General News BDA INVESTIGATING LEGAL OPTIONS TO CHALLENGE FD PAY CUT

T

he British Dental Association (BDA) condemns Government plans to impose a pay cut on foundation dentists (FDs) of more than £2,000 from September, and will be exploring all legal avenues to challenge this decision. Following a consultation on the issue, the Department of Health has confirmed that the pay cut will apply to FDs in England. It is also expected to apply to trainees in Wales and Northern Ireland.

The BDA believes that targeting young dentists with a pay cut of 8% at the start of their careers is a cynical move that exploits the most vulnerable members of the profession. The attack on FD salaries is expected to generate minimal savings for the NHS relative to the potential hardship it creates for dental graduates who are emerging from universities with an average debt of around £25,000. The BDA opposes any pay cut to FD salaries. We also believe that current final-year students have been placed in an intolerable situation where they will be forced to accept a financial arrangement that they were not aware of when they first applied. The Department of Health’s justification that the pay cut brings dental salaries in line with their medical equivalents simply does not add up, the

BDA believes. With the opportunity to earn additional NHS income, medical trainees at the same point in their career earn on average nearly £40,000, compared to this new salary of £28,076 for foundation dentists. 
 Chair of the British Dental Association’s Principal Executive Committee, Mick Armstrong, said: “We have made clear to the Department of Health on numerous occasions that seeking efficiency savings from FDs is inappropriate, and unfairly penalises young dentists who are not in a position to resist this pressure. “This is a view that is widely shared by the profession as we know from the 6,700 signatories who responded to our petition opposing these cuts. “We are both angry and disappointed over the failure to grasp the strength of feeling against these cuts,

which frankly many see as an attempt to prey on the most vulnerable members of the profession. “It is also unacceptable that these young dentists have been given ridiculously short notice of this change. “We will be seeking expert legal opinion to explore options to challenge this decision.”
 The BDA’s campaign to oppose the cuts has won strong support on Twitter, where the BDA is encouraging tweets using the hashtag #DFTPay, and on the BDA’s Facebook page. entists spend a lot of time considering which new suction or compressed air system is the best for their surgery. They look at a myriad of factors: price, reliability, size and space and yet one major factor that is often overlooked is noise. These suction and compression systems are a fundamental instrument that keeps your practice running on a day to day basis, with this in mind you are unlikely to want a piece of equipment that resembles the sounds levels of a pneumatic drill. ental decay is the world’s most common disease and the single biggest cause of oral pain, with over four billion people suffering from it. Experiencing pain, problems with communicating, eating and smiling due to dental decay leave people ostracised from communities, unable to work or attend school. Bridge2Aid has developed a model that provides training for local Health

Workers in emergency dental care, and has been supporting the government of Tanzania as they pioneer its execution in 5 regions of the country. The method has been proven to be effective and has resulted in millions of people having access to safe emergency treatment in their own community. For them, a life free of dental pain is now a real possibility. ‘Sarapion was trained on our Kagera programme in Karagwe district.

He works from Kakanja Dispensary and has been a Clinical Officer for seventeen years. Before participating in our dental training programme, Sarapion estimated he used to see approximately 120 patients per month of whom 10% were complaining of oral pain. He had to refer all of these patients to the district dental facilities, a two hour bus ride away which many of them did not end up making. After six days of practical training, Sarapion felt confident in administering infiltration and inferior dental nerve block injections safely. He was able to demonstrate a good knowledge and understanding of the cause of oral disease and basic principle of oral health and felt confident in carrying out safe and complete extraction techniques for adults and children. Within the three months after training, Sarapion had successfully treated 28 dental patients. 71% had received an extraction, 29% received antibiotics only and none were referred. After six months, Sarapion had treated another 49 patients who were suffering from dental pain. 69% of these patients received an extraction, 31% received antibiotics and no patients required referral.’ Bridge2Aid will once again be attending The Dentistry Show in 2015

