Treadmill versus Overground Running:

Is one more beneficial than the other? Thurmond Lanier DPM, MPH | Podiatry Aurora Health Center

Distance running is a popular activity for recreational and competitive athletes. When the weather is not the nicest many runners rely on the treadmill for exercise. The question arises is running on a moving belt equivalent or even better than running on the ground and is there an increase risk of injury with one way or the other. There are many studies comparing two methods of training. Here is a quick summary of the findings. Major Similarities of treadmill and overground running: • Motion of the hip, knee, and ankle are similar so there is no increase risk on either surface for injury at those joints. • Energy expenditure was relatively the same when comparing the two. This means both workouts will give you the equivalent cardiovascular workout.

Major Differences • Treadmill is monotonous event with very little variation with each step as overground running has significant variations throughout the run cycle. • Significantly higher stress/ strain rates in overground running that have the possibility to lead to stress fractures but can also strengthen tibial bone • When fatigued on treadmill flight time increases (time when both feet are off the belt) and strides stay the same. On treadmill, stride decreases as runners fatigue. • Pressure under the arch and

toes were higher on the treadmill but lower under the heel and great toe Bottom Line: Treadmill offers runners a way to stay in shape when conditions outside may not be the best. For the casual runner with no significant foot issues, the treadmill is equivalent to overground running and is a great alternative. The treadmill will allow a great workout in a controlled environment. The treadmill does increase the pressure in certain places in the foot so for runners who suffer with plantar fasciitis, hammertoes, flat feet or metatarsalgia (pain under the balls of the feet) overground running may be a better option.

Runners with bunions, arthritis in the great toe joint, or pain under the heel may favor the treadmill as there is less impact on these areas of the foot with treadmill running. For the competitive athlete, running technique is changed when comparing the two as evidenced by increased flight time on the treadmill. Ideally the treadmill should only be used when the competitive surface is unavailable for the athlete. Changes

Stress Lindsay Davel, APNP | Aurora Behavioral Health Aurora Health Center

Stress is your body’s reaction to change. It is, however our body’s reaction to change within our physical, mental and emotional well-being that can cause the stress to be either positive or negative. Stress is a normal part of life and many things happen to us daily that put stress on our body and mind. So, it is up to us to make sure we are reacting to the stress in the best way possible and that we are also healthy in mind, body and spirit to be better prepared for the stress that comes along each and every day. When not handled properly, stress can have negative effects on your body including: increase heartbeat, headaches, still neck and/or tight shoulders, back pain, fast breathing, increased sweating, and upset stomach. There are two types of stress, acute stress and chronic

stress. Acute, or short-term stress, is the body’s instant response to any situation that seems demanding or dangerous. Chronic stress, or long-term stress, is caused by stressful situations or events that last over a long period of time such as a difficult job or a health condition. Most of the time, our bodies will recover quickly from acute stress, however stress can cause problems if it happens too often or if we don’t have enough time to recover. Unfortunately, there are side effects to stress that are harder to manage such as weight gain, insomnia, depression, memory loss

and reduced concentration. So, how can we better manage our stress? The Mayo Clinic recommends the following strategies for stress management: physical activity, relaxation techniques, meditation, Yoga, and Tai chi. Also, be sure to get plenty of sleep, eat a balanced diet, and avoid tobacco use and excess caffeine and alcohol intake. Exercise, in any form, is a wonderful stress reliever. Exercise pumps up your endorphins (your body’s feel-good neurotransmitters) and gives you a natural “high” for your entire day. Exercise also helps you forget daily irritations

and allows you to focus on your body’s movement versus the stress of the day. Regular exercise can also increase self-confidence and decrease symptoms associated with anxiety and depression, improve sleep, and improve your mood. Dr. Herbert Benson identified methods that anyone could use to activate what he called “The Relaxation Response”. These methods include: abdominal breathing, progressive muscle relaxation, body scan exercise, stretching, listening, focusing, meditation, visualization, deep reCONTINUED on PAGE 9

CONTINUED from PAGE 8 laxation, and vigorous exercise. If one of these techniques works for you and you begin to use it on a regular basis, it could be life changing. Lindsay Davel, APNP, Aurora Behavioral Health says there are five points to remember when learning how to cope with stress: 1) Take Control of Your Stress. Davel says, “Give yourself a time out if that’s what your mind and body need.” 2) Prevent Self-Sabotage. “Don’t overuse caffeine, get plenty of sleep, don’t use alcohol and drugs to escape stress and monitor stress eating,” says Davel. 3) Increase Activity Level. “Choose activities that fit your body and lifestyle, use exercise sessions as “me” time,” adds Daval. 4) Relax with a 5-Second Deep Breath. “Try taking one minute worth of 5-second breaths and repeat it multiple times per day for maximum effect,” explains Daval. 5) Create a Positive Mind Set. “This means trying to use realistic thinking and let go of the uncontrollable,” explains Daval. “Take

