T

he Fort Bend business and civic communities have always done an excellent job of stepping up to help those in need – regardless of whether it’s financial or moral support. October is a month where we highlight the importance of Breast Cancer Awareness and early detection. Nearly every person in our community has been affected directly or knows someone who has been afflicted with this terrible disease. In fact, one of the greatest journalists in Fort Bend history– our own Bev Carter – fought Breast Cancer with the same amount of strength and zeal that she put into her legendary columns. This month’s Business Journal is dedicated to Bev and the millions of women who have fought or are fighting that battle throughout our community, nation and the world. With the strong support of hospitals such as Memorial Hermann, Methodist and M.D. Anderson, Fort Bend residents have resources nearby that many Americans aren’t fortunate enough to have. In this edition, we’ve spotlighted Cure & Co. owners Irma Klink and her daughter, Sasha Klein, both of whom are Breast Cancer survivors. The Sugar Land residents wanted to open a store where women could shop for wigs and have a friendly ear to listen to and a shoulder to cry on. Breast conserving therapy shows survival benefit compared to mastectomy in early-stage patients with hormone receptor positive disease

W

hen factoring in what is now known about breast cancer biology and heterogeneity, breast conserving therapy (BCT) may offer a greater survival benefit over mastectomy to women with early stage, hormone-receptor positive disease, according to research from The University of Texas MD Anderson Cancer Center. The study findings defy the conventional belief that the two treatment interventions offer equal survival, and show the need to revisit some standards of breast cancer practice in the modern era. The research was presented at the 2014 Breast Cancer Symposium by Catherine Parker, MD, formerly a fellow at MD Anderson, now at the University of Alabama Birmingham. In the 1980s, both US-based and international randomized clinical studies found that BCT and mastectomy offered women with early stage breast cancer equal survival benefit. However, those findings come from a period in time

when very little was understood about breast cancer biology, explains Isabelle Bedrosian, M.D., associate professor, surgical oncology at MD Anderson. “Forty years ago, very litIsabelle Bedrosian, M.D. tle was known about breast cancer disease biology – such as subtypes, differences in radio-sensitivities, radio-resistances, local recurrence and in metastatic potential,” explains Bedrosian, the study’s senior author. “Since then, there’s been a whole body of biology that’s been learned – none of which has been incorporated into patient survival outcomes for women undergoing BCT or a mastectomy. “We thought it was important to visit the issue of BCT versus mastectomy by tumor biology,” Bedrosian continues. The researchers hypothesized that they would find that patients’ surgical choice would matter and impact survival with tumor biology considered. For the retrospective, population-based

study, the researchers used the National Cancer Database (NCDB), a nation-wide outcomes registry of the American College of Surgeons, the American Cancer Society and the Commission on Cancer that captures approximately 70 percent of newly-diagnosed cases of cancer in the country. They identified 16,646 women in 2004-2005 with Stage I disease that underwent mastectomy, breast conserving surgery followed by six weeks of radiation (BCT), or breast conserving surgery without radiation (BCS). Bedrosian notes that it was important that the study focused solely on women with Stage I disease in order to keep the study group homogenous and because in this cohort few would be ineligible for BCT. Since estrogen receptor (ER) and progesterone receptor (PR) data were available and HER2 status was not, the researchers categorized the tumors as ER or PR positive (HR positive), or both ER and PR negative (HR negative). ugar Land residents Irma Klink and her daughter, Sasha Klein, both won their respective battles with Breast Cancer. “When mom and I were going through Breast Cancer, we didn’t find a one stop shop catering for people going through cancer,” Klein said. “The wig stores we found were extremely depressing, and the atmosphere was really dreary.” Klink assured Klein that someday they’d open a store that would be “beautiful and girly” and would help Breast Cancer patients. “After six months of treatment, my mom said ‘it’s now or never, let’s do it’,” Klein said. Together, the mother-daughter duo opened Cure & Co. at 2587 Town Center Blvd. –A Wig & Cancer Recovery Boutique in Sugar Land.

