MESSAGE FROM THE PRESIDENT

DECEMBER 2004

Thanks for another exciting and energetic GAP Meeting! With 40 new committee guests, eight new members voted in and six contributing members returning to Active (Welcome Back), we are moving forward!

Since several committees expressed concern about individual versus group authorship credit for reports and papers, I wanted to remind you that a few years ago, GAP members expressed similar concerns and the Board approved the recognition of individual authorship, rather than credit only for the GAP committee as a whole on publications. We hope this will encourage you to be productive and creative, still enable you to continue to advance your career at your home institutions, and also obtain important recognition for GAP. A list of the short reports published by committees is enclosed in the mailing and as you see from this list, GAP has been working very hard. If there are any omissions, please contact Frances with corrections.

There is a coupon book available for contributions to GAP to honor an individual as well as GAP. Again, the Board thanks all of those members who do not request any reimbursement for travel for the meetings.

ADMINISTRATIVE COMMITTEE REPORTS Fellowship Selection Committee The committee met to select a new group of Fellows for 2005 – 2007. A new group of fifteen Fellows was selected with one alternate named. This group will be notified and sent formal letters of acceptance.

The Committee proposed several new members to the President for appointment.

1. Update on Reports a. The Committee on Psychotherapy continues to negotiate with a publisher for its monograph on the Cost Effectiveness of Psychotherapy. b. The Committee on Psychiatry and the Law is negotiating with 2 publishers interested in their proposed monograph on Stalking. c. “Socially Sanctioned Coercion Mechanisms for Addiction Treatment” from the Committee on Addictions was approved. d. “Basic Family Skills for General Psychiatric Residents” from the Committee on the Family was approved. e. The International Relations Committee’s manuscript on Fundamentalism was given feedback with comments for reworking and resubmission to the Publications Board. f. The Committee on the College Student’s manuscript, “Student Mental Health Services in Crisis” was given rapid feedback at the meeting towards revision. g. The manuscript “The Internet as a Developmental Tool for Adolescence” was accepted for publication by JAACP in its Feb 2005 issue. h. The Committee on Psychopathology’s article “ADHD in Adults: Clinical Information for Primary Care

3 Physicians” appeared in the Sept. 2005 issue of Primary Psychiatry and is on the GAP website.

2. Membership change: Larry Gross agreed to replace Frances Cournos 3. Review of Publications from 1992-2004. We have 28 Committees including 4 new committees just formed. Of the 24 ongoing committees 20 have produced some type of written report in the past 13 years including: a. Published short reports: 27 from 12 Committees (16 of these were produced by 3 Committees) b. Approved but never published reports: 7 from 5 additional Committees (to the 12 above) of which 5 have been placed on the GAP website. c. 11 Books have been published (10 by APPI) from 10 Committees including 3 additional Committees (to the 17 above).

David A. Adler, Chair OPERATING COMMITTEE REPORTS Addictions Present: Nace, Suchinsky, Levin, Sullivan, Tamerin, Lewis, Galanter Fellow: Florian Birkmayer Guest: John Fromson The committee is working on a fast track article on “Coercion in Substance Abuse Treatment.” The anticipated completion is December 2004, targeted to mental health providers. The article will be published in Psychiatric Services.

The committee will finish revisions on approved draft on Part I and begin work on Part II, “Used Coercion in Substance Abuse Practice” and discuss new topics. Part II will be finished at the Spring GAP Meeting.

Aging Present: Sakauye, Liptzin, C Cohen Fellow: Jason Schillerstrom Guest: Susan Schultz The committee is working on a fast track article, “Medicare Managed Mental Health Care: A Looming Crisis for Psychiatric Services.” The draft is completed and will be submitted to the Publications Board at the end of this meeting. This article is targeted to psychiatrists and will be submitted to Psychiatric Services (invited).

The committee will work on plans for other publications venues for this topic (American Journal of Psychiatry, Psychiatric Times) and plans to post slides on aging issues on GAP website.

The committee will rewrite modified submissions and collect and review slides between meetings and at the next meeting, will review other submissions and focus on slide project.

Child Present: Knapp, Pfeffer, Jensen, Tanguay, Schowalter, Shapiro, Kruesi, McDermott Fellow: Mariana Mendez-Tadel Guest: Charles Zeanah The Committee is working on a fast track article, “Temperament: Biological Substrates, Modulation and Malleability,” first draft in preparation. The anticipated date of completion is 2005 with a target audience of mental health providers. The committee will be working on a review of the literature, clarification of themes.

