Helen Arteaga, Classroom/Nutrition Aid St. Francis Head Start Mrs. Arteaga was promoted to Classroom and Nutrition Aide on November 13, 2006. This has been my first school year working with Ms. Helen. She is a wonderful employee, and is dedicated to her job. She is never late, and rarely takes a day off of work. She ensures quality service when it comes to her doing her job. Mrs. Arteaga is an asset to the St. Francis team and we all appreciate the great service she provides for the program on a daily basis. James Ashe, Bus Driver, Head Start Transportation Mr. Ashe is a Bus Driver for Geminus Head Start. Everyone has a class clown and Mr. Ashe is mine. He has the greatest work ethic I have ever seen. Staff love working with him and the children look forward to seeing him each and every day. I thank Mr. Ashe sincerely for the time he has put into Head Start and for the safe delivery of the children. September 20, 2006
Lourdes Chavez, Family Support Specialist South Haven Head Start Ms. Chavez began as a Family Support Specialist at St. Mark’s Head Start. She transferred to South Haven Head Start on August 15, 2007. Lourdes has achieved a distinguished record of providing Family Support Services to the clients she serves in North Porter Co. Her attributes of passion and commitment to her profession, dedicated work ethic, brilliant time management skills and her genuine and sincere capacity for establishing the human connection, whether with clients or co-workers, assist her in realizing success in her work, and producing quality results. Shelanda Clay, Family Support Specialist
St. Mary’s Head Start Ms. Clay began as a Teacher Assistant at St. Francis Head Start. She was promoted to Family Support Specialist at St. Mary’s Head Start on August 15, 2011. Ms. Clay has brought a great deal of diversity, and flexibility to our FSS team here at St. Mary's. She is honest, professional, dependable, a Team player and a wonderful person, who has a laugh that makes you laugh, even when you are angry (which is a needed quality at times). In her first year as a Family Support Specialist, she has managed to maintain full enrollment of the cases that she handles, as well as provide additional support to families that were in need of further assistance. Jerie’ Decker, Teacher South Haven Head Start Ms. Decker began as a Family Support Specialist at Valparaiso Head Start. She transferred to South Haven Head Start as a Teacher on October 22, 2007. Jerie’ is an outstanding example of an Early Childhood professional; dedicated to teaching young children in a developmentally appropriate manner with compassion, enthusiasm and a strong desire to continue in the pursuit of providing quality child enrichment services to achieve child outcomes for school readiness. Providing for and assuring for every child's well-being and success under her supervision is her ultimate goal. October 13, 2006
Arlene McKinley, Receptionist Family Services Arlene McKinley started with us on February 5, 2007 as Receptionist at Geminus Corporation. Not only has Arlene been the friendly “Face of Geminus” as Receptionist, she has also been given the dual role as Office Assistant in Family Services, where she assists the programs of CAF, CCDF, Community Partners, and Real Fathers Initiative as needed. Arlene is a hard working individual who always wants to do her best and has been a great 6

asset to Family Services. I have counted on her to assist me on so many occasions. She completes her tasks without hesitation and in a timely manner, as well as covering the reception area and answering the many calls that come in daily. Arlene’s professionalism, commitment and dedication has definitely shown in the last five years. It is truly a pleasure to work with Arlene McKinley and have her on our team. Jo Salczynski, Teacher, St. Mary’s Head Start As a Teacher at St. Mary’s Head Start, Ms. Jo, as she is called by her students and parents, is recognized by many of her peers for the tremendous amount of patience she has with the students in her care. She is one of the most nurturing and loving teachers that I've ever seen. Jo's greatest contribution to Geminus is her experience working with children who have special needs. Her experience allows her to effectively support her students to achieve progressive outcomes, per our head start curriculum. September 15, 2006
Felicia Stone, Classroom/Nutrition Aid, Hobart Head Start Ms. Stone began as a Classroom and Nutrition Aide. Felicia upholds a professional image of Geminus Head Start and arrives to work daily with a positive attitude giving 100%. Felicia always acts as a team player and communicates well with her co-workers offering a helping hand when needed. Felicia works between the full-day/full year classroom and the AM/PM classrooms serving 5 meals and 2 snacks daily. When Felicia is not performing her daily responsibilities of serving food, running in-between classrooms, washing dishes, calling the Geminus kitchen, ordering supplies, mopping floors, calling the site manager because we ran out of milk and relieving teachers for breaks, she is cleaning and sanitizing for the teachers in their classrooms.

Ronda Velasquez, Health/Nutrition Coordinator Geminus Head Start Ronda has created vital partnerships with Valparaiso University and Ivy Tech Nursing programs to ensure support for the Family Support Specialists with growth assessments, heights, weights and blood pressures, as well as, lead and hemoglobin testing. Ronda has worked diligently with community agencies, staff and parents to create an effective Health Advisory Committee, that creates new and exciting ways to meet the health and nutrition needs of our children and families. Ronda prides herself on providing quality services to children and families, often accompanying Family Support Specialists and Home Visitors to families’ homes to address health related issues. Maria Williams, Bookkeeper Financial Services Maria Williams is a joy to supervise. She is a hard working, dedicated, and a detail-oriented individual who does an excellent job. If she sees that someone needs help, she is always there to lend a hand even if her plate is already full. Maria manages her work in a very structured manner so that all of the bills that come through the department are properly investigated and processed on time. April 2, 2007

