FROM THE DIRECTOR Over the years as Artistic Director here at Northlight, I have chosen work that I respond to personally. In many of my director’s notes I have relayed personal anecdotes that reflect my own response to the themes of the plays we have done. With Bruce Graham’s The Outgoing Tide, we examine an ethical question through the personal story of one family, which could be your own. Everyone must come to their own decision about their end—to the extent that they have a say, and that those who love them must come to some peace with that decision. These are ethical questions of course, and the subject of furious debate, both religious and political. But at the heart of the issue is the human cry for peace, and the right to leave this plane with dignity and with grace. My mother had emphysema. She was a life-long smoker which led to her condition. I wouldn’t wish emphysema on anyone, and the years leading up to her death were difficult for her and for her sister, my aunt, who cared for her. Finally, she entered the hospital and my brother and I got the call to come home. That afternoon, slipping in and out of her twilight sleep, she literally said goodbye to us several times. She was more than ready. It was the disease that had taken over, and it was not my mother in that bed. She’d told us what she wanted and so we summoned the doctor to ease her suffering. She died shortly after midnight, peacefully, in my arms, with my brother and her sister telling her we loved her. I felt blessed. I have been thinking a lot about Mom while working on Bruce’s play. Gunner’s situation of course is not the same, but really it’s not so different either. I have been in Gunner’s son’s shoes, and some day it will be Gunner’s shoes I will be kicking off. We all will. And when that day comes, we will all be yearning for a loving touch and that same dignity and peace.

PROGR AM NOTES The Ethics of Choice In The Outgoing Tide, Playwright Bruce Graham gracefully addresses end of life choices for a family that many of us can identify with, as they also try to reconcile relationships with each other. While advances in medicine and technology continue to extend human life, prolonged illness and aging persist to threaten quality of life for the elderly. With the lengthening of the human lifespan, the rate of suicide in the elderly continues to increase. For instance, the highest suicide rate in the United States is among elderly men over 85 years old; 40% of victims are over the age of 60 years old. In Charles McKhann’s A Time to Die: The Place for PhysicianAssistance, physicians shared their personal thoughts about their wishes if they found themselves diagnosed with Alzheimer’s disease: “Even the most conservative physicians, several of whom were totally opposed to the legalization of assisted dying, drew the line at Alzheimer’s disease, saying that they would prefer to have earlier and more dignified deaths than put themselves and their families through the full range of horrors.” An important issue regarding Alzheimer’s patients’ consideration of suicide or assisted dying is “whether a person who is intellectually intact […] can make a decision for the future demented self that is binding on those caring for him/her” (McKhann). For an Alzheimer’s patient, the decision to take his life is very dependent on the advancement of his disease. In addition, the individual’s state of mind, value system, and connections to loved ones influence this choice. McKhann, Charles F. A Time to Die: The Place for PhysicianAssistance. New Haven: Yale University Press, 1999.
Geriatric psychiatrists specialize in the psychological care of older adults, including the contemplation of end of life choices. Their work is the study of how their patients continue to live long and full lives, as well as the challenges that they encounter. Geriatric psychiatry is a relatively new field, stemming from the rise of the elderly and the medical innovations that keep humans alive longer. A number of geriatric psychiatrists were consulted during the rehearsal process for this production and related audience events.

P R O G R A M N O T E S ( c o n t .) Alzheimer’s Disease 5.3 million Americans suffer from Alzheimer’s disease, a form of prolonged and progressive dementia, described as a loss of memory and other mental capabilities interfering with daily life. Symptoms include challenges in problem solving; confusion with time, people and places; memory loss; changes in mood and personality; among others. The cause of Alzheimer’s is unknown: some assumptions include lifestyle, the environment, or genetics; according to the Mayo Clinic, genetics accounts for only 5% of cases. Although its source remains unclear, its physical effects on the brain are apparent. It is a slow and degenerative disease that causes damage to areas of the brain necessary for planning, reasoning, and remembering. One of the most challenging aspects of Alzheimer’s is its unpredictability; one day the patient is lucid, and, the next, she is lost in a void. Caretakers and loved ones experience a process that is mentally and emotionally grueling, as well as face the challenges of physically caring for once independent family members.
