In addition to the nine Women’s Championships, there will be more that 150 statewide One-Day Events open to women! Throughout the Men’s and Women’s schedules, the FSGA will be hosting events at some of the top golf courses in the state. Your commitment helps us preserve and protect this great game for future generations. ABOUT THE FSGA
Since 1913, the Florida State Golf Association (FSGA) has been preserving and protecting the game of golf for future generations. We are here to serve Florida’s golfers of all ages and abilities. The FSGA is a volunteer-based organization that includes over 800 member clubs and over 200,000 individual members from across the state. The FSGA provides a variety of services to its member clubs, their members and the general golfing community of Florida. The association conducts more than 450 tournament days each year, including 34 state championships and over 50 days of USGA qualifying events.

While conducting so many tournament days, the FSGA offers a variety of benefits and services to all Florida amateur golfers. The FSGA organizes and conducts high-quality, affordable competitions for junior golfers in Florida. The tour continually emphasizes to the players the best values of the game including integrity, honesty and person responsibility. The junior golfers playing on the tour are some of the finest players in the country and the FJT serves as a proving ground for their future in the game. 2012 Women’s Amateur Championship Weston Hills Country Club – Tour Course Fort Lauderdale, FL May 7-11, 2012 The 83rd Women’s Amateur Championship will be played at Weston Hills Country Club, created by world-renowned designer, Robert Trent Jones, Jr. Weston Hills is home to two magnificent 18-hole golf courses and has been the proud host to the PGA Honda Classic (1992-1995), the Dan Marino Celebrity Invitational (1992-2001), the LPGA Chrysler Plymouth Tournament of Champions (1997) as well as various USGA qualifying events with the FSGA. Entries Close: April 18 Eligibility: Must be a female amateur member of the FSGA. And must be (1) a resident of Florida (reside in Florida at least 4 months per year, have otherwise established legal residency or graduated from a Florida high school and presently attending an out-of-state college); or (2) a military personnel or a dependant of a military personnel based in Florida on active duty. Handicap Index: No higher than 34 Championship Fee: $245 (Championship Flight, qualifier) and $220 (Field Flights, non-qualifier). Entry fee includes cocktail reception the evening of May 8th. Cash bar. Championship Format: 18 holes of individual stroke play qualifying on Monday to determine Championship flight. Field flights will be predetermined based on handicaps. The first round matches will begin on Tuesday morning, with the second round matches and consolation matches to begin Wednesday morning. Quarterfinal matches will be played on Thursday morning followed by the semi-finals . Finals will be played on Friday.

2012 Women’s Senior Amateur Championship Fiddlesticks Country Club – Long Mean Course Fort Myers, FL June 7-8, 2012 Fiddlesticks’ Long Mean course is a Ron Garl design and among the top ranked courses in Florida. In 2010, Fiddlesticks was chosen to host southwest Florida's first USGA championship, the US Senior Women's Amateur. The club has also been home to several USGA and FSGA qualifiers, and will continue to be in the future. Entries Close: May 16 Eligibility: Must be age 50 or older as of June 7, 2012, and be a female, amateur member of the FSGA. Handicap Index: No higher than 34 Championship Fee: $175 (Both Divisions). Entry fee includes lunch following play on the final round. Championship Format: 36 holes of individual stroke play. In the Championship Division, flights will be determined after the first round and it will be gross play only. In the Field Division, players will be pre-flighted based on their handicap index. The Championship Flight will compete for gross prizes only. Gross and net prizes will be awarded in each Field flight.

After the first round Lang was up by one shot over Mary Jane Hiestand of Naples. It was a tight finish in the final round with Lang shooting a 3-over par 75 to capture the championship by two shots over Hiestand. This was Lang’s third Women’s Senior Championship title.

