I am honored to have been elected to serve as president of our Escambia and Santa Rosa County Medical Society. Given our rich 145-year heritage it is very humbling to follow those who preceded me. At our inaugural ball I asked … why medicine? And why the ECMS? I believe we all have an altruistic desire to help others—we practice medicine with a desire to make a difference as healers. Dr. Ellen McKnight spoke last year of adverse changes within our beloved profession. She explained that only through engagement and becoming activated physicians can we affect change. She certainly set the example as an activated physician and I hope to follow. Currently we cannot view medical social media without seeing references to physician burnout, depression and dissatisfaction—this is reflected by all time high rates of physician suicide and early retirement. Burnout is real. I have seen it in my colleagues and have a few times experienced it myself. I must confess, I didn’t join the ECMS to only help my fellow physicians and further the efforts of organized medicine, but I also joined for me. I need you, my physician colleagues, to share my journey in medicine. So many forces separate us as physicians and break us down: payor contracts, billing and coding, the quest for the “all mighty RVU”, malpractice and liability risk. Patient demands are unrealistic, and we strive for satisfaction scores that do not measure quality while also worrying about online physician ratings that are meaningless. Yet the greatest assault on our solidarity has been the EHR. We as physicians don’t even speak the same language. Our EHRs do not talk to each other. Disney using Magic Bands knows where their customers are, where they have been, and where they are going- all linked to their smartphone devices. They do this efficiently and at a fraction of the cost of our EHRs.

I still, however, believe we are incredibly fortunate to love what we do, just not how we have to do it. These challenges to our beloved profession may represent opportunities for us to band together and lean on each other. We did this as students and residents surviving rites of passage of malignant surgery rotations and grueling nights on call- we can do this now as well.

President’s Letter Continued Despite all that divides us, the ECMS is uniquely positioned to bring us together. It is the only organization in our area that represents physicians with an equal voice irrespective of status: employed, independent, military, academic, inpatient, outpatient, adult, pediatric, practicing, retired or still in training. Thus the ECMS offers the platform to unite us. We must have each other’s backs. It is my hope as your president that in 2019 we reach out to one another and get to know each other – initially putting faces with names of our colleagues but also by attendance and engagement getting to genuinely know one another. We will bond by sharing the trials and triumphs of our practices. This connection should lead to finding our common ground on those issues we will support at the FMA and beyond. We can then become a united voice fighting to protect the values we hold dear and will thus preserve the practice of medicine and the relationships with our patients. The government has heard and begun to respond to our frustrations but our legislators and CMS are trying to fix our issues for us in their way, not ours. We do not have to accept this and must be part of the solution to our own problems. In closing, The ECMS bringing us together allows us to endure the external forces that at times suck the joy out of medicine and by uniting we can rekindle that yearning we all began with….to be healers of our patients and each other. - Brett Parra, M.D.

Medical/Legal U.S. Surgeon General Discusses Public Health Issues The Doctors Company’s 2018 Executive Advisory Board meeting—a gathering of some of the leading figures in medicine in the United States—featured a conversation between The Doctors Company Chairman and Chief Executive Officer Richard E. Anderson, MD, FACP, and the United States Surgeon General, Vice Admiral Jerome M. Adams, MD, MPH. In this second of two articles highlighting key excerpts from the conversation, Dr. Anderson and Dr. Adams discuss the dangers of e-cigarettes and the movement to population health. Dr. Anderson: The Surgeon General’s office has been relentless in highlighting the dangers of cigarette smoking. My impression is that despite a decades-long reduction in the overall incidence of smoking, the percent of smokers in the United States is still very high—I think something around 20 percent, which leads us to a major controversy—and that is the role of e-cigarettes, vaping, and nicotine substitutes for cigarettes. It’s a complex issue with a lot of tentacles, but we would be grateful if you would give us some of your thoughts about that. Dr. Adams: This is a very important and personal topic for me. I’ve got three young kids. I’m not just dealing with this as a surgeon general or as a physician; I’m dealing with it as a dad. Respectfully, I think that folks are purposely making it a complicated issue. I don’t think it’s that complicated. We know that the combustible cigarettes are bad. We know that nicotine is addictive. E-cigarettes and vaping can be two things at once. There can be harm reduction compared to combustible cigarettes for current smokers, but they can also be a dangerous initiation into the world of smoking. I believe, and I hope all of you in the audience can say it loudly: No young person should be vaping. No young person should be using e-cigarettes. No one should be marketing these products with flavors like cotton candy and bubble gum to young children. I’m working with the FDA and Commissioner Scott Gottlieb on this issue. We know the brain continues to develop up until age 26, and we know the folks are susceptible to developmental delays and effects on the brain, which prime it. Dr. Anderson: The healthcare delivery system in the United States is undergoing a kind of once in a hundred-year cycle of change in the way care is delivered in almost every parameter of care; there are new technologies, new medications, new surgical potential, as well as different ways of accessing care throughout the system. But an important part of the conversation in that transformation is the discussion of population health versus the traditional doctorpatient and the individual patient and the individual office. It’s really struck me that this conversation is very much akin to the entire discipline of public health, which after all is focusing primarily on population health. How do you find that your background in public health helps you? How does it affect the way you see this national transformation? Dr. Adams: A lot of our problems stem from the fact that we don’t teach enough about public health and population health to our medical students or nursing students. We don’t teach enough about how to discuss proper diet, how to talk about exercise, or how to talk about smoking cessation—all things that are promoted as part of public health. That said, we’re all quickly being forced to get to grips with that because of changes in payment models. We found that in paying for procedures and cutting things up piecemeal and then reimbursing for them is not only ineffective, it’s costlier. And so now when we look at paying for outcomes, we’re having to embrace population health. The good news is doctors, nurses, and healthcare professionals are all more satisfied when they’re able to do that. When systems are put in place that give them that freedom, flexibility, and the tools they need, it allows them to serve the patient. It empowers professionals to refer patients or to write a prescription for food; it allows them to refer patients to community recreation centers where they can exercise—and ultimately, that patient becomes healthier because of something that you’ve been able to facilitate as a medical practitioner. So, we’re shifting in that direction; some people are kicking and screaming, others are embracing it. Dr. Adams is the 20th Surgeon General of the United States, a post created in 1871. He holds degrees in both biochemistry and psychology from the University of Maryland. In addition, the Surgeon General has a master’s degree in public health from the University of California at Berkeley, and a medical degree from the Indiana University School of Medicine. Dr. Adams is a boardcertified anesthesiologist and associate clinical professor of anesthesia at the University of Indiana. He has been active in a number of national medical organizations, including the American Society of Anesthesiologists and the American Medical Association.

