• Adaptations to your home e.g. stair lift, widening of doorways, ramps, specialist beds, • Assistive Technology • Access to community services such as luncheon clubs and day services • Domiciliary Care (home care) • Carer support schemes - respite stays in a care home for a short break • Shared Life Service - long and short stays within a family setting • Befriending schemes - run by voluntary organisations It may be that people want to move to accommodation that is more suited to helping them stay independent such as an ExtraCare Scheme or Sheltered Housing. These facilities can provide a higher level of on-site services including access to 24 hours care and support, meals etc.

Care at Home or Care Home?

As we get older we sometimes find it very difficult to look after ourselves and our homes in the way that we want to. Daily routines become more difficult and we can become more socially isolated. It is at this time that many people, along with their families and friends start to think about the future and whether or not they should remain in their own home or to look at moving into Residential / Nursing Care.

Care at Home

If care at home is needed support can be provided through a domiciliary care agency.

This could either be arranged by the Council, or through the person receiving a Direct Payment and buying in their own services. The Council can provide information and advice about this.

Trained staff will visit the person’s home at an agreed time during the day or evening to help with their care and support, for example help with dressing. This is sometimes referred to as “domiciliary or home care”.

Where providers of domiciliary care services carry out personal care (such as help with bathing or dressing) they have to be registered with the Care Quality Commission (CQC) and are inspected according to a set of national standards.

Care at Home

Many people prefer to stay in their own home and this may be possible with a combination of help from families, friends, the local authority, and independent community and voluntary organisations. Services available may include;

The help that a person receives, whether in their own home or in a care home, will be planned to meet their individual needs in such a way that their privacy, dignity, well-being, safety and independence are maintained at the highest level.

Care Home For some people, moving to a residential or nursing care home will offer the support and reassurance to keep them supported and safe.

There are many different types of care homes available: small homes, large homes, specialist services such as dementia care, homes for people experiencing mental health issues or those people who have a learning disability.

The choice of care homes ranges from small, family-run homes registered for a few residents through to large purpose-built homes operated by national companies as well as local authorities.

Support is available to help you with the funding application forms which will need to be completed and submitted before any decision about funding your care and support can be made. As part of this process adult social care usually need certain information regarding your personal finances, all information collected is held confidentially.

Currently, all care and nursing homes are registered and inspected by the Care Quality Commission (CQC) according to a set of national standards. These standards have been devised to ensure that the person receives the care and assistance they need in a safe and reliable way. However, under the new Care Act, there will be a new inspection regime, which is expected to come into force in October 2014.

Whichever home you choose there should be trained staff on duty throughout the day and night to help with activities for daily living and social activities which should be tailored to suit individual needs. Often homes have open visiting times and encourage friends and families to visit and socialise.

Care at Home

There is a list of the homes on pages 13 to 33 of this booklet. It also includes the capacity and the specialist client group each home is able to cater for.

Under the Care Act a new online provider profiles website has been set up on the NHS Choices website http://www.nhs.uk/

carersdirect/Pages/CarersDirectHome.aspx that will help people choose, compare and comment on care homes and other care services. The aim of the website is to give people more choice, better information in order to identify good care – and give them an opportunity to give feedback on the service they’re getting.

Legal advice Each of us hopes to remain healthy and independent for as long as possible, but there can come a time when we aren’t physically or mentally able to make decisions for ourselves.

For this reason it is worth planning well in advance so someone you know and trust can manage your affairs for you if the need arises.

You might need to grant someone you trust, or a number of people, the legal power to act on your behalf if and when you are not capable of making your own decisions. This legal power is known as Lasting Power of Attorney (LPA). Power of Attorney, is a legal agreement to enable third parties such as close family members to act on your behalf in your best interest. There are two types of Lasting Power of Attorney that can be arranged: • Property & Affairs Lasting Power of Attorney • Personal Welfare Lasting Power of Attorney You should contact your family solicitor or the Citizens Advice Bureau for more information.

It is strongly advisable to arrange both kinds of LPA so your affairs are fully covered. You may also want to ask about the Court of Protection (part of the Office of the Public Guardian).

from the Council or the NHS. Before your local authority can consider assisting with funding the cost of any care home placement, your needs will be assessed to establish whether you are eligible for funded support.

Currently, social care and NHS assessments comply with the relevant legislation and government guidance, including the National Assistance Act 1948 and the NHS and Community Care Act 1990. Even if you can afford to pay the care fees yourself, it is still advisable to have your needs assessed by your local authority.

Your rights Whether you receive assistance in your own home or in a care home, you have certain rights including the right to be consulted about the way your care and support is provided. If you are at home, it could be by contacting your social worker or if you are in a care home it may be by talking to the manager.

In some cases it may not be possible for the care and support provider to fully meet your request, maybe because of regulations or your request involves an expense that the provider would not usually be responsible for. If you feel that your care ought to be improved then you have the right to bring this to the attention of a person in authority such as the manager.

This will be reviewed annually but can be reviewed at any time if there is a change in your financial circumstances. However, if you choose not to seek advice from the Local Authority, it is advisable to look into the non means tested help that you may be entitled to, which may include; • • • •

Where possible your local authority will encourage and support people to stay in their own home using personal budgets to cover the cost of home care. Personal Budgets are calculated by completing a questionnaire to assess the level of care needed and the outcomes you wish to achieve through the support.