Nicole Ascar, Program Manager St. Mark’s Head Start Nicole began her employment on October 15, 2001 as a Family Support Specialist and was promoted to Program Manager on February 18, 2008. Ten years later, Ms. Ascar continues to be a very determined and dedicated professional. Ms. Ascar is responsible for our largest Geminus Head Start Site, which is St. Marks, located in Gary, Indiana. Ms. Ascar takes her responsibility as a Site Manager very seriously. She has set the bar very high for the St. Mark’s staff and families. Ms. Ascar works tirelessly with her staff to ensure that all the rigorous standards of the Head Start Program are met and or exceeded. Ms. Ascar continually impresses the Grantee staff with her creative ideas to ensure that the family needs are met. One of Nicole’s more notable ideas was to turn the parent room into a functioning lending library. Each month new themes are presented and the parents are encouraged to read to their children. Ms. Ascar is a big believer in taking care of the staff’s wellbeing. Nicole has found a way to turn the mundane job duties into friendly competitions. Prizes are awarded to the winners. She has invited professionals to come and speak to the staff about stress and other topics of wellness. It has been and continues to be a privilege to supervise Ms. Ascar. I am extremely proud to have Nicole Ascar on my team of Geminus Head Start site managers. October 15, 2001

Janie Askew, Teacher’s Assistant
Head Start St. Mark’s Ms. Askew began her employment as a Classroom and Nutrition Aide and was promoted to Teacher’s Assistant on August 18, 2003 at St. Mark’s Head Start. Ms. Askew was a Head Start parent, but is also is continuing to grow as a TA. Ms. Askew has stepped up many times to ride the bus when needed. She is known as the "Dancing Machine" and is always dancing with the August 13, 2001 children.
Adell Brown, Family Support Specialist Head Start St. Patrick’s Ms. Brown is the Family Support Specialist at St. Patrick’s Head Start. As a former Head Start parent, Adell is proof that the Head Start program works. She works with her families with humor, empathy and love. Thank you for all your years of hard October 15, 2001 work!
Lawanna Eaves, Teacher’s Assistant Head Start St. Mark’s Ms. Eaves began her employment as a Substitute Teacher. She moved to St. Mark’s Head Start as a Teacher’s Assistant on January 22, 2007. She is a good TA, and has a good relationship with her children and parents. The children love her. Lawanna does a good job engaging the children with IMIL Activities. Ms. Eaves always completes her work on time. August 13, 2001 Mimi Gardner, Vice President Family Services Mimi was recruited to fill a vacancy in 2002 to direct the Circle Around Families program that was started with a SAMHSA Child Mental Health Initiative Grant. She was recommended because of the reputation she developed while employed at Tri-City CMHC as a leader with demonstrated ability to partner effectively with DCS, Probation and Schools serving children and families. One of Mimi’s challenges was to assure CAF sustainability when the federal grants funds expired, which at the time 50% of federal grant-funded “Wrap 10

-around Systems of Care” begun with similar SAMHSA grants could not be sustained. Today, CAF has changed but is still viable and an important part of Lake County’s DCS and Probation referral options. Mimi understood that CAF alone would not assure its sustainability. She applied for and secured multiple grants and contracts that built a continuum of services for DCS and Juvenile Probation referrals that were extensions of CAF expertise and principles. Our Community Partners and Fatherhood programs are still relatively new but were recently cited by the Lake County DCS Director to be the best in the State. Our NW IN childcare voucher enrollment services serve families in five NW IN counties. That program consistently has one of the lowest error rates among state vendors. Mimi’s passion for quality has served NW IN children and families well and made Geminus Family Services an important component of the overall value Geminus brings to our community. Mimi’s heart and concern for low-income families and their children drive her passion for quality that has been evident throughout the past 10 years. She has worked to continue to build a Geminus Family Services market niche that is complementary to the shifting continuum of services provided by partner CMHCs. In addition she and her staff have built a remarkable network of community providers with whom Family Services partners with as well. Annual Vendor Fairs attract well over 100 vendors each year sufficient value to merit required attendance of DCS and Probation Staff. Annual Family Services Conferences continue to draw large numbers of Lake County participants from a wide variety of local organizations that serve children and their families. Mimi’s leadership and work illustrates, in a unique and special way, the meaning of our Geminus wordmark, “Partnering for the Future.” Family Services, under Mimi’s leadership, has been about building a better tomorrow for children and families through community partnerships. Albert Gay, Director, Prevention Services Over the last ten years Albert has been committed to our youth substance prevention programs. Initially, Albert ran programs for one of our providers, Boys and Girls Club. After leaving the area to complete his Master’s degree in Divinity, Albert returned and began providing management and oversight to youth programming in Lake County. He now provides oversight to providers in 12 counties spanning the northern region of Indiana serving thousands of adolescents. Albert has been instrumental in training prevention professionals throughout the state of Indiana and has worked nationally to provide training to a variety of groups seeking prevention training. Michelle Horton, Teacher, 4900 Broadway Head Start Ms. Horton began her employment as a Teacher’s Assistant. She was promoted to Teacher on January 3, 2005 and currently works at 4900 Ave. Head Start. She attended school to obtain her CDA and Associates Degree. After completing the requirements for her Associates Degree she obtained a teacher's position. She is currently in the process of pursuing a Bachelor's Degree in Early Childhood Education. Tameka Kinsey, Payroll Accountant Financial Services Tameka Kinsey is a hard working and dedicated employee. She puts forth every effort to make sure she gets her job done. Tameka is always eager to answer questions and find solutions to any problem that comes across her desk. She is a team player who can be called upon at any time. October 3, 2001
Kyle Marschke, Support Tech II Information Services Kyle plays a key role with our successes especially when it comes to supporting Head Start. His work over the past year consists of connecting all of the Head Start sites to our network, setting up the sites to scan and moved several locations. He takes great 12

