he practice of dental hygiene made its debut in North America in 1913 when a Connecticut doctor, Dr. A.C. Fones, opened the first dental hygiene school in his garage. The first graduating class consisted of 27 women. The benefits of their work exceeded expectations — cavity rates among participating children were reduced by about 75 percent. In Canada, Ontario became the first province to legally recognize dental hygiene with the amendment of the Dentistry Act (1911) in 1947. The first dental hygiene program in Canada was offered at the University of Toronto, Faculty of Dentistry. Five students signed up for the twoyear program which began in September 1951 with funding from the W.K. Kellogg Foundation. Today, dental hygiene is recognized and practiced worldwide — existing in more than 50 countries on six continents.1 “To become a dental hygienist in Canada, you must have completed a two-year diploma program typically at a local college before you may be registered. Dental hygiene across Canada is a well-respected career with many opportunities. These possibilities include working in clinical, administration, education, research and public health positions. The wages vary throughout the country; from approximately $32 per hour in some areas to as high as $55 per hour in others. A surplus of new dental hygiene graduates in recent years has resulted in a decrease in wages in some regions. Some of the downfalls to practicing in different provinces are the different regulations. For instance, in BC, the hygienist cannot provide treatment without the patient receiving a dental exam in the previous 365 days unless the practicing hygienist has an extended duty module (resident-care module). All BC and Manitoba hygienists may also administer local anesthesia. In

Ontario, you may take further training to become a restorative hygienist. Dental hygienists in BC, ON, and AB are able to open their own private clinics and practice without a dentist on staff.”2 According to the ‘latest figures’, the highest hourly average wages are earned in Calgary, Alberta at $53 per hour and the lowest average wages are earned in Montreal, Quebec at $17 per hour. The largest sector of employment for dental hygienists and dental therapists is health care and social assistance. The average weekly earnings for the health care and social assistance sector as a whole were $800.90 in February 2011, an increase of 3.3% on the previous year when average weekly earnings were $775.57. Assuming an average working week of 36 hours, hourly average earnings in the sector for February 2011 are $22.25.3 The profession is a noble one but not without its issues. At the top of the list is the word GLUT. In Ontario alone there are 8,300+ registered dentists, close to 12,000 registered dental hygienists and the growth rate for hygienists is three times higher than that for dentists. And while more than 3,000 hygienists are able to self-initiate, only 150 or so are listed in the CDHO website.4 While insurance companies and patients look kindly on the 25 to 30% reduced fees for cleanings, screenings, and education, patients aren’t exactly beating down the doors for appointments. And hygienists aren’t knocking down the doors of senior citizens’ homes to provide low-cost care, either. In fact, the concept is having a hard time taking off.5 A noble profession with many issues. So much to talk about that in fall, 2011, Oral Health launches Oral Hygiene. No axes to grind. No Associations to please. Just hygienists to serve. Let’s talk. DPM

AACD Scientific Session Provides Collaboration, Continuing Education Madison, WI—More than 2,000 dental professionals gathered in Boston in May for the American Academy of Cosmetic Dentistry’s (AACD) 27th Annual Scientific Session, making the event one of the Academy’s most successful. The conference offered attendees more than 144 continuing education classes to choose from. The event, themed, “The Rise of Collaboration,” also featured many new dental products in this year’s exhibit hall, with more than 167 exhibiting companies and 660 representatives. The AACD celebrated the achievements of its newly accredited members and newly accredited Fellow at the Celebration of Excellence Gala, and honored a number of individuals for their dedication and excellence in the field of cosmetic dentistry. We Don’t like Mondays Mobile, AB—More than 80 percent of practices are not currently marketing new products and services and, while 95 percent of practices have websites, only half have Facebook fan pages, according to a survey by TeleVox Software, Inc., a US-based provider of engagement communications technology solutions. The report reveals some interesting patient behavior, including: • more than 50 percent of respondents confirmed that Mondays are the worst days for missed appointments; • more than three quarters of missed appointments are by men; and • nearly half of missed appointments are from patients in the 14-25 age group.

