How to Obtain a U.S. Passport Cheat Sheet You must apply in person to obtain a U.S. Passport if:
You are applying for your first U.S. Passport You are under the age of 16 Your previous passport was issued to you when you were under age 16 Your previous passport was lost, stolen, or damaged Your previous passport was issued more than 15 years ago Your name has changed since your passport was issued to you and you were unable to legally document your name change
Step 1: Complete and Submit Form DS-11: Application for a U.S. Passport You may obtain this application at most U.S. Post Offices or online at www.travel.state.gov or www.usps.com/passport. Step 2: Obtain 2 Passport photos In order to be accepted the two photos must be identical, in color, 2 x 2 inches in size, have been taken in the last 6 months showing current appearance, be forward-facing view showing your full face, have a plain white or off-white background, and have between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head. You should take the photo in normal street attire. You should not wear a uniform, a hat or headgear and dark glasses or tinted non-prescription glasses unless you need them for medical reasons. If you normally wear prescription glasses, a hearing device, a wig or similar articles you should wear them in your photo.
In Hawaii you can obtain passport photos at FedEx Office locations, Long’s Drug Stores, Walmart locations, and AAA Offices. It is always best to call first to verify if a particular location offers passport services before traveling to that location.
Step 3: Report in Person to a Passport Acceptance Facility To find a post office that will accept passport applications visit www.usps.com/passport and click the locator link. The Schofield Barracks Post Office does NOT accept pass-
port applications, however the Wahiawa and Mililani Branches do accept them. You can only schedule an appointment at the regional passport office if you need your passport in less than 2 weeks for international travel or if you need your passport within 4 weeks to obtain a foreign visa.
Step 4: Submit Completed Form and Additional Documents in Person, Present Identification, and Provide a copy of your Identification Being Presented Additional requirements include: certified birth certificate issued by the city, county, or state Consular Report of Birth Abroad or Certification of Birth Naturalization Certificate Certificate of Citizenship
Acceptable forms of Identification are: Previously issued, undamaged U.S. passport Naturalization Certificate Valid Drivers License Current Government ID (City, State, or Federal) Current Military ID The photocopy MUST be of the ID Document that you will present when you apply:
Photocopy must be on plain white, 8 1/2 x 11" standard paper stock, showing the front and the back of your ID Photocopy must contain images on only one-side of each page submitted. - If copies cannot be made on the same side of one page, you may present two separate pages; one displaying the front of the ID and the second displaying the back of the ID. The paper should be free of other images and/or markings. The 8 1/2 x 11" paper size cannot be substituted with a larger or smaller size paper, even if the alternative folds down to the 8 1/2 x 11" size. You may enlarge the image of your ID on the 8 1/2 x 11" page, but you may not decrease the size of the image.

Passport Processing Times: Once your application has been accepted by the Acceptance Facility‌ Routine Service is taking about 4-6 weeks Expedited Service is taking about 2-3 weeks (if paying for overnight delivery)
Passport Status Check: You can check the status of your application about 5-7 days after you submitted your application by visiting www.travel.state.gov/passport/status You will need your Last name, date of birth, and the last four digits of your Social Security Number to access the system.
