une has arrived and with this mid-calendar month comes 92 sultry days of summer. It’s time for celebrating Father’s Day get-to-gethers, graduations, Flag Day, Juneteenth, just-because poolside parties, as well as, it’s being pegged as the preferred choice for many brides. And aren’t we glad to have such a varied selection of designers, gifts, foods, and outdoor grills that can be purchased locally to make them memorable occasions? So, it’s goodbye to the first half of 2013. May brought about numerous chamber programs and ribbon cuttings, golf tournaments, and a higher number of meetings and forums than usual due to local municipal and school district elections. As a county, we gained Santikos Theatre, I-69, and the title of most diverse county in the nation. Inside this issue, Memorial Hermann Southwest hoists the flag when it comes to the latest in medical technology and concerns such as mammograms, and urinary and bowel incontinence. From breast and bone health to gynecological and incontinence surgeries, MHS offers comprehensive, quality care for women throughout their lives. Recently County Judge Bob Hebert announced he will be running for a fourth four-year term and that by 2040 our population is likely to be 1.45 million versus the 627,293 businesses witnessed in 2012.

The Judge also noted that Texas has the highest uninsured persons rate of any state (24%), and that Fort Bend’s rate is 23%. Hospitals have provided some $5 billion dollars in uncompensated care. Fort Bend remains the responsible party and spent $1.75 million on indigent care in 2012. For state reimbursement, Fort Bend must spend more than 8% of its budget that’s $20 million. Hebert also states that our federal dollars will go to other states if Texas doesn’t join in the federal Medicaid expansion program. To achieve a better business world, politicians need to become better educators. They need to educate the country about the world in which we’re living, about the vital role education plays, and how only new innovative standards can raise new economic growth. Its their leadership by example that will show everyone honesty pays. Fort Bend is also facing numerous other issues: Water, infrastructure, global trade, education and safety issues. We’ll be bringing you more on that. Let this month be a bit more relaxed with vacations, more play time and longer weekends. One caution, June 1 also signals the beginning of the Atlantic Hurricane Season. Keep the cash register ringing. Sharon K. Wallingford

ising concerns about continued Gulf Coast regional and Texas drought are making headlines. San Antonio has set a goal of cutting outdoor water usage in half with heavy water usage fines that went into effect in May. Texas drought is causing strain in agricultural industries such as cattle, rice and wheat. Texas Combined Heat and Power Initiative (TXCHPI) is working to provide policy leaders solutions to ease water shortages and improve supply of power for Texas with CHP / cogeneration technology that reduces water needed for energy production. CHP systems also increase use of natural gas. The Texas natural gas industry supports nearly 12 percent of total employment and contributes more than $100 billion to the Texas economy each year. “Concerns about the adequacy of our power supply and ongoing water shortage have lead Texas to a policy crossroads,” said Paul Cauduro, Executive Director, TXCHPI. “Our solution is to remove regulatory barriers and treat CHP systems equally in the tax code in order to create a wave of CHP developments in the state’s industrial and commercial sector.” Research has determined that increasing CHP from 20 to 35% of total electricity generation Texas by 2025 would add roughly 14,000MW to the Texas grid while conserving roughly 161 billion gallons of water used in generation of electricity relative to “Business-as-Usual” projections. TXCHPI supports House Bill (HB) 2049, legislation currently being considered by the Texas Legislature. The bill would allow cogeneration facilities to sell electricity and thermal energy to the same customer thereby maximizing the efficiency of the cogenerating facilities while providing secure and reliable power to users at the location where it is needed without power transmission and distribution issues or line losses. Other bills supported by TXCHPI include: • HB 1864 which would authorize the State Energy Conservation Office (SECO) to develop guidelines for how CHP is evaluated, as currently required by law, prior to the construction or renovation of government facilities deemed to be “critical” for emergency preparedness and emergency response operations. Critical facilities include hospitals, prisons, commands centers and data centers. • HB 2074 which would include CHP in the Texas margin tax exemptions that currently exist for renewable energy sources by allowing a corporation to deduct a small portion of the CHP system investment from their state margins tax. • HB 1094 that makes needed changes to the long-term project financing mechanism known as Property Assessed Clean Energy (PACE) allowing owner and third-party operated CHP and waste heat recovery projects on commercial and industry property. The Texas Combined Heat & Power Initiative (TXCHPI) is a non-profit association of business interests that supports clean, energyefficient, CHP technology applications in industrial, commercial and institutional settings. TXCHPI champions CHP as the most effective, economical and environmentally sensible energy option for Texas.