By the end of the year, her practice will also have a Kodak 3D CT Pan to integrate with its existing digital imaging software. Utilizing TDO, a software program developed by Dr. Gary Carr, the office is entirely paperless Dr. Kilislian opened Kawartha Endodontics in downtown Peterborough, ON, on May 20th, 2010. The design is attracting local attention. Indeed, the practice has become a downtown landmark with the Peterborough Chamber of

ing is that everybody is now aware of the procedure, and patients often ask their dental professionals for guidance. In fact, the number of patients seeking tooth whitening at their dental office today is far greater than it was prior to the appearance of the OTC tooth whiteners. Most practices report that in-office tooth whitening (Figs. B, C, D) is far more popular among patients than the at-home variety. The reasons are quite obvious: instant gratification and no need to wear those bulky, pesky bleaching trays. Dentists are on board as well: no need to fabricate trays, the treatment is a delegatable procedure, and typically higher fees. The ubiquitous presence of super white teeth on television and stage, in movies and magazines all function to increase the overall public impact of, and demand for, tooth whitening. The procedure is gaining popularity throughout the patient pool. These factors have worked synergistically to propel this elective treatment modality to its leading role in clinical practice. In-office (or power) bleaching is the single most important driving force in aesthetic/cosmetic dentistry today. As a corollary, it is the single most important and powerful factor in promoting fee-for-service and elective dentistry. Given its popularity and demand, it is incredibly

ost practitioners recognize that tooth whitening remains the single most popular dental treatment worldwide, with more than 400 million cases completed successfully. Both dentist-delivered and overthe-counter techniques are available. Professional tooth whitening is further subdivided into the in-office and the at-home modalities. By far the most commonly utilized format is the OTC approach. Dental manufacturers have been far more proficient than dentists in the education and promotion of tooth whitening, its beneficial effects, and its safety. While the profession had a 15-year head start over the retailers, individual dentists agonized endlessly over the propriety and the delivery modes of the available destaining procedures. Healthcare companies, correctly sensing the almost infinite demand for enhanced esthetics, simply jumped into the market, and approached consumers directly (Fig. A). While the OTC products are generally relatively low in cost, and they still take the lion’s share of all whitening procedures, their generic approach does not fit all consumers and all stains, and many individuals are more comfortable whitening their teeth under the supervision of a dentist. The upside of the OTC wave in tooth whiten-

surprising that tooth whitening is occasionally offered as a loss-leader with a complete exam; in effect, the practitioner is giving away the most lucrative and patient-attractive component of dentistry for free. Once the new patient has achieved their cosmetic goal, there is little incentive to stay with the dentist for all the necessary treatment. In-office bleaching has been around since the earliest days of tooth whitening. Early on, it was overshadowed by the easier, less technique sensitive, less expensive and less chairtime-intensive at-home procedures. As mainstream bleaching is entering its third decade of clinical utilization, the popularity and general acceptance of at-home bleaching has increased patient expectations; at the same time, the desire for instant gratification has decreased their willingness to patiently await the several weeks that are typically needed for at-home results. Patients want whiter teeth within the hour, rather than within the month. One of the drawbacks with in-office tooth bleaching was the associated transient tooth sensitivity (more common with chairside procedures than at-home ones) that often accompanied treatment, and was reported to last up to several days. While the discomfort was generally shortlived, it was rather intense, and often required medicat-

