The annual gynecological exam for a woman is much more than just a pap test and review of the results. In fact, that is only a small part of the exam performed by Dr. Goldstein. While checking the cervix for abnormalities is important, Dr. Goldstein believes that for menopausal women bone health, breast health, and vaginal health are of the utmost importance.

A thorough internal exam begins with an examination of the skin of the reproductive area under a bright light, looking for any sores, lumps, bumps, or indications of an infection.

Next is the examination by speculum to check the cervix and vaginal walls such as white patches or ulcerations that look like canker sores – or a discharge that is unusual, a pap smear is taken. Cells are collected from two places, the surface of the cervix and the inside of the canal that leads up to uterus (called the endocervix).

The next is the bimanual exam, where the doctor inserts two gloved fingers into the vagina, places the other hand on the patient’s abdomen, and presses the pelvic organs. Many gynecologists also include a fecal occult blood test as part of the exam. In

addition to the bimanual exam, Dr Goldstein, a leading NYC Gyn, believes that there are enormous benefits to using transvaginal ultrasound as part of the routine exam.

For women closer to menopause the uterus and ovaries should also be checked by non-invasive transvaginal ultrasounds to prevent issues like abnormal uterine bleeding, painful periods, pelvic pain, vaginitis, urinary tract issues and more from arising.

The key is prevention. For instance, pre-cancerous lesions should be detected early before they become advanced and require a hysterectomy. The goal is to prevent the development of lesions that cannot be easily taken care of in an office setting with something as simple as an outpatient laser treatment.

Dr. Goldstein still believes that patients should be cared for one patient at a time with their individual risk factors, personal history, and family history taken into account regardless of the medical situation under discussion.
