About Us Healthwatch Oxfordshire Your voice on health and social care Healthwatch Oxfordshire is the independent watchdog and champion for people who use health and social care services in Oxfordshire.

Our vision is that people are actively involved in shaping the health and social care services they use in Oxfordshire.

We do this by: Gathering information about people’s experiences of using health and social care services in Oxfordshire and make it available to the public.

Using this information to make recommendations to relevant local and national organisations about how local services, policies and strategies need to change and improve and, where possible, secure their agreement to make improvements.

Helping local people hold those in charge of local health and social care services to account for improving services – and do this in a way that is visible to the public.

About Us How we gather information about your experience of health and social care services Healthwatch Oxfordshire finds out about local people’s need for, and experiences of, health and social care services in many ways.

•

We support other local organisations to publish reports that enable people who would otherwise find it really hard to get their views on services heard, to have a say.

For example last year we helped published reports that set out the views of deaf people, Asian women and young people using mental health services.

•

We talk to individual members of the public through an extensive programme of outreach events in local supermarkets and markets, and by attending community events and meetings of local groups – and we pass their feedback on to the hospitals, doctors and social care providers responsible.

•

We hold special listening events for groups of people with a particular interest in one service – for example we hold workshops every year for people who want to talk about their experiences as users of adult social care services and the County Council then makes improvements on the things they hear there.

•

We take up issues brought to us by local people. This year we have lobbied on the failure to meet some 62 day treatment time targets for cancer, the effectiveness of the healthcare being provided in our immigration detention centre and plans to change the way services are provided to people with learning disabilities.

•

We undertake large scale survey based projects on issues that you tell us are sometimes a problem – like being discharged from hospital and being treated with dignity.

•

We use our powers of Enter and View – this allows us to send volunteers into the places health and social care services are being delivered to talk to patients and to observe the quality of care.

Last year we heard directly about the experiences of approximately 4,500 people through our outreach, projects, events and information giving work. 1000 people have joined our mailing list and are in receipt of regular information from us. 3,614 unique visitors have made 5,500 hits on our website.

Some examples of what we heard through partnership projects included: e Sign Lingual UK – Access to Healthcare Services for Deaf People. This is being used to inform the new contract for British Sign Language interpreting services in Oxfordshire.

My Life My Choice – research into GP provision for people with Learning Disabilities. This has led the Clinical Commissioning Group to work actively with this group of service users to help improve take up of GP health checks.

Local Experience of National Concern. This report has been used by the Department of Health to inform development of an effective peer support service for families of young people with severe mental health needs, challenging behaviours, autism and learning disabilities.

Community Glue – Personal Budgets: Where next in Oxfordshire.

Oxfordshire County Council has committed to making it possible for informal groups to make care plans and to access information and advice together.

A report by university students. This led directly to the provision of paramedic care in the city centre on weekend evenings.

About Us Our own major projects In 2014/15 we published a major report into the problems local people experience accessing GP services in Oxfordshire.

Improving access to GPs What we did

We found that:

In 2014/15 we have focused on access to local GP services. We gathered the views of more than 1,500 people by.

• One third of those surveyed were unhappy with their wait for an appointment. “A three to four-week wait to see the GP you are registered with is completely unacceptable.”

• Working with the Patients Association to survey more than 800 local patients.

•

Funding organisations such as the Asian Women’s Group, My Life My Choice, and Sign Lingual, to explore the barriers facing Asian women, people with learning difficulties and local deaf people when they try to see a GP.

•

Holding a conference which was attended by more than 60 people from local voluntary organisations, and asking them to answer the questions which Care Quality Commission inspectors were going to be asking about local GP practices.

•

Talking to hundreds of local people in their communities and asking them to share their views on local GP provision.

• Two thirds of the people surveyed were happy with the service. “There is an entire culture of helpfulness at my practice….staff are excellent…my own GP is superb.”

•

Information-sharing between GPs and hospitals needs to be improved.

•

Dignity in care standards sometimes slip in reception and in consultations. “It seems I am putting someone out by asking to see a doctor.”

•

The public understand they have a responsibility to make sensible use of their GP, and think there is a need for a public education campaign on how to make more responsible use of NHS services.

How we used the information We shared the findings with the Care Quality Commission and they are using the information and the direct experiences of seldom heard groups to train their inspectors.

We presented our findings to the local Primary Care Board which brings together GPs, the Clinical Commissioning Group and NHS England.

They are developing a local “use your NHS wisely” campaign with our support and they used the data to help raise £5m from the Prime Minister’s Challenge Fund for pilot projects to test ways to improve things, such as: Setting up hubs where patients from groups of practices who need same day appointments can be seen, freeing up GPs in each practice to have longer appointments with other patients.

