• Adaptations to your home , such as stair lifts, widening of doorways, ramps, specialist beds, walk-in showers, low maintenance gardens, raised sockets and pull-down kitchen storage baskets; • Assistive technology – there are a number of different systems and devices which can help a person stay safe and supported in their own home, for example; pendants linked to community alarm systems, falls detectors, timed medication dispensers, trackers and memory prompts; • Access to community services such as luncheon clubs and groups; • Domiciliary care (home care) • Carer support schemes - respite stays in a care home for a short break • Shared Lives service - long and short stays within a family setting • Befriending schemes - run by voluntary organisations

What options? – Care home or care at home

If you need some care and support it is important to look at all the options available to you, to make sure your needs are met.

This could be care and support within your own home, moving into a residential or nursing home or specialist accommodation, including sheltered or retirement housing, assisted living and extra care housing, which is designed with the needs of frailer, older people and with varying levels of care and support available on site.

Care at home

The care and support you receive, in your own home or in a care home, will be planned to meet your individual needs in such a way that your privacy, dignity, well-being, safety and independence are maintained at the highest level.

If you need care and support at home it can be provided through an agency. Trained staff will visit you at home at an agreed time during the day or evening, to help.

This is sometimes referred to as domiciliary or home care and you can arrange home care privately, or contact your local authority, to see if you are eligible for services supported by them.

When home care providers help people with personal care, such as helping with bathing or dressing, they have to be registered with the Care Quality Commission (CQC) and they are inspected according to a set of national standards.

Many people prefer to stay in their own home and this may be possible with care and support from families, friends, the community, voluntary organisations and the councils.

Care homes For some people, moving to a residential or nursing care home will offer the necessary support and reassurance to keep them supported and safe. There are many different types of care homes available such as small homes, large homes, specialist services such as dementia care, and homes for people experiencing mental health issues or those people who have a learning disability. The choice of care homes ranges from small, family-run homes registered for a few residents through to large purpose-built homes run by national companies. Some are still owned by local authorities.

All care and nursing homes are registered and inspected by the Care Quality Commission (CQC) according to a set of national standards.

The purpose of these standards is to make sure that the person receives the care and assistance they need in a safe and reliable way. You can see a copy of of latest inspection reporters on care and nursing homes by going to the CQC website: www.cqc.org.uk or you can ask the home for a copy of their last inspection report.

Whichever home you choose there should be trained staff on duty throughout the day and night to help with activities for daily living and social events which should be tailored to suit individual needs. Often homes have open visiting times and encourage friends and families to visit and socialise.

There is a list of the homes in this booklet. It also outlines how many people can live in each home and if they offer specialist services for example dementia care.

Your rights If you have any care and support in your own home or in a care home, you have certain rights including being asked about the way your care and support is provided. If you are at home, it could be by talking to your social worker or if you are in a care home it may be by talking to the manager. In some cases, it may not be possible for the care and support provider to fully meet your request, maybe because of regulations or your request involves an expense that the provider would not usually be responsible for. If you feel that your care should be improved then you have the right to tell a person in authority such as the manager.

Legal and financial advice If you are planning your care and support needs for the future, it is important to get the right financial advice as you will be expected to contribute to the cost of your care service. The cost of care can be high so it’s really important that you get good advice on how you can best meet these costs. You can get information about this from the council or from the Financial Services Authority at http://www.fsa.gov.uk/register/home.do Each of us hopes to remain healthy and independent for as long as possible, but there can come a time when we aren’t physically or mentally able to make decisions for ourselves.

For this reason, it is worth planning well in advance so someone you know and trust can manage your affairs for you if the need arises.

You might need to give someone you trust, or a number of people, the legal power to act on your behalf if and when you are not capable of making your own decisions. This legal power is known as Lasting Power of Attorney (LPA).

You are encouraged to get independent advice to consider whether appointing a Lasting Power of Attorney or a Deputy under the Court of Protection would be in your best interests.

Paying for your residential/ nursing care The cost of care is one of the deciding factors when choosing a care home. Therefore it is important to know your financial situation before making any choices and to be aware of any financial help you may be entitled to from councils or the NHS. Your council can complete an assessment to see if you are eligible for care and support and this will include assessing your ability to contribute to the cost. The council will comply with the Care Act 2014 and associated legislation when they assess your ability to contribute to the cost.