College Student Present: Reich, Backus, Notman, Silber The committee is working on a fast track article on student mental health services in crisis. The report addresses the rising level of clinical need and utilization of services in student population and the relative decline of participation of psychiatrists in student mental health care.

The final draft is near completion, anticipated in January 2005. This will be targeted to psychiatrists, administrators and

4 mental health providers. This article will be submitted for publication to the American Journal of Psychiatry. The committee will continue to work on the manuscript with the selection of a new topic at the next meeting.

The committee is looking for new members.

Cultural Psychiatry Present: Lewis-Fernandez, Lu, Jacobsen, Desai Fellow: Anne O’Connell The committee is working on a fast track article on “Role of Concepts of Race, Ethnicity and Culture in mainstream psychiatric research. The first draft is near completion with the data gathering to be completed by April 2005 and the draft to be completed by November 2005. The target audience is mental health providers, medical anthropologists and sociologists.

The article will be submitted to American Journal of Psychiatry.

The committee will look for suggestions from the Publications Board on content and suggestions regarding journal publications.

The committee will work between meetings on the data gathering and the analyses. The committee will discuss the results and outline the discussion section of the article. The committee would appreciate recent references.

Family Present: Berman, Rolland, Heru, Grunebaum Fellow: Heidi Bruty Guests: Betsy Wood, Ph.D., Ittleson Consultant, John Sargent and Mariane Wamboldt The Committee is working on a fast track article, “Family Skills Curriculum” and follow up to the paper recently completed on proposal to operationalize RRC requirements for family skills in residency training.

A new topic is being chosen to be completed by 2006. The audience is psychiatrists involved in training psychiatric residents submitted to Academic Psychiatry for publication. The committee will work between meetings on a new topic and begin to work on sample curriculum and collect further references, present current paper to our own programs.

Gender and Mental Health Present: M Rosenthal, Lang Guests: Gail Robinson, Nada Stotland and Kim Yonkers Consultant: Ellen More The committee is discussing a report on the mental health aspects of abortion – consequences and context, critique of literature and recommendations for further research. Between meetings, the committee will be discussing the topic, doing a literature search and distributing reprints.

Human Sexuality Present: Friedman and Downey Guests: Goldenberg, Bartlik, Haas, Fogel, Schore, Adelson The committee is working on a topic, erotic desire: emergence, reservation and loss. Between meetings, the committee will work on assigning tasks, finalizing the topic and doing a literature search.

The committee is looking for members.

Medical Education Present: Pessar, Bernstein, Cabaniss, Dickstein, Hales, Nadelson Fellow: Sarah Graff Guests: Ruth Levine The Committee is working on a fast track article that has been reviewed. The committee is working on revisions suggested

by Academic Psychiatry per the review process. The paper will be submitted for publication.

The committee will be working on selecting a new topic.

5 Mental Health Services Present: Vergare, Lippincott, Sullivan Guests: Jules Ranz and Sigurd Ackerman The committee is working on a fast track article with the final draft nearly complete, expected to be completed in January targeted at psychiatrists. It will be submitted to Psychiatric Services for review.

The committee will begin working on a survey for electronic medical records.

Planning, Marketing, and Communications Committee Present: Paul Fink, Chairman, Jose Santiago, Seth Powsner, Harvey Ruben, Justin McCarthy, Jay Scully, Jack Bonner, and Michael Harrington, a guest from the Lilly Corporation.

The first order of business was our meeting with Mike Harrington, who had also attended the Contributions Committee the day before. We had invited him because he is an expert in the new Anti-Kickback rules that now exist between the psychiatric organizations and the pharmaceutical companies. Mike indicated that we are in a dynamically changing time with the pharmaceutical companies, but there was a need for change. He said, “Post Enron creeping criminalization of corporate behaviors,” is what has made the Justice Department and OIG of HHS become very, very intense about this problem. The codification needs to follow the OIG guidelines. It has to set up a set of external standards and it should also look at the ACCME guidelines. They have issued grant giving guidelines.

