “The initial advantage is bringing research culture into the day-to-day operations of the clinic. The collected data and samples will then help drive future research projects and collaborations.” The expanded focus on research at the Dugoni School will also help advance the work of Pacific’s Center for Innovation and Translation located on the San Francisco Campus. The center’s goal is to support students, residents, and faculty, so that clinical research becomes the easiest thing a clinician can do. The center also aims to build on existing and develop further collaborations with commercial companies and startups to test new products and innovations and facilitate translation of innovation into clinical practice.
A grant from the NIDCR / NIH will fund a collaboration between Pacific and Stanford to train the next generation of clinical oral health researchers.

Dugoni School student Sahar Rahnemabayat presented at the 28th annual Hinman Symposium which took place in November 10-12, 2023 in Memphis, TN, on the research topic “Inferior Alveolar Nerve Block in Pediatric Patients: A Quantitative Assessment. Gary Richards served as mentor on this project.

Exosomes in Multiple Sclerosis and Alzheimer’s Disease — Adversary and Ally What is it? A review of how exosomes play a role in several conditions and could also be the key to therapies for them and other conditions. Extracellular vesicles (EVs) are small, membrane-enclosed particles that cells secrete, and which carry proteins, lipids, and nucleic acids. Exosomes are a type of EV. As multivesicular endosomes form, they incorporate cytoplasmic bits and contents, transforming into exosomes. The exosomal cargo mirrors the identity and current physiological state of the cell that secretes them. EVs transport signaling molecules and play crucial roles in processes like myelination, trophic support, synaptic plasticity, and antigen presentation in a healthy state. What problem does it aim to solve? EVs can move between cells in the CNS and even cross the blood-brain barrier, influencing organs outside the CNS and vice versa. In disease, CNS-derived or peripherally-transported EVs can carry harmful molecules, contributing to neuroinflammation and neurodegeneration. However, they could also help deliver therapeutics. How does it work? EVs show promise for treating CNS diseases through various delivery methods like intravenous, intraperitoneal, subcutaneous, or intranasal routes. They’ve been successfully used both broadly and locally for therapeutic effects. To enhance precision, specific strategies are employed, such as using ligands or antibodies to target cells. In Alzheimer’s disease, intranasal administration is being explored, with a clinical trial underway for nasal administration of exosomes. Clinical trials with EVs have accelerated, driven partly by interest in COVID-19, but the field is still at an early stage, with most studies in early phases and no completed phase III trials. What are the real-world implications? Despite increased interest and research in the last decade, the use of EVs in treating CNS diseases is still in early stages. There’s growing potential for EVs to serve as biomarkers and therapeutics for conditions like multiple sclerosis (MS) and Alzheimer’s disease (AD). However, certain technical challenges need addressing before EVs can be used in patient care. What are the next steps? Ongoing technological advancements aim to unlock the potential of EVs for precise drug delivery to combat neuroinflammation and neurodegeneration, ultimately improving conditions like MS, AD, and other CNS diseases.