ing the patient prior to and after the procedure. In recent years, improvements in bleaching chemistry and in-office techniques have served to decrease the problem of bleaching sensitivity. This allows for greater patient comfort, and increases the confidence of the dental team. A majority of dentists now prefer in-office treatment for the additional control that it offers, the elimination of the overnight tray, and the immediate and instantly observable results. Dental consumers (dental patients) are far more focused on the color of their teeth today than ever before. Tooth whitening drives the patients’ initial interest from routine restorative to cosmetic dental treatments. In turn, cosmetic dentistry drives the greatly increased demand for restorative and tooth-retentive services that the profession has experienced over the past two decades. While this focus on aesthetics can be considered tangential, it has been instrumental in significantly improving the general level of dental health worldwide. aesthetically-minded patient. At-home bleaching, with its lower cost to the patient, is very easy, practical, and popular (Figs. E, F, G) Most dentists offer at-home bleaching, contributing to patients’ dental and oral health, as well as personal aesthetic satisfaction. Research and development are continually developing improved bleaching materials and simplifying clinical and at-home techniques. Effective in-treatment desensitization and pre-fabricated impression-less tray systems are relatively recent bleaching innovations. A growing clinical area is the retreatment of those patients who have previously bleached their teeth. Given the number of whitening treatments completed, this group represents a significant percentage of every dentist’s practice. These patients are motivated to maintain their optimal tooth whiteness level with regular touch-ups (Figs. H [sdi touch up kit], I). In fact, re-whitening should be a well-promoted component of every recall or recare visit; this element is a highly desirable motivating factor that encourages (from the patient’s perspective) the appointing and keeping of regular maintenance visits. In-office and at-home tooth whitening should not be thought of as completely separate procedures, however. They represent different approaches to the same end, and can offer complementary modalities to enhance the treatment process. It has long been recognized that in-office tooth whitening tends to relapse far more quickly when an at-home bleaching regimen is not used concurrently. Thus, in-office procedures should always be accompanied by an at-home component. Some of the more practical at-home whitening modal­i ties include the professionally administered whitening strips (Figs. J [Crest Supreme professional whitening strips], K [CAO whitening strips]). They require no impressions, no stone models, no tray fabrication, just a few moments of patient education and instruction. They have a higher level of hydrogen peroxide than their OTC counterparts and thus whiten teeth more rapidly and conveniently. They are also available only at the dental office. Bleaching lights have now been restricted to specific, safer wavelengths that produce less incidental heat and discomfort. While very popular with patients, and controversial among professionals, there is little documented evidence that they actually enhance or speed the whitening process. Light activated bleaching requires a photoreactive substance in the bleaching gel; no photo-initiator, no light bleaching effect. Recently, various catalytic agents have been introduced into bleaching gels to promote a more rapid breakdown of hydrogen peroxide, thereby generating more oxygen ions to speed the whitening process. The end point of tooth whitening is the same, regardless of the method, however. For difficult cases, such as tetracycline staining, there is every reason to use both in-office and at-home modalities in concert. The two techniques complement each other and are effective singly, or more effective together.

Considering that patients are likely be well versed with new tooth whitening treatment modalities and materials, it is imperative that the dental team be completely up-todate as well. Paradigm changes in destaining teeth are so common that practitioners should re-evaluate available treatment options on a regular basis (more or less annually), and then revise treatment accordingly. ZOOM Advanced Power System Fast, Effective Results

A “Whiter” Paradigm

Discus Dental introduced a chairside system that combines the Zoom Advanced Power Lamp with Zoom 25% hydrogen peroxide in-office bleaching gel to provide easier chairside whitening. To Light or Not to Light?

Light activation of bleaching gels has long been debated. Some studies with other chairside whitening units have demonstrated minimal, if any, noticeable differences in whitening when comparing teeth treated with the same bleaching gel without a chairside light. One drawback to chairside whitening has been redarkening after treatment. Recent studies have shown that Zoom gel used with the Zoom lamp works better than gel alone. Researchers at Loma Linda University have found that immediately after treatment teeth whitened with the Zoom System (lamp and gel) are 48% whiter than teeth whitened with gel alone. This conclusion was supported by Ontiveros and colleagues at the University of Texas Dental Branch Houston. Even 1 week after treatment, teeth whitened with the Zoom System remain 35% whiter than those whitened with gel alone.

Solving the Sensitivity Issue

Another drawback to chairside whitening is sensitivity as teeth rehydrate. Some patients demonstrate increased sensitivity after chairside whitening, especially those who have sensitivity before the procedure or have root exposure and enamel wear. The Zoom System uses a sensitivity protocol to maximize patient comfort. This includes the application of Relief ACP Oral Care Gel, which acts as a desensitizing agent that can be applied in trays or with a toothbrush.

