newton’s news
I first heard about Sepsis when
my sister, who is a nurse, told me about a particularly upsetting experience that she had gone through when losing a patient to Sepsis. What amazed me then was how prevalent and lethal Sepsis is, yet what little awareness there was about the condition and not enough action was being taken. Sepsis is a condition that claims the lives of 31,000 people in the UK every year, – more than breast cancer, bowel cancer and prostate cancer combined. Health professionals, led by the UK Sepsis Trust, have identified simple timely interventions and procedures, labelled the ‘Sepsis Six’, as a standard of care for patients when delivered within one hour. Early Sepsis treatment saves lives and is cost effective, reducing hospital and expensive critical care bed days for patients. If effectively implemented across the country, these interventions could prevent 12,500 unnecessary deaths and save much of the £2 billion that Sepsis costs the NHS. Put simply, a focus on Sepsis could deliver results quickly and save the NHS money. This has been proven in other countries and is relatively simple to enact. I decided to try and do something about this and a few years ago set up the All Party Parliamentary Group for Sepsis, a group of MPs and peers working with the UK Sepsis Trust to improve Sepsis care. The group has a range of objectives, as follows: Inform and educate Parliamentarians about sepsis; promote and encourage actions that lead to early and reliable recognition and treatment of the condition in the community, in hospitals and at the interface; help increase public and professional awareness of the condition and facilitate the development of a national registry for sepsis. Following on from the report of the Parliamentary and Health Service Ombudsman titled ‘Time to act: severe sepsis: rapid diagnosis and treatment saves lives’ I held meetings with Ron Daniels, Consultant in Emergency Medicine and Chief Executive of the UK Sepsis Trust that led to the Secretary of State for Health taking up the Trust's recommendations and take action. The Government’s proposed measures included an audit of every GP surgery in England on their effectiveness at diagnosing the condition; the creation of new NICE clinical guidelines; the introduction of an incentive scheme for hospitals to improve training and awareness, the
launch of a new public awareness campaign and the development of a new diagnostic tool for GPs to recognise symptoms in children under five. The Secretary of State for Health Jeremy Hunt MP said he wanted to make tackling sepsis as important to the NHS as tackling the superbugs Clostridium difficile (C.diff) and meticillin-resistant staphylococcus aureusis (MRSA). “I want the NHS to become the safest healthcare system in the world,” he said. “There has already been good progress; we have virtually halved C.diff and MRSA infection rates in the last 4 years. “Sepsis is a devastating condition that kills more than 80 people in England every day. It’s time to apply the lessons we’ve already learnt on patient safety and reduce the number of lives that are needlessly lost to this silent killer.” The recently reported deaths of two babies from Penryn, deaths that could have avoided, are tragic. I was dismayed to learn of the details of their deaths and how their symptoms went undiagnosed by a series of heath professionals. Mr and Mrs Mead bravely insisted on a full inquiry into the death of their son that has been undertaken. Recently the findings of the inquiry were presented to Parliament and the Secretary of State made a statement in Parliament including a firm commitment that the recommendations of the inquiry will be fully implemented. I will be working hard to enable that lessons are learned from the death of Mr and Mrs Jermyn's and action taken. Clearly more training of NHS staff and the consistent implementation of guidelines as well as building awareness of the symptoms amongst the public are all essential. I will continuing to do everything I can to prevent more suffering and avoidable deaths from Sepsis. I encourage all readers of this article to visit the Sepsis Trust website to learn more about Sepsis.