“We knew it was really well-needed,” Klein said. “Women can come here in the comfort of their hometown and there’s a group for support if they need it. They have an ear of someone who has walked the walk, and we can help you through that journey.” In a friendly atmosphere, Cure & Co. sells wigs and a variety of products (hats & surgery recovery items) that can help Breast Cancer patients. They also offer Spa services, including complementary facials to Breast Cancer patients, said Klein, a professional esthetician. For every non-patient that gets Spa service at Cure & Co., a free Spa service is donated to a Breast Cancer patient. Klink and Klein are part of a family that has the BRCA1 gene mutation that results in high percentages of breast cancer. Seven women in their family have been diagnosed with breast cancer, including Klink’s sister, Patricia Ortiz, a two-time October 2014 H Fort Bend / Katy Business Journal

In addition to patients, caregivers, husbands, family members, oncologists and nurses also shop at Cure & Co. Customers visit the store from throughout Fort Bend County as well as Katy and Houston. Klein emphasizes the importance of early detection. Her mother was diagnosed during Stage 3, and she was diagnosed in Stage 2. Klein said Breast Cancer Awareness Month in October is important because it usually results in more women getting mammograms. “More women are diagnosed after the month of October because of Breast Cancer Awareness,” Klein said. Cure & Co. regularly partners to combat cancer alongside M.D. Anderson Cancer Center and the American Cancer Society.

Novel immunotherapy vaccine decreases recurrence in HER2 positive breast cancer patients

A

new breast cancer vaccine candidate, (GP2), provides further evidence of the potential of immunotherapy in preventing disease recurrence. This is especially the case for high-risk patients when it is combined with a powerful immunotherapy drug. These findings are being presented by The University of Texas MD Anderson Cancer Center at the 2014 American Society of Clinical Oncology’s Breast Cancer Symposium in San Francisco. One of only a few vaccines of its kind in development, GP2 has been shown to be safe and effective for breast cancer patients, reducing recurrence rates by 57%. Further, women with the highest overexpression of HER2 (known as HER2 +3) had no cancer recurrences when they were administered the vaccine after completing trastuzumab (Herceptin), a type of immunotherapy drug known as a monoclonal antibody. HER2 is an oncoprotein that promotes tumor growth and is expressed to some extent in 75-80% of breast cancers. “This is an important and different avenue in immunotherapy research, in that we are investigating ways to prevent cancer recurrence by stimulating the immune system to treat Breast conserving, continued from page 6

score for a broad range of variables, including age, receiving hormone therapy and/or chemotherapy, as well as type of center where patients were treated and comorbidities. Of the 16,646 women: 1,845 (11 percent) received BCS; 11,214 (67 percent) received BCT and 3,857 (22 percent) underwent a mastectomy. Women that had BCT had superior survival to those that had a mastectomy or BCS – the five-year overall survival was 96 percent, 90 percent and 87 percent, respectively. After adjusting for other risk factors, the researchers again found an overall survival benefit for BCT compared to BCS and mastectomy. In a matched cohort of 1,706 patients in each arm, the researchers still found an overall survival benefit with BCT over mastectomy in the HR positive subset but not in the HR negative subset.

cancer,” says principal investigator Elizabeth Mittendorf, M.D., Ph.D., associate professor of Surgical Oncology. “The ultimate goal is to develop a preventative tool that will Elizabeth Mittendorf, minimize the risk of M.D., Ph.D. recurrence in women who have already had breast cancer and for whom standard therapies have failed.” The findings are the result of a phase II randomized trial that paired the GP2 vaccine, designed to stimulate the CD8+ cells, commonly known as “killer” or “toxic” T cells, with an immune stimulant known as granulocyte/macrophage colony stimulating factor (GM-CSF). The trial included 190 patients with varying levels of HER2; 89 women received the GP2 vaccine with a GMCSF adjuvant and a control group of 91 patients received GM-CSF alone. Eight patients experienced early recurrence or developed a second malignancy and did not complete the vaccine trial. The vaccine is injected subcutaneously and the initial series consisted of monthly inoculations for six months, followed by four cycles of booster shots administered every six months thereafter. The patients were

monitored for nearly three years. For all 190 patients, including those who did not complete the trial, the disease-free survival (DFS) rate was 88% among those who received the vaccine and 81% in the control group – representing a 37% reduction in recurrence. Excluding the patients who did not complete the vaccine series, the results are higher – 94% DFS rate versus 85% who did not get GP2 – a 57% risk reduction. Women with HER2 +3 who were administered trastuzumab as part of the standard of care prior to receiving the vaccine experienced no cases of cancer recurrence. According to Mittendorf, trastuzumab may act like a primer for the vaccine. Trastuzumab stimulates CD4+ T cells to release substances that fight cancer cells and initiates an antibody response. Thus, it may prepare the immune system, making the vaccine even more effective. MD Anderson is now testing this combination of immunotherapies in other clinical trials.
 The GP2 study supports previous MD Anderson research on similar breast cancer vaccines, such as AE37, which showed a significant immune response and improved recurrence rates in triple-negative breast cancer patients.