2012 Girls’ Junior Championship Black Diamond Ranch – Ranch Course Lecanto, FL June 11-12, 2012 The Ranch was carefully crafted to gently blend into its natural surroundings. Reminiscent of Pine Valley, the Ranch Course sports rolling fairways and dramatic elevation changes of over 100 feet. The Ranch's 16th, 17th and 18th holes “represent the three best finishing holes I've ever designed,” according to Architect Tom Fazio. Entries Close: May 23 Eligibility: Open to female amateur golfers ages 6-18 as of June 11, 2012. Entrant must not have reached their 19th birthday by June 12, 2012 and not attending college if graduated from high school. Players must be (1) a resident of Florida; or (2) a dependant of a military personnel or a military personnel based in Florida on active duty. Handicap Index: No higher than 34 Championship Fee: Ages 6 to 12 - $60; Ages 13 to18 - $95. Entry fee includes lunch on both days. Championship Format: 36 holes of individual stroke play for Regular (16-18) and Junior (13-15) Divisions. Age divisions 10-12 and 6-9 will only play nine (9) holes of stroke play each day.

2012 Summer Mixed Team Championship Hobe Sound Golf Club Hobe Sound, FL June 30 – July 1, 2012 The 2012 Summer Mixed Team Championship will be played at Hobe Sound Golf Club, a Joe Lee signature course designed around preserved wetlands and massive oak hammocks. Established in 1988, the magnificent 6,664-yard course has hosted multiple FSGA Mid-Senior Four-Ball South Championships.
Entries Close: June 13 Eligibility: At least one entrant of each team must be a member of the FSGA. Handicap Index: No higher than 34 Championship Fee: $385 (includes cart for Championship rounds) Championship Format: 36 holes of four-ball stroke play

2012 Parent-Child Championship Walt Disney World Lake Buena Vista, FL July 28-29, 2012 Disney's Palm and Magnolia courses play host to the Children's Miracle Network Classic, a mainstay on the PGA Tour schedule for the past 30 years. Previous winners of the event include Tiger Woods, Vijay Singh, Lucas Glover, and David Duval. The FSGA’s Parent-Child Championship has grown to be one of the most successful events on the championship schedule. It is a great opportunity for families to enjoy the game while in a fun atmosphere.
Entries Close: July 11 Eligibility: Any parent and child of which at least one entrant of each team must be a member of the FSGA. There is no maximum age limit for parent or child. Professionals can play but are not eligible for trophies or prizes in the regular division but are fully eligible in the junior division. Grandparents may also play with grandchildren. Championship Fee: 18 holer - $280 ($300); 9 holer - $140 ($160) - members who do not have an active FSGA/GHIN Handicap do not receive the $20 discount. Championship Format: For the 18 holer divisions, the format of play is 36 holes of Modified Alternate Shot stroke play. The format of play for the Junior Division is Modified Alternate Shot using Stableford scoring each day. Both players hit tee shots from their respective tees. Both players then hit the second shot with the other's ball. After the second shot, the team chooses which ball it will use to complete play of the hole hitting the chosen ball alternately for the remainder of the hole.

2012 Women’s Stroke Play Championship Ritz-Carlton Golf Club Orlando, FL July 6-8, 2012 The Ritz-Carlton Golf Club, Orlando, Grande Lakes provides golfers with an experience not typically found in Florida. The design, by two-time British Open Champion Greg Norman, blends a variety of hazards to provide a challenging but fair test of a golfer's abilities.

2012 Women’s Four-Ball Stroke Play Championship Lake Jovita Golf Club – South Course Dade City, FL August 4-5, 2012 Lake Jovita’s South Course is interwoven through lakes, ponds, valleys, hills, and lush vegetation make it a premier golfing destination. The course has been home to many FSGA qualifiers and championships, including the Junior Match Play Championship and Florida Open and Senior qualifiers.
Entries Close: July 18 Eligibility: Entrants must both be amateur members of the FSGA Handicap Index: No higher than 34 Championship Fee: $375 Championship Format: 36 holes of four-ball stroke play. The lowest team score for 36 holes will be the Champion. If necessary, there will be a hole by hole play-off to determine the Champion.