tMB, PhD, MRCVS PAUL COCKERHAM MA, Ve ll animal and large animal veterinary h sma Has over 30 years’ experience in bot e eon at Rosemullion Veterinary Practic practice and is a senior veterinary surg
This is one of these canine diseases (and be reassured it doesn’t really affect cats) which is just what it says it is: it’s a bug – actually sometimes two bugs – which produce a cough in dogs, frequently when they’ve been in kennels. It’s most often caused by a bacteria called “Bordetella bronchiseptica”, but there’s often a virus involved as well, called “Parainfluenza-3”. Either by themselves, or together, these bugs invade the lining of a dog’s larynx (the voicebox) and windpipe, causing inflammation, so that when air passes over these sore areas it results in an irritating cough.
HOW IS KENNEL COUGH TRANSMITTED?
Kennel cough is a highly contagious disease. It goes from one dog to another much the same way as a human cold transmits in humans – after all, coughing is a great way of spreading bugs into the air! – or it can go via direct contact of one dog with another, like sniffing noses and suchlike. So, it’s often spread in areas where many dogs are confined together, such as boarding kennels or animal shelters. Other ‘at risk’ situations are dog groomers and dog shows. One infected dog can soon infect many others even if it is not showing any symptoms of kennel cough at the time. That’s because the incubation period – which is the time the dog first gets the infection to the time that signs start showing, is typically between 7 to 10 days.
KENNEL COUGH
Unfortunately, that means that you can have an infected dog in kennels, at a show, in the groomers, without anyone knowing it, and so the bugs spread from one dog to another with nobody any the wiser.
WHAT ARE THE SYMPTOMS OF KENNEL COUGH?
Kennel Cough starts off as a very dry, hacking, coarse cough, with urging and gagging. It often sounds like your dog has got something caught in the back of his throat and he is trying to cough it up and many owners mistakenly think that their dog has a bone caught in his throat. The coughing can be quite violent, so they may also cough up white frothy material almost like a mild vomiting. All that said, they’re usually quite well (apart from the cough) with a normal temperature and they’re very keen for their daily routine – walks, running around and all the rest of it – and they very rarely stop eating! Labradors never stop eating, obviously, but even other breeds less dedicated to their guts don’t seem too badly affected.
What’s always intrigued me is that the cough starts incredibly suddenly – middle of the night, halfway through the morning, whenever – and starts off as it means to go on. There’s no gradual build up to it like with other diseases, but it’s full on right from the start.
The coughing can get worse on exercise and often goes on and on, day and night, which can become very distressing not only for your pet but also for you!
WHAT’S TO DO?
If the cough is mild and your pet seemingly otherwise fine, then probably you don’t need to do anything immediately, but you can just see how the cough goes on. Uncomplicated cases of Kennel Cough often resolve without any treatment within 7-14 days. But it helps to remove their collar and use a harness if you can, so there’s no pressure on the throat if they pull on the lead. Soft food usually goes down well, too. However, if the coughing is getting worse or your pet is struggling, then go and see your vet. They will probably prescribe an antiinflammatory drug, as a cough suppressant, and may advise a course of antibiotics if the signs are severe. If left untreated sometimes these cases deteriorate into bronchopneumonia, and that’s no joke. Honey is also quite a good cough suppressant – particularly last thing at night, and most dogs love it!
SHOULD WE VACCINATE?
There are two vaccines, one for each of the two bugs. The Parainfluenza virus is incorporated
into the booster vaccination, but the Bordetella one is separate, and is given down the nose. Now this isn’t always the easiest of jobs, particularly with dogs who are old hands at the game as they think it’s the canine equivalent of waterboarding! The reason for giving it down the
nose is that the vaccine produces an immunity in lining of the dog’s nose, so it kicks the bug out before it’s got a chance to start invading. So, if your dog is going into kennels, or mixes regularly with other dogs – such a dog-walking group, or goes to training / obedience / agility classes, or is a working dog – then yes, vaccinate, vaccinate, vaccinate! Especially in winter, as just like human colds, these bugs survive much better in wet weather. It can be given at the same time as your dog’s usual ‘booster’ too!

Well we certainly had a lot of weather in February!! Hopefully the weather for March will be a lot better and we can all get back on the garden. March is always an exciting time of year with the new gardening season arriving. At the nursery we are just starting our third year and we have more stock than ever before. We are constantly building our stock levels and hopefully this year create a retail shop.

FROM WHEEL ALIGNMENT AND BALANCING TO SERVICING AND MOT’S, XPRESS GARAGE & TYRES OFFER A QUALITY SERVICE AT HIGHLY COMPETITIVE RATES.
ROADSIDE REPAIRS This demonstrates the benefit of a workshop that’s manned 24/7, necessary to handle out-of-hours repairs and to service supermarket vehicles often operating until late at night. It also, as Peter points out, follows the principle of everyone pitching in: “Our recovery drivers are also motor technicians so they can carry out roadside repairs if necessary. But we can recover vehicles to the workshop and turn them round in the evening.” The company provides several specialist service that include full diagnostics using equipment such as Jaltest on anything from a car to a 44-tonne truck. It also has a machine to skim and balance discs on high performance cars in particular and a 3D Hunter wheel alignment system, the only one in Cornwall, that is recommended by every motor manufacturer and uses the same hawkeye technology employed in cricket and tennis. The extensive capabilities are backed by a range of accreditations that include membership of the RMI, the Trust My Garage scheme and the AA Approved Select scheme as well as being an approved RAC Dedicated Service Centre, having ISO 9001 for quality and being only the second garage in Cornwall to be issued with the BSI kitemark. Despite the diversification, Xpress remains a specialist tyre centre. “We have four staff specialising in purely tyres who have ten to 28 years’ experience fitting tyres,” remarks Peter. “Customers get the best tyre knowledge in Cornwall and fitting is to an exceptionally high standard.” RECOVERY PLANNING A recent recovery customer would have benefited from that service, having come off the road at a roundabout due to his tyres not being bedded in properly. That required recovery from down a 25 foot embankment and, as Peter explains, showed the
need for careful planning: “Some incidents, especially rollovers and offroad, require much skill and care to recover a vehicle safely while limiting damage to the environment and the vehicle we’re trying to recover.” Most planning now is taken up with the new fleet and commercial workshop, which at 5,000 square feet will allow the servicing and repair of vehicles up to 44
tonnes. “It opens up a lot more doors to the haulage industry due to having the workshop to deal with that size of vehicle,” says Peter. “We’re getting all staff trained on hybrids and other specialised vehicles we’ll be involved with and are looking to expand our MOT work to deal with vehicles that have to go to VoSA testing stations.”