While provocative, Bedrosian cautions that the findings are not practice changing, as the study is retrospective. Still, the research complements other recent studies that showed BCT was associated with a survival benefit compared to mastectomy. Also, she points to the delivery of radiation therapy as the possible driver of the overall survival benefit. “We’ve historically considered surgery and radiation therapy as tools to improve local control,” says Bedrosian. Yet recent studies suggest that there are survival-related benefits to radiation in excess of local control benefits. Therefore, radiation may be doing something beyond just helping with local control. Also, we know hormone receptive positive tumors are much more sensitive to radiation, which could explain why we found the survival benefit in this group of patients.” As follow up, Bedrosian and her team hope

to mine the randomized controlled trial findings from the 1980s, matching those cohorts to current NCDB patients to see if a similar survival benefit could be observed. “While retrospective, I think our findings should give the breast cancer community pause. In the future, we may need to reconsider the paradigm that BCT and mastectomy are equivalent,” she says. “When factoring in what we know about tumor biology, that paradigm may no longer hold true.” The study was institutionally funded. In addition to Bedrosian and Parker other authors on the MD Anderson study include: Heather Y. Lin, Ph.D., Yu Shen, Ph.D., Liang Li, Ph.D.; all Biostatistics, Henry Kuerer, M.D., Ph.D. and Gildy Babiera, M.D., both Surgical Oncology; and Simona Flora Shaitelman, M.D., radiation oncology. Mammosafe provides mammogram checkups convenient for women with mobile unit

D

r. Daniel Roubein, a radiologist with more than 20 years of experience, recommends that women 40 years old and older get a mammogram once a year. There are exceptions when it should be done earlier, such as family history. Roubein wanted to make it easier for women to get access to mammograms, so last October, he developed a fully certified mobile mammography unit for patients. Mammosafe works with Houston area businesses to set up appointments for mammograms in the business’ parking lot, so employees don’t have to travel to make a doctor’s appointment. “A high percentage of women were over 40 and never had a mammogram,” Roubein

said. “When we’re right outside the parking lot, it made it impossible for them to skip it.” While Mammosafe isn’t the first mobile Mammogram unit, Dr. Daniel Roubein Roubein said it is thorough and has a strong attention to customer service and details. The results for the mammograms are revealed in a short period of time to ease patients’ anxiety, and everything is done in a “secure, safe” manner that respects patients’ privacy. Roubein goes over the images with the patient and talks about the results. If there is a problem, he puts the patient in touch with a health care professional or communicates with the patient’s doctor.

If more scans are necessary, Roubein’s staff takes care of it on the scene. In traditional medical settings, patients may set up a follow-up appointment that is 2-3 weeks away. Mammosafe can often spend 2-3 hours at a location because it will serve as many patients as possible. Recently, it visited a school district north of Houston, and teachers were able to get their mammograms during their planning or lunch times. “We feel real good about being able to get out to the patients,” Roubein said. UTHealth School of Nursing Pacesetter BSN model for nursing education

A

re designing of the four-semester B.S.N. curriculum, the Pacesetter track gives students their in-classroom course work, plus an opportunity to work a full-time clinical rotation schedule over 16 weeks during their final semester of school. “Our goal was to create an educational experience that will get the student living the life of a nurse while still in nursing school and help develop the strong critical thinking skills demanded by a clinical workplace,” said UTHealth School of Nursing Associate Dean for Academic Affairs, Cathy Rozmus, Ph.D., R.N. Thanks to clinical partners such as the Memorial Hermann Healthcare System, the Pacesetters have endured 12-hour work shifts, experienced continuity of care with patients and become acquainted with the hospital’s system of care. Results of a published study in the Journal of Nursing Education and Practice by Dr. Rozmus and others found that first-year job retention rates were 63 percent for 92 graduates from the traditional curriculum and 100 percent for 50 randomly selected Pacesetter alumni.