The pharmaceutical companies will each have either an internal or external autonomous funding office. There will be an arm’s length between sales and marketing and the academic and clinical entities that normally would go through their sales representative to get money. We agreed to discover whether each company that is important in the pharmaceutical world has an internal agency, such as Lilly, or an external agency handling their grant giving. We listed ten pharmaceutical companies we should investigate: a. b. c. d. e.

It was also suggested that we create guidelines for interacting with the pharmaceutical companies which would not only be used by GAP, but by the entire psychiatric community using the Federal law as our guide. There are approximately 100 FBI agents functioning in Philadelphia and Boston, all dedicated to this idea of stopping the kickbacks to doctors and medical agencies. They have been given incentives. To get this information we can go to the company website and I think we will very quickly learn how they are individually going to handle this matter and operate within the rules. The key thing is no sales managers or the marketing department. We could access people through the medical department and talk to some of the medical directors.

It was suggested we have one of the high level medical directors for one or two of the companies come to GAP, particularly as it relates to the internal or external agencies. We might want to invite some of them as well.

We then discussed what we might need the money for or reasons to get the money.

1. I proposed that if we could get $50,000 in a reserve fund to be used only for honoraria and travel to the plenary session, even though it would not be enough, it would be a lot more than we are doing now, which is zero.

2. GAP has stayed pristine with little pharmacologic money. GAP could serve in a variety of purposes for the pharmaceutical companies: a. If the pharmaceutical companies wanted to have focus groups where we have mutual interests, that would be possible. Or they could contract with us for certain services, documents, advertising materials, etc. b. Educational grants to provide for publication dissemination, but without any influence or evident tampering.

Another item that is clear that is any program we do needs to ensure that nothing is discussed that is off label.

Furthermore, there may be some room for us to help them in market research. These would be hands-off grants regarding various treatments, de-stigmatization of mental illness, but we cannot sell their organization or ours. c. The other thing is that whatever we ask for has to ultimately go back to patient benefit. Areas such as access to medication, adherence, compliance, ethical issues, technological issues such as stem cell research, etc., would all be legitimate. Our think tanks can rarely be utilized in this fashion and we need to go back to the committees and ask them if they have any thoughts about this.

6 Someone in the group mentioned that we have a valuable brand with GAP – authority, expertise, and high quality. One of our tasks is how to get the brand out into the community.

The Web We had a presentation from Seth that was repeated the next morning at the Steering Committee meeting. He markedly improved member’s ability to get onto the net and to work on projects together. Seth went ahead and, for a small amount of money, purchased PLONE. There are a lot of things we can do with PLONE. The software is now free for us except for an entry fee and Seth has already listed all of the committees and their chairs. What he needs is to have each of those committees filled in with the entire membership. Then it will be very simple for them to log onto their committee and they can easily make what they put on the committee private.

Seth reported that in the last six months we had approximately 68 hits a day. Most of them were just for the first page. One out of five downloaded something from it. It is safe to say that most of our members are not using it. The biggest problem in the development of PLONE is that we will have two separate websites which could eventually be merged. The current website will have all of our documents, as it now does. The PLONE website is www.ourgap.org and the first website is www.groupadpsych.org.

Linkages We then discussed the fact that we need to have all the other organizations with whom we link, link to us. I will ask Frances write to each of them requesting this link. First we have to go to each of the linkages we have and see if they are linked to us.

Steve Katz and Buzz Goodstein Harvey reported on two of our members. We were overjoyed to hear that Steve Katz, who had a medical error that almost killed him, is back walking with a cane and improving every day.

We then had a visit with David Pollock, who has completed the adherence project and who has prepared a letter to send to the pharmaceutical firms. We have sent it to Mike Harrington to get his comments, but it was a beautifully written and well developed letter. We also heard at the Steering Committee from Ken Sakauye from the Aging Committee that they are doing Medicare or managed care, which is another area that would lend itself to funding. It is clear that we need to create a central filter for appeals for money and for applications. We have to decide which committee is going to do this. It was felt that the most logical committee would be the Contributions Committee.

We then decided that we have to get a C&L Committee. Frances found out that we overlap in November, 2005, but they do not have any future dates listed. I have written to Tom Wise asking him if we can try to find a way to not compete and gave him the dates of our next three or four upcoming November GAP meetings in an effort to coordinate dates. We also thought that if they would get started now, if we give them a waiver next November to meet at the Psychosomatic meeting for that one time, we could get it started quicker.

