Introduction When we invited people to take part in our first Grand Finale fundraiser many years ago, we thought about a one-time event. We wanted to organize something that would be fun and at the same time would bring real benefits to the children in need of help. What kind of benefits? Nobody could have predicted. The immense success of the first Grand Finale enabled us to divide the collected funds between all children’s heart surgery units in Poland. Our action became nationwide in character, and our first purchases substantially improved working conditions for Polish children’s heart surgeons. Jurek Owsiak, Founder & Chairman of the GOCC Foundation The success of that first Grand Finale fundraiser encouraged us to set up a Foundation which would take on a mission of organizing a nationwide money collection in the following years. The Great Orchestra of Christmas Charity Foundation was officially established in Warsaw on March 2nd 1993. Today, the GOCC Foundation enjoys the most trust among all non-governmental organizations and public institutions in Poland. Proof? Public opinion polls, numerous awards and massive support for the Grand Finale fundraisers which each year engage 120 000 volunteers. We work to advance the level of care offered to the youngest and the oldest patients in public hospitals in Poland. We purchase medical equipment for Polish children’s hospitals and geriatric units. The donations made by the GOCC Foundation have contributed enormously to changing the face of children’s medicine in Poland and improving the level of care offered to the elderly. The Foundation also runs and finances five all-year-round nationwide medical programmes, which have been praised by world-class specialists in children’s medicine. Interested? Read on!

Leadership and Governance Jurek Owsiak, who is the originator and founder of the GOCC, combines the role of Chairman of the Board with his long-term voluntary service for the Foundation. Other key people on the Board of the Foundation are:

Piotr Burczyński Lidia Niedźwiedzka-Owsiak Bohdan Maruszewski

http://en.wosp.org.pl/foundation/about_us According to the findings Public Opinion Research Centre (CBOS), the GOCC Foundation enjoys the trust of an overwhelming majority of Poles. 85% of citizens of Poland trust us! We are followed by Caritas, Polish Red Cross, and the Polish Armed Forces.

The Grand Finale The Grand Finale is a day-long nationwide fundraiser organized by the Great Orchestra of Christmas Charity in order to support the Polish public healthcare system. Held once a year, usually on the second Sunday in January (with 25th Grand Finale being an exception, planned for third Sunday of January 2017) the affair combines a telethon, public and online fundraisers, charity auctions with a plethora of accompanying events taking place across the country. On the day of the Grand Finale, approximately 120 000 volunteers take part in the public fundraiser. The fundraiser is coordinated locally by almost 1600 so-called Collection Centres, and by private companies which display stationary collection boxes on their premises. We play globally, with grassroots initiatives of holding a Grand Finale fundraiser springing up everywhere, where there is a thriving Polish community. At 8:00 pm, we invite everyone to enjoy a fireworks and lights display dubbed “Light to the Heavens“. Join us in sending up above a symbolic signal – light a match, a candle, a torch – to the Good Angels up above. Let the winter night spark with a million of lights! The display is one of the most anticipated moments of the night, and we all come together as it blazes through the winter darkness. Another traditional element of our fundraiser is the Golden Hearts Auction. The emotions run high at 11 pm as the most prestigious lot, the Golden Heart №1, is auctioned out. Right after midnight we always publicise the initial estimates of the amount of funds we have raised. The online auction service Allegro.pl, which has been playing with the Orchestra since 2001, enables our supporters to bid for incredible items donated by many famous and successful musicians, actors, sportspeople, and politicians. Online auctions are an ever more popular way of supporting our cause. Read more at: www.wosp.org.pl/final

How to become a GOCC volunteer? 1. Come late October, you can check on our website and see if there is a Collection Centre established in your area. 2. If there is no Collection Centre in your area yet, you can begin thinking about establishing one yourself…or remain patient and see if there is one in the works! 3. You found a local Collection Centre! Well done, now you can get in touch with them and explain how you can help them coordinate the Grand Finale. 4. High time to get ready to embark on an adventure with the crew at the Collection Centre – now is the time to prepare a picture for your ID card. 5. All in the Collection Centre’s hands now – wait for them to get back to you!

Are there any age restrictions for volunteering during the Grand Finale?