Breast Care Center offers short wait times, advanced technology and more

A

ny woman who has ever had an abnormal screening mammogram or a breast biopsy knows the hardest part of receiving the news or undergoing the procedure is waiting for the diagnosis. That’s why the Breast Care Center at Memorial Hermann Southwest Hospital provides patients with a followup appointment within five days of an abnormal screening mammogram. It’s the same reason Arlene Ricardo, M.D., a breast surgeon affiliated with Memorial Hermann Southwest, visits with newly diagnosed cancer patients within a few days of their diagnosis. “It’s the unknown and the waiting that drive patients crazy,” said Dr. Ricardo, who specializes in surgical treatment of diseases of the breast. “The Breast Care Center recognizes how important it is to get results to patients quickly. If patients need a follow-up appointment, they often schedule it before the patient leaves the Center. By the time my patients have their mammograms at the Center and walk over to my office, the radiologists have read their test and sent me a report.” The Breast Care Center is fully accredited by the National Accreditation Program for Breast Centers, a program administered by the American College of Surgeons. Besides short wait times, the Center offers several advantages over general imaging centers. All mammograms are read by boardcertified breast radiologists. Unlike general radiologists who see everything from broken bones to breasts, these dedicated breast radiologists read only breast mammograms so they are skilled in detecting abnormalities and performing diagnostic breast procedures. eliminating the problem of overlapping breast tissue where potential cancers can hide with traditional mammography. It is particularly beneficial to women with dense or fatty breast tissue. Another advantage is the availability of Nurse Navigators, specially trained oncology nurses who help guide patients from a possible cancer diagnosis through survivorship. Also helpful are the Center’s Saturday hours.

Understanding your options Educating cancer patients and providing guidance for them to make good choices is something Dr. Ricardo takes to heart. When she was just 14, she lost her own mother to breast cancer and is passionate about her patients achieving the best outcomes. Additionally, Dr. Ricardo began her medical career in pathology before attending medical school and completing her residency in general surgery. This unique combination of knowledge, dedication and compassion links Dr. Ricardo in lifelong relationships with her patients.

“When my patients are first diagnosed with breast cancer, I spend an hour with them and their families,” she said. “I explain their pathology report line by line and we discuss all their treatment options. I draw pictures and use arrows to show how things connect. I think this empowers them.” Always putting patients first, Dr. Ricardo made the choice years ago to specialize in breast surgery instead of general surgery. It also enables her to work in an area that is personally important. “When you specialize in any particular field you raise the bar as far as the quality of care you deliver and staying up to date with new data, techniques and information that comes out,” she said.

Be prepared for the health care act’s “play or pay” provision The Patient Protection and Affordable Care Act of 2010’s shared responsibility provision, commonly referred to as “play or pay,” is scheduled to take effect Jan. 1, 2014. It doesn’t require employers to provide health care coverage, but it in some cases imposes penalties on larger employers that don’t offer coverage or that provide coverage that is “unaffordable” or that doesn’t provide “minimum value.” A “Large Employer” is smaller than you might think - A large employer is one with at least 50 full-time employees, or a combination of full-time and part-time employees that’s “equivalent” to at least 50 full-time employees (FTE’s). The nondeductible penalties for those who must pay generally are $2,000 per full-time employee excluding the first 30 employees, on employers with more than 50 employees that