Fulshear Scarecrow Festival returns to Cross Creek Ranch, Oct. 18

est Houston’s biggest collection of scarecrows once again will come to life when the Fulshear Scarecrow Festival returns to the masterplanned community of Cross Creek Ranch on Saturday, October 18, 2014 from 11 a.m. – 4 p.m. Admission into the festival is free and open to the public. NRG coal plant works on clean energy project By Michael Sudhalter msudhalter @ fortbendstar.com

N

RG, the Houston-based owner of the WA Parish Generating Station in south Fort Bend County, is coordinating a $1 billion project to bring cleaner air to local residents. W.A. Parish, a 4,650 acre plant on the Brazos River, opened in the 1950s as a natural gas plant and added coal in the late 70s and early 80s. While some coal plants have closed around the country, NRG is looking for a long-term future with the world’s largest Post-Combustion Carbon Capture Project, which is expected to be completed by late 2016. Carbon Dioxide produced by the burning of the coal in one of the units at the plant, generates about 20 percent of the electricity for the Greater Houston

area, will be separated and redirected to the West Ranch oil field 80 miles southwest of the plant in Jackson County. “The goal is to reduce greenhouse emissions in a way that provides no economic penalty to our shareholders and add no cost to electricity generated for the customer,” said David Greeson, Vice President of Development for Petra Nova, a special purpose company that’s part of NRG. “It’s going to have a number of very beneficial impacts. It will make our air a lot cleaner and provide economic growth. That is something people can rally around.” The project was financed through NRG/Petra Nova, along with its partner in the oil and gas industry, Hilcorp Energy, as well as JX Nippon, a Japanese company. They also received a $167 million grant from the U.S. Department of Energy. The West Ranch Oilfield currently

produces 500 barrels per day, but that will be increased to 15,000 barrels per day. “The dramatic increase will produce the revenue that makes this viable,” Greeson said. There are only two other PostCombustion projects in North America – one in Estevan, Saskatchewan, Canada that’s about half the size of the carbon capture capability being installed at W.A. Parish, and another in Mobile, Ala. that’s one/tenth the size. Carbon Dioxide is clear and odorless so people won’t notice the change but according to Greeson, the air will be cleaner. Greeson said there will likely be increased regulations on Carbon Dioxide in the future, and NRG/Petra Nova will be one step ahead, in terms of compliance. He said other plants may follow in W.A. Parish’s steps, nationally and globally, if the project is successful. “This is the beginning of something potentially huge,” Greeson said.

Martin selected as new Joint Center Medical director at Memorial Hermann Southwest

pecializing in total joint replacement, Stacey K. Instead of a traditional instructor, there’s an electronic kiosk with 300 classes. "We have a catalogue of workouts that we keep fresh, and we give our members the ability to reserve the workout room," Black said. "The members can do the workouts on their own or with a group. There's an element of privacy there, too." Snap also has a tanning booth where customers can safely work on their tan. Black said that he and his staff of trainers are willing to be as involved — uninvolved — in a member's fitness experience as requested. "If people want to be motivated/held accountable, we're happy to do that," Black said. "We'll call people to remind them to show up if they ask to do that." Black trains high school athletes from Fort Bend Christian Academy — where he's the line coach — as well as Fort Bend ISD schools. We work with athletes to jump higher, run faster and be stronger." "I show them a lot of technique stuff they've never been exposed to," Black said. "I've trained with the best there is from college, forward. Work ethic is something that's very important for the athletes. We want them to go out and work for something instead of having a right to it. Work ethic is a crucial part of being successful. Athleticism is just half the battle." Snap is currently in the process of planning an addition to its building. They're going to build a 2,000 square feet Mixed Martial Arts to the existing gym that's just a tad under 5,000 square feet. Black said he's proud of the training staff with each trainer specializing in a unique area. Roxy Hagy helps members turn around their personal fitness and well-being. "She makes a commitment to fitness and being healthy," Black said. "She brings so much passion it's contagious." Tim Payne is someone that Black describes as a "strength and conditioning/ power lifting guru." "Crossfit is appealing and creates a bond between people. It gets people passionate about fitness, staying healthy and working out." Black said Snap isn't just about becoming a stronger person, but noted that an increase in strength often pays dividends in the other areas, such as weight loss. "Every other goal will fall into place when people get stronger," Black said. Black, who played in the NFL at 315 pounds, discovered that first-hand when he slimmed down to 270. "The fastest way to burn fat is to add more muscle," Black said. Looking forward, Black is considering expanding to a new location, possibly in the Richmond/Rosenberg area. Black said some of the skills involved in MMA are similar to those used in football, specifically for offensive and defensive linemen. "You have the hand-to-hand fighting and body control that transfers to other sports," Black said. Snap started in Minnesota in the late 1990s and arrived in Texas about five years ago. Some of the locations are corporateowned, while the location in Sienna is a franchise owned by Black. Black also wants to create a Crossfit

