Echosens, a French SME, has introduced in 2003 the first product to respond to expressed, unfulfilled needs of Hepatologists and paved the way to a new measurement in hepatology: the determination of liver stiffness thanks to the FibroScan® FibroScan® is a totally non invasive medical device dedicated to hepatologists. It uses the Echosens patented technology Vibration Controlled Transient Elastography (VCTEtm) at 50Hz to diagnose and monitor liver diseases. FibroScan® allows exploring a volume at least 100 times larger than a liver biopsy in a rapid, safe, repeatable, and non-operator- dependent way. It quantifies liver stiffness (E) in kPa and ultrasound attenuation (CAP) in dB/m, which are effective markers of the quality of the hepatic parenchyma correlated to fibrosis and steatosis, respectively. Technology! Medical Technology over the past ten years has transformed the healthcare sector by several cutting edge solutions in the areas of molecular diagnostics, molecular imaging, minimally invasive therapies, implantable medical devices etc. The size of the Indian medical technology industry may touch US$ 14 billion by 2020 on account of strong economic growth, higher public spending and private investments in healthcare, increased penetration of health insurance and emergence of new models of healthcare delivery. The global trends display promising numbers, as the global healthcare IT market is estimated to reach US $53.8 billion by 2014, growing at a CAGR of 16.1 percent. It is expected that the market for general applications in health IT will grow at an overall CAGR of 13 percent from 2009 to 2014 in India. ealth IT has now revamped the healthcare system providing strength of character to the healthcare infrastructure. IT is gradually picking up pace to integrate the economy from the forefront. Escalating demand for services and innovative solutions by the healthcare industry has pumped the IT sector to bring pioneering resolution to bridge the gap that lack of medical devices and software creates. According to a report by markets and markets in August 2011, the world healthcare IT market is expected to grow from US$ 99.6 billion in 2010 to US$ 162.2 billion in 2015, at a CAGR of 10.2 percent.

While US$ 4.5 trillion is spent on global healthcare annually, over 100 million people fall into poverty each year due to expenses incurred upon healthcare and approximately two billion people worldwide live without even basic healthcare. With greying populations, rising incidence of chronic diseases, and growing demand for tools tailored to local healthcare, markets healthcare lies in one of the world’s most pressing and complex problems. provide effective treatment. Acquisition in the healthcare and life-sciences space for IT service providers seems to be quite proactive in the current market. Today, technology is a key enabler for every industry. From imaging to tele-diagnosis, technology has proven useful in reaching the unreached masses as well. Health being the primary concern for an individual, needs to be closely looked at by providing affordable healthcare with the use of latest technology. Companies like Wipro-another IT major – are scouting for a niche player in the Health IT segment. Healthcare trends have changed from diagnosing patients in clinic to advising patients through the use of mHealth and telemedicine. Health IT has demonstrably come a long way since its inception. Electronic tools are becoming common in following five areas- treatment, exchanging clinical data with other physicians, notes on patient care, generating reminders, and writing prescriptions. With the use of tools such as EHR, EMR, e-Prescriptions and eReminders, exchange of clinical data has become much more efficient and streamlined. Uses of Health IT • Reducing paperwork by eliminating the need for handwritten medical records • Reducing medical errors by transmitting accurate information electronically and eliminating mistakes due to misreading of the doctor’s handwriting • Reducing health care costs by decreasing the need for repeat medical tests by different doctors and eliminating storage space and staff time to maintain medical records • Improving the quality of care by minimising medical errors and assuring that all health care providers have accurate and timely information
The gadgets also maintain the database of medical information. At the time of emergency the information is transferred to the effective treatment. MedicAlert has more than 4 million members worldwide and is available in 140 languages. Customers can access the medical records on their mobile phones as well. More than 40 percent of the company’s online traffic takes place after business hours. Using such tools, physicians can edit their patient’s records and spend more time in viewing patients’ records to ensure there are no errors and update the patient for any risks. A typical EHR has information about an individual’s health conditions, allergies, treatments, tests, and medications. Many EHRs can connect with health care providers outside the doctor’s office such as specialists, labs, imaging facilities (X-rays, CT Scans, MRIs), and the local hospital. Some EHRs have warning systems built in to let the doctor know about drug allergies or potential problems with drug interactions. Also, some EHRs have medical alerts to remind the doctor to perform certain tests or procedures. Electronic prescribing or e-prescribing (eRx) is a way for the doctor and other health care providers to send the prescriptions to the pharmacy electronically. This reduces chances of error due to incorrect interpretation of a written prescription, and also helps in avoiding harmful drug interactions. According to a recent survey published in the Business Wire, 75 percent of survey respondents claimed meaningful use of EMR is to be their top budget priority for 2011 IT investments, with 53.5 percent citing physician adoption of EMRs to be their greatest challenge in meeting that goal. 87 percent of respondents said time to access an EMR impacts physician satisfaction and 73 percent said that it also impacts

