SUBJECT: DoD Vehicle Decals, Joint Base Pearl Harbor Hickman Access.
Joint Base Pearl Harbor Hickman (JBPHH) has agreed to issue DoD vehicle Decals to Soldiers who require frequent access to JBPHH. Approval of request will be based on required or desired usage. Personnel requesting a decal will be vetted by DES who will then issue a Memorandum to JBPHH Access Control Office. Soldiers can obtain the required documents at building 79, Schofield Barracks, near Foote gate, 655-8405, Mon- Fri 0800-1600 or by e-mail at jose.m.javier2.civ@mail.mil to obtain the electronic request document. Completed request document will need to be turned in CPL Javier at building 79, Schofield Barracks. Applicants will also need to provide current driver’s license, vehicle registration, proof of safety inspection and valid insurance. Once the request has been approved and signed, the approved memorandum can be picked up at building 79 or can be e-mailed to the applicant. POC is CPL Javier, 655-8405 NO ACKNOWLEDGEMENT REQUIRED