do not offer coverage and have at least one full-time employee who receives a premium tax credit. Employers with more than 50 employees that offer coverage but have at least one full-time employee receiving a premium

tax credit, will pay the lesser of $3,000 for each employee receiving a premium credit or $2,000 for each full-time employee, excluding the first 30 employees. 2013 Sets the Baseline - Although the shared responsibility provisions don’t take effect until 2014, employers will use information about the workers they employ in 2013 to determine whether they’re subject to the provisions and face the potential for penalties in 2014. The rules are complex, so be sure to learn whether you might be subject to penalties and what steps you can take to avoid, or at least minimize them. M

ost American workers today do not realize that if they were to become disabled, under ERISA law, when they fall off their employers’ payroll, they can no longer make elective deferrals into their 401(k) retirement plans. In addition, the employer match, if one is offered, is also prohibited from being deposited. Until recently, pension disability insurance only existed on an individual basis. What this meant for the American worker is it made pension disability insurance difficult to qualify for, as that individual would not only be scrutinized heavily by underwriting but it is also quite expensive. Pension disability insurance is designed for individuals who want to insure their retirement contributions continue after they become disabled. Today, pension disability insurance is offered on a group basis, which means it’s easier to qualify for, it is considerably less expensive than the individual product and it protects one of your most valuable assets—your retirement plan. One of the most overlooked benefits in American corporate culture today is pension disability insurance. Richmond-based Pension Advisory Group is the distributor for a new product called the Retirement Income Assurance Policy (RIAP). Pension Advisory Group built RIAP to address this specific problem. The Retirement Income Assurance Policy insures that when a disability strikes, the disabled employee’s retirement plan will not be deactivated. “The time has come for American employers with defined contribution retirement plans to implement RIAP for the benefit of their covered workforce,” said Paul D. Hinson, President of Pension Advisory Group. “In life insurance, waiver of premium is a well-established benefit. A similar ‘waiver of contributions’ benefit is needed

in qualified retirement programs to ensure that elective deferrals by plan participants, and contributions by their employer, will continue as planned if the participant should become disabled.” RIAP insures the amount of the contribution for each employee, as well as the employer match. Should a disability occur, the policy activates and begins making monthly deposits into an annuity contract held by a bank trustee on behalf of the disabled participant. The money accumulates tax deferred, just like the 401(k) plan, and the cost to the employer is minimal (approximately 1.75% of the total contributions to the plan). The result is that when the employee reaches normal retirement age, he/she will have the security of a retirement plan waiting for them, just when disability payments cease. According to the Council for Disability Awareness (CDA), a non-profit group dedicated to helping the American June 2013 H Fort Bend / Katy Business Journal

workforce become aware of the likelihood of disability and its financial consequences, enumerated that more than four million Americans between the ages of 21 and 65 are expected to suffer a disabling injury or illness in 2013 alone. That compares with more than 36 million Americans classified as disabled by the U.S. Census Bureau, or about 12% of the total population. Of those workers who were disabled, more than 50% were between the ages of 18 and 64. Based on the CDA Personal Disability Quotient (PDQ) calculator, a typical 35year old female, nonsmoker, with normal height/weight, who works mostly in an office setting with some outdoor physical responsibilities and who leads a healthy lifestyle, has a 24% chance of becoming disabled for three months or longer during her working career. There’s a 38% chance that the disability will last five years or longer, with the average disability for someone like her lasting 82 months. One