2012 Women’s Four-Ball Match Play Championship Grand Harbor Golf & Beach Club – River Course Vero Beach, FL September 22-23, 2012 The River Course, created by Joe Lee, is a masterpiece of course design woven into the remarkable native landscape. Situated along the Indian River, the course is visually stunning as well as athletically challenging. The 14th hole has become Grand Harbor’s signature hole, garnering international praise.

2012 Fall Mixed Team Championship MetroWest Golf Club Orlando, FL October 6-7, 2012 Opened in 1987, this Robert Trent Jones Sr. signature design continues to reign as one of the finest public golf courses in the Orlando area. The course features spring-fed lakes, mature native trees, expansive fairways and large, undulating greens strategically surrounded by sculpted bunkers. Unlike most Florida golf courses, MetroWest features 100-foot elevation changes that gracefully roll among water hazards and expansive bunkers. Entries Close: September 19 Eligibility: At least one entrant of each team must be a member of the FSGA. Handicap Index: No higher than 34 Championship Fee: $385 Championship Format: : 18 holes of four-ball stroke play on Saturday, and 18 holes of modified alternate shot on Sunday.

2012 SWATCA Team Matches Black Diamond Ranch – Quarry Course Lecanto, FL October 22-24, 2012 The Quarry Course at Black Diamond is simply breathtaking. The 14th, 15th and 16th holes are carved over and around canyon walls and combine challenging shot making opportunities with more awesome views. Black Diamond’s Quarry Course was home of the FSGA’s 2011 Florida Open, as well as many other qualifiers and championships. Eligibility: Invitation only Championship Format: The first day consists of four-ball match play, the second day is a "Chapman's" foursome match play, and singles matches are played on the third day. One point is awarded for a match won, while a tie earns 1/2 point to each side. Each day's matches are determined the night before in a blind draw among the states and teams. Eight players from each team compete in a round from the Team Captain's selection. Teams: 8 members per team Defending Champion: South Carolina won with 12.5 points. Florida finished with a close second having 11 points.

Men & Women One-Day Events (140+): •Eligibility: Must be an amateur golfer with an FSGA/GHIN handicap (24.0 or less for men and 34.0 or less for women). First time players without an established GHIN handicap will be eligible to compete, but not for prizes. •Yardage: Approximately 5,000 yards •Format: 18 holes of individual net stroke play within the Women’s Flight. Players will play to 100% of their course handicap. Shotgun start.
Women Only One-Day Events (9): •Entry Fee: All women only One-Day events include lunch in the entry fee. •Eligibility: Must be a female amateur golfer with a GHIN handicap (34.0 or less for women). •Yardage: Approximately 5,000 yards •Format: 18 holes of individual stroke play or 18 holes of Four-Ball stroke play. Players must select which division they wish to compete in upon entry. Both divisions compete for Net and Gross prizes. The 1st & 2nd low net and gross winners of the event will move on to play in the Tournament of Champions. Shotgun start.