Who is the Escambia County Medical Society Foundation? The Escambia County Medical Society Foundation is a non-profit organization dedicated to providing healthcare services on a volunteer and funding basis through its members. The Foundation was created in 1994. The primary goal is to assure access to adequate healthcare for the medically indigent citizens of the area, to study and promote improved methods and facilities for healthcare, to pursue the protection of public health, implement the means of financing healthcare at reasonable costs to cooperate with other organizations and institutions interested in pursuing these goals, and disseminate information concerning healthcare in general. Foundation ECMS Foundation Newsletter Dear County Medical Society Members; I hope that you all had a good holiday and have now settled down to a reasonable schedule. We have an exciting calendar planned for 2019 and we hope you will join us in support of our growing programs: • The Physician Wellness Program - Physicians have begun to take advantage of the “Helping Healers Heal” program to combat physician burnout. It is completely confidential and the first three sessions are free for Active members. Review our website for more information. We will also be adding a female psychologist this year. • The Blood Pressure Program continues to distribute blood pressure cuffs to patients who cannot afford them through the four local Free Clinics. Your Foundation has recently purchased another 100 cuffs to be distributed. • The We Care Program continues to provide needed medical care to indigent patients. Please contact Dr. Lanza if you wish to participate. • The Go Senior Voucher Program provides transportation vouchers for patients’ doctor visits. Contact Erica if you are unclear how to enable this for your patients. • The FSU Medical Student Scholarship Program, and The Pensacola State College Endowment continue to be strengthened through the Foundation. • The 1873 Society has gained additional members since our last newsletter. He completed his internship at Oakland Naval Hospital in Oakland, California and his residency at the University of California in San Francisco. BHC Recognized for Orthopedic Program Baptist Health Care has been named to the Becker’s Healthcare 100 hospitals and health systems with great orthopedic programs list for 2018. Hospitals featured on the list earned recognition for quality of care and patient satisfaction for orthopedic and spine surgery. Andrews Institute for Orthopaedics & Sports Medicine is an affiliate of Baptist Health Care, and is the area’s only orthopaedic program on the list and one of three orthopaedic programs in the state of Florida.
Sacred Heart Health Care SHMG Welcomes Sidney Mallenbaum Neurologist Dr. Sidney Mallenbaum, MD, has joined Sacred Heart Medical Group’s regional physician network. Dr. Mallenbaum practices at Sacred Heart Hospital Pensacola. He joins the office of neurologists Dr. Terry Neill, Dr. George Dmytrenko and Dr. Lance King. Dr. Mallenbaum has more than 30 years of experience practicing neurology. He is triple board-certified in neurology, electrodiagnostic medicine and neuroimaging. He comes to Sacred Heart from serving as medical director of the Primary Stroke Center at Carilion Medical Center and an associate professor of neurology at Virginia Tech Carilion School of Medicine. Dr. Mallenbaum was also a graduate of the Carilion Clinic Physician Leadership Academy. Aaron Wallender joins SHMG Aaron Wallender, MD, DDS, a pediatric craniofacial surgeon, has joined Sacred Heart Medical Group. Dr. Wallender performs surgeries at the Studer Family Children’s Hospital at Sacred Heart and sees patients at his office on the Sacred Heart Hospital Pensacola campus. Dr. Wallender is the only pediatric craniofacial surgeon in West Florida and South Alabama. He specializes in managing cleft and craniofacial disorders in children. Neurosurgeon Theodore Spinks joins SHMG Pediatric neurosurgeon Dr. Theodore Spinks has joined Sacred Heart Medical Group. He joins the office of pediatric neurosurgeon Dr. Matthew Pearson. Dr. Spinks was an assistant professor at the Texas A&M Health Science Center. He’s also served on the clinical faculty of Texas A&M University College of Medicine and the University of Pittsburgh School of Medicine. He received his medical degree from Columbia University in New York, completed his residency training in neurological surgery at the University of California in Los Angeles, and performed a fellowship in pediatric neurosurgery at Children’s Hospital of Pittsburgh. Sonia Smith, DNP, joins SHMG Nurse practitioner Sonia Smith, DNP, has joined SHMG’s pediatric pulmonology and sleep medicine team. Sonia practices with pediatric pulmonologist and sleep-medicine specialist Dr. Gulnur Com. They focus on treating lung problems and sleep conditions in children, including asthma, cystic fibrosis, prematurity-related lung disease, sleep apnea, sleep-maintenance problems and narcolepsy. She received a bachelor’s degree in Nursing from the University of North Carolina at Greensboro, a Master of Nursing in Child Health from Emory University in Atlanta and a Doctor of Nursing Practice from Troy University in Alabama.