in eight American workers will be disabled for five years or longer during their working career. RIAP offers a win-win solution both for the employer and the employee. While the employee is working, the disability premiums are paid by the employer and are tax deductible on the employer’s tax return. Rates are calculated at unisex rates with guaranteed issue, meaning that no individual underwriting of insurablilty occurs. The process is simple, clean and cost efficient for the employer while offering an excellent benefit option to its employees. If an employee does become disabled and has met the specified elimination period in the policy, the employer is no longer responsible for making the premiums payments and now the insurer steps up and takes over making payments into the policy for the disabled employee. RIAP offers a winning solution for the employee as an excellent benefit perk that reduces stress and worry to the employee if he/she becomes disabled before retirement age. Premiums continue to be made and growth in the employee’s plan continues. Disability benefits are deferred until retirement and held in trust for the disabled employee until normal retirement age. Just when disability ends at retirement age, a steady stream of income is now available and payable to the participant (employee). RIAP offers the perfect solution that an employee won’t erode his/her retirement long before retirement age when those monies are most needed. With proper planning, understanding and execution, RIAP avoids financial catastrophic ruin to an employee when an employer implements this cost-efficient benefit to its employees.

Local officials lend decades to expanding US 59 to I-69

I-69 finally a reality for Fort Bend County

s you travel along U.S. 59 in the Greater Houston area, keep an eye out for the segments of the highway that are now officially designated as Interstate 69. Texas Department of Transportation’s Houston District crews have installed the signs that co-designate another 28 miles of U.S. 59 (from I-610 West to Rosenberg) as a part of this vital interstate. Ultimately, I-69 will ultimately become a 1,600-milelong highway stretching from Michigan to Texas. “This is one small part of a much larger, critical link connecting Texans from Laredo and the Rio Grande Valley all the way up through Texarkana,” said Texas Transportation Commissioner Jeff

Recently, Commissioner Austin, Judge John Thompson of Polk County and Chairman of the I-69 Texas Alliance, Fort Bend Commissioners Richard Morrison and James Patterson, Wharton Mayor Domingo Montalvo, Jr., Rosenberg Mayor Vincent Morales and other dignitaries were all on-site to lend a hand to TxDOT crews installing the new signs along US 59 in Fort Bend County. “Each section added means opportunity for economic development, enhanced freight movement, as well as safety and mobility improvements,” said Judge Thompson. “While attaining the I-69 designation for U.S. 59 in Fort Bend County is an economic benchmark, the maximum benefit for the county and Texas will not be achieved until I-69 is completed from Texarkana to Mexico,” said Judge Robert Hebert of Fort Bend County. “We can take a moment to celebrate and then it’s back to work making all of I-69 a reality.” Interstate 69 is an invaluable transportation asset. To the Houston region, I-69 will be a critical link for sea ports to more efficiently move cargo to destinations within the state and beyond our borders. For area businesses expanding or relocating, convenient Interstate access will be a factor in site selections; and for the community, I-69 will increase safety on our roads and provide additional hurricane evacuation routes. I-69 Texas is important to the connectivity of the state because it provides access to inland ports, sea ports along the Gulf Coast and connects major east-west Interstates in Texas. Nearly 70 miles of existing highways now having federal Interstate designation in Texas and over 100 additional miles are currently under consideration for designation. Additionally, over $700 million dollars have been dedicated to studies and project development for I-69 Texas.

It’s just beginning. As transportation needs continue to increase and funding tightens due to budget cuts, groups of leaders from Fort Bend, Brazoria, and Waller counties, along with representatives from Port Freeport have gathered to form a board called the Highway 36A Coalition. The Highway 36A Coalition was founded to address a regional need for an action-oriented and unified advocacy organization focused on current and future infrastructure issues, says Richard Fields, representing Aguirre & Fields Engineers and Planners in Sugar Land. The Coalition seeks to represent the interests of the Region’s stakeholders to decision making bodies and funding agencies. The Highway 36A Coalition promotes public and private investment in regional free-flowing transportation facilities originating from the Freeport area of the Gulf Coast in, around, and through southern Brazoria, western Fort Bend, and Waller Counties, to provide opportunities for economic growth, hurricane evacuation, and quality of life through these facilities. “The Coalition will bring together Brazoria, Fort Bend, and Waller Counties, their cities, property owners and other stakeholders to advocate for better northsouth access in the three counties from the Gulf to US 290/Highway 6 in Waller County,” says Commission Andy Meyers, Precinct 4. Highway 36 runs from Port Freeport to Rosenberg and then west to Austin County. Highway 36A is defined as the northern extension north through western Fort Bend County connecting to and following the proposed Waller County Prairie Parkway as defined by the Waller County Thoroughfare Plan, connecting to SH 6 north of Hempstead.