All ages are welcome at the Grand Finale! If you are underage, we ask your legal guardians to sign your Declaration. If you are under 16 years old on the day of the Finale, you should be accompanied by an adult.

Where do the volunteers fundraise?

We never do door-to-door fundraising. Volunteers raise money locally, in public places. You can meet them in towns, cities, and villages across the country.

Will I receive a document conforming my voluntary service at the Grand Finale?

Collection Centres receive certificate forms, which serve to confirm the amount of money you have raised and testify your participation in the Grand Finale. We rely on Chiefs of individual Collection Centres to pass the documents onto you. Unfortunately, we are unable to provide you with additional certificates.

6

How to set up a Collection Centre? First of all, check if there is a Collection Centre established locally. If there is a Collection Centre gearing up to hold the Grand Finale, why not join forces to organize an event on an even bigger scale? Building bridges and bringing different people, groups, and organizations together is a part of our mission. If there is no Collection Centre established in your area, it’s your call to take up action! 1. Try to find people, who will commit themselves to organising the work at the Collection Centre. They will be your Collection Centre crew – the core of the initiative. 2. Get to know the Rules and Regulations of the Collection Centre. Make sure your crew follow them! 3. Choose the Chief of the Collection Centre, who is going take charge of the Collection Centre’s work and organisation. 4. The Chief of the Collection Centre then registers in a database, fills in an application form, and accepts the Rules and Regulations, agreeing to follow its provisions. The GOCC Foundation reserves the right to refuse to register a Collection Centre without justification. We do our best to provide you with an explanation, but usually the registration process for Collection Centres runs smoothly. Locally run Collection Centres hold events across Poland and abroad. We are always amazed by your creativity, ingenuity, and logistic skills. The effort put into organising such a beautiful, colourful, and above all, safe fundraiser is huge, and we applaud you for it. We play till the end of the time – and one day longer. You rock!

The GOCC Foundation has equipped 108 wards with 198 pieces of equipment purchased for the sum of 11 822446, 27 PLN. We purchased 122 ophthalmoscopes and 24 photocoagulators. By the end of November 2015, 193 094 preemies have had their eyes tested, and 7 950 laser procedures have been performed. Thanks to this programme we saved eyesight of 7 609 babies. http://en.wosp.org.pl/medical * as of February 2016

PROGRAM PRZESIEWOWYCH BADAŃ SŁUCHU PRZEBADALIŚMY JUŻ PONAD 5 MILIONÓW NOWORODKÓW

.

Nationwide Programme for New-born Hearing Screening The aim of this programme is to ensure that each child born in Poland has had its hearing tested – we have recently reached the magical mark of 5 million screenings*! We estimate that daily around 1 000 tests are performed, and thanks to the programme we have been able to diagnose hearing defects in 13 525 children, enabling doctors to begin treatment as soon as possible.

PROGRAM NIEINWAZYJNEGO WSPOMAGANIA ODDECHU U NOWORODKÓW

Nationwide Programme for Non-invasive Respiratory Support of New-borns - Infant Flow The aim of this programme is to limit the number of cases of mechanical ventilation in neonates and premature babies, because this procedure often leads to acute lung complications. What we offer instead is a modern and non-invasive method, which does not require intubation. We have purchased 464 Infant Flow devices, 50 oscillating respirators, 140 mobile SiPAP stands, and 32 respirators with automatic oxygen supply control for 245 neonate units all over Poland http://en.wosp.org.pl/medical * as of October 2016

NARODOWY PROGRAM WCZESNEJ DIAGNOSTYKI ONKOLOGICZNEJ DZIECI

Nationwide Programme for Early Cancer Diagnosis in Children The aim of this programme is to develop a nationwide system for cancer diagnostics for children. It has enabled us to establish 80 cancer diagnostic centres all over Poland, equip children’s oncology clinics with state-of-the-art medical devices, buy 80 high-class ultrasound scanners and 6 CAT scanners. In 2016 we have purchased devices for 2.5 million PLN in order to launch and equip a Central Laboratory of Genetic Research in Children’s Oncology – ONCOLAB in Łódź. http://en.wosp.org.pl//our_programs/early_cancer_diagnostics

OGÓLNOPOLSKI PROGRAM LECZENIA POMPAMI INSULINOWYMI DZIECI Z CUKRZYCĄ

Nationwide Programme for Personal Insulin Pump Treatment of Children with Diabetes The aim of the programme was to provide diabetic children with insulin pumps in order to give them a chance for an easier and more active life despite their illness. Once it has been correctly programmed, a personal insulin pump automatically delivers the necessary amount of insulin. The programme, run between the years 2001 and 2008, enabled us to buy a total of 2978 personal insulin pumps for children under 10 years of age.