CLINICAL TIPS

Successful Chairside Whitening with Zoom Advanced Power System

1. Isolation is critical because chairside whitening solutions typically use hydrogen peroxide bleaching gel, which is stronger than carbamide peroxide and can cause softtissue damage if it comes in contact with the gingiva or lips. Lip and tongue retractors, such as Discus Dental’s IsoPrep Retractor and Liquidam light-curable dam, can completely isolate the soft tissue from contact with the bleaching gel and exposure to the chairside whitening lights. 2. The whitening gel should be applied in a 1-mm to 2-mm thick layer over all tooth surfaces that will be whitened to ensure that adequate bleaching gel is present to whiten teeth when the light is applied. To avoid spreading the bleaching gel onto the soft tissue or under the isolation, remove the gel from the teeth using a surgical suction tip (now included in the new redesigned Zoom kit) after each 15-minute session. Avoid using the air/water syringe because this can dislodge the isolation and push bleaching gel onto the soft tissue. Any remaining gel can be removed with moist gauze. 3. Tooth sensitivity can be addressed by having the patient use a desensitizing agent. The Zoom System includes a unique Patient Post Care & Maintenance Kit that includes a syringe Relief ACP (Discus Dental). Relief ACP can be administered in either a take-home tray or applied with a toothbrush daily for 10 to 30 minutes. 4. Application of vitamin E oil to the lips before beginning whitening will minimize irritation. 5. Patients also should be advised that consuming coffee, tea, or red wine or smoking during the first 2 hours after chairside whitening can lead to staining and darkening of the teeth. It will take about 2 hours for a pellicle to reform on the enamel surface. 6. To maintain and further enhance the chairside whitening results, it is advisable to fabricate take-home trays to be worn by the patient for 30 minutes weekly. efore we can discuss fraud, we have to understand the difference between fraud and theft. Theft is a simple crime of opportunity. Theft is often not deliberate and is usually a one-time thing. Theft is also easier to detect. Fraud, on the other hand, becomes more complicated because it involves more deceit, planning and intent. It requires someone to perpetrate a crime and cover it up. It requires access to the records that must be manipulated in order to hide the action. And fraud usually occurs over a period of time — often measured in years. Once a system is compromised, it is often victimized in small increments repeatedly over an extended period of time. One reason is to keep individual transactions to small amounts that won’t attract attention. So, to make it worthwhile, the perpetrator needs to repeat the behavior many times.

Who commits fraud and why?

Eighty-five percent of people who commit fraudulent acts have no previous criminal record. The average profile of a fraud perpetrator is typically a long-term, trusted employee. According to statistics, people can be characterized by a 20/20/60 rule: 20 percent of people will always steal; 20 percent will never steal; 60 percent will exercise situational ethics. In other words, good moral people will undertake aberrant behavior when their personal circumstances are so dire that they feel they have no other choice. Often this situation is fueled by financial troubles. These financial troubles may be caused by gambling or drug problems, to major health issues, to the loss of family income, or even an unexpected tragedy in their personal lives.

Why focus on professional practices?

All commercial businesses can be targets for fraud but professional practices are at a higher risk than other commercial enterprises for many reasons. Fraud hits professionals particularly hard since it is damaging both professionally and personally. The professional is the face of the business and the anchor of the practice’s brand. So, while the practice absorbs most of the impact of fraud, the effects spill over into the personal lives of the professionals and their families. According to the 2010 Association of Certified Fraud Examiners (ACFE) Report to the Nations, the average loss against professional practices from fraud schemes is $110,000 per year due to billing schemes, cheque or payment tampering, expense or payables fraud and skimming of cash.

Why are professionals more susceptible?

There are several things that are common among professional practices that make them more likely to be victims of fraud. Among professionals, however, the group most often victimized by fraud is dental practices. While other professionals should also understand how fraud occurs in professional environments, the following information will focus on the dental industry. Some of the reasons for the

higher risk in dental practices are: • Dentists spend more time with patients, which leaves less time to manage their practices; • Dentists are highly trained in their profession but do not typically have much business training. They place great reliance on others to conduct the business aspects of the practice; • Dentists are ethical and trusting and attribute these same traits to their employees; • Typically, a lot of trust is placed in few people. Think of the typical staff structure of a dental practice — there are very few employees involved in the front office and they do everything related to the business aspects of the practice; • Dental practices are generally quite profitable so make attractive targets; • D ocumentation of processes and procedures is often lacking; • Anti-fraud controls and peer or management reviews are often lacking; • Red flags of fraudulent activity are often missed; • Employee turnover is often quite low. Long-term employees know the systems better and how to beat them. These long-term employees are also inherently more trusted by the dentist.

How can you assess your risk of fraud?

