The Alliance will prioritise efforts to enable older people live longer in their own homes. Gather and provide information on the availability of a range of „ageing-in-place‟ options for older people throughout the county, including a checklist of things to consider about your own home, and disseminate through appropriate channels. ACTION 4 GOAL: Ensure that future investments in housing supply or adaptation are „age-proofed‟ against agreed criteria. Consolidate existing guidelines and develop a housing Age Proofing tool, based on the evidenced based learning acquired from the above actions, and in particular from partners working within the EIP programme. Apply the tool to the assessment of future provision.

HEALTH Ageing in Place including Long Term Care. Assumption based on feedback from Older People—Older People need a relevant support service which can step up and step down to meet their needs at appropriate times. ACTION 1 GOAL: Improve the care-pathways and setting for „at risk‟ HSE , LFA, older people on the boundary of hospital and/or LTC admission to NGOs access integrated services to promote their independence. Maximise the potential of the Cottage Hospital „transition hub concept‟ to connect older people to appropriate, relevant and timely services so that they can continue to live in the place of their choice. Increase the use of ICT to support the quality of service between locations, and to improve peoples capacity to self-manage. ACTION 2 GOAL: Improve the awareness of, and options for, Health & people with dementia so that they can continue to live flourishing lives Active in the community. Ageing Group Develop a programme to support people with dementia, their families and carers to support ageing in place while linking to appropriate services in the public, private and third sector and extending the Cultaca role. ACTION 3 GOAL: Reduce the incidence of loneliness through Health & making connections for „at risk‟ older people using personal, Active technology (phone/ICT) and community supports. TRANSPORT Aging in Place—Getting & Keeping connected Assumption based on feedback from Older People—Older People remain connected to community, services and stay actively involved in all aspects of life for as long as possible. ACTION 1 GOAL: Develop a personalised transport programme in Louth at community level using a community planning based approach. Capture the learning from the Clogherhead pilot project and adopt and apply to other areas, recognising the scale up requirement of appropriate coordination and technology . ACTION 2 GOAL: Improve the experience of the passengers using, and connecting between public and community transport services.