Offering email consultations to free up appointment slots.

Setting up a home visiting service that can respond early in the day to requests for home visits, making more appointments available for those who can get to the surgery.

Employing extra staff who can spend more time with patients who need help accessing care.

Recommendations Using information to get services changed All Healthwatch Oxfordshire activity results in reports or letters which set out clear recommendations about how local services ought to be improved, based on what local people tell us.

Each report is promoted extensively in the local media and sent to the leaders of the organisations who can do something about the issues raised, with a request that they tell us what action they are going to take.

Most of our recommendations are made to one or more of the following organisations: •

Oxfordshire Clinical Commissioning Group – commissioner of hospital and community services and

co-commissioner of General Practitioners’ services.

•

Oxfordshire County Council - commissioner of adult and children’s social care.

•

Oxford University Hospitals Foundation Trust – provider of acute hospital services.

•

Oxford Health Foundation Trust – provider of community and mental health services.

•

NHS England – commissioner of specialist and primary care services.

•

South Central Ambulance Services – provider of the 999 and emergency ambulance services, the NHS 111 service for non-emergency calls and patient transport services.

•

Southern Health Foundation Trust – provider of Learning Disability Services.

At the end of every year we write back to each organisation asking them to update us on their progress with making the changes they promised. We then compile a detailed report that is presented to our local Health Overview and Scrutiny Committee, to our local Health and Wellbeing Board and to the Press.

In 2014/15 this approach led to over 40 changes to local services, policy or staff training – all of which were based on feedback from local people, and changes were made by all these major health and social care organisations.

•

Oxfordshire County Council committed to: publishing performance data of individual care agencies showing the number of missed visits and late visits; is developing customer standards for care homes, personal assistants, supported living, social workers and occupational therapists; has committed to incorporating dementia awareness into their training and is changing the way it gives information and advice to make it easier to understand.

• Oxfordshire Clinical Commissioning Group raised £5m from the Prime Minister’s Challenge Fund for a range of projects to improve access to GP services. The projects will address the concerns patients told us matter to them, such as longer appointments for complex problems and making it easier to get an urgent appointment for a minor ailment.

•

Oxford Health Foundation Trust is putting mental health services in all secondary schools.

•

Southern Health Foundation Trust offered much better intensive support to people in the community, reducing the need for people to be admitted to hospital.

•

Oxford University Hospitals Foundation Trust is revising its staff training on using translators and interpreters.

• South Central Ambulance Service has put new paramedic services into the city centre on weekend evenings.

•

NHS England has committed to revising and re-issuing its guidance on using interpreters and translators to all primary care providers in Thames Valley

Advocacy Putting local people at the heart of improving services Healthwatch Oxfordshire supports the involvement of local people in the commissioning, provision and management of local health and social care services by: • Helping to recruit local people to sit on formal committees where decisions get made - like the Oxfordshire Children’s Trust Board and the local Health Improvement Board • Widely publicising all local consultations on service change.

• Recruiting and training volunteers to undertake Enter and View activity.

• Supporting and advising groups and individuals who want to comment directly to commissioners and providers • Formally raising concerns and questions with commissioners and providers on behalf of the local population

Providing information One of our core functions is to provide information to the public about the services available to them.

The Healthwatch Oxfordshire team answers questions from the public every week about local services.

Last year we received information requests from more than 500 people. Requests for information cover a huge range of topics from how to make a complaint , how to access patient records, the number needed to get through to a particular service, where to get support for a particular issue or sometimes just explaining how the health and social care system works.

The organisations we most commonly signpost people too are Support Advocate Empower Promote (SEAP) – who help individuals make complaints to the NHS, the PALS (Patient Advice and Liaison) teams in each hospital and the Oxfordshire County Council comments and complaints team.

Volunteer

Healthwatch Oxfordshire needs you!

Healthwatch Volunteers are vital to our work.

Healthwatch Oxfordshire has a comprehensive strategy for recruiting, training, using and supporting volunteers, which is overseen by our Business Manager.

In the past year, we have recruited 25 local people, who have been trained to help us promote our work and to undertake structured interviews with patients and carers as part of our enter and view programme.

Volunteers have helped us to design questionnaires and to think about how we capture and analyse the data coming into the organisation from every source.

Volunteers also go out and about across the county with our staff to talk to the public about their experiences of receiving health and social care.

Each volunteer is asked to apply in writing, is interviewed, then undergoes a full Disclosure and Barring Service check. Volunteers are then invited to participate in a one-day Healthwatch Oxfordshire induction and training event, and for those who want to help us with our enter and view activity we offer a further one-day enter-andview training event.

For each enter and view project in which the volunteers are involved, we provide additional project-based training so that they have a full understanding of why we are carrying out the project and what we want to find out.