There are many things you can do to prevent fraud from happening or to detect it and lessen the impact. Below is a Top 10 list for all professionals -ask yourself whether or not you have done these: • I have created an internal culture that is intolerant of fraud. • I know the history and background of all my employees and require regular vacations be taken. • I know where I am vulnerable. • I have appropriately segregated duties in my front office. • I have software controls in place to track access to financial data. • Accounting reconciliations are performed in a timely fashion and reviewed. • I know what my results should be and I regularly monitor the accounts and reports personally. • I have a system of management oversight to review the work of my support staff. • I regularly assess the personal and professional ethics of my staff. • Banking activities require two signatures or management oversight. ince August 2009, dentists Jeff Diefenbacher and Jacqueline Sieber have based their practice in Kitchener, Ontario, a town in which finding an ideal location took five years. The new premises were ultimately chosen to keep the dental office in the town in which Dr. Nyle Diefenbacher-Jeff’s father-first founded the practice in 1953. The renovated building, featuring multi-textural stone walls, wrought iron chandeliers, wood ceiling beams, and relaxing earth-toned colours, resembles a comfortable cottage and reflects the vision of interior designer Rita Valente. Adding to the décor’s uniqueness are Dr. Sieber’s framed photographic compositions, which are displayed throughout the office. During the renovation, their contractor Van Del Construction experienced some challenges with the building — a former Royal Bank containing two vaults. One vault was converted into a bathroom while the other was transformed into dry storage, since its 28

AN RRSP on steroids

L

ooking for a way to defer tax today on excess earnings in your professional corporation, while at the same time salting away a far larger nest egg for your retirement? Then check out an Individual Pension Plan (IPP)-what we call an RRSP on steroids. An IPP is similar to an RRSP in that it uses an investment account that accumulates over time to provide you monthly retirement income. But unlike the RRSP, the IPP provides retirement guarantees that are known in advance, much like a teacher’s or government employee’s pension. IPP plan contributions are determined by a series of actuarial valuation reports to provide defined income at retirement. As a result, much larger amounts can be contributed to your IPP by your sponsoring professional corporation compared with an RRSP that you fund personally. This is especially attractive when income in the professional corporation exceeds the $500,000 active business income threshold.

Consider the following IPP advantages:

• larger tax deductions-up to 65% more versus an RRSP; • creditor protection; • top-up deductible contributions if the return on plan assets is less than 7.5%; • tax deductible “balloon” contributions available at retirement; • tax deductible administrative costs; • following your death, plan assets can transfer tax-free to your spouse or children when they are non-voting shareholders of the professional corporation;

• a dditional past service amounts going back to the professional corporation’s inception date increase the immediate tax deduction and the retirement benefits; and • I PP assets can be transferred taxfree to a locked-in RRSP or RRIF at retirement. Take, for instance, “Dr. H. Smith,” age 59, who established his professional corporation in 2006, drew a salary of $110,000 from 2006 to 2009 and will earn $125,000 this year, and has RRSP assets of $500,000. His wife Sharon, 57, who also works at his practice, has drawn a yearly salary of $50,000 since 2006, and has a $250,000 spousal RRSP and $125,000 personal RRSP. Together, the Smiths have no unused RRSP carry-forward room.

So, what benefits does an IPP offer the Smiths?

• Right out of the gate (or over the next several years), Dr. Smith’s professional corporation can make a tax deductible past service deposit of $133,500, and the Smiths can transfer RRSP assets of $90,700 to the IPP. • The professional corporation can also deduct the first-year IPP deposit of $48,000. • The deposit rises to $51,600 in 2011 and to $55,400 in 2012. • The IPP has a targeted rate of return of 7.5% annually and if the investment returns are less than the 7.5%, the professional corporation can top up the IPP on a tax-deductible basis. • At retirement, the most likely outcome will see the plan assets transferred to a

locked-in RRSP or RRIF from which the Smiths will draw a retirement income that is substantially higher than what would have been provided under the RRSP scenario.

Will an IPP work for you?

Where a medical professional has a pro­f essional corporation and draws employment income from it, satisfying the following conditions will determine if the right set of circumstances exist to create an IPP. • Are employment earnings (T4 income) more than $90,000 per year, inclusive of bonuses? • Is the medical professional 40 or older? • Has he or she been incorporated for several years to take advantage of past service room? • Have RRSP contributions been maximized to date and are they north of $350,000? In summary, the benefits for medical pro­fessionals to have an IPP funded by their professional corporation are significant. This includes a greater amount of tax-deferred growth when compared with an RRSP. As well, the professional corporation will receive the tax deduction for past service plus ongoing contributions.