time out for yourself, seek out friends and above all, laugh!” According to the American Psychological Association, the top two sources of stress are job stress and financial stress. The Occupational Safety and Health Administration (OSHA) declared stress a hazard of the workplace costing American industry more than $300 billion annually. Here are some tips to reduce work related stress: have a “to-do” list, be helpful to your coworkers, develop a genuine friendship with your boss, be on time and prepared for meetings and appointments, focus on actionable items, delegate work, reward yourself, listen to your favorite music as you work, be the master of your time, focus on what you love about your job, and unwind after a day of hard work. Here are a few strategies developed by Dave Ramsey each year to teach community

members about financial stress management; the seven baby steps of this program include: 1) Start an emergency fund 2) Pay off all debt 3) Put three to six months of expenses into a savings account 4) Invest 15% of household income into pre-tax retirement 5) Begin a college fund for your children 6) Pay off your home early 7) Build wealth and give. If you feel like you could benefit from a stress management program, contact Aurora Behavioral Health services at the Aurora Health Center at (920)907-7000.

Pregnancy

Pre Pregnancy Health Helps Ensure a Health Pregnancy Dr. Rob Southwick | OB-GYN Aurora Health Center

Almost everyone realizes the importance of prenatal medical care to assure the health of a pregnant woman and her baby. If you decide to see your health care provider even before you become pregnant, he or she can offer advice about exercise, eating and lifestyle habits that can increase your chances of becoming pregnant and may reduce the risk of miscarriage or birth defects. Women who are considering getting pregnant often ask if there is anything they should be doing (or avoid doing) before they become pregnant. Here are some suggestions I routinely share with women who plan to conceive in the near future. • If you are thinking about becoming pregnant, you should take 0.4 mg of folic acid every day. Folic acid, which is found in

leafy green vegetables and in fortified flour and rice products, has been shown to lower the risk of certain birth defects called neural tube defects. In addition to eating a well balanced diet, it’s a good idea to take a multivitamin with folic acid daily for three months before pregnancy and at least three months into pregnancy. If there is a history of neural tube defects, then women should be placed on a higher dose of folic acid. • If you need to update your vaccines, plan to do so at least one month before you try getting pregnant. vaccines such as the MMR (measles-mumps-rubella) and others can increase the risk of birth defects. • Avoid drugs and alcohol. You should ask your health care provider about taking any medications (both prescription and over-the-counter drugs) as well as any herbal supplements. • Stop smoking. Smoking can make it harder to get pregnant, and it poses risks to the fetus. • Eat well and exercise. Being either significantly over- or underweight can increase risks during pregnancy. Most

women who are trying to conceive are encouraged to pursue moderate, but not extremely strenuous, exercise. Even women who ovulate regularly may face a higher risk of miscarriage if they engage in strenuous exercise, such as marathon training. Your doctor may also recommend that you avoid eating certain types of fish during pregnancy that could contain high levels of mercury that can cause problems. • Carefully manage any medical conditions you may have. If you are being treated for a CONTINUED on PAGE 14

CONTINUED from PAGE 10 pre-existing medical condition (asthma, diabetes, hypertension) it’s important to obtain medical advise before getting pregnant. Not only is it imperative to keep these illnesses under control during pregnancy, but some common medications used to treat these conditions may have an adverse affect on your pregnancy. If this is the case, your health care provider may be able to suggest a suitable substitute. • Visit your dentist. Research has shown that periodontal gum disease — an infection of the gums caused by plaque — is especially common in pregnant women and is related to problems during pregnancy, such as premature labor and low birth weight. Even if you are healthy, it’s a good idea to make an appointment to see your health care provider for a complete medical check-up, before you attempt to get pregnant. For more information or to schedule an appointment with Dr. Southwick or any of our Women’s Health Providers please call the Aurora Health Center at (920) 907-7000 or www. aurora.org.

Bladder or bowel control

Incontinence Tiffany Fisher dpt | Physical Therapist Aurora Health Center

Incontinence, the loss of bladder or bowel control, is not a disease but a symptom of other underlying disorders. Women of all ages, whether they have borne children or not, experience incontinence. Incontinence affects a person’s self-esteem, their emotional well-being and their independence. Pelvic pain that makes it difficult to sit, causes pain with intercourse, or pain in the hips, lower abdomen or lower back also affects many women. These problems may be caused by spasms of the muscles that support the bladder, uterus and rectum and are a symptom of muscle joint or nerve problems. Unfortunately, many women suffer in silence because they are too embarrassed to discuss these issues with their health care provider or are fearful that surgery is the

• Urinary or vaginal infections

only treatment option. The good news is that the majority of women can be successfully treated using non-surgical techniques. What factors contribute to incontinence and pelvic pain?

