! MESSAGE%FROM%THE%CEO% ! Death,% Dying% and% Bereavement% within% the% UK% are% changing.% Major% changes%in%demographics%will%mean%that%a%growing%older%population%will% leave%health%and%social%care%services%with%significant%challenges.%Many% people%will%live%longer%and%will%die%with%multiple%chronic%illnesses%which% will% include% dementia.% This% change% in% itself% will% be% significant% for% hospices.% There% are% three% times% as% many% care% home% beds% as% NHS% hospital%beds%in%the%United%Kingdom%and%currently%one%in%five%people%in% the%UK%die%in%residential%or%nursing%care%homes.%We%will%need%to%ensure% that% both% our% workforce% and% organisational% structures% and% service% offerings,%are%developed%accordingly%to%meet%these,%and%other,%changing% needs.! ! In% the% publication% ‘Dying% for% Change’% (DEMOS% 2010),% hospices% are% charged% with% ‘providing% intimacy% at% scale’.% At% Earl% Mountbatten% Hospice,%we%place%achieving%this%at%the%heart%of%our%strategy%for%the%next% five%years.% % Our%2015–2020%strategy%at%EMH%also%reflects%our%desire%to%remain%at%the%forefront%of%caring%for% people%living%with%death,%dying%and%bereavement%across%the%Isle%of%Wight.%We%realise%that%we% cannot% face% these% significant% challenges% alone,% and% we% aspire% to% work% in% partnership% with% others%to%achieve%our%aims.%The%final%report%of%the%Commission%into%the%Future%of%Hospice%Care% was%published%almost%two%years%ago.%Much%of%the%focus%of%our%EMH%strategy%responds%to%key% areas%published%within%this%report%and%we%align%ourselves%strongly%to%our%national%membership% organisation,%Hospice%UK,%and%commit%to%sharing%and%learning%from%our%fellow%hospices%as%we% move%forward%into%the%future.% % We%know%that%everyone%will%die%and%everyone%will%be%bereaved.%Over%the%next%five%years,%EMH% will%be%needed%more%than%ever%in%order%to%support%our%Island%both%to%live%and%to%die%well.%We% continue%to%depend%on%our%local%community%for%over%two%thirds%of%our%annual%costs,%and%we%are% grateful% to% them% for% continuing% to% support% our% work% and% aims.% However,% we% also% know% that% most%of%us%are%not%comfortable%in%talking%about%death,%dying%and%bereavement.%We%believe%that% we% have% a% responsibility% to% change% this,% and% you% will% notice% that% our% strategy% contains% a% number% of% key% initiatives% around% engaging% with% our% local% community% positively% in% order% to% challenge%misconceptions%and%to%change%attitudes%towards%the%work%that%we,%and%others,%do.% % It% is% important% for% EMH% that% now,% more% than% ever,% we% get% it% right.% The% modern% hospice% movement% grew% out% of% a% revolution% against% terrible% deficits% in% the% care% of% the% dying.% As% a% charity,%we%must%not%forget%that%we%are%here%to%fight%for%social%justice,%to%care%for%people%when% they% are% at% their% most% vulnerable,% and% to% challenge% the% status% quo.% We% aim% to% remain% slightly% disruptive%as%the%next%five%years%unfold.%

Main%challenges% !
•! EMH's! mission! to! promote! and! to! provide! good! care! and! support! for! those! people! living! with,! affected! by,! or! curious! about! death,! dying! and! bereavement! across! the! Isle!of!Wight!faces!considerable!challenges.!Over!the!next!few!years!hospices!will!be! confronted! by! increasing! and! unusual! demands! against! a! background! of! continued! and!unprecedented!changes!in!health!and!social!care.!!! •! There!is!a!subsequent!necessity!to!develop!models!of!care!that!are!less!demanding! of! resources,! both! financial! and! professionalZ! whilst! holding! fast! the! respect! for! the! dignity! of! each! individual! and! the! support! for! those! close! to! them,! as! well! as! the! attention! to! symptom! control,! that! have! rested! at! the! core! of! hospice! care! since! Dame!Cicely!Saunders!founded!the!hospice!movement!in!1967.!!! •! By!2035!3.5!million!of!the!UK!population!will!be!aged!over!85!and!will!account!for!half! of! all! deathsZ! one! in! three! of! them! will! die! with! dementia! and! most! with! multiple! chronic!conditions.!!These!demographic!changes!are!occurring!sooner!on!the!Island.! EMH!will!need!to!be!financially!and!organisationally!secure!to!compete!successfully! within! the! changing! NHS! commissioning! environment! and! will! need! to! develop! and! articulate!appropriate!evidence!about!what!support!is!most!effective,!for!whom,!and! at!what!cost.!!Many!hospices!are!unable!to!articulate!accurately!the!true!costs!of!the! care! they! provide.! The! Palliative! Care! Funding! Review! will! support! us! to! achieve! greater! accuracy! and! transparency! regarding! costs! of! hospice! care! in! different! settings.! •! Hospices! need! to! develop! outcome! measures! which! give! them! the! ability! to! really! prove! their! worth.! We! are! committed! to! working! with! others! to! develop! a! set! of! national! measures! in! order! to! achieve! our! aims.! We! are! told! that! between! 5! and! 9! per!cent!of!all!deaths!occur!in!hospices!across!the!country.!However,!we!know!that! like! EMH,! many! hospices! care! for! people! dying! at! home,! in! care! homes! and! within! hospitals.! Our%commitment%to%meet%their%needs% ! ! We! provide! a! needs9led! service,! rather! than! one! that! is! driven! by! our! own! aspirations! and! areas!of!interest.!We!identify!the!changing!needs!of!our!society!through!working!closely!with! our!community!at!all!times.!This!is!achieved!by!working!in!close!partnership!with!other!health! and! social! care! providers,! as! well! as! maintaining! regular! contact! with! individuals! and! community!groups!to!obtain!feedback!and!new!information!in!order!to!continuously!improve! and!develop!our!services.! ! The!following!diagram!draws!together!the!current!levels!of!services!provided!by!the!Hospice.! The! services! incorporated!in!the!model!are!all!seen!as!“core”!to!our!mission.!The!differentiation!in!costs!of! the!services!will!not!affect!our!drive!to!provide!high!quality!care!at!all!times!and!at!all!levels.! We! believe! that! if! we! can! provide! these! different! services,! then! we! will! reach! our! three! beneficiary!groups!–!those!approaching!the!end!of!life,!those!who!are!bereaved,!and!those! who!are!curious!about!death,!dying!and!bereavement.! !