“Care to Chat” caring for kids on and off the playing field

Fort Bend Exchange Club hosts another ‘Duck Golf’ Classic

This year the Duck Golf Classic will take place on Oct. 20, at Pecan Grove Country Club.

Exchange Club of Fort Bend are at work planning the 2014 Duck Golf Classic which will take place on Oct. 20 at Pecan Grove Country Club. The Exchange Club of Fort Bend was known for the “’Duck Race” which used to be held at the Fort Bend County Fair, and although the club are no longer hosts the race, the members still wanted to have a “Duck” connection – something that “Quacky” the club mascot was delighted about! Katina Scott, Event Chair says “We wanted to give our club an identity and as so many people already knew us for the Duck Race this seemed like a great idea to carry the “Duck” theme through to our fundraising golf tournament.” A new tradition will start at this year’s event. New Texana Autism program opens Opening Celebration of the Children’s Center for Autism at Sugar Land took place on Sept. 4 and will serve Medicaid and low income families in the Sugar Land, Stafford and Missouri City Area. The renovation of the building and the operation expenses are being funded through the Texas Healthcare Transformation and Quality Improvement Program (1115 Waiver). However, community donations have also been received from the Exchange Club of Sugar Land, Fort Bend Cares, Fred & Mabel R. Parks Foundation to help equip and furnish the classrooms. Kate Johnson-Patagoc, Director, Specialized Children’s Services said “The center has been specifically designed to meet the needs of children with an autism diagnosis. For example, children with an autism diagnosis often have difficulty generalizing to novel situations and have to be taught specifically how to respond in different environments. To address this issue, we installed a wide-variety of different types of faucets, hand drying devices, soap dispensers, drinking fountains, and dining seating, etc. We use this variety as part of our applied behavior analysis intervention so that children are successful when they encounter different situations in the world. We are extremely pleased at the way the center turned out. The only thing lacking at present is a playground, which will cost around $70,000. A playground is an important place for a child with an autism diagnosis to develop needed language, play, social skills, problem-solving, and motor skills, so we are hoping that the community will assist us in making this a reality.” The center will also house the Texana Behavior Improvement Center programs and Therapy for Tots for children ages 0-3 years. Lee A. Palmer Receives 2014 Texas Association for Home Care & Hospice Award The Texas Association for Home Care & Hospice (TAHC&H) announced August 20 during their 45th annual meeting, Lee A. Palmer, Administrator of Consolidated Home Health Lee Palmer in Sugar Land, as the recipient of the Chairperson of the Year Award for his work in the home care community in Texas. “I am pleased to award Lee with the Chairperson of the Year Award,” said Rachel Hammon, executive director of TAHC&H. “He is a true leader and is passionate about

making our industry the best it can be. He is a ‘can-do’ person who always steps up to the plate.” For the last several years, Palmer has been an integral member of the TAHC&H Membership and Public Affairs committee. As Vice-Chair of the committee, he helped spearhead the TAHC&H Gifting Program that raises money for charities in Texas; helping to raise over $5,000 for the Texas food bank. He also founded the TAHC&H Young Professionals Organization (TYPO), a program to engage the younger members of TAHC&H and encourage them to step up into leadership roles within the association.