PROGRAM LECZENIA OSOBISTYMI POMPAMI INSULINOWYMI DLA KOBIET CIĘŻARNYCH Z CUKRZYCĄ

Nationwide Programme for Personal Insulin Pump Pump Treatment of Pregnant Women with Diabetes The aim of this programme is to make personal insulin pumps available to women with diabetes. The use of an insulin pump by a pregnant diabetic woman substantially lowers the risk of miscarriage, innate defects and medical complications during pregnancy and childbirth. It bolsters the correct development of the baby and improves the physical and mental state of the mother. Loan of the personal insulin pump is free of charge. Between 2004 and the end of November 2015, our pumps were rented to a total of 2 485 women. http://en.wosp.org.pl/medical

Providing dignified medical care for the elderly When you consider the demand for good geriatric care, you begin to see that there are still too few geriatric wards in Poland. It is, without doubt, one of the most neglected branches of medicine in the country. Still, there is hope that thanks to the buzz surrounding the Grand Finale fundraisers, we have been able to draw public attention to this matter. It is high time we came face to face with the truth: due to the changing social model of family and systematic ageing of the population of the country there is a huge need to establish a system ensuring that the dignity and respect for the elderly patients go hand in hand with modern methods of treatment.

We decided to come to aid of geriatric units and help long-term care wards, where patients usually remain for an indeterminate time. As soon as we began to explore the needs of these units, we realized that they are in dire need of the most basic pieces of equipment.

All in all, in the span of four years, we have purchased equipment for 70 917 586, 98 PLN, providing much-needed support to 242 wards across the country.

GOCC Library These handbooks include latest research and contemporary standards and developments in medicine. The handbooks are especially useful for students of medicine and medical professionals looking to extend their knowledge in the field.

Get Even with Diabetes Diabetes is a global problem. The illness strikes regardless of age, sex, race, nation or religion. Get Even with Diabetes is a campaign aimed at raising awareness of diabetes in Polish society. We support effective treatment for patients with diabetes, promote healthy lifestyle as a way of preventing the illness, and fight against prejudice and intolerance of diabetics. One of the key elements of Get Even with Diabetes initiative is encouraging leading food producers in Poland to place important information on carbohydrate, protein and fat exchange units on the labels of their products. The data helps diabetics quickly assess the amount of insulin which should be taken after the meal and avoid dangerous dosage errors. Look for products with the Get Even with Diabetes logo!

Get Even with Diabetes Run In aid of raising diabetes awareness and showing our support to those afflicted by the disease, we hold a run dedicated to diabetes on the day of the Grand Finale. Let’s show our solidarity and fight the stigma surrounding the disease. The event is held in towns across the country, promoting healthy lifestyle and bringing people together in the same time. Since the 8th edition of the run in 2014, the GOCC Foundation has been charging each runner a participation fee. All the funds raised this way are used to purchase personal insulin pumps, which are rented free of charge to pregnant women with diabetes.

Individual Help So far, we have managed to help 4 361 children by purchasing the equipment worth 17 583 750, 52 PLN*. The main objective of the Great Orchestra of Christmas Charity foundation is to purchase medical equipment for Polish public hospitals. However, due to an overwhelming amount of personal requests for help, we have decided to launch the programme addressing the needs of children who undergo rehabilitation in their homes. We are unable to finance operations or treatments of adults or children – the money raised during the Grand Finale can be allotted only for the purchase of equipment necessary for rehabilitation at home. *as of October 2016

13

Educational Programme CPR for Schools In 2006 we have decided to launch a programme aiming to teach CPR and first aid to children in primary schools in Poland. We wanted to establish a consistent CPR training programme in primary schools in the country. By introducing children to CPR we foster good habits from very early age. Helping others becomes their second nature! So far, we have trained 27 701 teachers, who then taught CPR lessons in 12 216 schools, introducing CPR to around 2.5 million children. It means that the programme has reached 91% schools in the country.