Paying for care in your own home

Where possible, your council will encourage and support people to stay in their own home.

This can be paid for privately or using personal budgets given to you by the council. Before you are given a personal budget for your care and support, the council will complete an assessment to see if you are eligible and this will include assessing your ability to contribute to your personal budget. The council will comply with the Care Act 2014 and associated legislation when they assess your ability to contribute to the cost.

People can get a personal budget in different ways. Some of these are explained below and people can choose more than one option;

If your care is arranged by councils, your assessment will be reviewed annually but can be done at any time. If there is a change in your needs or financial circumstances you should request a reassessment. Even if you can afford to pay the care fees yourself, you should still ask for an assessment in case you can’t afford to pay them in the future. If this happens the council will need to have agreed you are eligible for the type of care you are planning to have.

If you have not done this you may need to move care home when you require financial help from the council.

Direct Payment This involves an amount of money being paid to you, or your agreed representative, normally on a 4 weekly basis so that you can pay for the support that has been agreed. This is the most flexible method of receiving a Personal Budget because you are fully in control of the money and the support or services being provided.

The payments are made less any contribution that you are required to make towards your care services.

Personal Health Budgets Personal Health Budgets (PHBs) help people to know how much money is available to spend on parts of their NHS care. This means they can discuss and agree the best and most helpful ways to spend their money with their families, carers and the professionals involved in their care. The idea of PHBs in health follows the successful introduction of direct payments and personal budgets in social care.

Leicester City, West Leicestershire and East Leicestershire & Rutland clinical commissioning groups are offering PHBs to adults eligible for NHS continuing healthcare.

People with PHBs will be able to choose the health outcomes they want to achieve, know how much money they have for their care and support, create their own care plan, choose how their budget is held and managed, including the right to ask for a direct payment and ensure their assessed needs are met in ways and at times which make sense to them. PHBs will not be the answer for everyone, but they may be a good way for some people to get the healthcare services which are right for them.

Things to consider when choosing a care home or moving to extra care housing Making the decision to move home is one that should be considered carefully.

You should visit the home with family and friends if possible, try and make more than one visit, and perhaps arrange visits at different times of the day. The first care home you see won’t necessarily be the right one, so make sure you visit a range of homes to see which will suit you.

If you receive help from an independent organisation with choosing a care home, make sure that there are no‘hidden’administration charges, and that you will not have to pay a higher fee to the care home because of extra charges made to the home. It’s also important to make sure that any independent organisation can show you all the homes that can meet your needs in the areas of your choice as some organisations only work with a few homes with which they have interests.

Below is a list of things to ask and consider when you have decided that this is the right time to move into a care home or specialist accommodation.

There may be other things that you want to find out before making your final choice: Location and building • Where is the home, will my visitors be able to get there easily?

• Will I be able to get to shops, pubs, parks, local community facilities and places of worship easily?

• Will it be easy for me to get into and out of the building, and to move around the home and between different floors?

• Can I bring my own things, such as furniture & pictures?

• Will I be helped to make choices about my day to day routine, can I get up and go to bed at times that I choose?

• Are there any restrictions on visiting times or the number of visitors I can have?

• Where can I spend time with my relatives when they visit?

• Can the home meet my cultural and religious needs?

• Is there a choice of food, and can I decide when and where I can eat?

• Can I use a computer at the home, or am I able to use my own computer equipment?

• Is there a range of activities provided?

• Will I be able to keep my hobbies and interests?

Care needs • Is the home registered to provide the Ievel of care that I need?

• Do the other residents seem to have a similar level of need to me?

• Can I choose how support with personal care is to be provided?

• Does there seem to be enough staff on duty to meet the needs of residents?

• Do the staff talk to residents in ways that show they are respected and their opinions are listened to?

• How does the home plan to meet my needs in a dignified, respectful manner?

What are the homes fees?

• Is it clear what the fees cover, and whether you may be charged any extra fees and what these would cover?

• Are fees to be paid weekly, four-weekly or monthly? Are they paid in advance or in arrears?