The right to know one’s accuser and the exact charges is a basic due process right. “The Texas Medical Board withholds complaints from physicians, and stalkers have exploited this secret complaint process to harass their targets, as one witness explained to the Texas Senate Jurisprudence Committee on April 9. The process can also facilitate racial discrimination. “SB 1193 also requires the Board to supply the targeted physician with reports that it compiles in investigating his case, including credentials of experts.

“The Texas Medical Association (TMA) opposes the bill. It calls it a ‘medical bullying’ bill, and claims it will ‘weaken’ the Board and imperil tort reform passed more than a decade ago. These arguments are without merit. “In fact, it is targeted physicians who are the victims of bullying and intimidation by persons who can file malicious complaints with impunity. A completely baseless complaint can subject a physician to enormous stress and expense, and even destroy his livelihood. The current secret process can make it impossible for the doctor to defend himself appropriately. “Simple justice requires the Board to operate with transparency. It is unfair for doctors to be subjected to a Star Chamber process that is not tolerated for other professionals, or even for defendants in malpractice cases or accused criminals.” The Association of American Physicians and Surgeons (AAPS), which was founded in 1943, is a national organization representing physicians in all specialties.

Online mapping allows public participation in drainage update Sugar Land citizens are encouraged to participate in an online mapping activity that will help guide an update to the City’s Master Drainage Plan. The activity allows citizens to identify locations of drainage-related issues such as street flooding, standing water or detention pond maintenance. Feedback will be used to help create an assessment of the City’s drainage needs. The Master Drainage Plan was last updated in 2007. The update is intended to address the following items: • Asset ownership and responsibilities of agencies with drainage responsibilities

have changed and need to be updated. • Critical drainage projects have been completed or are underway. • Improved detention pond maintenance policies are necessary to ensure detention ponds are properly designed, constructed and maintained. • The plan will also identify capital improvements to address concerns. Citizens may visit www.sugarlandtx. gov/drainage to participate in the mapping activity, learn more about the project, sign up for email updates and find out about scheduled public meetings.

Fort Bend, Houston region continues national job growth trend The Greater Houston region continues to lead national job growth trends, according to a recent report released by the U.S. Bureau of Labor Statistics (BLS). Fort Bend and Montgomery counties are among the Top 10 in the nation for rate of employment growth (Sept. 2011 – Sept. 2012). The BLS report lists Harris county, which includes the City of Houston, as second in the U.S. in absolute employment growth during the same time period. As a corollary to the BLS statistics, the Houston Metropolitan Statistical Area’s (MSA) GDP growth has outpaced the nation, ranking first above all other metro areas (U.S. Bureau of Economic Analysis). A national corporate indicator echoing the government data is the 2012 U-Haul National Migration Trend Report, in which the Houston region ranks as the top destination for one-way family moves for the fourth year in a row. “This new BLS data is consistent with our region’s ongoing performance as a leader in a variety of economic metrics,” said Jeffrey Wiley, president and CEO of the Greater Fort Bend Economic Development Council

