Appointment: To set an appointment or for answers for any questions, call 1-800866-7872 from USA (or 1-800-918-7575 form LATAM) any day from 8:00 am to 12 midnight, except holidays. Site of appointment: Demographic Registry, Plaza Las Américas Shopping Center, 525 Roosevelt Avenue, Hato Rey, San Juan, PR 00918 Laboratory Tests: You must provide medical certification that shows compliance with tests required for marriage in your place of residence. Be aware that this certification is only valid for ten days, so you must say I do within this ten day time period. Residency Requirements: You don’t need to be a resident, but you must present an affidavit stating that you are not a resident of Puerto Rico and that the sole purpose of your visit is to get married. Non-US citizens must also include that you will not be staying in Puerto Rico for more than what is allowed. Affidavits written in the United States must be accompanied by certification of your state’s county clerk. For affidavits written in Republics or Sovereign Countries, the Apostille seal of the Hague Treaty of 1961 is required. Identifications: Don’t forget to bring valid

photo identification, passport, state issued ID, visa or green card. Authentication of documents: An Internal Revenue Stamp of $20 gets your license authenticated. These are available in the Demographic Registry. Birth Certificate: In order to record your official name, please bring a copy of your birth certificate. Other Certificates: If you have been divorced or are a widower, please bring the corresponding certificates. Marriage Certificate: Once the proper document are submitted and revised for compliance by the registrar, the marriage certificate is issued and signed. It’s very important you bring it to the ceremony! A properly identified parent must accompany couples under 21 so they can then sign the marriage certificate in the presence of the minor(s). Certificate Delivery: For the last and final step, you must deliver the certificate, marriage license and all other attached documents to the Demographic Registry of the municipality where the ceremony took place no more than ten days after the marriage.