Before every enter-and-view visit, each volunteer is given a detailed pack with the details of where they are going, who they will be working with, who they will be

interviewing and when, which Healthwatch Manager will be on site and who the contact manager is in the site hosting the visit, as well as a copy of the questionnaire they will be using in the structured interviews they will be undertaking.

Every visit is followed by a debrief and every project is followed by a full evaluation involving the volunteers.

Once the data provided to us by the volunteers has been entered and analysed by our staff, and staff have drafted the project report, the volunteers will be asked to comment on how well it reflects what they heard and observed, and to suggest improvements.

Simple gadgets A range of gadgets exist to remind or enable you to do something, or alert a family member or carer living with you that you need help.

These gadgets are sometimes referred to as standalone Assistive Technology. Here are some examples of gadgets that Oxfordshire County Council can help to provide:

Good Neighbour Schemes If you want help with transport, odd jobs, gardening, general errands, or someone to chat with, a Good Neighbour Scheme is for you.

Oxfordshire County Council is encouraging Good Neighbour Schemes in Oxfordshire to form and develop. The Community Information Network works closely with Good Neighbour Schemes, and can help you to find one that will meet your needs.

Equipment, aids and gadgets Assistive Technology - devices to help you stay safe and independent Assistive Technology refers to devices that can be used to increase your independence and safety and help you with day to day tasks. It can give you, your family, carers or professionals the peace of mind that help is available when you need it and can help determine how you are managing at home if you have memory problems.

Assistive Technology can help you to: - do something, such as remember to have something to eat or drink, take medication or go to an appointment; - keep safe at home, through smoke, flood, or fall detectors; - request immediate help; - keep safe when you’re out and about; - reassure family, friends and carers that you are safe when they are not with you.

Assistive Technology ranges from very simple gadgets, such as medication aids (boxes with compartments for particular days of the week and times of day), to Telecare; devices that can automatically and remotely monitor emergencies and call for help.

Telecare Telecare is used to describe devices (for example, movement, flood, gas, smoke or fall detectors) that automatically send a signal when a problem is detected and calls for assistance.

Personal pendants are also available as part of Telecare; when pressed, a fully trained operator assesses the nature of the emergency and reacts accordingly.

Memory prompts - these are used to assist people with memory difficulties by providing daily memory prompts using personally recorded voice messages at specific times of the day to do specific activities, for example, take medicine or eat lunch.

Movement activated memory prompts - a movement activated message player allows a message to be recorded and played back when a person walks past the sensor.

E.g. “take your keys with you and lock the door” or “make sure you have turned the cooker off”.

Locating devices - location devices are designed for people who may be at risk of getting lost when they go out on their own and can help them to return home safely.

The can be used to locate a person when needed, allow the wearer to call for help and alert friends and family if a person goes outside of areas they know and feel safe in, has a fall or is uses transport. People can also have two way conversations using these devices.

Will I have to pay for my Assistive Technology?

If you arrange this through Oxfordshire County Council, they will undertake a financial assessment to work out what, if any, contributions to the cost of Telecare you need to make.

Gadgets (standalone assistive technology) are provided to you based on your need for the equipment and do not require a financial assessment.

You can arrange for an assessment either for yourself or for someone else:

If you have difficulty with day-to-day activities such as getting washed and dressed or getting yourself something to eat, you may be eligible for support from Oxfordshire County Council.

Being ‘eligible’ means that, because of illness or disability, you need support and care to manage some of the basics of everyday life, such as eating, using the toilet, maintaining relationships or keeping yourself safe at home. There are national criteria for the council to use to work out if you are eligible.

To find out if you are eligible you can have an assessment.

This is an opportunity to understand your situation, the needs you may have and your strengths and capabilities, and will involve talking to you, your partner, carer, relatives or friends as appropriate to consider the factors that affect your health, wellbeing and independence.

The assessment will include the outcomes that you want to achieve in your day-to-day life as well as consideration of any needs you may have for care and support. The assessment will consider how the provision of care and support could help you achieve what you want to and promote your wellbeing.

You have the right to have an assessment regardless of your ability to pay for the support you may need. The assessment may be over the telephone or face-to-face, and in the future you will also be able to choose to complete the assessment online.

If the assessment finds that you do have needs for care and support which are eligible to be met by the council, you will be helped to make a support plan. This will include the council telling you how much meeting your needs is likely to cost known as a personal budget.

Having a personal budget helps you to have control over your life, identify your needs and the things you want to do and, if you wish, to manage your own support. You can choose to have your personal budget paid to you as a direct payment, which means you can plan exactly how to use it to meet your needs.