This information is then used to identify how much money is needed to pay for this.

Support might take the form of traditional services like home care and day service attendance, or other more flexible and creative approaches but other more flexible and creative methods can also be used to achieve the goals that you identify. Your Local Authority will discuss

Cash Personal Budget This involves an amount of money being paid to you, or your agreed representative, on a 4 weekly basis so that you can pay for the support that has been agreed.

The money has to be paid into a separate bank account. These payments are made less any contribution that you are required to make toward the budget, so you need to pay your contribution into this bank account.

This is the most flexible method of receiving a Personal Budget because you are fully in control of the money and the support or services being provided.

Provider Managed Account This is a variation of a cash budget but in this case the money is paid to a service provider of your choice. Again the payments are made less any contribution that you are required to make toward the budget.

However, in this case the provider will bill you for your contribution so that they can use the full budget to pay for the services that they provide. This offers a little less flexibility than a cash payment made to yourself, but you do not need to set up a separate bank account.

Council Managed Budget In this case you are billed for your contribution on a 4 weekly basis in advance. The council arranges the agreed support, but this is restricted only to services that are provided under contract to the council so is less flexible.

Due to the administration required to manage this, there is less scope for variations in service.

Care Home considerations Making the decision to move into a care home is one that should be considered carefully.

You should visit the home with family and friends if possible, try and make more than one visit, and perhaps arrange visits at different times of day. The first care home you see won’t necessarily be the right one, so make sure you visit a range of homes to see which will suit you or your loved one best.

If you receive help from an independent organisation with choosing a care home, make sure that there are no ‘hidden’ administration charges, and that you will not have to pay a higher fee to the care home because of extra charges made to the home. Please ensure that the independent organisation show you all the homes that can meet your needs, in the geographical area of your choice as some organisations only work with homes whom they have a commercial agreement or interest.

Below is a list of things to ask and consider when you have decided that this is the right time to move into a care home. • Will I be helped to make choices about my day to day routine, can I get up and go to bed at times that I choose?

• Are there any restrictions on visiting times or the number of visitors I can have?

• Where can I spend time with my relatives when they visit?

• Can the home meet my cultural and religious needs?

• Is there a choice of food, and can I decide when and where it is to be eaten?

• Can I use a computer at the home, or am I able to use my own equipment?

• Is there a range of activities provided? Will I be able to keep my hobbies and interests?

Care needs • Is the home registered to provide the level of care that I need?

• Do the other residents seem to have a similar level of need to me?

• What will happen if my needs change or increase? What about end of life care?

• Can I choose how support with personal care is to be provided?

• Can I or my family and friends be included in making decisions about how care is to be provided?

• Does there seem to be enough staff on duty to meet the needs of residents?

• Do the staff talk to residents in ways that show they are respected and their opinions are listened to?

• How does the home plan meet my needs in a dignified, respectful manner?

Staying safe More information Everyone has the right to live a life free from abuse and neglect. That’s why there are dedicated policies, procedures and trained workers to make sure that the protection of vulnerable adults is a key priority when services are being provided to the public.

A vulnerable adult is someone aged 18 or over who is or may be in need of community care services, and is or maybe unable to protect themselves from significant harm or from being exploited by others. This may be because: • They have a mental health problem • They have a disability • They have a sensory impairment (eg vision or hearing loss) • They are old and frail • They are ill Vulnerable adults are more likely to experience abuse than the rest of the population, and may be less likely to be able to take steps to keep themselves safe or use services that will help them to stay safe.

Forms of abuse include: • Physical abuse such as hitting, pushing, pinching, shaking, misusing medication, scalding, restraining, hair-pulling • Sexual abuse such as rape, sexual assault or sexual acts to which a vulnerable adult could not have consented, or where they were pressurised into consenting • Psychological or emotional abuse such as threats of harm or abandonment, being deprived of social contact, humiliation, blaming, controlling, intimidation, coercion, harassment, verbal abuse, or being prevented from receiving services or support • Financial or material abuse such as theft, fraud or exploitation, pressure in connection with wills, property, or inheritance, misuse of property, possessions or benefits • Neglect such as ignoring medical or physical care needs, preventing access to services or withholding food, drink or heating • Discriminatory abuse – harassment or slurs based on a person’s race, sexuality or disability • Institutional abuse – abuse in residential homes, nursing homes or hospitals when people are mistreated because of poor or inadequate care, neglect or poor working practices.

Any of these forms of abuse can be deliberate or the result of neglect or acts of omission.

Who might be causing the abuse?

The person (or people) responsible for the abuse are often very well known to the person who is being abused.

What is abuse?

They could be: • A relative, friend or neighbour A health worker, social worker or support worker • Another resident or service user • An occasional visitor or someone who is providing a service • Someone who deliberately exploits vulnerable people

Care at Home

Abuse is mistreatment by any other person that violates someone’s human and civil rights. For example, abuse can vary from treating someone with disrespect in a way that significantly affects their quality of life, to causing actual physical suffering. Abuse can happen anywhere – in a residential or nursing home, in a hospital, in the workplace, at a

The Leicester, Leicestershire and Rutland Integrated Community Equipment Service is urging people to return equipment that they no longer need or want. Often, items that have been prescribed by health or social care professionals get stored by people when they are no longer required.