2011 PLAYERS-OF-THE-YEAR Amateur Player of the Year: Meghan Stasi Meghan, of Oakland Park, had a top 20 finish in the prestigious Harder Hall Championship, a top 10 finish at the South Atlantic Amateur Championship and made it to the quarterfinal round in the Ione Jones/Doherty Amateur Championship. She also tied for 2nd at the Women’s Stroke Play , made it to the quarterfinal round at the Women’s State Amateur Match Play, made it to the quarterfinal round at the North/South Women’s Championship and qualified for the USGA Women’s Amateur Championship, the USGA Women’s Mid-Amateur, and was a member of the Florida Team at the USGA Women’s Team Championship.
Senior Player of the Year: Mary Jane Hiestand Mary Jane, of Naples, won the prestigious Ione Jones/Doherty Senior Amateur Championship. She finished second in the Women’s Senior, had a top 10 finish in the Women’s Stroke Play, and made it to the round of 16 at the Women’s State Amateur Match Play. Mary Jane was selected to represent the State of Florida on both the FL/GA and SWATCA teams, and she qualified, and eventually made it to USGA Women’s Senior Amateur Championship.
Junior Player of the Year: Robbie Howard Robbi, of Bradenton, finished in the top 20 at the Dixie Amateur Championship, in the top 25 at the AJGA Taylor Made Junior Championship, tied for 9th at the AJGA Webb Simpson Junior Championship, had a 4th place finish at the AJGA Florida Junior, and tied for 7th at the ArmourBite Junior Open. At the Junior PGA Championship Qualifier, Robbi tied for 1st. She also finished in the top 20 at the Girls’ Junior Championship and made it to the Semifinals at the Women’s State Amateur Match Play Championship.

CALLING ALL VOLUNTEERS! The FSGA is proud to have one of the largest and finest volunteer based golf associations in the country. FSGA Committee Members act as tournament volunteers and/or course rating volunteers. There are many volunteers that participate in both and help us a great deal. And we would love you to be a part of what make us so successful! If you are interested in becoming an FSGA Committee Member, please complete the following steps: Step 1: •Contact Tracy at the FSGA office at (813) 868-5816 for additional information about becoming an FSGA Committee Member. She will gladly answer any questions you have! •Complete the Committee Information Form. •Once you have completed the Committee Member Form please return the completed form to the FSGA. Step 2: •Observe at a Tournament or Course Rating anywhere across the state. We make it easy for you to be involved! •Once we receive your Committee Member Form, we will help you coordinate a day to volunteer in your area with your desired interest. Step 3: •After these initial steps and you feel you want to continue volunteering with the FSGA, we ask that you join the FSGA as a member and support the state golf association. •Once you have completed all of the above steps, you will receive your official welcome letter, FSGA volunteer uniform and additional volunteer and training information. 2012 CHAMPIONSHIP POLICIES
Entries are subject to rejection at any time (including during the event) by the FSGA. The reason for rejection may include false statement of handicap index, improper attire and/or unbecoming conduct. Any person whose entry is accepted shall be subject to the Rules and Conditions of Competitions set forth by the FSGA. In all FSGA championships, an official event may be constituted by reverting to the results of an earlier round. Eighteen holes may constitute an official event. Eligibility In order to participate in an FSGA tournament, you must: a) be a female b) be an amateur golfer c) be an individual member of the FSGA for the current year d) have a valid USGA GHIN Handicap Index of 34.0 or less e) meet the specific eligibility requirements of the tournament involved Exceptions – Membership is not required for the Girls’ Junior Championship. Residency – Contestants in the Women’s Amateur and Junior Girls Championships must meet one of the residency conditions below: 1) a person who maintains her permanent legal place of abode, where physical presence and possessions are maintained, for a period of not less than four consecutive months prior to last day of the Championship; or 2) a student who physically attends a high school or college in Florida at any time during the four months prior to the last day of the Championship; or 3) a student who graduated from a Florida high school and who attends an out-of-state college but remains a legal dependent of a parent or guardian who meets the Florida residency requirement; or 4) a military personnel based in Florida on active duty, their spouse, and dependent children as of the date of close of entries. Entry Forms Entry forms for all Champions are available online at www.FSGA.org. There are no paper entries. Entry Priority In order to accommodate online entries, ALL COMPLETE ENTRIES for ALL EVENTS will be accepted based upon the order of receipt. Entries received after the competitive field is full or after the close of entries, will be placed on an alternates list. In the event a FSGA Championship field (non-qualifying required) receives a number of entries that exceeds maximum capacity, the Tournament Committee reserves the right to hold open one spot for the defending Champion provided the defending Champion submits a complete entry on time. Should the defending Champion not submit a complete timely valid entry, the spot will be filled by an alternate.