Local doctor awarded two state honors OakBend Medical Group’s Dr. Joe Anzaldúa has been named the Member of the Month by the Texas Academy of Family Physicians. Anzaldúa has been in practicing family medicine in our area since Dr. Joe Anzaldúa 1985. A graduate of the University of Texas Medical School in San Antonio, Anzaldúa chose family medicine because of the variety of medical care it offers. According to the doctor, “Family medicine provides a holistic approach to both wellness and illness care, which provides a perfect balance in my daily practice. I’ve always been intrigued with the psycho-social aspects of the family unit, both nuclear and extended – from delivering babies to discussing end-of-life issues with the family of an elderly Alzheimer’s patient.” Anzaldúa firmly believes that one of the most important qualities a family physician should have is “allowing the patient to be a

partner in their own health care. That means listening, caring, involving, and educating the patient.” Additionally, Dr. Anzaldúa has been asked to serve on the Texas Department of State Health Services Texas Crisis Standards of Care Panel. This panel was tasked by Dr. David Lakey, Texas Commissioner of Health, to examine ethical issues and priorities related to standards of care during a public health emergency or disaster and to develop a framework specific to Texas. Within the medical, legal, and ethical construct, this framework will address the provision of health and medical resources by emergency medical service personnel, hospitals, alternate care systems, and other public health and emergency management and response disciplines during times of crisis or scarce resource availability. “I am honored to have been asked to serve on this Panel,” said Anzaldúa, “it is a wonderful opportunity for me to give back to the community.”

FBISD names Michael Brassfield as new Director of Transportation Fort Bend ISD names Michael Brassfield as New Director of Transportation FORT BEND ISD - The Fort Bend ISD Board of Trustees approved Michael Brassfield as the new Director of Transportation at its August 11 Special Called Meeting. Brassfield has more than 30 years of experience in transportation, and as a former Assistant Director of Transportation, is no stranger to Fort Bend ISD. Brassfield most recently served as Coordinator of Transportation in Pflugerville ISD. Prior to that, he served as Director of Transportation for Manor ISD and General Manager of Transportation for Lubbock ISD. He also held supervisory positions in transportation at Frenship ISD, Seguin ISD and Waco ISD. “We are proud to have Mr. Brassfield rejoin the Fort Bend ISD transportation team,” said Max Cleaver, FBISD Chief Operations Officer. “Given his experience, knowledge and outstanding leadership abilities, I am confident he will help improve the quality of transportation services provided to our students.” Brassfield has studied at Abilene Christian University, Texas Tech University and Lubbock Christian University. Harish Jajoo elected president of TML Sugar Land Mayor Pro Tem and City Council Member Harish Jajoo has been elected president of Texas Municipal League (TML) Region 14 for the incoming term, which includes cities and municipalities in 17 county region all the way from Houston county to Matagorda county including cities of Houston, Galveston and Sugar Land. As regional director of TML’s Region 14, the largest region in Texas, Jajoo will promote interest in municipal government on a regional level, facilitate the exchange of information among cities in the region, and assist the TML Board of Directors in formulating policy that represents the diverse interests of the state. TML was formed in 1913 and now serves the needs and advocates the interests of more than 1,110 cities and towns comprising the membership. More than 18,000 mayors, councilmembers, city managers, city attorneys and city department heads are members of the League. The TML has 16 regions. “I am excited and honored to be elected to this key role within the Texas Municipal League and will focus my energy and atten-

New Nursing and Rehabilitation facility opens in Missouri City Welcoming their first three residents, Windsor Quail-Valley Post-Acute Healthcare Center opened its doors in the Missouri City community Tuesday, Sept. 2. The first admission kicked off with Administrator Sam Pack driving resident Roger Bateman to the building in a bright red Camaro. Staff greeted Roger along with residents Long Duong and Zohra Begum at the door with applause. Bateman said he and the other residents feel good about being at Windsor Quail Valley. “It’s very nice and clean and the food is outstanding,” Bateman said. “The staff is very friendly and we enjoy the activities. Discussing history is our favorite past time.” Administrator Sam Pack said this is the start of something special. “This is an important day for our residents. This is now their home and all of us here at Windsor Quail Valley are excited to get to know them and their families,” Pack said. The 120 bed facility offers 75 dually certified Medicaid/Medicare beds and 45 Medicare Certified beds. The building covers nearly 50,000 square feet and has a spacious therapy area for residents.