(EDC). “Fort Bend County continues to benefit from our favorable business climate, strong leaders, diversity of industries, infrastructure and proximity to the Greater Houston area.” Fort Bend earns national and regional accolades for job growth, population growth, and related economic and workforce development milestones. Maintaining a decade-long trend as a leader in economic opportunity, Fort Bend County continues to grow and diversify. The county’s job growth has contributed to an increase in population and diversity. Recent U.S. Census Bureau data estimates the county’s 2012 population at 627,293, an increase of 7.2 percent since 2010. The County has also been named the most diverse in the nation in a new study by Rice University. Fort Bend residents also benefit from a distinct quality of life, with five of the top 20 master planned communities in the nation (and seven of the top 24 MPCs) having an address within county lines (RCLCO, 2013). Fluor feeds more than 33,000 hungry children in 30 minutes

Fluor recently partnered with Kids Against Hunger to prepare 33,000 meals for malnourished families in Fort Bend County, Romania and Mexico. More than 300 employees teamed up by department and participated in a friendly competition to see who could prepare the most meals. The event was fun and rewarding for all who volunteered. There was a spirit of excitement and competition, but ultimately everyone was there for a common goal- to save the lives of hungry children. “The energy among our employees while supporting this cause was amazing to see,” said Richard Meserole, Houston vice president and general manager, Fluor. ”Hunger is not just an issue for other parts of the world; it is a relevant issue in our own backyards. I hope in some small way, our 30,000 plus meals helped make a difference in the lives of those in need.” A significant amount of the meals were distributed to Lunches of Love, a Fort Bend County meal service program committed to ending childhood hunger. “Witnessing the operation was fantastic,” said Adriane Gray, Lunches of Love founder. “Fluor’s community outreach is simply the best. On behalf of Lunches of Love (LOL) and the children in Fort Bend County, I thank Fluor and Fluor Cares for all you do. I know these meals will make a positive impact on LOL and the children we feed. Adamson selected for top George Foundation post The George Foundation Trustees announce the retirement of longtime Executive Director Roland Adamson, who has served the foundation for 35 years. The effective date of his retirement is May 1. “After interviewing the top three candidates, the foundation trustees recruited Roger Adamson to fill the open position of Chief Executive Officer of The George Foundation,” said Chairman of the Board of Trustees Pat McDonald. Adamson, the foundation’s newly appointed CEO, is a 1999 graduate of the United States Military Academy, receiving a BS degree in Systems Engineering. From West Point, he fulfilled his military obligations from 1999-2004, training at Fort Hood, Texas and with a tour in Iraq. While in Iraq, Adamson served as the Company Executive Officer of the 1st Battalion’s 67th Armor Regiment in Ba’qubah and as Operations Officer at Headquarters of the 4thInfantry Division in Tikrit. Following his military service, Adamson received a MBA from the Kellogg School of Management at Northwestern University in 2006, majoring in Marketing, Finance, and Management & Strategy. He comes to the foundation with seven years of experience in management and marketing with a Fortune 500 Company, Caesars Entertainment, where his last position was Vice President of marketing. Adamson is returning to the community where he was reared and graduated from Lamar Consolidated High School. He brings with him his family: “... my wife Jennie and two children, who will now get to experience the richness of growing up in Fort Bend County,” said Adamson.

UH Alumni Association honors Jacquie Baly as “Rising Star” In April the University of Houston Alumni Association honored Jacqueline Baly as a 2013 Alumni Award recipient. Ms. Baly was the recipient of UH’s “Rising Star” Jacqueline Baly award. Her achievements include serving as Mayor Pro Tem and Councilmember for the City of Sugar Land as well as President & CEO of BalyProjects, LLC. She is a former Commissioner of the Brazos River Authority, a statewide appointment she received from Governor Perry, where she represented Harris, Fort Bend and surrounding counties on water

and wastewater issues. She has been featured on KUHF, KTRH, KTRK and several radio stations discussing policy matters. She also lectures on public policy and urban politics at the University of Houston. This year’s honorees include names that are prominent both nationally and locally as well as on the UH campus. Distinguished Alumni Awards are given to individuals whose professional accomplishments bring credit to the University. The Rising Star Award, given for the first time in 2010, recognizes the accomplishments of alumni under the age of 45 for demonstrated achievement in their field above and beyond that of their peers.