• Is a top up fee needed, that is, do the home charge more than the amount the council has agreed to pay for my place?

• What are the arrangements for handling my personal money?

• How will my valuables be kept safe and secure?

Support for carers If you care for an adult living in Leicestershire, and it’s having an impact on your health or home life, you might be eligible for a carer’s assessment.

The county council is supporting carers – the people who are looking after a friend or family member who isn’t able to look after themselves and needs care because of long-term illness, disability, mental health problems, old age or alcohol/drug abuse.

You must not be paid to do the care – or be doing the care as part of your work for a voluntary organisation.

• Financial or material abuse such as theft, fraud or exploitation, pressure in connection with wills, property, or inheritance, misuse of property, possessions or benefits.

• Neglect such as ignoring medical or physical care needs, preventing access to services or withholding food, drink or heating • Discriminatory abuse – harassment based on a person’s race, sexuality or disability • They have a sensory impairment, e.g. vision or hearing loss • They are old and frail • They are ill Vulnerable adults are more likely to experience abuse than the rest of the population, and may be less likely to be able to take steps to keep themselves safe or use services that will help them to stay safe.

What is abuse?

Abuse is mistreatment by any other person that violates someone’s human and civil rights. For example, abuse can vary from treating someone with disrespect in a way that significantly affects their quality of life to causing actual physical suffering. Abuse can happen anywhere; in a residential or nursing home, in a hospital, in the workplace, at a day centre or educational establishment, in supported housing, in your own home or in a public place (e.g. the street).

Forms of abuse include:

• Institutional abuse – abuse in residential homes, nursing homes or hospitals when people are mistreated because of poor or inadequate care, neglect or poor working practices.

Any of these forms of abuse can be deliberate or the result of neglect or not doing something.

Who might be causing the abuse?

The person (or people) responsible for the abuse are often very well known to the person who is being abused.

They could be: • A relative, friend or neighbour A health worker, social worker or support worker • Another resident or service user • An occasional visitor or someone who is providing a service • Someone who deliberately exploits vulnerable people What should you do if you think someone is being abused?

• Physical abuse such as hitting, pushing, pinching, shaking, misusing medication, scalding, restraining, hair-pulling • Sexual abuse such as rape, sexual assault or sexual acts to which a vulnerable adult could not have consented, or where they were pressurised into consenting

Anyone who is concerned that an adult is being harmed abused or neglected should contact us using the relevant contact details.

• Psychological or emotional abuse such as threats of harm or abandonment, being deprived of social contact, humiliation, blaming, controlling, intimidation, coercion, harassment, verbal abuse, or being prevented from receiving services or support

Our unit is Nurse led and provides high standards of care for the Nursing and Specialist Care residents.

We provide this care across the adult age range from 18 years and for both male and female residents.

The value of our service can be measured in the Units ability to assist and co-operate in the training of Student Nurses in partnership with De Monfort University. We also undertake the Overseas Nursing Course in conjunction with Northampton University.

We have for a number of years past held contract with the local Primary Care Trust to provide nursing care for patients with Generic or Acquired Neurological conditions. This includes Motor Neurone, Huntington’s, Multiple Sclerosis, Brain Injury, Long-term Chronic stable Respirator dependent residents.

Twenty four hour nursing care is provided by our team of trained nurses using individual care plans drawn up in consultation with resident’s families and General practitioners to enhance Independence and ensure individuals choice of privacy.

Short and long term places are available including respite, convalescence, and week end or night placement.

The Building was enlarged and developed some years ago to serve as a private hospital.

Consequently, the accommodation is generously proportioned with rooms for 30 residents.

Our Chef’s provide a high standard of home cuisine and special dietary needs are catered for.

We advise all clients to make a Lasting Power of Attorney (LPA) in respect of their financial affairs and their health and welfare.

Things to consider include making sure that a Lasting Power of Attorney and an up-to-date Will are in place. If there’s a property to sell to enable a move into a care home then we can also take care of the conveyancing.

A variety of circumstances can make a Will outdated. These include getting married after a Will was made (if the Will was not made in contemplation of the marriage), a change in your wishes or a change in the value of your assets. It may also be the case that your Executors are no longer able to fulfil their role.