Courses and Teaching Aids are free!

We rely on Peace Patrol volunteers, who have been certified by AHA, to provide courses to teachers. The foundation is the first NGO in Poland which has been authorised to use AHA system to teach CPR.

Check if first aid is taught at your child’s school Use a simple search tool to check if your child’s school is a part of the CPR for schools programme. You can check if there are teachers trained to teach CPR, and if the school has received all necessary teaching aids. www.szkoly.wosp.org.pl (Polish only)

14

Poland Knows how to Save Lives! Children who have learned how to save lives are very proud of their skills, and they enjoy sharing this knowledge with adults, taking up the roles of teachers and instructors. A lot of schools take inspiration from our programme to spread knowledge about CPR in their communities. We encourage everyone to get involved in World First Aid Day and raise awareness. We also encourage schools to take up action and participate in our annual CPR for Schools Competition. European Restart a Heart Day is a great opportunity to promote CPR and first aid to the community, and that’s why we have decided to set ourselves a challenge – we come together to set new records. In 2013 there has been 83 111 participants attempting to set a new record. In 2015, there has been 92 049 people taking part in our attempt. This year, we have received an estimate of 85 000 participants! www.rekord.wosp.org.pl

Encourage your local schools to join the program CPR for Schools is a programme dedicated to children from grades 1-3. Limited resources make us unable to provide CPR courses to children in other age groups. However, if you wish to organize a course for older children, you can always get in touch with a school, which has been participating in our programme. They have both the skills and the equipment. We know that many schools are happy to help, and seeing a community come together is extremely rewarding!

The Peace Patrol is a community of volunteers, who come together to take part in different events organised by the foundation. These amazing people are present during the Grand Finales, and they help, taking up different roles, during Woodstock Festival Poland. Each member of the Peace Patrol has completed a first aid, survival, and team-building course at our training centre at Szadowo-Młyn. They are prepared to work as event stewards and assist in organization of the largest open-air music festival in Europe. Peace Patrol is not only about first aid training and survival exercises – often it is the beginning of amazing adventures and a is, first and foremost, a close-knit community of people spanning continents and generations.

All volunteers who have completed the course meet at Woodstock Festival Poland. The work of the Patrollers is one of the main reasons why professional services cite the festival as be the most professionally organized and safest mass event in Poland. We all work hard to retain its unique character best summarized by our mottos: Love! Friendship! Music! Rock & roll! Stop violence! Stop drugs!

Tell the Teddy Bear’s Tale Teddy’s tale is not a happy story… The GOCC foundation has been working tirelessly to improve the situation in paediatric wards across the country, and we decided that it is the time to make a stand against violence towards children. We refuse to close our eyes to the fact that there are still little patients who land in hospitals, because they are the victims of violence or abuse. We launched a public awareness initiative. By displaying plush toys sporting signs of violence and abuse, we draw attention to the smallest victims. Listen to the Teddy Bears heartfelt plea - don’t hurt me, love me! You too can join our action and take a stand against violence. You need to: 1. Gather some soft toys. 2. Bandage them. 3. Go to our website to download and print of paper slips saying ‘Don’t Hit Me, Love Me’ and attach them to toys. 4. Make sure to leave the soft toys in various public locations across town. 5. Take some photographs and post them online using #HistoriaMisia #TeddysTale. 6. Go to our website to download and print thank-you cards and give them to all those who helped you. Tips:

* Why not involve local children in the action? Encourage them to colour slips of paper and raise awareness of the problem. * If you are a teacher, scouter, or a tutor, consider organizing a lesson about children’s rights.

We encourage Woodstockians to travel to our festival by public transport rather than cars and to carpool. We take part in workshops and conferences, learning about how to be more eco-friendly. Of course, we understand that no festival can be truly environmentally friendly without the help from its participants. We are slowly but surely introducing more and more people to the green way of life.