Resource Centre NEW RESEARCH TO IMPROVE QUALITY OF LIFE FOR DENTURE PATIENTS

T

he University of Manchester is leading a first of its kind trial into the quality of life of people wearing dental implants. Around 3.7 million people in Britain have no natural teeth and many find it difficult to wear lower dentures, causing problems in chewing and speaking that have a significant effect on their quality of life. Most of those who have lost their teeth are over the age of 65, and the impact of ill-fitting dentures on their lives is so severe that the World Health Organization has classified individuals with such difficulties as disabled. One solution to this problem is to fit titanium implants into the lower jaw to hold dentures in place, but this requires significant surgery, which not all patients are able to undergo. An alternative for these patients is to opt for newly developed mini-implants, which are less traumatic to put in place. The University is working with Central Manchester University Hospitals

NHS Foundation Trust (CMFT) to lead the trial which is the first of its kind to provide a cost-benefit analysis and undertake a robust assessment of the impact of mini implants on people’s quality of life, compared with dentures secured by conventional implants. Managed by Manchester Academic Health Science Centre (MAHSC), a partnership between the University and seven leading NHS partners, the trial is funded by the National Institute for Health Research (NIHR) Research for Patient Benefit Programme. Professor Martin Tickle, a Senior Research Professor in the School of Dentistry in the Faculty of Medical and Human Sciences, who leads on population health for MAHSC, said: “This pilot trial is one step towards MAHSC’s aim of improving quality of life and it is of particular importance for an ageing population. “We hope the results of this study will inform a much larger trial that we hope to roll-out across the UK.”

The trial has recently started and so far, 16 patients have been recruited. Overall, 44 participants will be selected to take part, 22 of which will receive conventional implants, while the others will be provided with mini implants. Over the course of six months, the researchers will follow-up with the patients to collect information on their experiences, including the level of pain they have encountered, their ability to chew, and the overall impact of treatment on their quality of life. “When dentists try to help people whose lives are severely affected by denture problems, it is of critical importance that they know which product is most effective and best suited to the individual patient before treatment goes ahead,” Dr Craig Barclay, Consultant at The University Dental Hospital of Manchester, part of CMFT, and Honorary Senior Lecturer in Restorative Dentistry at The University of Manchester, said. “We hope that the study will improve quality of life for patients with

dentures, by enabling clinicians to provide the most appropriate treatment.” MAHSC is one of only six UK academic and health partnerships currently designated as an Academic Health Science Centre (AHSC) – a mark of international excellence in healthcare research – and the only one north of Cambridge. Its role is to join world-leading academics with NHS Trusts and Clinical Commissioning Groups in order to ensure the best ideas emerging from the scientific community are translated into standard healthcare to benefit patients more quickly and more effectively. Rushing into a treatment plan, or sometimes even just a straightforward extraction with a patient you don’t know, can lead to unforeseen misunderstandings. 2. Spend time on your notes and your discussions with patients.Time spent discussing treatment plans, risks, options, costs, may seem like time wasted not doing dentistry- it never is. It is an investment in gaining a satisfied patient. Time spent on detailed notes may save you hours of stress (and possibly a legal case) if the patient is not satisfied. Many cases are indefensible simply because the notes were inadequate to prove that the dentist did do what he said he did. 3. Watch out for the occlusion - beware of wear. The signs of clenching and grinding are

commonplace - but you ignore them at your peril. With patients retaining their teeth, and having high expectations of porcelain and composite restorations, and endodontically treated teethbe realistic about the forces that will be incurred in function, warn patients clearly about their habits, and recommend a night guard if appropriate. Acid burns- phosphoric acid causes nasty burns if inadvertently left in contact with skin or mucosa. Watch out for acid etch seeping under the rubber dam during rinsing, or getting on gloves, instruments or the suction, and being smeared onto the cheek or lips whilst you are working. 4. Keep on training - maximise your learning to improve your skillset. Your BDS is just the first step in lifelong learning- that’s why we call it ‘practice’!