Kevin Mikalouski, Support Tech II Information Services Kevin’s work over the past year consists of the new Avatar system. He is able to troubleshoot and find a solution for the most difficult problems. He was able to find a solution for the Avatar scanning and signature pads modules that Netsmart said couldn’t be done. When the team needs to move quickly, this is where he excels. Kevin really enjoys a challenge and finding solutions and he is an instrumental part of the success of this organization. Andria Weems-Lewis, Teacher, Roxanna Head Start Andria began her employment as a Teacher’s Assistant. She was promoted to Teacher August 31, 2004 at the Roxanna Head Start site. Ms Lewis is a team player, and is always helpful in any given capacity. She takes pride in whatever she does, and I have never seen a frown on her face because she is always smiling. Ms Lewis has a positive outlook on any situation. It's a September 4, 2001 joy to have Ms Lewis on the Roxana team.

Margaret Anderson, Billing Specialist Electronic Billing Congratulations Peggy on celebrating 15 years with Geminus. During her 15 years, Peggy has had various responsibilities and has seen many changes in the Billing Process. She’s seen the evolution of billing from paper claims on a monthly basis to electronic claims being sent out daily. In her current position, she has been the “go to” person for submitting Medicare claims and the follow up on denials to resubmit for payment. Thank you for being part of our Team! May 14, 1997

Carolina DeLaPaz, Teacher’s Assistant Roxanna Head Start Ms. DeLaPaz began her employment as a Teacher’s Assistant. She is currently at Roxanna Head Start. Ms DeLaPaz has very high expectations of herself and that is reflected in the work she produces. There is an overwhelming passion that overcomes her when she is involved with the children and it's a joy to observe. She takes pride in whatever she does. Dolores Irizarry, Software Engineer
Information Services Dolores has filled many roles in her 20 years at Geminus. She has excelled in her data processing skills and made major contributions to the stability and integrity of the data processing environment. Her knowledge of the behavioral healthcare field and data processing, her work ethic, and selfless willingness to do whatever it takes have been significant factors in the stability of daily operations and success of major projects over the years. We have all benefited, on many levels, from Dolores Irizarry’s time at Geminus. January 15, 1992
Sherrie Johnson, Teacher Salk Head Start Ms. Johnson began as a Teacher at Hobart Montessori Head Start and transferred to Salk School Head Start on October 22, 2002. Sherrie continues teaching at Salk where returning families request her as their child’s teacher. Sherrie arrives to work ready to touch the lives of all the children in her care and team-teach to the fullest. Sherrie is comfortable with her everyday responsibilities and in her role as a teacher. I am taken aback by her daily devotion to her teaching and wish all teachers were as engaged with their students as Sherrie. I always enjoy walking into Sherrie’s classroom and regularly observe her busy in activities and taking pleasure in her job as a teacher. Sherrie has had a number of practicum students working under her at Salk. Teachers look up to Sherrie and begin to model what she has taught them through this experience. One of my favorite times to observe Ms. Sherrie is when she brings out her puppets and brings them to life right in front of the children. The expression on the children’s faces is priceless! I feel very fortunate to have Sherrie as part of my Salk Head Start teaching team. September 1, 1991 16

Mrs. McGee began her employment as a Teacher at St. Mark’s Head Start.
Mrs. McGee is an outstanding teacher. On several occasion, Mrs. McGee has gone above her job description, always completing her work on time and is always doing creative April 1, 1992 activities with the children.
Catherine Tomczak, Billing Specialist Patient Financial Services Cathy has been a dedicated and reliable employee for her twenty years at Geminus. For the past ten years, she has taken great ownership over the Title IVB billing for Regional and later, Geminus. However, her responsibilities have always been varied, and she has the same level of commitment on any task she is assigned. Mary Gordon, Bus Driver, Transportation Ms. Gordon is all about families and children. If she could, I think she would adopt families to keep them under her wing helping her children get to school every day to enjoy what Head Start offers them. Pamela Frame, Benefits Coordinator Human Resources Pam has been employed with this organization for 35 years beginning her career at Tri-City Mental Health in Human Resources and transitioning to Geminus Corporation at its inception in 1992. Pam is responsible for comparing and finalizing the benefits plans giving our employees the best options to help make Geminus and Regional a quality place for our employees to work. Pam has grown with the position, along with its many legal complexities, to be the best benefits coordinator I have ever worked with. December 13, 1976
Paula Jusko, HRIS Analyst Human Resources Paula has spent the last 35 years as part of this organization, and her experience and service has been a key factor in our success. Her tenure began at Southlake Mental Health before moving to an executive position at Geminus. Although she chose another path over the years, she has continued to work in Human Resources. In my experience and involvement in several Human Resource chapters and affiliations, I have never met anyone that approaches her knowledge and excellence in the Human Resource field. We are truly fortunate to have someone of Paula’s quality at Geminus. November 1, 1976
Stella Perez, Intake Specialist, Registration Stella Perez sets the standard for compassion and devotion to the clients of Regional Mental Health. Her hard work and extra effort during transitions in our department has not gone unnoticed. Her willingness to cover shifts at Stark and Strawhun, and work until all tasks for the day are finished, is one specific example of Stella giving 110%. May 5, 1977 18