What else do we do? The GOCC Foundation often engages in different projects and initiatives In 1997, we prepared 156 aid transports to help those who suffered when a huge flood hit southern part of Poland. In 1999, we organized a transport of medicines for refugees in Albania. In 2005, we provided help to tsunami victims in Sri Lanka by equipping five hospitals all over the island. In 2006, after a roof of an exhibition hall in Katowice collapsed we bought equipment for 5 specialist fire brigade rescue units.

In 2010, a mini-Finale fundraiser with the motto Stop Floods, helped us raise 2 655 507, 34 PLN and buy equipment for 564 units of voluntary fire brigade. In 2012, we donated 100 Braille typewriters for visually impaired children to a missionary centre run by Sisters of St. Francis in Kibeho, Rwanda.

In 2013, the first edition of our conference ‘Now! Go Online’ helped us meet other non-governmental organizations and discus how the Internet and new technologies can be used in our everyday work.

In 2014, we invited other NGOs to workshops and started a series of meetings at which we try to combine the work of an NGO with the online world and inspire others to engage in joint projects. The year 2014 also saw the opening of the portal ‘Good Site of the Internet’ (www.dobrastronainternetu.pl) dedicated to show the good practices of NGOs online.

How to help our cause? If you wish to support the GOCC Foundation from abroad you can make a donation

SWIFT: PKO PPL PW IBAN: PL 58 1240 1112 1111 0010 0944 9739 For more ways of supporting we go to: http://en.wosp.org.pl/the_grand_finale/how_to_help Each and every donation helps us to keep going – keep working to improve the level of medical care offered in public hospitals in Poland.

1% of tax A special financial instrument in the Polish taxation system allows taxpayers to donate 1% of their income tax to NGO of their choice. Only NGOs registered as public benefit organizations can be beneficiaries of the 1% of tax system. Thanks to the generous support of the taxpayers, the GOCC Foundation was able to: Buy an MRI scanner for the Children’s Memorial Health Institute in Warsaw; Equip 220 public institutions with AEDs; Run the CPR for Schools Programme; Support the National Programme for Personal Insulin Pump Treatment of Diabetic Children; Support children’s hospices in Poland and purchase a fleet of cars and equipment needed in palliative care, including aspirators, inhalators, oxygen concentrators, cough assistors, syringe pumps, pulse oximeters and rehabilitation beds with anti-sore mattresses; Buy additional equipment for the National Early Cancer Diagnostics in Children programme in order to improve health care standard in all children’s oncological units in Poland. This year we asked the public to donate 1% of their tax in support of two causes - we want to purchase equipment such as external fixation devices used in orthopaedic surgery to lengthen, reshape limb bones and to treat complex bone fractures, as well as to finance the purchase a fleet of cars to be used by children’s hospices nationwide.

We have raised 4 157 390, 67*! * as of October 2016

Finances How do we raise money? Once in a year, the Great Orchestra of Christmas Charity Foundation holds a huge one-day fundraiser called the Grand Finale. On this day, 120 000 volunteers coordinated by 1 600 Collection Centres take to the streets to raise money. There are online auctions and fundraisers, concerts, fire shows, workshops, and much more – with proceeds all going to support medicine in Poland. We also rely on individual donations, 1% charitable tax deductions and corporate support. We devote 92% of the funds collected at the public fundraiser to purchase of medical equipment and financing medical and educational programmes. The remaining 8% are used to cover our administrative costs. This data is taken from the report produced by the Ministry of Labour and Social Affairs. The spokesperson for the Ministry has added that this result represents the best result of relationship of costs vs donations represented by an NGO in the country.

How do we account for the raised money? It takes us about 1, 5 years to account for the Grand Finale. This much time is needed to identify the needs of hospitals, purchase the equipment, sign and fulfil the contracts, transport the equipment to hospitals and train the medical personnel in using the equipment.

We are always very precise, transparent and honest. Every year, our work is audited by independent companies. The GOCC Foundation creates scrupulous reports and sends them to all public bodies supervising our work: Ministry of Health, Ministry of the Interior and Administration, Ministry of Labour and Social Policy, Ministry of Finance and the relevant Tax Office. Detailed financial accounts and reports of the Foundation and the reports from individual Grand Finale fundraisers are available on our websites.

