The Trust is hosting a prestigious Celebration Ball on Friday November 5th 2010 at the Tower Ballroom, Blackpool. The Celebrating Success Awards will take place on the night to celebrate the achievements of staff and recognise the fantastic progress we continue to make as an organisation.

Who can apply for or nominate colleagues for an award? All staff working directly or indirectly (e.g. contract staff) for the Blackpool, Fylde and Wyre Hospitals NHS Foundation Trust are eligible to apply for or nominate colleagues for an award according to nomination rules for each award category. Trust volunteers are eligible to be nominated for Radio Wave’s Unsung Hero Award and the Gazette’s Patients’ Award categories only.

Who can attend the Celebration Ball? All staff working directly or indirectly for the Blackpool, Fylde and Wyre Hospitals NHS Foundation Trust are eligible to attend the Celebration Ball. The event includes reception drinks, three course dinner, Celebrating Success Awards Ceremony, live entertainment and disco. This information pack gives you everything you need to know about the Celebrating Success Awards 2010 including how you can personally apply for or nominate an individual or team to win an award. Simply select an award category and submit an Application Form either online or by post to enter. A designated number of free tickets for the Celebration Ball are available for those shortlisted to receive an award.

The awards recognise and honour outstanding performance by individuals and teams within the Trust.

This event creates welcome positive publicity for hospital staff and the Trust. It also raises the internal profile of all those shortlisted to receive an award.

A short film to be shown at the Celebration Ball ensures that the achievements of shortlisted individuals and teams are communicated to everyone attending the event and will support even higher standards of patient care and innovative working practices.

The awards are about far more than simply winning prizes: they provide a platform for entrants to contribute to raising standards in healthcare throughout the Trust.

An opportunity to showcase everything that is good about our people and our organisation. Simply going through the application process for an award allows applicants to take a step back, appraise their own or their team’s work and focus on positive achievements.

Award categories 2010 The eight award categories for 2010 are listed below. Trust volunteers are eligible to be nominated for Radio Wave’s Unsung Hero Award and the Gazette’s Patients’ Award categories only. Please submit a separate Application Form for each award category entered.

Team of the Year Award: Prize £500 Recognises the outstanding teamwork that goes on throughout the Trust. Applicants will have to demonstrate how working as a team has helped to bring about service improvement or quality of patient experience. Staff can nominate their own team or any other team within the Trust, whether clinical or non-clinical, for the Team of the Year Award.

Radio Wave’s Unsung Hero Award: Prize £500 For the person who has consistently gone that extra mile to provide the best service possible for patients, visitors or other Trust staff. Unsung Heroes are to be nominated by other members of staff only. Staff cannot put themselves forward as an Unsung Hero. This category is also open to Trust volunteers to nominate a colleague or be nominated.

Partnership Award: Prize £500 Recognises an individual or team who has developed successful partnership initiatives with other organisations such as PCTs, Social Services, Local Authorities, Voluntary Organisations, Charities etc. Staff can nominate themselves, their own team or any other individual or team within the Trust for the Partnership Award.

Inspirational Leadership Award: Prize £500 Recognises excellent leadership approaches or leaders that can demonstrate positive and sustained impact on patients, service users and staff. Teamwork is the bedrock of effective service delivery but sometimes it is the contribution of an individual that makes the critical difference between a good service and an outstanding one. Staff can nominate themselves or any other individual within the Trust for the Inspirational Leadership Award.

Service Transformation Award: Prize £500 Recognises a service for patients that has been truly transformed. Transformation means fundamental change. Typically change that is transformational is wide in its scope and fast in its pace. Staff can nominate themselves, their own team or any other individual or team within the Trust for the Service Transformation Award.

Patient Safety Award: Prize £500 This year we have introduced a new award category called the Patient Safety Award. This award recognises a team which has consistently provided high quality care and shown continuing dedication to patient safety. Applications could include examples of how teams have reduced the numbers of patient falls, medication errors, infection rates, pressure sores and performed consistently well in areas such as nursing indicators. Staff can nominate their own team or another from within the Trust for this award.