overall patient care. The survey found that adoption of Electronic Health Record System, which covers 8.7 million beneficiaries, eliminated 1,000 tons of paper records and 68 tons of x-ray film. Adoption of the system also lowered gasoline consumption among patients who otherwise would have made trips to the doctor by at least three million gallons per year. This shows that electronic health records have a positive net effect on the environment, and that our model for evaluating their impact can be used to determine whether their use can improve communities’ health. Online portals are gradually picking up pace, and hold potential for promising growth in the next five years, particularly in the areas of telephone counseling, self-help and web-based support services. Adoption of online services can help overcome distance, as well as financial and attitudinal barriers faced by the developing countries. For example, the I-med network developed by Queensland software helps doctors to log in and see the patient’s radiology images regardless of where they happen to be. Remote-I, an initiative of CSIRO health scientists uses broadband network to treat eye disease in the remote Pilbara region of Western Australia. It uses static imagery, store-and-forward, and video conferencing (although not all at once) to take high-resolution images – some of them in 3D – of the retina and then sends them to specialists in urban centres for analysis. The information is then transferred to the computer for analysis. After this, treatment guidelines are transmitted back to clinic staff at the remote location. The system uses static imagery for low-bandwidth situations and, video conferencing imagery for high bandwidth use. It is based around the HL7 industry standard, ensuring interoperability with a wide range of hardware and software solutions. Over the years, tele-health has linked physicians to each other, fostering better collaboration and problem-solving, rather than linking patients to the physicians. When reimbursement for telemedicine services is slow, the effort to gain physician’s role may be better served by focusing on the improvement in clinical outcomes through access to resources. There are patient rooms dedicated to create a virtual “exam room” linking real-time audio and video communications with mobile devices. P&G Future Works, the MNC’s entrepreneurial engine recently funded Healthpoint Services – a startup that runs rural clinics to provide affordable primary healthcare in developing countries. It provides rural communities with access to four basic services: clean drinking water, access to qualified doctors via telemedicine supported by two-way video and electronic medical records; advanced on-site diagnostic capa-

bility, enabling evidence-based healthcare; and highquality medicines via an onsite licensed pharmacy. Healthpoint offers 22,000 medical consultations and serves over 50,000 people daily with drinking water. This may help in overcoming infrastructural challenges at the PHCs level in the developing countries. As hospitals automate their manual, paper-based processes, IT leaders are rethinking their server and desktop infrastructure strategies. The consumerisation of IT has CIOs scrambling to deal with demands for iPads, iPhones and other personal devices brought into the hospital by a technology-comfortable physician. The industry has shown an unprecedented response from the Health IT providers to the existing users that were apparently resisting the change from paper to electronic, who now want to entirely convert everything on the latest and smart devices. Medical imaging has certainly been a driving case in improvements of graphics rendering for the virtualization vendors. USB device support has improved, with a number of successful deployments that justify desktop virtualization as a viable solution. The benefits of PACS are many, including the ability to view imagesfrom multiple locations, accessibility to reports, collaborative diagnosis, quicker image delivery and reducing the incidence of lost films with advanced 3D post processing and easy-to-use technology. The Secure Reporting Sytem (SRS) provides a secure electronic “mailbox” for physicians to automatically receive medical transcription, lab results and other pertinent patient related reports. Access is restricted to the physician and their chosen delegates and the reports are only for those patients where the provider is listed on the case.
Challenges With increasing integration of IT in healthcare, privacy and security issues are acquiring more significance. A few companies have installed firewall and intrusion