Geminus Administrative Service Organization (ASO) Employees of the Month May 2011 — Dawn Ward Financial Services Dawn is very patient and professional and has never complained. She helps Family Services has relied heavily on Dawn to correct SALs for the department and has probably received a trillion corrections. She just smiles and never complains. Dawn always greets me with a smile and is very helpful to the Prevention Department. She helps us understand our monthly ledger reports and is often a great investigator when a line item seems out of place. I would like to thank Dawn for all that she does for us. When numbers run rampant With no rhyme or reason And all seems dark and dismal Hold on for your life Cause in like the light Comes the Dawn of the Day To bring reason to all of that dismay. She helps us in a pinch Making it look like a cinch Chasing all our worries away This poem is dedicated to one fantastic professional that really goes the extra mile for her customers and clients. She’s very helpful and attentive to our specific grant needs and does a wonderful job of assisting us in all aspects of finanacial situations. She is our DAWN of the DAY! 22

June 2011 — John Washington Information Services
John has exceeded my expectations while working on the Regional Dashboard reports. John is a very competent Database Administrator and has moved the SQL system to another level. He has created dashboards that are disseminated to the Clinical Managers at Regional Mental Health. He has worked with the VP of Quality Improvement to meet the reporting needs of Regional. The dashboards were so well received that he was then asked to develop dashboards for the clinical staff. The dashboards are synched with the Active Directory system and open when the clinician signs into the Network. John works well with members of other teams and takes pride in his work. Bringing data into the hands of the clinical staff in a timely efficient way is something new for Regional.
July 2011 — Nancy Brnicky Patient Financial Services I worked with Nancy when she was an administrative assistant, and she was wonderful at the Hobart office. I’ve maintained contact with Nancy as she transitioned to her position in the Strawhun business office managing money with day treatment and intake. I’ve witnessed her friendly and warm approach to clients as they purchase meal tickets, etc., and she continues to display that same inviting demeanor at her current position which she had at our office. Nancy is always very helpful to both clients and staff at Regional Mental Health. She is always willing to lend a helping hand without being asked. Nancy assists clients on a daily basis to help them understand their statements, discount status and 23

Regional MHC procedures. This allows clients to understand what their statements mean and what staff at Regional may be able to assist them in their request for additional services.
August 201l — Scott Harris Information Services Scott has exceeded my expectations while working on the SAMSHA grant for the Regional Integrated Care Initiative. Scott completed 11 forms in a very quick timeframe to meet the requirements for clinical documentation of the grant. Even though Scott was beginning the Avatar implementation, he was able to manage his time wisely and meet the deadline. The impact of Scott’s efforts are that Dr. Kern and his team can chart on the patients that fall in the guidelines of the grant. They have the documentation necessary to make this grant successful for all involved. Scott always exceeds my expectations. He is conscientious and dependable. He is self-motivated and has moved my department forward on many occasions. To expand on the comments above, when new forms were needed for the new Stark Residential unit, Scott was able to complete them in a matter of hours. This is consistent with every interaction I have ever had with Scott: warm, friendly, extremely helpful and very quick to solve problems or get things done. I agree with the comments of others; Scott has exemplified what I would consider a genuinely caring, responsive and helpful employee. He has simply been a delight to interact with as we have gone through the process of starting up a new unit. Where others have sighed and responded, “I will get to it,” Scott said, “We can do this right now—while I have you on the phone—no problem.” 24

September 2011 — Norma Jean Juel President’s Office
Norma Jean has always demonstrated an excellent work behavior. She seems to go above and beyond her call of duty. She is always willing to help her fellow employees. As an example, at the Geminus picnic she made sure that employees required to work were still taken care of and didn’t miss out on the raffles; if gifts were won, she made sure they were forwarded to the employees’ bosses to give to the winners. Another example is when there is an employee luncheon, she is willing to fix plates of food when we are unable to attend. She even takes time to teach her fellow employees how to crochet. You could not ask for a better recipient with a heart of gold. She deserves to be employee of the month.
October 2011 — Elizabeth Buckley Information Services Liz has worked very hard to develop the ANSA, CANS and CANS 0-5 in the new AVATAR software. This has been very challenging as there have been so many issues associated with Scriptlinc and Webservices (two technologies from NTST). There has been no formal training and little assistance from the Software Company for these products. Dr. Sharon Kraus has asked that the client rating improvements be noted within the software so that the clinician knows the client outcome before they submit the form. Liz has programmed a very “intelligent” product that provides needed information to the end user. She has met the needs of the Clinicians and the Clinical Managers at Regional. This improvement in the application should increase the clinician’s awareness of the status of the client. Liz never gives up and keeps working hard until the product is complete. Liz is also working on scripting to enable PFS at Geminus to bill for things that the new system does not accommodate. 25

