THREE CONSIDERATIONS FOR WOMEN SEEKING HORMONE REPLACEMENT THERAPY FOR MENOPAUSE.
Doctor Steven R. Goldstein MD is a Certified Menopause Practitioner, a past President of the North American Menopause Society and a past President of the International Menopause Society. He has also coauthored several books on hormone replacement therapy such as “The Estrogen Alternative”. For over 25 years Dr Goldstein has helped women to overcome and at least cope with the symptoms of menopause in his role as a Menopause Specialist in NYC.
It is noteworthy that while many women seek relief from the symptoms of menopause, there are many who do not find it or wind up in a worse condition. To illustrate this, a patient of Dr Goldstein's private practice in NYC recently left this review:
"Amazing experience. After years of working with doctors who don't fully understand hormone replacement it has been a blessing to find Dr.
Goldstein. The regimen he has me on has me sleeping better with no more night sweats. Thank you!!"
Name withheld to protect her privacy. However, it is worth noting that this patient sought out several doctors for the symptoms that she was experiencing due to menopause. If you are a patient seeking treatment with hormone replacement therapy (HRT) for menopause Dr Goldstein, a top HRT Specialist in NYC has mentioned several considerations in his writings that you must take into account. Three of them are listed below, although there are more that he discusses with patients that he sees personally.
First, it is important to remember that there is no blanket Hormone Replacement Therapy regimen that can be applied to each woman. It is not as simple as just writing a prescription. HRT is NOT formulaic, meaning it is not applied in a slavishly followed rule. Each woman must be evaluated as an individual. Her symptoms, her overall health, her gynecological health are all factors in determining what type of HRT is best for her. Dr Goldstein examines each of his patients thoroughly and listens to what they are experiencing. This is not a five-minute examination; patients must allot at least an hour for a visit with Dr Goldstein.
Second, the endometrium (uterine lining), MUST be examined. It is a biomarker for what is going on with the ovaries that produce the hormones estrogen and progesterone and factored into the hormonal status whether a woman is pre, peri, or post menopause. This examination is done via Transvaginal Ultrasound, a painless procedure done in Dr Goldstein’s office in a few minutes.
Third, hormone replacement therapy must treat the symptoms, and NOT the level of the hormones in menopausal women. This is the problem with using some of what is known as "bioidentical hormones". Some practitioners who use bioidentical hormones perform salivary
testing on patients to determine their levels of hormones. Hormone Replacement Therapy, unlike thyroid disease, is not based on levels of hormones, it is based on symptoms. Doses are varied until symptoms are relieved.
As mentioned earlier, each woman's body is different, each is experiencing different symptoms or a combination of different symptoms. This will determine what is to be recommended to the patient.