detection software to protect the information. Since all health plans are now drifting towards transforming all information into electronic format, the risk of unauthorised access to medical records increases as records would be available to the patient, doctors as well as to all the healthcare providers. Over the past several years there have been a number of instances of security breaches in hospitals and doctors’ offices resulting in medical identify theft. Access to personal information may allow thieves to bill for medical services in the patient’s name. The challenge to the health IT fraternity is to create the next-generation telemedicine system that can provide a true multi-point communication platform, allowing everyone involved in the medical case to securely view the same data and medical records at the same time regardless of the communication device used. With the advent of cloud-based technologies, services and software applications, a dedicated enterprise network in a single location should be developed, enabling better medical collaboration and information sharing via secure real-time audio, video, and data links. Using clinical IT systems as the engine for quality improvement is a logical endeavour. Until about a decade ago, IT systems were intended to automate transactions in hospital departments. When the business became more enterprise-focused, the emphasis shifted to improving processes across departments. IT departments today may have accomplished a lot of good, but the traditional processes did not allow the department to get data capture in a way that can be aggregated and use it to really drive improvements in quality. With the advent of rampant research and development in the IT sector, many hospitals are buying practices, or looking for innovative ways to provide services that allow practices to improve patient care by applying the latest technologies, without having to worry about implementation and management.

Technology that Cares
India’s healthcare sector is in a dynamic phase of growth but technology must work towards affordable healthcare

ealthcare has emerged as one of the most progressive and largest service sectors in India with an expected GDP spend from 5.5 per cent in 2009 to 8 per cent by 2012. At present the sector is estimated to be around US$ 40 billion and will grow to US$ 78.6 billion by 2012. The Indian healthcare sector is expected to become a US$ 280 billion industry by 2020 with spending on health estimated to grow 14 at the rate of per cent annually.

The size of the Indian medical technology industry may increase from US $ 3 billion in 2010 to US$ 14 billion by 2020 on account of strong economic growth, higher public spending and private investments in healthcare, increased penetration of health insurance and emergence of new models of healthcare delivery. molecular diagnostics, molecular imaging, minimally invasive therapies, implantable medical devices etc., the sector has witnessed a metamorphosis. For example, the Rashtriya Swasthya Bima Yojana (RSBY) has transformed the lives of millions of people living below the poverty line by providing them cashless and paperless health insurance. This would not have been possible without the innovative use of technology that the scheme has made.
A proper regulatory mechanism will ensure incentives to manufacture high-tech devices, standardisation of equipments and fair pricing. It will also control inappropriate diffusion of technology. The only way to motivate indigenous manufacturing initiatives is through right infrastructure and conducive policies
In today’s age when diabetes, heart disease, cancer and obesity-induced diseases are becoming increasingly common among all age groups, the need of high-tech applications is undeniable. Sophisticated devices and diagnostics not only assure patients’ security, accurate results and better health outcomes, but also save time and resources by shortening hospital stays as patients recover faster. On the contrary, such high-end machinery introduced by medical observers is a reason for rising health care costs as most of them are imported and the cost of procuring and maintaining them is extremely high. There is thus a critical need to strike a balance between quality and affordability of healthcare.

Currently, about 65 per cent of the medical equipments are imported. This has thus become a key area for forging partnerships across borders. Factors such as engineering excellence, cost-effective labour, increasing emphasis on intellectual property rights and most importantly, a fast growing domestic market make India an ideal manufacturing base.
Innovation The pace of medical innovation is accelerating and technologies that were considered to be part of fiction till recently are already a reality today. The next decade and beyond will bring rapid changes in this space. Minimally invasive therapies designed to help patients heal faster, while at the same time they streamline surgical procedures and reduce hospitalisation expenses. We have medical devices which collect patient information, transmit them to doctors miles away and make necessary adjustments in the settings for better control of condition. Similarly, implantable devices combined with drug therapies that target chronic conditions at their point of origination and miniaturisation across many kinds of device technologies to provide advanced therapies are on the horizon. In the past, taking scans and making films out of them was a huge task. Transferring these images from one place to another was also difficult and required a lot of time and effort. Today, with development of infrastructure and technology, capturing, storing and transferring images has become much easier. The field of neuro science has seen significant progress in the last three decades. This has made diagnostics speedy, accurate and easy with the availability of technologies such as Multislice CT, MRI Scan, PET CT and SPECT. Improvements in therapeutic modalities include image guidance, neuro navigation and minimally-accessed surgery in brain. Today, Indian doctors are performing all types of neuro-surgical procedures and tests which were earlier available only in the West.. However, India is still heavily