Paying for Support and Care Unlike health care, social care is not free and Oxfordshire County Council will carry out a financial assessment to check whether you need to contribute to the cost of your support.

Depending on your financial circumstances you may have to pay for some or all of the cost of support and care you need.

Most people pay something towards their support.

You may not see yourself as a ‘carer’ - you see yourself as a son, daughter, partner, friend or neighbour. But if you regularly support and care for someone who could not manage without you then you are a carer and there is help available to support you to carry on with this vital work.

If you provide unpaid support to someone who could not manage without your support, the law says you have a right to have your own needs assessed, even if the person you care for does not have support and care arranged by the council, and even if their needs are not assessed as being ‘eligible’ This assessment, called a ‘Carer’s Assessment’, gives you the chance to say what you need as a carer, and to find out what support might be available to help you.

The assessment can give you information about services available, and other ideas for supporting you in your caring role. Your Carer’s Assessment will show if you qualify for support through the council.

From April 2015, many carers will be entitled to receive a support plan and personal budget in their own right, for services that enable them to continue in their caring role while still being able to pursue their own interests or lifestyle.

Carers will not be charged a contribution to services arranged through the council.

How does it work?

The NHS 111 service is staffed by a team of fully trained advisers, supported by experienced nurses and paramedics. They will ask you questions to assess your symptoms, then give you the healthcare advice you need or direct you straightaway to the local service that can help you best. That could be A&E, an out-of-hours doctor, an urgent care centre or a walk-in centre, a community nurse, an emergency dentist or a late-opening chemist.

Where possible, the NHS 111 team will book you an appointment or transfer you directly to the people you need to speak to.

If NHS 111 advisers think you need an ambulance, they will immediately arrange for one to be sent to you.

Calls to 111 are recorded. All calls and the records created are maintained securely, and will only be shared with others directly involved with your care.

Choosing the Right Care Home

Choice of Accommodation The law says that where the local authority is funding accommodation it must allow the person entering residential care to choose which care home they would prefer. Social services must first agree that the home is suitable for the person’s needs and that it would not cost more than they would normally pay for a home that would meet those needs. If the person chooses to go into a more expensive home, a relative or friend may be able to ‘top up’ the difference in cost.

Choosing a Care Home Care homes may be arranged through the local authority but many people will want to arrange them independently. It is a good idea to visit several homes before making a choice. Make sure you spend enough time in each home to get a good idea of what it is like.

Respite and Short Term Stays Residential care home stays don’t necessarily have to be permanent. Temporary stays can be arranged for respite care (in which you take a break from caring for somebody else), or as a trial period before a permanent stay.

Temporary stays can give you flexibility when covering unexpected events, such as: • Palliative care (which manages or reduces pain) after a hospital stay or illness • Support for newly disabled people and their carers • Enabling someone to continue living independently if they live alone and suddenly require care • Giving someone a chance to try potential future homes What your choices are A list of nursing and residential care homes within your locality can be found on the following pages.

Every care home in England must be registered with the national regulatory body Care Quality Commission (CQC).

They inspect each care home on a regular basis and write an inspection report for you to read. It is advisable to read this report before making a final decision.

Nursing & Residential Care What is the difference?

There are two main types of residential care homes: A care home will provide accommodation, meals and personal care. This includes help with washing, dressing, toileting and help getting up from bed and chairs. Nurses based within the community will carry out any nursing tasks.

A care home with nursing provides the same services but has nurses present at all times to supervise nursing tasks. Residents are usually more dependent than those in residential care.

Some homes are registered to provide both of these types of services and there are some specialist care homes for people who require extra support often due to dementia, mental health or learning disabilities.

How are care homes checked and inspected?

Every care home in the United Kingdom must be registered with the national regulatory body which is the Care Quality Commission (CQC).

They inspect each care home on a regular basis and write an inspection report. www.cqc.org.uk Choosing a Care home When you visit a care home it’s easy to be wowed by the glossy brochure, tasty sample menus and stimulating timetable of activities.

It’s harder to know whether they represent the real conditions in the home, and whether it’s right for you or your relative. Use these tips and questions to help you choose the right care home.

What Is Mental Health?

Mental health includes our emotional, psychological, and social well-being. It can affect the way you think, feel and behave. It also helps determine how we handle stress, relate to others, and make everyday choices. Mental health is important at every stage of our lives.

Mental health problems can affect the way you think, feel and behave. Some mental health problems are described using words that are in everyday use, for example ‘depression’ or ‘anxiety’.

This can make them seem easier to understand, but can also mean people underestimate how serious they can be.

Mental health problems range from the worries we all experience as part of everyday life to serious long-term conditions.

The majority of people who experience mental health problems can get better and many recover completely, especially if they get help early on.
