hen a patient walks through the doors of North Colorado Medical Center, there isn’t a moment when he or she isn’t seen. Patients are continuously monitored, with charts — which show their medical history and treatments — immediately and electronically available wherever they go. In the intensive care unit, patients are continuously monitored, even if doctors and nurses aren’t physically in the room. “We’re able to look up anything at a moment’s notice,” said Robert Janata, a doctor in NCMC’s pulmonary, critical care and sleep center. Phoenix-based Banner Health manages NCMC and operates 23 hospitals in seven states across the West. Janata said NCMC has joined in a Bannerwide initiative to use its systems to focus on sepsis, a potentially lethal infection-related condition that leads to inflammation and organ dysfunction. Historically, sepsis led to a 40 percent mortality rate, although it’s hovered around 20 to 30 percent since the 1970s. It’s still the No. 10 killer in the U.S. after Alzheimer’s disease and kidney disease, according to a 2004 article published by the American Medical Association. Maggi Basinger, director of NCMC’s ICU and Burn Center, said the hospital uses its electronic medical records to track patients in real time, in general checking up on patients once every half-hour. If a nurse or doctor can’t reach a patient, Banner Health monitors patients from its hub in Arizona. The electronic monitoring system, called iCare, was implemented in 2006, but it’s grown since then as the hospital adds clinical alerts and “best practice measures,” which have allowed doctors to track and alert patients with sepsis symptoms.

The system isn’t unique to NCMC; about 10 percent of intensive care units across the country use it. But Banner has one of the largest systems in the country, according to its website. Patients’ rooms also come equipped with what Basinger calls the “eye in the sky,” a camera above the patient’s bedroom that comes equipped with a microphone. If a patient’s monitor detects a problem and a doctor or nurse isn’t available, a Banner doctor can look in at any time with a camera that can zoom in far enough to detect pupil changes of two millimeters. Doctors can use the system to speak directly to patients. But eventually it will include a screen that will visually connect patients to the doctors looking

in on them, which Basinger said will make the surveillance concept “a little less disconcerting.” The monitoring system runs on the pricey side. According to Robert Groves, Banner Health medical director for critical care, the program cost between $2 and $5 million in initial capital and costs about $28,000 annually to run the program for each of the 52 beds. But between 2007 and 2010, patients spent about 26,300 fewer days in the ICU, translating to lower hospital costs and patient bills. Sara Quale, a Banner spokeswoman, compared the initiative to how people respond to a heart attack. “You don’t just wait around, you call 911. You

get going when you feel the chest pain,” Quale said. “Now one of the focuses nationwide and for Banner is to focus on sepsis more or less in the same way.” Banner has aimed to keep the sepsis mortality rate below 7 percent throughout its system. So far this year, NCMC has maintained a 2 percent mortality rate. Janata said the difference is in the consistency of patient care, saying the doctor-patient ratio and the frequency of system checks is “quite an achievement” for a hospital in an area such as Greeley. “I don’t know if I’d consider Greeley rural or not … but we have pulmonary critical care docs that are running the ICU every day of the week every moment of the day,” Janata said. “Not to say other hospitals don’t have good critical care doctors, but if you’re a more local hospital, you don’t have critical care doctors looking at every patient.” The system also tracks other health problems — such as heart failure, pneumonia and urinary tract infections — but generally to a lesser degree. Banner’s live data collection system has been running for about two years, Janata said, but it continually adds information and changes procedures based on the feedback it receives from its hospitals. NCMC continuously makes changes to what gets monitored based on what needs more tracking. Basinger said the system also allows NCMC and other hospitals to standardize care, creating digital “checklists” for the treatment of illnesses that allow doctors and nurses to follow tried-and-true methods, which is especially important if the medical worker isn’t familiar with a particular illness. Both Janata and Basinger acknowledged that every person is different and needs individual care, but they said that standardizing care that works prevents mistakes and saves time. “In that past you’d have 10 different approaches to treating sepsis, and probably some of those ways led to bad outcomes,” Janata said. “So treating things that have proven again and again to work … is probably the better way, as opposed to cookbook medicine.” NCMC has used the same idea to strictly enforce a standard order of steps, which makes sure doctors perform the correct procedures on patients (such as not performing consecutive CT scans). It also ensures doctors and nurses use sterile procedures to limit infection. Basinger said one of the biggest problems with any corporation is its inability to quickly innovate. The new systems will continue to build on themselves and change quickly. “In the hospital business, you don’t have time to sit back and fiddle around with things,” she said. “You want to make sure you get information out there if it’s something that saves patients’ lives.”