Liz has also exceeded my expectations while working on the SAMSHA grant for the Regional Integrated Care Initiative. She completed an application in SQL that will be used by the Regional Integrated Care team to document required items that must be reported. Even though Liz was working on other assignments, she took the time to create an excellent, well written application in SQL and placed it on the desktop of the Nurse Case Manager of the Grant program. The impact of Liz’s efforts are that Dr. Kern and his team can track data on patients that fall within the guidelines of the grant. They have the application and database that is necessary to make this grant reporting successful for all involved. Liz’s programming skills have moved my department to another level. She is an excellent programmer and has accomplished many great things since she has been here.
November 2011 — Juan Huizar Patient Financial Services Juan has really gone out of his way to assist Residential Services with gaining and understanding of the new MRO guidelines that we are now facing. He always responds to questions quickly, and takes the time to return my calls whenever there is a question, even if it’s not directly related to his job, he goes out of his way taking the time to help. Juan even called Residential Services with suggestions to improve our prior authorization requests. He has always been so helpful and so calm in dealing with these PAs that we send to the state. He seems so organized sending out ticklers letting us know we should send a PA or to monitor the units so that we don’t run out. These PAs are a source of revenue for Regional, so they are extremely important. Completing PAs is also a rather tedious and frustrating process, but Juan has really helped us keep on top of things, so I know he’s helpful to others as well. 26

December 2011 — Darlene Spencer Financial Services
For years Darlene has worked in the trenches of payroll, producing near perfect payrolls every other week...but that’s what we expect of her. Lately, however, Darlene has been on the front line of software conversion having to be 1st up in three software rollouts at once. In April and May 2011, she worked on the new Vista HRIS software, learning and testing. In May and June 2011, she was 1st up in the SAGE software, having to commence invoicing before the prior year end so that checks could be produced beginning with July 1. In August and September 2011 she assisted in the set up of the new timekeeping software “Time Star.” That software rolled out on October 9, 2011. In the week that proceeded the rollout, Darlene had to cancel her vacation to help prepare; then she worked late every night...including Friday night when time clocks were deployed after the end of the weekday shifts. Under normal circumstances we might have expected Darlene to be somewhat surly...as she is usually dry of whit, but she has been pleasant and helpful taking the lead in the department for understanding the software. I want to say “ATTA GIRL” to Darlene!
January 2012 — Christine Cokusis Information Services Christine maintains the day-to-day administrative processes within the department with very little direction. She makes sure that off-site tape rotations, mail, license management, purchase orders and requests for services (RFS) are done timely and correctly. The last two or three months have seen a surge in RFSs of well over 100 per month. Christine makes sure they are routed and processed through the three different processes within IS to insure appropriate access rights 27

are provided in a timely manner. Christine also participates in new employee orientation. She provides training on Outlook, MS -Office, etc. she also makes sure that new employees have the correct access during this orientation. The number of projects underway within IS has put a strain on the staff. Christine has pitched in wherever she can. She has learned and undertaken the creation of Crystal Reports for the AVATAR project, she has assisted in the ASO projects by helping HR with data entry, keying hours of time punches for the new Payroll system and other tasks asked of her by other member of IS. Christine was assigned the task of registering the Eligible Providers for the first round of ARRA Meaningful Use incentives. Wading through the various federal and state agencies to get this processed required persistence and good record keeping. Christine acquired several contacts and followed up diligently to insure that all requirements were met. This process required her to coordinate efforts with PFS, Regional and Geminus. Her efforts resulted in the awarding of the firstyear incentive amounts to Regional with a total sum of $255,000.
February 2012 — Dolores Irizarry Information Services Dolores has been instrumental in the MRO Transformation project with HAP submissions as the affect MRO Prior Authorizations. Dolores has gone above and beyond to independently develop report tools and mechanisms to improve processes for the HAP submissions, prior authorizations, and the loss of revenue through PA tracking. Recently she developed a menu of reports for Patient Financial Services to utilize because she was “losing sleep” worrying we would be able to have all the information we needed. 28

Dolores has devoted so much of her time to IT projects. She has spent weeks exporting data for Payroll, HR and Accounting. Dolores has loaded patches and created conversion files for AVATAR. Her extensive knowledge of our operating system and the various assessment tools we use to report to the state is impressive. Whether it’s during the day or during off hours, she gets back to you and helps resolve any issues you may have. Dolores has provided not only valuable input, but independently developed solutions. She is an absolute pleasure to work with. She is always willing to help assist you whenever she is asked, regardless of whether or not its her job. She is always pleasant and smiling and never seems to get frustrated no matter how long it takes to grasp what she is saying. I have enjoyed working with Dolores for over 18 years while she has held a variety of positions, and she has always been a valuable resource person with a willingness to help. It is no wonder she is considered one of our most valued employees.
March 2012 — Douglas Ward Financial Services
Doug is always willing to do what’s asked of him, and does it completely and in a timely fashion. He went out of his way to inform Regional of a business opportunity that we eventually “landed” and also put together a HUD application that ended up being the only one funded in this last cycle of awards. He serves his customers very well.

April 2012 — Hulette Williams Information Services
Hulette Williams continues to help Human Resources, Payroll and now Patient Finance with transitions involving new software. She is a true life saver. She has figured solutions out with the new payroll system when we were unable to reach the software consultant. Hulette continues to help everyone transition smoothly with new software issues. She comes in with a smile and remains calm despite our panic episodes. Hulette has even stayed over, and at times late (midnight), to assist the payroll department with resolving issues with new software. She continues to even help us despite the fact that she is now helping the PFS staff get ready to launch their new software. I need to add to the last recommendation...Hulette has gone beyond the call of duty to help payroll with our new system. She comes up to our department or calls to make sure everything is going okay. Hulette stayed to make sure our first payroll out with the new system was submitted without any errors. Hulette truly deserves to be awarded the employee of the month.
April 2012 — Trish Rosich Patient Financial Services Trish has been dedicating all of her time to the Avatar program. She has spent countless hours to ensure that PFS has a good product that works! The time she has dedicated has taken her away from her family, and has taken a toll on her physically. Through it all she has kept a good attitude and still takes time to assist staff both at Geminus and at Regional. She has stepped up when we needed a leader on this project and done a great job. She is an asset to our department and has helped me overcome obstacles in my position and continues to offer support. 30