I was given names of the possible leaders for the Neuroscience Committee and each of us will take one or two names and try to get someone to set up that committee.

We discussed the idea of an Emergency Psychiatry Committee and three names were brought up – Gordon Straus, Bill Dubin, and Henry Wiesman. I will look into that.

Finally, we spent some time talking about greater visibility for GAP and had our usual conversation about the unhappiness with the dues which turns people off.

Preventive Psychiatry Present: Pruitt, Wissow, Koplan, Powers The committee is in the process of reviewing the progress on the slide presentation “Preventive Psychiatry” and begin to develop strategic plan to disseminate the slide presentation. The second part of this is review of interview of psychiatry residency training directors based on a survey we developed to determine current inclusion of prevention/promotion training in psychiatry training programs.

The committee will work between meetings on: I. Revising several slides and refine speakers notes. Develop core biography.

Ethnographic study of prevention awareness and programs in psychiatry training programs. Continue interviewing directors and coding responses to survey questions.

Psychiatry and Community Present: Pollack, Cournos, Lamb, Roth, Munetz, Merlino Fellow: Michelle Pent The committee is working on the topic for a report: 1. Treatment adherence training module – CD-rom or other electronic format 2. Hospital bed capacity issue The report will be targeted at: 1. Psychiatric providers, advocates 2. Psychiatric and mental health administrators and policy matters The committee would like to have free distribution through professional and advocacy channels. It will be submitted to Psychiatric Services.

The Publications Board could help in obtaining industry support for production and distribution of the module.

During this meeting, the committee reviewed slides and re-established tasks needed to complete project. Also, worked on developing initial ideas for project focus.

Between meetings, the committee will follow up with Paul Fink regarding GAP participation and ask members to develop future topics.

At the Spring 2005 Meeting, the committee will finish plans for production/distribution and finalize the focus and outline for the paper.

Psychiatry and the Law Present: Pinals, Resnick, Knoll, Mossman, Giorgi-Guarnieri, Phillips, Newman, Ash Guest: Graham Glancy The committee continues to work on a book on stalking in the final draft to be completed in December. The target audience is MHP’s, lawyers, law enforcement and the public.

The committee is working with publishers to finalize an agreement for publication.

The committee has reviewed the publication agreements and working on final content areas.

Psychiatry and Religion Present: Lewis, Meador, Ostow, Snyder The committee is working on a fast track article on “Morality Issues in Psychiatric Treatment.” The article should be completed by next Fall 2005, targeted at mental health providers.

Between meetings, the committee will be working on the draft.

Psychopathology Present: Dulit, Adler, Berlant, Braun, Fornari, Siris, Dixon, Sonis The committee is working on a fast track article on the problems and challenges of the implementation of evidence-based practice in psychiatry and hope to move into a discussion of how to tackle the challenges. The project should be completed

by November 2005, targeted at mental health providers. The committee defined the topic and completed an outline.

Between meetings, the committee will complete the literature search and begin writing the paper.

Psychopharmacology Present: Baron, Sumner, Slaby, Gross

8 Fellow: Ned Bhanji Visitor: Alan Gruenberg The committee is working on a fast track article on direct to consumer marketing. This will be submitted for presentation at the IPS 2005 Meeting, targeted at psychiatrists. The article will be submitted to Psychiatric Services.

The committee completed the raw analysis of 1250 surveys. Between meetings, the committee will work on the IPS submission and the completion of the paper for submission for publication.

Research Present: Gardner, Sutton, Verhulst, Wamboldt, Kerbeshian, Bearhs, Swann Fellow: Morton Sosland The committee is working on a fast track manuscript, “Development of Social Brain Model in Psychiatry.” The anticipated completion date is mid-2005, targeted at mental health providers. The American Journal of Psychiatry is negotiating with the committee for a review/lead article.

Between meetings, the committee will research references and refine the content of the article.

Sexual Minorities Present: Hire, Drescher, Hanin, Mitchell, McCommon, Barber, Rosario, Guss, Volpp Guest: Scott Masters The committee is working on a web-based ten module curriculum for psychiatric residents on gay and lesbian issues.

The first draft is in preparation with a completion target of 1-2 years, targeted at psychiatric residents, medical students and psychiatry faculty. The committee focused on completing module on history of psychiatry and homosexuality.