Entry Procedures All entries must be received by the Florida State Golf Association prior to 4:30 p.m. on the deadline date indicated. However, late entries will be accepted and placed on an alternate’s list to fill spots if they become available up until the time that the Tournament Committee establishes final pairings. Entries will not be accepted unless they are accompanied by the correct entry fee. Telephone and fax entries will not be accepted. Loss of Exemptions If a player turns professional, she loses any exemption(s) she previously earned as an amateur player with the FSGA. Tournament Information Information concerning practice rounds, course assignment, pairings, starting times, etc. will be mailed or emailed no later than one week prior to the event for those players opting for the paper or electronic option. Information will be posted at the FSGA website, www.fsga.org. If you have not received your information within five days of the event, it is YOUR responsibility to contract the FSGA Tournament Office. Do not call more than five days prior to the event. Handicap Verification All players must have a valid USGA GHIN Handicap Index of 34.0 or less, and will be ineligible if their handicap index cannot be verified. Withdrawals Players who have applied to participate in a FSGA event and wish to withdraw for any reason MUST withdraw through the FSGA website or notify the FSGA Office by email at championships@fsga.org or by telephone at (813) 632-3742 prior to the event. Players who fail to follow this procedure will be listed as a NO SHOW. No Shows A player who does not show up and does not notify the FSGA may, at the election of the Tournament Committee, have their entry rejected for any FSGA event for up to one calendar year. Refund Policy Requests for refunds received prior to the entry closing date will be honored (less $20 per entry administration fee). Requests received after the closing date will be honored (less 50% of the entry fee). No requests will be honored within 48 hours of the first scheduled tee time. Spikeless Policy It is a condition of the Florida State Golf Association that shoes with traditionally-designed spikes (regardless of composition, i.e., ceramic, plastic, etc.) or spikes, regardless of design, comprised either entirely or partially of metal (when such metal comes in contact with the surface of the putting green) are prohibited. Penalty for breach of this condition: Disqualification.

Cart Fees and Spectators The cost of cart fees for Championships is included in the entry fee. Only participants may ride in carts. Neither caddies nor spectators may ride in a cart regardless of the availability of seating. There will be a maximum of two carts per group and a maximum of two people in a cart at one time. Each player’s golf bag must be on the cart and players should ride. Exception - Junior Girls Championship: All players must walk and carry their own clubs. Pull carts are permitted if course policy allows. Use of a caddie is prohibited. Based upon availability, spectators may be authorized to rent carts, but must follow the published policy (available on site). Spectator Attire Proper attire for spectators is requested at all times at the golf facility. Attire Proper attire for competitors and caddies is required at all times at the golf facility. Failure to comply may result in withdrawal of entry. The dress code prohibits tank tops, T-shirts, denim shorts, jeans, and short shorts. Bottom of shorts must be approximately four inches above the knee (as a measuring rule, shorts must come below the finger tips when arms are held to the side). All hats must be worn properly. Collared shirts or shirts specifically designed for golf must be worn and tucked in at all times. Girls may wear shirts with sleeves and no collar, but not sleeveless and no collar. This code supplements any course or club guidelines, which shall take precedence. Publicity and Promotion; Broadcasting By participating in FSGA Events you grant the FSGA and those acting with the FSGA, permission, authority and the permanent rights to use your name or likeness for publicity and promotional purposes. Participants understand that all media, including film, photographic prints, audio or digital files are the exclusive property of the FSGA. Participants in FSGA events waive their rights to inspect or approve all media produced by the FSGA and those acting on the FSGA's behalf. Participants waive their permanent rights to compensation for use of their name and likeness by the FSGA. Flighting If a flighted round is not completed by the entire flight, the flighting will be ignored for the purposes of prize distribution. Official Time The official time will be maintained at the starting tee(s) only. Return Checks All checks returned due to insufficient funds will carry a $30.00 service charge.