In addition to the above recommendation, I too feel Trish has been extremely helpful with anything I have asked of her. I feel that during this hectic time with Avatar, she takes the time to answer my questions and explain them so that I am able to understand and able to continue with my work. Even though she is not my immediate supervisor, she is always there when I need her. I also need to support Trish as employee of the month. For the past year she has been an excellent example of perseverance. She worked tirelessly, even pushing through her own personal obstacles in order to stay positive, always keeping the “go live” goal in sight. Not only did she play an instrumental part in building the system (showing much foresight), but she is working very hard to get the rest of Regional and Geminus up to speed. Without her, this project would not have run as smoothly as it has been. What can be said that has not already been said. Trish is always willing to help you with your inquiry. Even if it doesn’t pertain directly to billing, she is very knowledgeable in all aspects of billing and follow-up collections, and is always willing to teach you something new. She has given many hours of her time, heart and soul to the Avatar conversion, and has never let the stress of that task overwhelm her or the staff. She is a smart troubleshooter, and if it needs to get done, she is the person to get it done. Trish is a huge asset to Geminus and especially to the Patient Finance Department.

Geminus Head Start Employees of the Month May2011 — Lawanna Eaves St. Mark’s Head Start Lawanna comes to work everyday and cares for the children in the class. She is eager to learn about new programs introduced for our site. She develops great relationships with the parents and provides open communication and a comfortable atmosphere in the class. Ms. Eaves is a great co-worker, all around hard worker and a good listener.
June 2011 — Tondelia McVea Geminus Head Start Tondelia deserves to be recognized for her commitment to her role of Disability Coordinator, and her devotion to children with special needs. As a Disability Coordinator, she attends team meetings with Head Start staff and case conferences with parents at the LEAs. On weekends she sets up tables and banners to help promote and explain the importance of Head Start. Being the parent of a child with special needs, Tondelia has an exceptional understanding of the needs and services available. She helps support the Head Start staff, parents and all children by quietly offering her expertise and knowledge. The impact of her workload is reflected in the relationships and MOU’s she has created. She deserves this award and recognition.

July 2011 — Abigail Cruz Roxanna Head Start
Ms. Abigail has one of the warmest and sweetest personalities that anyone could have, making her the perfect Family Support Specialist. She is always offering her assistance in the classroom. In our site meetings, she is the translator, and all the materials that are sent out from the Roxanna site that require translator input are sent with the help of Ms. Cruz. She does not hesitate to offer her assistance. She is loved by all of our parents, but most of all, our Spanish speaking families speak highly of her. There was a family with a child that needed some special help services, but Mom was hesitant (as the parent put it, “I am afraid to go by myself.”) to attend, and without being asked, Ms. Abigail volunteered. This is the type of support she gives to our families, and for that I am thankful.
August 2011 — Lullaby Robinson 4900 Broadway Head Start Lullaby has a heart the size of Texas. She always has a smile on her face, a kind word for each child, treats others as she wants to be treated and always with a positive attitude. I have watched Ms. Lullaby blossom since her move to 4900, and I think she has found her place to shine. Ms. Lullaby is one of the most nurturing teachers I’ve ever worked with. Ms. Robinson is in her 12th year with Geminus, and is not slowing down. She is dependable, honest, reliable, professional and most importantly, she is there for the children and the parents we serve each day. I witnessed Ms. Robinson in action working with one of our more difficult, challenging children, and she was as cool as a cucumber. 33

Lullaby managed to not only redirect our friend, but she managed to do so in the most moderate tone possible. Ms Lullaby’s redirections skills are those that any manager can appreciate. Ms. Robinson is also a great reader, and loved by the parents in her room for her nurturing spirit. and patience with the children. She is well respected by her Teacher and her fellow co-workers.
September 2011 — Elizabeth Pavuk South Haven Head Start Ms Liz is a wonderful teacher and co-worker. She bends over backwards to try and fulfill every child’s need. She will do anything to help a co-worker. Most of the time you don’t even have to ask her; she already knows what is needed. Ms. Liz will even help you in the evenings and weekends to give you suggestions with lesson plans, activities or just an ear to listen. She gives lots of encouragement to the children and her fellow co-workers. Ms. Liz started with Head Start as a parent many many moons ago, and now she has worked her way up to a teacher. I am proud to work with Ms. Liz. Any child would be lucky to have her as a teacher. I know I am lucky to have had the chance to work and learn from her.
October 2011 — Jesenia Morales St. Francis Head Start Ms. Jessie is an engaged employee. She is consistently productive and passionate about the children and families she serves. She’s been a family support specialist for almost a year, and in that time, she has managed to connect with her families on a level that most veteran Family Support Specialists have yet to encompass. Not only does Ms. Jessie care for the families on her caseload, she is becoming a “go to” person for the teachers throughout the site. The staff is comfortable and 34