• Smoking/chronic coughing • Straining with the evacuation of stool • Chronic constipation

• Pregnancy and childbirth • Pelvic injury or surgery • Weakened sphincter muscles • Neurological conditions • Pelvic organ prolapse • High impact physical activities • Menopause • Obesity

Pamper yourself at our booth at the Women’s Expo!

Your physical therapist will develop a plan that addresses your specific needs and may use exercise, relaxation, fluid/ food intake, manual techniques and/or biomechanics/ biofeedback as non-invasive treat-

ment options. If it’s determined that surgery is needed for your treatment, a variety of procedures are available by board-certified gynecologists and urologists who will discuss other treatment options with you. To schedule an appointment with Tiffany Fisher, please call the Aurora Health Center at 920-907-7000 or go online at www.AuroraHealthCare.org for more information.

CVS

5 things you can do to prevent computer vision syndrome Dr. Rob Southwick | OB-GYN Aurora Health Center

Spend a lot of time in front of a computer? You may suffer from CVS – short for Computer Vision Syndrome. The symptoms are probably familiar: headaches, blurred vision, eyestrain, dry eyes, and neck and shoulder pain. The more you stare at a screen, the more likely you’ll have CVS symptoms. Especially if you already have vision problems: farsightedness, aging eyes with cataracts or “presbyopia” (the reason the 40+ crowd reaches for their reading glasses), astigmatism or trouble focusing. Poor lighting, screen glare, poor posture or sitting too close or too far from your monitor can also make symptoms worse. The problem with computer screens Computer viewing is demanding on your

eyes. The distance and viewing angle between your eyes and monitor aren’t the same as reading a magazine or book. Pixilated print is typically not as sharp as type in printed materials. And screens can cause further strain due to reduced contrast, flashing, light reflections or glare. If you wear eyeglasses or contact lenses, your prescription was likely calibrated to read a chart 20 feet away, not a computer screen 20 inches in front of you. Adjusting to the shorter distance can put strain on your eyes. If you have multifocals, you may tilt your head

forward or backward to see your computer screen clearly.

Use low wattage light bulbs in desk lamps. (If you can’t stop the glare, get a screen glare filter to decrease the light reflected from your screen.)

Avoiding computer vision syndrome The cure for CVS is simple: get away from your computer. But if you must spend hours staring at a screen, symptoms will continue to recur and may worsen over time. If CVS has become “a pain in your neck,” here are some things to consider: 1. Lighting – Make sure overhead lighting, desk lamps or light from windows is not causing glare on your screen.

2. Your chair and posture – Select a chair with back support that allows you to sit up straight comfortably. Your feet should rest flat on the floor. Chair arms should support your arms while you are typing. Never rest your wrists on the keyboard CONTINUED on PAGE 19

AUTUMN

Allergy Signs and Symptoms Anita Gheller-Rigoni, DO | Allergist/Immunologist Aurora Health Center- Fond du Lac

The leaves will be changing, and the wind will blowing….and you will be miserable. Get out your handkerchief, because seasonal allergies are here, and if you suffer from “seasonal allergies”, you may notice that your symptoms are present through most of the year. And you’re not alone. Over 50 million people in the United States suffer from allergies, costing $10 billion dollars annually in the United States! Of these, about 35 million have allergic rhinitis and 15 million carry the diagnosis of asthma. Allergic rhinitis is the most common of the allergic diseases and can be both seasonal and perennial (throughout the year). Symptoms of seasonal allergies result from pollen in the air. In the Midwest, if you start sneezing in midMarch, it’s likely that trees are your foe. Grasses

pollinate from early May through most of July, weeds from mid-August to late October and outdoor molds from early September to the first frost. If your symptoms persist, even in the winter, then you may have perennial rhinitis. Perennial allergens include animal dander, dust mites and indoor molds. Signs and symptoms that indicate you may benefit from seeing an allergist are: • Nasal symptoms: congestion, runny nose, itching, sneezing, decreased sense of smell, frequent sinus infec-

tions • Eye symptoms: redness, itching, tearing, sensation of something in your eyes • Respiratory symptoms: frequent cough, especially at night or with exercise • Dry, itchy skin and/or hives • Stinging insect reactions: Symptoms such as breathing difficulty, loss of consciousness, swelling of throat • Recurrent infections Dr. Anita Gheller-Rigoni is an allergist/immunologist. An allergist/

immunologist is a physician who has completed two to three years of specialized training in addition to three years of training in either internal medicine and/or pediatrics. After this training, an allergist/immunologist can become “board certified”, in this specialty and helps to diagnose and treat both children and adults with allergies, asthma and immune deficiencies. The diagnosis of allergies include; skin and/or blood testing. Asthma and allergies often occur together, CONTINUED on PAGE 22