dependent on foreign manufacturers for the import of 85 to 90 percent of items of day to day use such as guide wires, micro catheter and coils.
Diagnostics The fast growing diagnostic sector opens several avenues for partnership between Indian and International diagnostic companies. There is huge potential in the clinical research and trials market by combining the unique strengths of Indian and US companies, as these companies are the leader in pharmaceuticals, biologics research and development. India has 17 percent of the world’s population but poorest healthcare infrastructure among growing economies. Spending on healthcare accounts for only about 5 percent of the GDP. Also, the diagnostic services in India are highly fragmented and unorganised. There is uneven penetration of services with greater concentration in South West India and scarcity in the North-East. As the healthcare industry matures in terms of infrastructure and innovative technologies, the next goal of care delivery demands clinical excellence which can be achieved through through evidencebased medicine tools and benchmarks. Evidencebased medicine is already being promoted globally and has proved to be beneficial to all the stakeholders – clinical teams, hospital managements as well as patients. Evidence-based medicine tools such as clinical protocols and clinical pathways provide explicit and well-defined standards of care for the clinical teams and support multi-disciplinary care planning. From a management perspective, these tools reduce healthcare costs, reduce patient documentation, optimise management of resources and help continuous clinical audit. They improve clinical care by delivering superior outcomes, improved clinical effectiveness and patient satisfaction. ICT plays a crucial role in high quality healthcare. Government Initiatives The Department of Science and Technology has constituted a committee to develop a biomedical devices and technology vision through a national consultation mechanism. The Department has also tied up with Indian Council of Medical Research (ICMR) for

this purpose. The Department has drafted a Bill to establish a Biomedical Devices Regulatory Authority. With the objective of addressing the manpower requirements of this sector, the Government is working towards developing enough pull factors that would attract youth to the sector, through an inter-agency programme. This is being done with the help of the All India Institute of Medical Sciences. In order to make health-care affordable and accessible to rural India, we need a congenial environment for indigenous manufacturers that encourages them to produce cost-effective high-quality equipment and lessen our dependence on imports. The medical devices in India are still covered by the Indian Drug Cosmetics Act of 1940. A medical device is a totally different category and cannot be clubbed with pharmaceuticals because there is hardly any similarity between the two. For years now, the Indian government has been trying to formulate a Medical Device Act but there is little clarity on the time by which a new law might be in place.In the US, FDA approval is required for all diagnostic technologies. However, in India, there is still no system for approval of any authority for producing diagnostic kits. Also, lack of enough research and development in the country restricts technology innovations and most of the technologies used in diagnostic labs are imported from outside. This is the era of cutting-edge technology. Even today, a routine protocol for screening all newborns for inborn errors of metabolism does not exist. Another step that can protect and

promote the domestic industry against the onslaught of imported goods, and motivate domestic players is to give a price differential of say 15 to 20 percent on a blanket basis, as is done by countries such as Malaysia and Thailand. Such a step will encourage MNCs to establish manufacturing bases in India, and thus reduce our dependence on imports. A proper regulatory mechanism will encourage manufacture of high-tech devices. The only way to motivate indigenous manufacturing initiatives is through the right infrastructure and conducive policies. Similarly, the confluence of advances in computing technology (high performance PCs), communications (broadband) and solutions (video conferencing) have made telemedicine a reality. A shift in policy treating education and healthcare as core infrastructure and a 1-2 per cent shift in budgetary allocation can make wonders happen in the healthcare sector. Molecular imaging (MI) will assume an ever more important role in furthering our understanding of human disease and patient care in the future. Strategic planning for investment on MI modalities is a key to success. Future in radiology will see enhanced efforts towards greater use of molecular imaging to be able to detect lesions before they attain even pin-head size and which are yet only at cellular levels. By understanding the molecular basis of disease and developing methods to detect and treat changes in the body at the molecular level, physicians will be able to identify diseases in the earliest possible stages. This will by far remain the major focus of research and development in clinical medicine in the 21st century.