seeks her guidance when planning activities. They rely on her creativity when programs are planned. She is a hard worker who creates a friendly environment here at St. Mary’s.
November 2011 — Amalia Galvan Hobart Head Start Amalia is a Family Support Specialist that I have had an opportunity to work with now for three years. She is a great candidate for Employee of the Month because her work performance and dedication to the Head Start program is excellent. Her knowledge of Head Start along with family support responsibilities are shown daily. Parents turn to Amalia regularly for support and reassurance. They look forward to seeing Amalia and request her by name at enrollment time. Amalia works with the community regularly to help families of all socio-economic levels. In particular, around the holidays Amalia ensures our neediest families are able to enjoy donations from local agencies. Some of these include Thanksgiving baskets and gifts at Christmas. Amalia brought in the Lions Club for vision screenings and lined up a presentation at the October parent site meeting. Sandra Monroe, from the Purdue University Cooperative Extension Service came to speak with families about nutrition. Following the presentation six families signed up to receive family nutrition services in their homes. Amalia also informed parents about the “Get Coats for Kids” drive happening at Hobart Middle School in November. One parent came up to me after the coat drive and was almost in tears because she was able to receive brand new coats and hats for all three of her children. Amalia is currently assisting seven families with receiving services for the children through NISEC. She also works hard with the teaching team to bring in In-kind each month.

Amalia not only services families and children, she helps other employees when needed and acts as a team player. Amalia Galvan has exemplified engagement over the last few months more than usual. She is always showing how much enjoyment she gets from her job and helping others.
December 2011 — Cynthia Rossi 4900 Broadway Head Start Cynthia Rossi has proven daily that she is one of the best Teacher Assistants under my supervision. Ms Rossi is creative, energetic, dependable and motivated. She is offering ideas that can further support not just her classroom, but the entire site. Upon her arrival at 4900, all of her parents immediately fell in love with her as a teacher, mostly accredited to her calm and friendly demeanor. The parents in her room have nothing but good things to say about her when I’ve inquired about performance, especially her customer service skills. Ms. Rossi should also be recognized for her commitment to furthering her career in the early childhood arena. She is currently in the process of completing her Bachelors degree in Human Services. Cynthia aspires to be a Family Support Specialist for Geminuis Head Start when she is done with her undergrad degree.
January 2012 — Vanessa Fernandez Geminus Head Start
I have seen Vanessa grow professionally by leaps and bounds. She began her professional career with Head Start straight out of college in early 2009, and she has become a leader within the program. She has implemented many new processes to improve the quality of services for families and children. Vanessa has 36

begun tracking the progress of child outcomes for dual-language learners to ensure equality and progress for all children and their kindergarten readiness goals. Vanessa has truly become an advocate for the children and families that she works with, but best yet, she is empowering them to become advocates for themselves. She embodies Head Start’s mission. I, along with the rest of those who come in contact with her, cannot say enough positive things about her. She is truly deserving of this award, if not employee of the year. I am lucky to have her on my team.
February 2012 — Vicki Dillon St. Mark’s Head Start Vicki is always looking out for staff, families and children. She cares a lot about what she does. Vicki loves to teach, and introduces new learning to children and parents. She always looks for ways to help others. Vicki never expresses any negativity, is a very hard worker, goes beyond the means of accomplishing her job and she looks for ways to make everything work. That’s why I love working with Ms. Vicki. She has shown me how to grow as a teacher, and that’s a wonderful feeling. Keep up the Great Job Ms. Vicki, and continue to reach out to others. You DO make a difference in the lives of people you touch.
March 2012 — Milagros Vazquez Evans Elementary Head Start Ms. Vazquez has shown exemplary service to our Head Start children and families. She is a constant advocate for the children and the rights of their parents. She is a wealth of information when we have families in need. She sets up annual food baskets for families in need and gives freely of herself. Ms. Vazquez also used her community contacts to locate toys for the children over the Christmas holiday. She recently went to the local DCFS office 37

with a parent and helped them receive benefits that had previously been denied. Milagros is dedicated to her job and the families that she services. I have worked with Ms. Vasquez over the past three years, and her love and compassion for the families has never waivered. She knows all the children by name and never fails to hug them and tell them she loves them. I believe that Milagros has been an excellent example, and I am proud to call her my co-worker.
April 2012 — Chiana Goodlett St. Mary’s Head Start In light of recent changes to our program, my first task was to identify which staff would be the most helpful, proactive and supportive in making the change process as smooth as possible. Mrs. Goodlett (aka “Mayor” Goodlett) has been a truly valuable resource to me throughout the transition. Mrs. Goodlett has exceeded my expectations by displaying such personal qualities as:’ Leadership: She and her fellow Family Support Specialists were charged with proactively planning schedules for parent activities, recruiting opportunities, etc. Mrs. Goodlett was the first one to step up and offer suggestions and ideas for these tasks, and was open to suggestions for further planning. Collaborative: By being part of the Miller community, Mrs. Goodlett has utilized her ties to collaborate with other organizations for recruitment purposes, as well as to discover potential resources for the parents at St. Mary’s; this action has allowed me to become more familiar with the families we serve and provides valuable information as to how our site can better meet the needs of the community.

Geminus Managers of the Month May 2011 — Cathey Wirtz Patient Financial Services I feel that Cathey Wirtz is worthy of the Spirit of Geminus Award because she has been a tireless partner in helping my department negotiate the complex, and often frustrating, process of DCS IV-B referrals and MRO. Cathey has remained positive and upbeat., and she and her staff have provided invaluable guidance to my managers and direct care staff. Cathey takes time out of her schedule to attend bi-weekly telephone calls with DCS, during which she is an active participant. She is well informed, and able to ask questions that are pertinent to the topic. Cathey always responds quickly to requests from both me and my managers, or other staff. She has the ability to prioritize, and will keep us informed of her progress on lengthy tasks. Cathey’s extra effort has resulted in my staff feeling more supported, has increased their ability to do their jobs better, which has in turn, resulted in better provision of services for our clients. I feel that Cathey deserves this award not only because she excels at her job, but because she embraces the idea of cooperation and collaboration between Regional and Geminus. She is interested in making the relationship seamless, efficient and collegial.
June 2011 — Jean Thurman Geminus Head Start Jean has demonstrated a tremendous amount of leadership with all content areas and Early Head Start Coordinators at Dunebrook and Parents as Teachers, Lake County.