26

How do we buy the equipment? It might sound simple, but the key to successful equipment purchase is defining the need of public hospitals, consulting experts, and purchasing the best equipment - at the best price! We aim to support public medicine in Poland and we do so by purchasing equipment at an open tender, which is held after we receive requests for equipment from the hospitals across the country. This allows us to purchase devices which will be used to the full extent of their capacity and just the devices which are necessary.

Check where your money went From the very beginning of its existence, The Great Orchestra of Christmas Charity Foundation has put much effort in creating honest financial reports from each Grand Finale fundraiser. With the support of The Story Company we came up with a website where you can check exactly how your donation contributed to the cause. More information here: www.nacoidamojepieniadze.wosp.org.pl

Złoty Melon is a company established and wholly owned by the GOCC Foundation. It was established in order to clearly separate charitable activity from business activity. The aim of Złoty Melon is to support the GOCC Foundation in its work, help it optimize costs and obtain extra funding. Quite importantly, the Foundation is eligible to tax deductions which would not be possible if it operated as a business entity directly rather than through a company it owns. Złoty Melon: Sells products marked with the logo of the Great Orchestra of Christmas Charity Foundation. The proceeds from the sales is used to pursue the statutory aims of the GOCC Foundation; Organizes events, including Woodstock Festival Poland, one of the biggest open-air festival in Europe; Has released more live DVDs with Polish rock music than any other publisher; Is actively involved in the organization of the Grand Finale fundraisers; In cooperation with the GOCC Foundation and certified American Heart Association instructors organizes training in CPR, first aid and AED use; As of today, Złoty Melon has transferred to the GOCC Foundation 3 125 227, 04 PLN by way of dividend. The commercial activity of Złoty Melon supports the running and operations of Szadowo Młyn training centre, where over 9 000 people have learned CPR and first aid! Złoty Melon is a limited liability company. It works independently, pays taxes, cover social insurance contributions and operates legally – just like any other limited liability company in Poland. It employs 7 people. As a company owned by the GOCC Foundation, Złoty Melon also provides services to the Foundations partners and sponsors. These are mostly companies which help us organize the two most important projects: the Grand Finale and Woodstock Festival Poland. Profits generated this way are used to carry out projects undertaken by the Foundation while substantially lowering the Foundations costs.

FAQ: How do I get in touch with you? Our offices are located in Warsaw, on Dominikańska 19C. You can find our address and telephone number on our site. You can also reach out to us via our social media channels.

Have you got local chapters of foundation? No, we are based only in Warsaw, and we don’t have any local chapters. The Collection Centres, which coordinate the Grand Finale fundraising drives are not local representatives or chapters of the GOCC foundation. Collection Centres allow people, who would like to support the case to come together, but they do not work for us, per se.

Do you work all year round? The foundation works all year round, coordinating equipment purchases and medical and educational programmes, which are active in hospitals in the country.

How can I support your work? There’s something that everyone can do to help us! You can pitch in by helping us organise the massive events such as the Grand Finale or Woodstock Festival Poland. If you don’t have enough time to volunteer, but you would still like to contribute, you can make a donation on our website.

Under what conditions is equipment donated to hospitals? Each piece of equipment is passed to the hospital as a donation. As a granter, the foundation reserves the right to control the beneficiary (according to the provision in the donation contract). What’s more, according to the Article 893 of the Code of Civil Proceedings, the beneficiary is obliged to comply with a series of demands. A complete list of demands, which apply in this case can be found on our website.

How can a hospital apply for equipment? A request for the equipment, which should comply with the theme of the current Grand Finale or should function within the framework of one of the medical programmes, has to be sent to the GOCC office by mail. The official document should bear a signature of the director of the hospital. The request for the equipment should be appropriately motivated: it has to contain the description of the state of the hospital and it should list the equipment. It needs to state the amount of patients, who receive treatment and list all reasons why the hospital needs to receive the support. The request should clearly state the hospital’s legal entity form. We also need to receive documents which prove that the hospital is offering treatment free of charge, within the framework of publicly available medical care.