The Gazette’s Patient’s Award: Prize £500 For the person who deserves special recognition of their service excellence. This award will receive nominations for Trust staff and volunteers from patients, their carers and/or relatives. It is a chance to thank staff members who have gone that extra mile to achieve high quality patient care. Staff and volunteers cannot nominate themselves or any other member of staff to receive the Patients’ Award unless they themselves have been a patient recently and wish to nominate the staff who cared for them during this time. Requests for patient nominations will be made via a number of communications channels: • Local press and media such as The Gazette • Trust website at www.bfwh.nhs.uk/about/success • Posters at all Trust sites Patients will be able to submit their nominations either online or by post via: • • • •

Chairman’s Award: Prize £1,000 This award will go to one of the winners of the other seven award categories. It will go to the individual or team the Chairman feels has made the most outstanding contribution overall. The recipient of this award will be decided solely by the Chairman and this award category is not open to applications or nominations.

The judging process The Judging Panel will shortlist three individuals or teams per category. Each entry will be judged on the guidelines outlined below...

Rules • Individuals and/or teams can enter more than one award category but must submit a separate application form for each award category • By entering, the applicant(s) agree to co-operate in the judging process • Shortlists will be published in the middle of August 2010

• Shortlisted individuals and teams will receive a designated number of free tickets for the Celebration Ball 2010 • Shortlisted individuals and teams automatically give their consent to appear in a short film highlighting their achievements to be screened at the Celebrating Success Awards Ceremony • The judging panel will choose one winner and two runners up per award category • The judging panel’s decision is final • Prize money is not a personal reward but to be reinvested in the Trust to further develop your award-winning project or within the winner’s area of the Trust to benefit patients


Guidelines on completing your Application Form The guidelines below may assist you in completing your Application Form. Summary Please provide a concise summary of your award application in no more than 200 words. It may be easier to draft this section after completion of the main body of your application, when you are clear on the key points of your submission.

Team of the Year Award You may like to support your application by including details of: • Developing and delivering excellent relationships with patients, staff and partner organisations • Achievement against the odds • Changing the way services are delivered to bring about continuous improvement for patients and service users • Successful delivery of goals and targets in a harmonious team environment

Radio Wave’s Unsung Hero Award You may like to support your application by including details of: • The nature of their role and length of service • Examples of their loyalty, outstanding service and devotion to their role, patients and/or staff • The personal and professional qualities that make this individual stand out

Partnership Award You may like to support your application by including details of: • Effective multi-agency working and partnership arrangements with local agencies • Shared vision and values across all teams involved • A creative approach to developing services • Actively encouraging the engagement of all those in support services


Inspirational Leadership Award You may like to support your application by including details of: • Has the individual made a contribution above and beyond that of colleagues or peers in comparable organisations? • A focus on innovation and improvement for the benefit of users and staff • Full engagement of staff and service users • Directly enhancing the quality and experience of care for service users and their families • Team working, particularly across professional and organisational boundaries to achieve improvements in the wellbeing of patients and people who use services • Communicating and demonstrating clear values, aims and approach • How team development has contributed to the sustainability of improvements in service delivery • Evaluation and measurement of the impact of leadership on service delivery

Service Transformation Award You may like to support your application by including details of: • Fundamental change that has resulted in services that will meet the needs of the future as well as today • A fast pace of change • Changes to the people or roles involved in delivering care and when, where or how it is delivered • How the resulting care service has been transformed in ways that have a positive impact on patients/ service users, staff and the organisation • Large gains in performance, such as increased efficiency and effectiveness of the service, increased safety and improved access • Feedback from patient/service users and staff that they find the transformed service to be an exceptional experience in terms of both receiving and delivering care

Patient Safety Award You may like to support your application by including details of: • How the team has consistently strived for excellent patient care and safety • The direct effect that the work has had on patients and examples of goals achieved through teamwork • Examples of changes made to work enviroment and practices for the safety of the patient