Wong chair of Texas Board of Professional Engineers Daniel Wong of Missouri City has been appointed chair of the Texas Board of Professional Engineers. Governor Rick Perry also appointed two members to the board. The Daniel Wong board licenses qualified engineers, enforces the Texas Engineering Practice Act and regulates the practice of professional engineering in Texas. Wong is CEO of Rick Perry Tolunay-Wong Engineers. He is a member of American Society of Civil Engineers, Texas Society of Professional Engineers and University of Houston Civil Engineering Advisory Board, past president of the Asian American Architects and Engineers Association, and a past board member of the

Houston Council of Engineering Companies. He is also a trustee of the Fort Bend Economic Development Council, past member of the Sugar Land City Council, and past board member of the Fort Bend Chamber of Commerce, Houston-Galveston Area Council, and University of Houston Alumni Association. Wong received a bachelor’s degree, a master’s degree in civil engineering and a doctorate of civil engineering from the University of Houston. He will serve as chair of the board for a term to expire at the pleasure of the governor. Sina Nejad of Beaumont is president of Sigma Engineers Inc and Kyle Womack of Horseshoe Bay, a licensed professional engineer and vice president of business expansion at Parkhill, Smith and Cooper, Inc. were also appointed. Urinary and bowel incontinence are common, treatable conditions

M

any women feel embarrassed to talk about urinary or bowel incontinence, yet it is a common medical condition, particularly for women. The good news is that there are numerous treatments to improve both conditions. “Many of my patients experience urine leakage during pregnancy,” said Jayshree Patel, M.D., an obstetrician/gynecologist affiliated with Memorial Hermann Southwest Hospital. “Other factors can contribute to urinary incontinence, including number of deliveries, birth weight of children, obesity, chronic respiratory problems, constipation and soft connective tissue diseases.” Different types of urinary incontinence respond to different treatments. Stress incontinence, which occurs when coughing, laughing, sneezing or during other activities that put pressure on the bladder, is most frequently the result of full-term pregnancies. Overactive bladder, in which the bladder muscle is unable to stay relaxed, occurs in response to an urge to urinate. Mixed incontinence is a combination of the other types. “Medication can provide the most immediate results for overactive bladder, but sometimes we have to try different medications and doses,” said Dr. Patel. “For stress incontinence, a plastic pessary can be inserted into the vagina to support the bladder and urethra. Other conservative treatments include dietary changes, bladder training and other behavioral therapies. Kegel exercises can also help, but as with any exercise, it takes time and must be continued to be beneficial.” Kegel exercises strengthen the pelvic floor muscles, which support the uterus, bladder, small intestine and rectum. Anne Le, M.D., a colorectal surgeon with Memorial Hermann Medical Group and affiliated with Memorial Hermann Southwest, said noninvasive therapies, dietary changes, medications and Kegel exercises are always the first line of treatment for bowel incontinence.

“You’d be surprised how often we add fiber bulking agents to the patient’s diet and that’s all they need to control fecal incontinence,” she said. “Biofeedback to retrain the pelvic and anal sphincter muscles is also a noninvasive option.” A relatively new treatment for bowel incontinence is Solesta®, a gel that acts as a bulking agent. It is usually administered once as an in-office procedure, but can be repeated twice. “We inject it into the anal canal to essentially create a mechanical barrier to stool leakage, said Dr. Le. “It’s a little over 50 percent effective, but risks are extremely low, so for many patients, it’s worth trying before surgical treatments.”

Overall, patients report an 85 percent improvement in their symptoms with InterStim.” When it comes to the surgical treatment of stress or mixed urinary incontinence, many patients find relief with a bladder sling. The two most common are the TOT (transobturator tape) and the TVT (tensionfree vaginal tape) slings, which act like a hammock to provide support under the urethra. Success rates for the slings range from 75 to 90 percent.