Between meetings, each member has assignments for different modules.

Social Issues Present: Lurie, Alger, J Rosenthal, Ottenberg Fellow: Jinger Hoop The committee is working on an adult comic book for college students or high school students about controversial psychiatric public issues and a “I’m depressed, why should I take an antidepressant?” The comic book would be targeted at high school students and college students.

Between meetings, the committee will be looking at comic book illustrators and may invite artists as consultants to the next meeting.

Terrorism Present: Stoddard, Aldrich, Post, Shanfield, Weine no fellow 1. Guests: Lubit (2nd time), Joel Gold MD – see submitted biosketch. Purpose of visit – interested in becoming a member and joining committee.

2. Type of Manuscript: Journal article. Aiming for 5000 words. May be fasttrack.

3. Title and or general subject: Title not definite. The subject is Roles of Psychiatrists organized by causes, consequences and responses to terrorism.

4. Status: 4. First draft mostly completed.

5. Anticipated date of completion: April , 2005.

6. Anticipated audience: Psychiatrists, other physicians and mental health providers.

7. Am J Psychiatry or similarly wide circulation journal.

8. Possibly – but not certain— prior to next meeting. If so, I will send the draft.

9. Focus of work: reorganizing writing and abstract. Abstract below: ABSTRACT Terrorism, a strategy designed to accomplish political, religious or ideological goals through fear and intimidation promises to dominate the domestic and international landscape for an indeterminate future. The causes and consequences of, and responses to terrorism pose a considerable challenge to psychiatrists and other health professionals. Psychiatrists have multiple roles in these realms. The concern of this paper is to provide an understanding of how and why individuals become

9 terrorists, understand the psychological and behavioral effects of terrorism and what can be done about it including policies designed to inhibit terrorism. This paper will review the relevant knowledge in these areas. · · ·

Focus for next meeting: completion of paper or next draft.

Ways in which GAP members can be helpful: It would be most helpful to discuss and consider how to retain within GAP government employees on low salaries who cannot afford GAP. They are key players in the terrorism response, and some have come but left for economic reasons.

Therapeutic Care Present: Lipkin, Gruenberg, Fidler, Kramer, Hammersley Guest: Ryan Finkenbine The committee worked on IMG issues, revisited therapeutic care in psychiatric inpatient care in short stay era – milieu protocols. The anticipated date of completion is 18 months targeted at hospital psychiatrists. At this meeting, the committee edited the letter to editor, revising several collaborative projects.

Between meetings, the committee will review sanctuary model before topics for focus as: milieu, protocols, role of MD, and role of hospital.

Therapy Present: Lazar, Rockland The committee is finishing a monograph, “Is Psychotherapy Worth It,” costs, social costs, and cost effectiveness is being considered by a publisher. This publication will be targeted at policy makers, psychotherapists, advocates for psychotherapy benefits, resource in psychiatric residency training. The committee is negotiating with a large publishing house.

The committee is considering a new topic for an intermediate length report, “The Cause of Cultures in Psychotherapy.”

This would be targeted at psychotherapists. This will likely be a journal article. There will be continued discussion of this project.

Work and Organizations Members Present: Josh Gibson., Brian Grant, Barry Greiff, Duane Hagen, Stephen Heidel, Robert Larsen, Barbara Long, Daven Morrison, David Morrison Guests: None Type of Manuscript: Monograph for submission for publication Title and/or general subject of report: The general topic is changing relationships at work. The book will examine 1. How these are caused by technology, litigation, and globalization; 2. The concept of psychological contracts as a tool to understand the causes and consequences of the changes; 3. The roles of courage, resilience, performance appraisals, and psychiatric support in helping organizations manage the changes in constructive ways.

Status First draft completed.

The relevance of the ideas and need for such a book was addressed in a meeting with four executives and the committee a month before this meeting. The executives were enthused about the ideas and gave many ideas for improving the structure and writing.

Anticipated date of completion: Years Anticipated audience: Managers Focus of work accomplished during meeting: Incorporating the ideas of the executives who met with the committee. In particular we worked on inserting the concepts of the psychological contract into each chapter.

10 Plans for between meeting work: Each chapter will be rewritten and reviewed by at least two committee members. It will then be rewritten for the Spring meeting.