She insured all aspects of the Head Start performance standards were woven into home visiting and socialization practices. Jean takes a solid proactive systems approach to programming; insuring that quality is emphasized in all aspects of service delivery. Jean has also shown tremendous leadership with the completion of the Early Head Start center-based facility; she has trained the EHS staff on all aspects of infant/toddler development as it pertains to both Head Start performance standards and the State of Indiana Child Care Licensing Standards. As Jean has come to Geminus with a wealth of knowledge pertaining to Head Start/Early Head Start, she has taken the initiative to engrain herself in partnering with the Grantee content area specialists to ensure that she is supporting the systems that they have created, respectfully offering guidance to her colleagues to insure a seamless delivery system for 0 to 5 for children and families. Jean is coined for her mantra, “it is about relationship building.” She focuses on all aspects of service delivery bringing her strengths of a multi-disciplinary approach to our team!!!
August 2011— Izdemi Tamayo St. Francis Head Start Ms. Izzy is more than a Program Manager; she has done so much, it’s hard to know where to begin. She is a leader, mentor and team player. Ms. Izzy is a “hands-on” manager who works side by side with her staff, never asking us to do anything she is not willing to do herself. She rolls up her sleeves and has a “how-can-WE-fix-it” attitude with a clear sense of respect and support for the people she supervises. She is no nonsense, direct and firm yet full of good humor and encouragement. She has a thorough grounding in policies and procedures, but avoids turning them into grim limitations or 41

chores. She clearly sees them as the framework for providing quality programming for our families and managers to convey the necessary information and responsibilities without the need for micro-managing or draining the fun out of the reason we are all here. Through her leadership and strong sense of teamwork, she has turned St. Francis into a “model” site with 100% compliance.
September 2011 — Joelynn Miller-Johnson RBA Administration Mrs. Johnson has, without a doubt, given her all to our RBA facility this year. She has been wearing several hats, from Director of Nursing to Business Office Manager along with various supervisory hats. Joelynn has managed to be a support beam for nursing, taking on the business office issues, passing out snacks, providing activities for residents, such as popping popcorn and even singing karaoke, and implementing better care for our residents. She has worked almost everyday for several months with not even a complaint. Not to brag about her, but she just completely filled our census with her diligent work and her ability-to-get-the-job-done attitude. She drove back and forth to South Bend, Indiana, over the last few weeks to make this happen. I could go on and on about the great works and leadership skills of Mrs. Johnson, but I’d be typing all day!
November 2011 — Walter Gustafson Information Services
I think often IT services are taken for granted. We only notice when things don’t go right. Over the past several months we have taken on the project of replacing nearly every software program (including report generator software) that we use in the day-to-day functions of these organizations. It’s true that Rob has developed some persistent and patient project managers. These projects are basically going 42

as well or better than one might expect. On top of that, Rob’s department applied for and received the 2011 Netsmart Customer Innovation Award. Rob and his team have applied for and received ARRA meaningful use (Stage One) funding to the tune of $250,000, and we were the first CMHC in the State of Indiana to do so. Rob’s department has applied for and received a major infrastructure grant to expand bandwidth between Geminus and the Regional sites. His department took advantage of the opportunity to access special funding for integrated care grant recipients (Regional) which will pay for a large chunk of the Avatar development project. While Rob has not done this singlehandedly, he has been able to motivate his staff to complete all these things at the same time—a true feat.
December 2011 — Camille Propes Patient Financial Services Two months ago we made a change in the PFS reporting and duty assignments to accommodate allowing Avatar development staff to work on that project full time. Day-to -day electronic billing supervision and event rule maintenance (among other things) was transferred to Camille. She was tentative at first and attempting to “endure” the assignments rather than seeing it as an opportunity. As time wore on, Camille had to begin to really learn new functions. She is now reporting excitement and curiosity to learn more, as well as a sense of accomplishment. She is now able to do things she never thought she would or could. We’re very proud of her growth!

February 2012 — Toni Lach Purchasing Services
Toni has gone the extra mile to help me in my new position. She has remained at work after hours and even come in on her day off to assist in running checks to make sure our employees were paid on time. I don’t think the transition would have gone quite as smoothly had she not taken the extra time and effort.
March 2012 — Deborah Hartzler Hebron, Valparaiso & Holy Name Head Start Debbie, as we like to call her, always has a positive attitude and never backs away from “too much work.” Debbie is truly a team player whom I think exemplifies the “Spirit of Geminus.” Debbie is often the first manager to take on a newly assigned task. With our TSG software, she was able to learn the proficiencies and share her skills with others. Debbie took on Docuware with a blink of an eye. She is responsible for three sites, and she never complains about the travel. Debbie has high expectations for her staff and her staff meets or exceeds the challenges with her support. When I first started my new position, Debbie was my “go-to person.” She was willing to work with me, demonstrated patience and was very supportive. I am very pleased to have Debbie as part of my staff.
